My 2013 Prius has 86,500 miles on it. It has been regularly maintained. A routine state inspection revealed a severely rusted rear axle beam and a front axle that is heading in that direction. Other Prius' in [XXX] have had this issue, and in some cases, the wheels have detached from the axle while the car is in motion. That is documented. That is a major safety concern, as who would expect to see an axle issue, especially in a low mileage car. I found a similar NHTSA complaint, XXX, on a 2013 Prius in [XXX], as well as issues in [XXX] with 2010 and 2012 Prius vehicles. The Toyota dealership that found this issue deemed it to be "a major safety concern" as the beam is "rusted apart". There were no warning signs, no issues, until the visual inspection. My concern is not simply the cost of repairing an older vehicle. I am concerned that a structural component has corroded to the point of being describe as "rusted apart", creating a major safety concern on a vehicle with limited mileage. The threat of a wheel separation or loss of vehicle control is a serious risk that needs exploration. I called Toyota Headquarters and they indicated that the car was out of warranty and there had been no recalls. The case number for that contact is: [XXX]. Toyota has previously recognized that corrosion of structural rear components in salt-belt vehicles can create a safety problem, but I do not know if that commentary has been applied to Gen-3 Prius'. I respectfully request the NHTSA review its complaint records and manufacture submissions to determine any pattern of premature corrosion or structural failure involving the rear axle beam/rear cross-member of 2010-2015 Toyota Prius vehicles, particularly vehicles operated in states where road salt is routinely used. Safety concern: This issue affects braking performance and disables ABS and traction control, creating a safety hazard, especially in emergency braking situations. This appears to be a known issue with Toyota Prius brake actuator assemblies. Toyota previously issued a warranty extension, but my vehicle is now outside the time window despite low mileage. This failure occurred prematurely and without warning. What happened: The warning lights temporarily went off after adding brake fluid but returned shortly after. The pump continues to cycle frequently, indicating system failure.

I was driving and notice ABS light and other light came on, then noticed that when I tried to brake it felt weak, had to pump brake, then later noticed a leak on brake fluid. I took car to Toyota and they checked it and said brake booster and booster pump needed replacing and they wanted to charge me 3,000 dollars. I could not do the job so I checked around and the auto one in my area informed me that many Toyota Prius’s had a recall on that issue. I made an appointment with Parks Toyota in Deland, Florida to fix problem but when he got there they told me that my car was not on recall for that but was for battery EV fuse but couldn’t do because my engine light was off. I am having such a difficult time with this car. I cannot risk my life and that of others driving with this brake condition if I can’t have the manufacturer or who ever has to to take care of this issue. So far I was able to get a mechanic to do some adjustments so I can drive. I am being very careful and only use it as needed. I stopped looking for a part-time job because I need this fixed first. The Toyota brake booster system failed and is no longer safe to be on the road. The car will no longer brake properly and is a danger to anyone else on the road. It was inspected by Toyota prior. The entire dashboard warning lights for the brake system lit up 2 years ago and has fully failed now.

Pulled out of the driveway drove about a 100 ft heard large clunking sound front end dropped down as if there was a flat tire pulled off the highway drove through the grass 100 ft back to my home the car started smoking from the engine compartment and burst into flames. Tried to extinguish with fire extinguishers and water hose while calling 911. Fire Department extinguished the fire. Car is a complete loss. There was an oil leak trail from where the car first drop down through the grass. The car did not have a previous oil leak. The car had 300,000 miles on it. No one was injured.

my 2013, Toyota Prius recently suffered a sudden hybrid system failure with DTC space P0A0F and a check hybrid system warning. This matches the symptoms described in NHTS a safety recall 20V369000. Your recall look up tool shows my Vin is not included. However, I have never received this recall repair, and my failure is identical to the documented safety issue. I am requesting help in getting Toyota to complete a goodwill repair or warranty extension review as this is clearly a safety related inverter failure for which Toyota has acknowledged responsibility and similar vehicles.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2013-2015 Prius and 2014-2017 Prius V vehicles not included in recall 18V-684. Excessive voltage in the Intelligent Power Module (IPM) within the inverter may cause the hybrid system to shut down, causing the vehicle to stall while being driven.

Consequence & remedy

Consequence: An unexpected hybrid power system shut down may increase the risk of a crash.

Remedy: Toyota will notify owners, and dealers will perform a software update for the hybrid system. If the vehicle has experienced an inverter failure with certain hybrid system faults related to this condition, the inverter assembly will be repaired or replaced, prior to software update. All repairs will be performed free of charge. This recall began July 24, 2020. Owners may contact Toyota customer service at 1-888-270-9371. Toyota's number for this recall is 20TB10 / 20TA10.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2010-2014 Toyota Prius and 2012-2014 Toyota Prius V vehicles. Excessive voltage in the Intelligent Power Module (IPM) within the inverter may cause the hybrid system to shut down, causing the vehicle to stall while being driven.

Consequence & remedy

Consequence: If the hybrid system shuts down unexpectedly, it can increase the risk of a crash.

Remedy: Toyota will notify owners, and dealers will update the software for the motor/generator control electronic control unit (ECU) and the hybrid control ECU as necessary, free of charge. The recall began January 22, 2019. Owners may contact Toyota customer service at 1-888-270-9371. Toyota's number for this recall is J0V. Note: The software updates in this recall are needed even if the vehicle had the software previously updated under recalls 14V-053 or 15V-449.

In the affected vehicles, the Intelligent Power Module (IPM) inside the inverter module (a component of the hybrid system) contains transistors that may become damaged from high operating temperatures. If this occurs, various warning lamps will be illuminated on the instrument panel and the vehicle will have reduced power allowing it to only drive a short distance.

Consequence & remedy

Consequence: The vehicle may enter a fail-safe/limp-home mode that limits the drivability of the vehicle. The hybrid system could also shut down completely resulting in a vehicle stall, increasing the risk of a crash.

Remedy: Toyota will notify owners and dealers will update the software for both the motor/generator control electronic control unit (ECU) and the hybrid control ECU, free of charge. If an owner experiences a failure of the inverter before the vehicle receives updated software, the dealer will repair or replace the inverter assembly with a new one at no charge. NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. On September 19, 2019, the Office of Defects Investigation (ODI) received a defect petition from Mr. Roger Hogan requesting that the Agency investigate and recall certain Toyota Prius, Prius PHV, Camry Hybrid and Avalon Hybrid vehicles for a brake actuator valve wear condition covered under Toyota Customer Support Programs ZJB and ZKK (subject CSP?s). In support of his request, the petitioner identified 117 NHTSA complaints allegedly related to the subject CSP condition, including 60 reporting crashes. On September 30, 2019, ODI opened Defect Petition DP19-004 to evaluate the petitioner?s request for an investigation. In August 2020, the petitioner notified the Agency that he was withdrawing his petition. Because the petitioner has withdrawn the petition, we are denying the petition as moot and no further analysis of this petition is necessary.NHTSA is authorized to issue an order requiring notification and remedy of a defect if the Agency?s investigation shows a defect in the design, construction, or performance of a motor vehicle that presents an unreasonable risk to safety. 49 U.S.C. ?? 30102(a)(9), 30118. Because the petitioner has with withdrawn the petition, the petition is denied as moot. This action does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency notes that it is not required to receive a defect petition prior to opening a defect investigation and will take further action if warranted by future circumstances.
