INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6) SEVERAL ELECTRONIC MALFUNCTIONS, ALL OCCURRING WHEN MY BLUE TOOTH IS CONNECTED. SOME INADEQUATELY SHIELDED WIRING OR CONNECTOR(S) COULD ACCOUNT FOR THIS. I SAW IN THE CONGRESSIONAL TESTIMONY BY A LADY IN A LEXUS USED HER BLUE TOOTH CONNECTION TO CALL HER HUSBAND DURING THE 6 MINUTES OF STRUGGLE TO STOP HER CAR'S OUT OF CONTROL ACCELERATION BY VARIOUS METHODS INCLUDING BOTH FEET ONN THE BRAKE. I HAVE A 2009 TOYOTA PRIUS THAT THE DEALER SAYS IS WORKING "NORMAL." THESE EVENTS HAVE NOT BEEN NORMAL, AND ON ONE OCCASION MY ENERGY SCREEN THAT WAS DOING WIERD THINGS ACTUALLY CHANGED TO THE SPEED DIAL SCREEN. IN ANOTHER INCIDENT, (SMART KEY FEATURE IS DISABLED BECAUSE IT DRAINS THE BATTERY) I LOCKED THE CAR WITH MY KEY FOB AS USUAL, AND WHEN I RETURNED COULD NOT UNLOCK IT THAT WAY AND HAD TO DIG OUT THE "KEY" TO OPEN THE DOOR SO I COULD START THE CAR. MY BATTERY HAS DIED 3 TIMES AND WAS FINALLY REPLACED. I BOUGHT THIS PRIUS TOURING MARCH 26, 2009. THE DEALER HAS NO IDEA OF WHAT'S GOING ON, ONLY TELLS ME THE CAR IS "NORMAL." I SUSPECT SOMETHING SO SIMPLE AS SHIELDING FROM THE WEAK BLUE TOOTH SIGNAL COULD WELL BE CAUSING A VARIETY OF PROBLEMS, PROBABLY DEPENDING ON WHERE THE FAULTY WIRING IS LOCATED. IF NOBODY IS INVESTIGATING THIS, IIT IS A MISTAKE!. [XXX] *TR

2009 TOYOTA PRIUS: WHEN DRIVING OVER POT-HOLES, BUMPS, OR UNEVEN PATCHES OF ROAD AT ALL DIFFERENT SPEEDS, THE CAR'S "TRACTION LIGHT" WILL FLASH AND THE CAR WILL MOMENTARILY SURGE FORWARD. IT SEEMS AT THIS TIME THAT THE BRAKES ALSO TEMPORARILY LOSE POWER. THIS HAPPENS SEVERAL TIMES A WEEK. THIS WORRIES ME AS I FEEL THIS COULD LEAD TO CAR ACCIDENTS OR PUT PEDESTRIANS IN DANGER. SPOKE WITH MANY OTHER PRIUS OWNERS AND THEY REPORT THE SAME PROBLEM. CONTACTED THE DEALERSHIP WHERE I BOUGHT THE CAR AND THE SERVICE MANAGER TOLD ME THIS WAS A "NORMAL FUNCTION" OF THE ABS. I STILL FEEL THIS IS A SAFETY ISSUE THAT IS NOT BEING PROPERLY ADDRESSED. I'VE DRIVEN MANY OTHER CARS WITH ABS AND NONE OF THEM BEHAVED THIS ERRATICALLY. *TR

AFTER GETTING THE VEHICLE PULLED OVER TO THE SIDE OF THE INTERSTATE, THERE WAS A LOT OF SMOKE COMING FROM THE FRONT BRAKE PADS. I RESTARTED THE VEHICLE IN AN ATTEMPT TO GET IT OFF THE INTERSTATE FOR FURTHER INSPECTION. AS I EXITED THE HIGHWAY, I TURNED THE VEHICLE ONTO THE OFF RAMP BUT THE VEHICLE DID NOT TURN AND DROVE US DIRECTLY INTO THE GUARD RAIL. I CONTACTED TOYOTA AND THEY HAVE DENIED THERE WAS ANYTHING WRONG WITH THE VEHICLE AND DISMISSED MY COMPLAINT. *TR

