The contact owns a 2024 Toyota Rav4. The contact stated that while driving at 5 MPH into a parking space, the vehicle experienced unintended acceleration three times and crashed into a guard pipe. No warning light was illuminated. The air bags failed to deploy. The front passenger's side seat belt was loose and failed to restrain the contact's husband in his seat. The contact stated that the front passenger's side seat belt failed to retract after the crash. The contact and her husband did not sustain any injuries, and no medical attention was received. A police report was filed. The vehicle was towed to the dealer, but had not yet been diagnosed or repaired. The dealer informed the contact that an engineer would inspect the Event Data Recorder (EDR) in three months. The manufacturer was not made aware of the failure. The failure mileage was approximately 9,000.

The contact owns a 2024 Toyota Rav4. The contact received notification of NHTSA Campaign Number: 25V595000 (Electrical system); however, the part for the recall repair was not available. The contact stated that while the vehicle was at the dealer for service, the contact inquired about the unrepaired repair. The contact was advised not to drive the vehicle because of the safety concerns. In addition, the contact was informed that if the failure occurred while driving the vehicle, the instrument cluster would be disabled. The contact was informed that the safety features would not be available because of the unrepaired recall. The contact was not satisfied with the information. The contact was very frightened to drive the vehicle. The contact stated that the vehicle was the only means of transportation. The manufacturer was made aware of the failure, and a case was filed. The contact had not experienced a failure regarding the unrepaired recall repair.

Driving on a city portion of Highway 69 between Prescott and Prescott Valley, traveling about 45mph, the panoramic moonroof suddenly exploded. There was no one near us and the road is newly paved so no obstacles. Glass shards rained down on us. This happened on a normal day about 75 degrees. Totally unexpected. I did sustain a cut on a finger. This could have harmed one of us with flying glass. We have contacted our insurance company and directed us to a collision compare place. In checking online, I see that others have experienced the same problem and there are lawsuits about this issue.

The first time I was stopped at a red light and the car started acting like it was missing and then it shut off this was during rain the message on the dash said engine stopped steering power low. The next time this happened I was parking it a parking lot and the engine did the same it rpm started going up and down like it was missing then it shut off and showed the message engine stopped steering power low this was also during rain after I had been driving in the rain. After about 20 minutes both times the car would start and drive as normal. I have looked online and multiple people have had the same issue.

The contact owns a 2024 Toyota Rav4. The contact stated that while driving at approximately 10 MPH, the vehicle came from an intersection with a stop sign and turned left, but the contact was unable to see oncoming traffic. The contact then noticed another vehicle speeding towards her and she depressed the brake pedal; however, the other vehicle crashed into the front of the contact’s vehicle. Several unknown warning lights were illuminated after the crash. The air bags did not deploy. The contact stated that her head was jarred backwards, and she received medical attention at the hospital. A Police report was filed but the contact had not yet received a physical copy. The vehicle was towed to a tow yard but was not yet deemed totaled by the insurance company. The manufacturer was not made aware of the failure. The failure mileage was approximately 15,300.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Consequence & remedy
Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.
Consequence & remedy
Consequence: An instrument panel display that does not show critical information can increase the risk of a crash or injury.
Remedy: Dealers will update the instrument panel software over-the-air (OTA) for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Additional letters will be sent, anticipated in May 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB08 and 25TA08. Lexus' numbers for this recall are 25LB05 and 25LA05.
