TOYOTA PRIUS PROBLEM - CRUISE CONTROL MALFUNCTION: ON 2 JAN 2010 (ABOUT 0600 HRS) I WAS DRIVING A RENTED PRIUS (ALAMO) FROM PALM BEACH, FL TO MIAMI, FL. I WAS USING CRUISE CONTROL AND GOING THE LISTED SPEED LIMIT FOR I-95 (65MPH). THE CAR SEEMED TO BE PERFORMING CORRECTLY FOR THE FIRST 35 PLUS MILES BUT THEN I GOT INTO HEAVY TRAFFIC AND HAD TO BRAKE. THE CRUISE CONTROL RELEASED NORMALLY AND I SLOWED TO ABOUT 60 MPH. WHEN THE TRAFFIC CLEARED I PUSHED THE CRUISE CONTROL "RESUME" BUTTON AND THE PRIUS STARTED TO ACCELERATE AS NORMAL. HOWEVER, THE ACCELERATION DID NOT STOP AT THE ORIGINAL SPEED I HAD SET AND HAD BEEN DRIVING AT (65 MPH) BUT CONTINUED TO CLIMB TO 70 MPH AND HIGHER TO NEAR 75 MPH WHEN I HAD TO HIT THE BRAKES AND SHUT THE CRUISE CONTROL OFF TO SLOW THE CAR. AFTER THAT I DID NOT USE THE CRUISE CONTROL FOR THE REMAINDER OF MY TRIP. I JUST THOUGHT THERE MAY HAVE BEEN A GLITCH OR OPERATOR ERROR AND I DID NOT THINK OF IT AS THE PROBLEM I RECENTLY SAW ON THE NEWS. I LIVE IN THE REPUBLIC OF PANAMA AND DID NOT SEE ANYTHING ABOUT THE PRIUS PROBLEMS UNTIL RECENTLY. AS I SAID THE CAR WAS A RENTAL AND I JUST MENTIONED TO THE CHECK IN PERSON THAT I HAD A SLIGHT PROBLEM WITH THE CRUISE CONTROL BUT I DO NOT KNOW IF IT WAS REPORTED. *TR

MODEL YEARS PRIOR TO 2010 SHOULD BE INCLUDED IN THE INVESTIGATION. I AM GOING TO MAKE AN APPOINTMENT WITH MY DEALER. THANK YOU *TR

THE CAR WAS SERVICED 1/4/10 AT TOYOTA DEALER FOR STATE INSPETION AND USUAL MT. NEVER MENTIONED RECALL OF MATS WHICH I NOW FIND WAS A WELL KNOWN FACT IN SEPT.09. THEY ARE INSISTING IT WILL DRY OUT AND BE FINE,SALT WATER ON ANY CAR IS DEADLY BUTI HYBRID. THE HOUR IT SAT IN WATER PRIOR TO TOW,THE CAR STILL WOULD NOT SHUT OFF. I CALLED TOYOTA THAT DAY AS TOW TOOK CAR TO IT,THEY INSISTED THAT THERE WAS NO ISSUES WITH 2008 PRIUS. THE CAR WAS MOVING MUCH FASTER THAN MY 10MPH OF ORIGIN AND WOULD NOT BRAKE,SHIFT OR SHUT OFF. WATER WAS POURING OUT STEREO SPEAKERS AND UP UNDER HOOD ON/OR UNDER ENGINE BLOCK,THE ENGINE RAN FOR THE HOUR THE CAR SAT IN SALT WATER. TOYOTA HSA BEEN ABSOLUTELY NO HELP AND STILL INSISTS DESPITE AN OFFICAL RECALL THAT THERE WERE TROUBLES WITH 2008 PRIUS. THE POLICE PHOTOS SAY IT ALL. *TR

TOYOTA PRIUS, 2008, INVOLVED IN ACCIDENT , AUGUST 13, 2009 WHEN IT ACCELERATED RAPIDLY OUT OF CONTROL AFTER BEING AT A STOP SIGN FOR 4 MINUTES STOPPED IN A LINE OF TRAFFIC, HITTING CAR IN FRONT AND BEING VERY DIFFICULT TO BRING TO A STOP. WENT FROM 0 TO 45 MPH VERY QUICKLY AND HARD TO STOP .TOYOTA HAS NOT RESPONDED TO COMPLAINT OR MY INSISTENCE THAT IT BE FIXED. WAS DEEMED BY JUDGE WHO HEARD THE APPEAL OF TICKET TO BE A MECHANICAL FAILURE. FIRST ACCIDENT FOR ME IN 48.5 YEARS OF DRIVING. INITIALLY I THOUGHT IT WAS BRAKE FAILURE, BUT AFTER THE INFO CAME OUT, IT MATCHES EXACTLY THE ACCELERATOR STICKING THAT HAS BEEN DESCRIBED. FILED COMPLAINT WITH TOYOTA IN OCTOBER AND HAVE NOT HEARD BACK. HAVEN WAS THE CONTACT, CLAIM NUMBER AT TOYOTA IS #0910145720. ALSO NOTIFIED DEALER WHERE IT WAS PURCHASED AND HAVE NOT HEARD FROM THEM. WAS ASSURED I'D HEAR IN JANUARY AND GOT A LETTER INDICATING THAT I SHOULD TAKE THE FLOOR MAT OUT. IT HAPPENED AGAIN IN NOVEMBER OF 2009, ALTHOUGH I IMMEDIATELY RECOGNIZED IT AND GOT THE CAR STOPPED QUICKLY. I'M NOW AFRAID TO DRIVE IT BUT NEED MY CAR TO WORK. *TR

