I was driving my Toyota Sienna on a busy 6 lane highway in the second to the left lane, on a bridge. Suddenly without any warning AT ALL, my vehicle lost all power and came to an abrupt halt in the middle of high speed traffic. My wife and children were all in the car and we were in imminent danger of getting hit by another vehicle. It took the police approximately 10 minutes to arrive during which we were in life threatening danger. After the vehicle stopped a message on the dashboard read Hybrid System Failure. The police tried to push our vehicle to a safe spot but the wheels wouldn't turn and the car had to be towed. Upon inspection by the mechanic it appears that there is a broken shaft in the transmission that may have caused this problem. I want to emphasize that there was NO WARNING at all and we did not have a chance to move to safety.

At the begining of 2024, I received a recall letter from Toyota regarding the airbag malfunction. I was told to wait for a remedy by Q3 of 2024. I patiently waited and was very anxious driving my vehicle. I recently did talk to my local dealer during my maintenance and they stated that they do not have any information regarding that and that I should call Toyota. I tried calling Toyota and was instructed to wait as they still have not got a remedy and that if I was getting in any accidents Toyota will be fully responsible. I bought the car to drive for my safety an this is the level of safety that I am getting. Thus I am here looking for help on how I can get this matter resolved as driving knowing that airbag has failed on the passenger side is very unsafe.

1. The vehicle displayed the Rear Cross Traffic Alert (RCTA) warning light along with the Blind Spot Monitoring (BSM) warning light on the dash board. The vehicle was brought to Toyota of Ann Arbor for an inspection. After inspecting the vehicle, Toyota of Ann Arbor reported that road debris (snow, dirt, mud, etc.) was trapped between the bumper and the sensor resulting in the sensor to malfunction. The vehicle has been taken through a car wash with an underbody wash option, but due to the position of the sensor, the sensor is not able to be cleaned from a car wash. The sensor can only be cleaned by removing the bumper. But when the bumper is removed, the sensors have to be re-calibrated and the re-calibration of the sensors can only be completed by trained professionals. 2. Due to the malfunctioning sensor, the driver's safety is put at risk by not being alerted of obstructions (both objects or persons) behind the vehicle, nor oncoming cross traffic, nor detecting vehicles in blind spots. This could cause collisions or worse, fatality, due to the malfunction sensor not alerting the driver. 3. This vehicle has been able to reproduced the problem multiple times and as mentioned above, the vehicle was inspected and confirmed by the Toyota of Ann Arbor dealership. The dealership (Toyota of Ann Arbor) mentioned that there are multiple reports of this issue on other vehicles of the same make and model. 4. The vehicle or component has not been inspected by the manufacturer, police, insurance representative, or others. 5. No prior symptoms were seen or displayed prior to the failure.

The breaks give out to a point where you run the risk of running into someone or something. We've visited Toyota 3 times now complaining of this issue yet they just state it's the regenerative breaks and how they operate. Although the breaking never got to this level of safety concern until we reached around 30,000 miles. I filed a complaint with Toyota that resulted in nothing being done. A couple examples would be sitting at a stop light and the breaks release almost causing us to run into the car in front of us. Pulling into daycare trying to stop while pulling into the parking spot and the breaks release. The last one is while going down hills the breaks seem to let go and the vehicle speeds up. It scared my wife so much that we actually sold the vehicle as Toyota deemed it safe. We loved the vehicle but can't risk the safety of our family.

The contact owns a 2021 Toyota Sienna. The contact stated that while his wife was fueling the vehicle, his 5-year-old son pressed the automatic rear driver's side sliding door switch and the door opened, bumped, and scraped the fuel door. The contact's wife immediately stopped fueling the vehicle. The contact stated that the vehicle was designed with an automatic door lock that should engage to prevent the rear driver's side sliding door from opening and bumping into the fuel door while fueling the vehicle; however, the automatic door lock failed to engage. The contact stated that he was concerned that the failure had caused metal fragments intrusion in the fuel tank. The vehicle was taken to the dealer, where the cause for the failure could not be determined. The vehicle was not diagnosed nor repaired. The manufacturer was notified of the failure and referred the contact to the dealer for assistance. The failure mileage was approximately 49,000.

I would like to make formal complaint in regards to a manufacture defect. I purchased my Toyota Sienna XSE brand new in August of 2021 at Cerritos Toyota in California. On December 24th at approximately 1:30 p.m. on Christmas eve, after filling up gas at Sam's club in Fountain Valley, CA. I pull out on to the street heading northbound on Warner approaching intersection as I come to complete stop on red. All of a sudden, my Sienna completely shut itself off and the engine light came on. I put it back on park and restart the car. As I attempt to accelerate forward. The car jerk and the brake didn't work and the engine light came on. Fortunately, I was able to make a U turn and pull into a gas station near by ( Shell) and I thank the lord almighty there was no car right behind me. I could of gotten rear ended. It was the most terrifying moment of my life. My wife and I was in the car at the time of the incident. We waited patiently over an hour for Triple AAA to have it tow the nearest dealership next to my house in Long Beach, CA. My 2021 Toyota Sienna currently has 52,023 miles and sitting at the dealership to be repair at Long Beach West Coast Toyota. I have contacted Toyota of this incident and they refused to fully covered repair instead they offered me 700 payment assistant the rest would come out of my pockets. I believed the car should be part of the campaign recall and nothing should come out my expense. I would like you you guys to investigate the incident and look into this matter as soon as possible because there thousands like my mind that could there have the same issue but was not part the campaign recall. A bulletin was published on April 13, 2022 affecting 2021 -2022 Toyota Sienna. Production Period March 3rd, 2020 through April 7th 2022. Manufacture's Campaign Number: Toyota: 22TB03

Toyota Engineering & Manufacturing (Toyota) is recalling certain 2022 Lexus LX 600, Lexus NX 350h, Lexus NX 450h+, Toytoa RAV4 Hybrid, 2021-2022 Lexus LS 500h, Toyota Mirai, RAV4 PRIME, Sienna, Venza, and 2020-2022 Toyota Highlander Hybrid vehicles. The Skid Control ECU software may not turn on the Vehicle Stability Control (VSC) system when the vehicle is restarted after the driver has disabled the VSC system. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 126, "Electronic Stability Control Systems."

Consequence & remedy

Consequence: Driving with a deactivated stability control system increases the risk of a crash.

Remedy: Dealers will update the Skid Control ECU software, free of charge. Owner notification letters were mailed May 27, 2022. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 22TA03. Lexus' number for this recall is 22LA01.
