WHILE ENTERING A HIGHWAY ONRAMP IN MY COMPANY'S 2008 TOYOTA PRIUS, THE CAR SUDDENLY BEGAN ACCELERATING OUT OF CONTROL WITHOUT APPLICATION OF THE GAS PEDAL. I HAD TO PRESS HARD ON THE BRAKE JUST TO HOLD THE CAR AT 65 MPH. THE CAR WAS SURGING AND LUNGING FORWARD, AND I WORRIED I WOULDN'T BE ABLE TO FIND A PLACE TO PULL OVER BEFORE LOSING CONTROL COMPLETELY. FOR THE NEXT 5-10 MINUTES, AS I APPROACHED TRAFFIC IN FRONT OF ME, THE CAR CONTINUED TO ACCELERATE, EVEN THOUGH I HAD MY FOOT ON THE BRAKE THE ENTIRE TIME. AT ONE POINT I NOTICED THE CAR WAS REACHING SPEEDS IN EXCESS OF 95 MPH. A LOUD RATTLING NOISE ENSUED AND AN ELECTRICAL SMELL FILLED THE CAR. EVENTUALLY, THE BRAKE COMPLETELY FAILED TO HAVE ANY EFFECT, AND, BARRELING DOWN THE HIGHWAY AT 90 MPH, I PUT THE CAR IN NEUTRAL, BUT IT CONTINUED TO ACCELERATE. AT THIS POINT I HAD MY RIGHT FOOT ON THE BRAKE PEDAL AND DEPRESSED THE PARKING BREAK ALL THE WAY TO THE FLOOR WITH MY LEFT FOOT, THIS SLOWED THE CAR SLIGHTLY, BUT NOT ENOUGH TO REGAIN CONTROL. FINALLY, I TURNED THE CAR OFF COMPLETELY, AND WITH MY HAZARD LIGHTS ON, MANAGED TO PULL THE CAR AWAY FROM TRAFFIC AND OFF ON THE SHOULDER OF THE HIGHWAY, WHERE I COASTED FOR A LONG DISTANCE, SPEEDS DECREASING GRADUALLY UNTIL EVENTUALLY IT STOPPED. SMOKE AND A BURNING ELECTRICAL SMELL FILLED THE CAR. I FILED A POLICE REPORT WITH AN OFFICER WHO HAPPENED TO ALSO BE ON THE SHOULDER, GIVING SOMEONE A SPEEDING TICKET. MY COMPANY TOWED THE CAR TO A DEALERSHIP, AND AN INVESTIGATION IS SUPPOSEDLY UNDERWAY, THOUGH I'M NOT OPTIMISTIC ABOUT ANY SORT OF RESOLUTION BASED ON THE TESTIMONIES I HAVE READ FROM OTHERS WHO HAVE EXPERIENCED THE SAME THING. LUCKILY, IN MY CASE, THIS DIDN'T RESULT IN AN ACCIDENT, AND NOONE WAS KILLED. *TR