ARTICLE: "GOING DOWNHILL AND APPROACHING A STOP SIGN. MY FOOT IS STEADY ON THE BRAKE AND I AM GRADUALLY COMING TO A STOP. THEN, I INADVERTENTLY GO OVER A BUMP IN THE ROAD AND THE PRIUS LUNGES FORWARD. I HAD TO IMMEDIATELY SLAM HARD ON THE BRAKES IN ORDER TO KEEP THE CAR FROM GOING INTO THE INTERSECTION. MULTIPLE ISSUES WITH 2009 PRIUS. FIRST: LOSS OF AIR PRESSURE IN FRONT LEFT TIRE FROM TIME I PICKED UP THE VEHICLE TO PRESENT. I CONTINUALLY NEED TO PUT AIR IN THE TIRE, USUALLY ON A WEEKLY/BI-WEEKLY BASIS. THE OTHER TIRES ARE FINE. SECOND: FOR THE SAME TIRE, THE TIRE PRESSURE LIGHT WOULD NOT BE TRIGGERED UNTIL ~23 POUNDS, SHOULD HAVE BEEN TRIGGERED IN SAME TIRE AT 33 LBS. THIRD: VEHICLE WOULD NOT STOP WHEN APPLYING PRESSURE ON BRAKES AT SLOW SPEED. THIS HAS OCCURRED ABOUT 6 TIMES IN A 6 MONTH PERIOD. SIMULTANEOUSLY, THE VSC LIGHT WENT ON AND THE SLID INDICATOR WENT ON. THE TERRAIN WAS ON EITHER ASPHALT OR CONCRETE ROADS, SLIGHTLY DOWNHILL (CONCRETE) AND ONE STREET IS A BIT ROUGH (FILLED IN POT HOLES). ALSO, BOTH AREAS ARE WITHIN A QUARTER MILE OF EACH OTHER AND NEAR THE OCEAN. WITH EACH OCCURRENCE THE VSC WENT INTO EFFECT. ON 11/21/09 IT JOLTED HARD MUCH MORE THAN THE OTHER OCCASIONS; THIS DATE WAS THE INCIDENT ON THE BRIDGE WHEN THE CAR WAS GOING IN A SLIGHT DOWNWARD DIRECTION. I LOST CONTROL OF THE VEHICLE FOR SEVERAL SECONDS AND WAS AFRAID THAT I WOULD HIT THE CAR IN FRONT OF ME; I BEEPED MY HORN TO WARN THE DRIVER IN FRONT OF ME. MY VEHICLE DID STOP IN TIME BUT IT TOOK ALOT LONGER THAN IT SHOULD HAVE TAKEN. ON A LATER OCCASION, I WAS GOING ABOUT 5-10 MPH FASTER ON A NEARBY STREET AND ATTEMPTED TO STOP FOR A STOP SIGN AND IT DID THE SAME THING, HOWEVER THE VSC DID NOT JOLT BACK AS HARD. THE VEHICLE WAS NOT ANGLED DOWN AS MUCH AS THE 11/21/09 INCIDENT. ON 11/21/09 I TOOK THE VEHICLE TO CABE TOYOTA DEALERSHIP AND THEY LOOKED AT THE CAR BUT FOUND NOTHING WRONG WITH IT. I ALSO HAD THE CAR LOOKED AT BY TOYOTA CORP TODAY AND WAS TOLD THAT THE DRIVER FEELS THAT I FELT THE ABS BRAKES ONLY. THIS IS NOT THE CASE. WHEN I STEPPED ON THE BRAKES EACH OCCASION IT WAS AS THOUGH THERE WAS NOTHING THERE AND I HAD NO CONTROL IN STOPPING IT. THIS WAS ESPECIALLY TRUE WHEN THE VSC KICKED IN (THE JERKING). EACH TIME IT LASTED MANY, SEVERAL OR MORE SECONDS. *TR

Southeast Toyota Distributors, LLC (SET) is recalling certain models interspersed through model years 2009 through 2013 as follows: model year 2009-2012 Tacoma, 4Runner, Camry, Camry Hybrid, Prius, and RAV4; model year 2009-2010 Avalon, FJ Cruiser, and Highlander Hybrid; model year 2010-2013 model year Corolla, Sienna and Tundra; model year 2009-2013 Highlander and Venza; model year 2012 Prius V; and model year 2010-2012 Sequoia. During modification by SET to include accessories such as leather seat covers, seat heaters or headrest DVD systems, these vehicles may not have had the passenger seat occupant sensing system calibration tested. Without passing the calibration test, the occupant sensing system may not operate as designed.

Consequence & remedy

Consequence: If the front passenger seat occupant sensing system is out of calibration, the front passenger airbags may not deploy or they may deploy inappropriately for the passenger's size and position. This could increase the risk of personal injury during the event of a vehicle crash necessitating airbag deployment.

Remedy: Southeast Toyota will notify owners, and dealers will test the sensitivity of the occupant detection sensors, and recalibrate them as necessary. The recall began on March 21, 2013. Owners may contact Southeast Toyota at 1-800-301-6859.

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

DP13001 · Intermediate Steering Shaft Separation

Opened Feb 21, 2013 · Closed Apr 8, 2020

Status: closed (inferred from source dates) · Steering; Steering:column; Steering:linkages

The Office of Defects Investigation (ODI) received a January 2013 defect petition requesting that it open an investigation on Model Year (MY) 2004-2009 Toyota Prius vehicles to address a possible manufacturing safety defect related to the steering column intermediate shaft.During the defect petition evaluation, ODI: 1.Reviewed the petition and its enclosures; 2.Reviewed the subject steering system layout; 3.Examined the Petition Vehicle?s history, including its repair history; 4.Reviewed and followed-up potentially related consumer complaints; 5.Assessed Safety Recalls 06V-188 and 12V-537; and, 6.Requested and examined complaint, claim, field report, service history, and warranty information from Toyota.ODI's review revealed that the petition vehicle had been recalled (Recall 06V188) to remedy a defect in the system identified in the petition with the remedy taking place prior to the failure that prompted the petition. Circumstances point to incorrect reassembly of the petitioner's vehicle by those who performed the recall remedy.ODI examined the issues raised by this petition to identify any related safety defect trend in the general subject vehicle population. That review identified a low rate of related consumer complaints (twenty-five over a eight year period from a population of over 600,000 vehicles) and no pattern that would identify either a safety defect not already covered by the two existing steering safety recalls by Toyota or a systematic problem with either recall remedy.Based on this evaluation, ODI does not believe that additional investigation will lead to a finding that a defect related to motor vehicle safety exists in the subject vehicles. Therefore, in view of the need to allocate and prioritize NHTSA?s limited resources to best accomplish the agency's safety mission, the petition is denied. The denial of this petition does not constitute a finding by NHTSA that a safety-related defect does not exist.