CAR WILL NOT STAY IN DRIVE AFTER IT HAS BEEN IDLING, IN PARK, ON A HOT DAY. WHEN GETTING BACK IN CAR AND PUTTING THE CAR IN DRIVE, IT SLIPS BACK TO NEUTRAL AFTER ACCELERATOR PEDAL IS PRESSED. CAR GOES A FEW FEET THEN SLIPS BACK TO NEUTRAL. AFTER PUTTING IT BACK INTO DRIVE AND PRESSING THE PEDAL AGAIN, IT SLIPS BACK TO NEUTRAL. THE ONLY SOLUTION IS TO SHUT THE CAR OFF COMPLETELY AND RESTART AFTER 15-20 MINUTES. THIS HAS TWICE LEFT ME NOSED OUT INTO A BUSY STREET. I FIRST TRIED TO RESOLVE THIS ISSUE WITH MY LOCAL DEALERSHIP, TOYOTA, ON 9/17/10 . THEY CLAIMED THEY COULD NOT DUPLICATE THE PROBLEM AND SAID THEY HAD NEVER HEARD OF IT EVEN THOUGH I BROUGHT THEM SEVERAL INTERNET ACCOUNTS OF OTHERS WHO REPORTED THE EXACT SAME PROBLEM. THIS YEAR, WHEN THE PROBLEM STARTED AGAIN WITH THE HOT WEATHER, I REPORTED IT TO TOYOTA USA (CASE # ). THEY ALSO SAID THEY'VE NEVER HEARD OF THE PROBLEM BEFORE AND INSISTED THAT I MUST HAVE THE PROBLEM DUPLICATED AT THE DEALERSHIP. SO, I RETURNED TO MY DEALERSHIP BUT THIS TIME WITH A DVD COPY OF THE PROBLEM THAT I HAD TAKEN WITH MY MOVIE CAMERA. THIS DVD SHOWS THE ISSUE PERFECTLY. THEY RESPONDED THAT A TOYOTA TECH HAS FOUND AN "UPGRADE" THAT THEY CAN PROVIDE ME FOR $240 LABOR. THEY COMMENTED THAT MY CAR HAD 52,000 MILES ON IT WHEN THE PROBLEM FIRST STARTED SO THEY'RE BEING REALLY NICE IN PROVIDING THE PART "UPGRADE". IT'S ALMOST AS IF THEY'RE SAYING THAT AT 52,000 MILES I SHOULDN'T EXPECT THE TRANSMISSION TO STAY IN GEAR!?!? I TOLD THEM THAT THE POWER-TRAIN IS WARRENTED FOR 100,000 MILES AND IF THIS ISN'T A POWER-TRAIN ISSUE I DON'T KNOW WHAT IS. FUTHERMORE, THEY MAY REGARD IT AS AN "UPGRADE" BUT I REGARD IT AS A SAFETY DEFECT THAT SHOULD BE REPLACED BY TOYOTA AT NO CHARGE. I HAVE A COPY OF THE DVD, CLEARLY SHOWING THE ISSUE, THAT I WOULD BE HAPPY TO PROVIDE TO YOU. THANK YOU FOR YOUR TIME. 12V536000 · Engine And Engine Cooling; Hybrid Propulsion System: Inverter

Nov 14, 2012

Toyota is recalling certain model year 2004-2009 Prius vehicles manufactured August 6, 2003, through March 30, 2009 and FCHV-adv vehicles manufactured December 12, 2008, through September 13, 2011. During manufacturing, a scratch may have occurred inside of the electrically driven water pump at the coil wire. The coil wire may corrode at the scratched portion.

Consequence & remedy

Consequence: The corroded coil wire may break and the water pump could stop. The corroded coil wire could cause a short circuit between coil wires and a possible open fuse, creating a stall-like condition of the hybrid system while the vehicle is being driven. This may increase the risk of a vehicle crash.

Remedy: Toyota will notify its owners, and dealers will replace the electric water pump, free of charge. The recall began on January 17, 2013. Owners may contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331.

TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.

Consequence & remedy

Consequence: A STUCK OPEN ACCELERATOR PEDAL MAY RESULT IN VERY HIGH VEHICLE SPEEDS AND MAKE IT DIFFICULT TO STOP THE VEHICLE, WHICH COULD CAUSE A CRASH, SERIOUS INJURY OR DEATH.

Remedy: TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. The Office of Defects Investigation (ODI) received a January 2013 defect petition requesting that it open an investigation on Model Year (MY) 2004-2009 Toyota Prius vehicles to address a possible manufacturing safety defect related to the steering column intermediate shaft.During the defect petition evaluation, ODI: 1.Reviewed the petition and its enclosures; 2.Reviewed the subject steering system layout; 3.Examined the Petition Vehicle?s history, including its repair history; 4.Reviewed and followed-up potentially related consumer complaints; 5.Assessed Safety Recalls 06V-188 and 12V-537; and, 6.Requested and examined complaint, claim, field report, service history, and warranty information from Toyota.ODI's review revealed that the petition vehicle had been recalled (Recall 06V188) to remedy a defect in the system identified in the petition with the remedy taking place prior to the failure that prompted the petition. Circumstances point to incorrect reassembly of the petitioner's vehicle by those who performed the recall remedy.ODI examined the issues raised by this petition to identify any related safety defect trend in the general subject vehicle population. That review identified a low rate of related consumer complaints (twenty-five over a eight year period from a population of over 600,000 vehicles) and no pattern that would identify either a safety defect not already covered by the two existing steering safety recalls by Toyota or a systematic problem with either recall remedy.Based on this evaluation, ODI does not believe that additional investigation will lead to a finding that a defect related to motor vehicle safety exists in the subject vehicles. Therefore, in view of the need to allocate and prioritize NHTSA?s limited resources to best accomplish the agency's safety mission, the petition is denied. The denial of this petition does not constitute a finding by NHTSA that a safety-related defect does not exist.