I CALLED IN AND COMPLAINED TO BROOKLYN TOYOTA AND DROVE IN FOR SERVICE THE SAME DAY ( INCLUDING TO DO THE TIRE INSPECTION). TOYOTA DEALER DIDNT FIND ANY PROBLEMS WITH THE CAR'S ACCELERATION OR TIRES BUT AS A RESULT OF THE DEALER INCOMPETENCE THAT WAS UNABLE TO DETECT A NAIL IN THE TIRE -I HAD A FLAT TIRE JUST 20 MIN AFTER I LEFT THE DEALER. I CALLED AAA FOR ASSISTANCE INCIDENT # 2181 ON 11/18/09. I ALSO CALLED ADVANTAGE TOYOTA IN LYNBROOK , LI TO INSPECT AND POSSIBLY FIX THE ACCELERATION AND THE BUMPER. THEY ALSO SAID THEY DIDNT HAVE ANY RECALL NOTICES TO FIX AND COULDNT FIX THE BUMPER UNLESS TAKEN TO A PAID COLLISION SHOP. 2) I ALSO HAD THE CAR IN THE ADVANTAGE TOYOTA SERVICE ON 11/4/09 FOR OIL, FILTER, CHECK UP AND THE BRAKES AND TIRES. I TOLD THEM I EXPERIENCE LOOSING TRACTION / BRAKES AFTER HITTING THE BUMPS OR THE POTHOLE OR MAN HOLES.

FOR SOME UNKNOWN REASON, THE CAR'S ENGINE REVVED (VERY HIGH RPMS - I COULD HEAR IT FROM THE BASEMENT). THE CAR GRAZED THE SIDE OF THE HOUSE, WENT THROUGH A SHORT WALL (BRICK - ABOUT 1 FOOT TALL, 1 FOOT THICK AND 5 FEET LONG) AND AN IRON DECK GATE. THE CAR ENDED UP ON THE DECK AFTER BREAKING OFF TWO 6X6 SUPPORT POSTS. THE DECK HAS HORIZONTAL STAINLESS STEEL CABLES INSTEAD OF PICKETS, AND I BELIEVE THAT THESE ARE WHAT KEPT THE CAR FROM GOING OVER THE EDGE - A 25 FOOT DROP. *TR

Nov 14, 2012

Toyota is recalling certain model year 2004-2009 Prius vehicles manufactured August 6, 2003, through March 30, 2009 and FCHV-adv vehicles manufactured December 12, 2008, through September 13, 2011. During manufacturing, a scratch may have occurred inside of the electrically driven water pump at the coil wire. The coil wire may corrode at the scratched portion.

Consequence & remedy

Consequence: The corroded coil wire may break and the water pump could stop. The corroded coil wire could cause a short circuit between coil wires and a possible open fuse, creating a stall-like condition of the hybrid system while the vehicle is being driven. This may increase the risk of a vehicle crash.

Remedy: Toyota will notify its owners, and dealers will replace the electric water pump, free of charge. The recall began on January 17, 2013. Owners may contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331.

TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.

Consequence & remedy

Consequence: A STUCK OPEN ACCELERATOR PEDAL MAY RESULT IN VERY HIGH VEHICLE SPEEDS AND MAKE IT DIFFICULT TO STOP THE VEHICLE, WHICH COULD CAUSE A CRASH, SERIOUS INJURY OR DEATH.

Remedy: TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

The Office of Defects Investigation (ODI) received a January 2013 defect petition requesting that it open an investigation on Model Year (MY) 2004-2009 Toyota Prius vehicles to address a possible manufacturing safety defect related to the steering column intermediate shaft.During the defect petition evaluation, ODI: 1.Reviewed the petition and its enclosures; 2.Reviewed the subject steering system layout; 3.Examined the Petition Vehicle?s history, including its repair history; 4.Reviewed and followed-up potentially related consumer complaints; 5.Assessed Safety Recalls 06V-188 and 12V-537; and, 6.Requested and examined complaint, claim, field report, service history, and warranty information from Toyota.ODI's review revealed that the petition vehicle had been recalled (Recall 06V188) to remedy a defect in the system identified in the petition with the remedy taking place prior to the failure that prompted the petition. Circumstances point to incorrect reassembly of the petitioner's vehicle by those who performed the recall remedy.ODI examined the issues raised by this petition to identify any related safety defect trend in the general subject vehicle population. That review identified a low rate of related consumer complaints (twenty-five over a eight year period from a population of over 600,000 vehicles) and no pattern that would identify either a safety defect not already covered by the two existing steering safety recalls by Toyota or a systematic problem with either recall remedy.Based on this evaluation, ODI does not believe that additional investigation will lead to a finding that a defect related to motor vehicle safety exists in the subject vehicles. Therefore, in view of the need to allocate and prioritize NHTSA?s limited resources to best accomplish the agency's safety mission, the petition is denied. The denial of this petition does not constitute a finding by NHTSA that a safety-related defect does not exist. The agency reserves the right to take further action if warranted by the circumstances. For more information, see the Federal Register notice in the public file for this action (https://www.nhtsa.gov/recalls#vehicle, select NHTSA ID, enter: DP13001)The ODI reports cited above can be reviewed on-line (https://www.nhtsa.gov/recalls#vehicle) under the following NHTSA ID numbers:10407520, 10418710, 10429551, 10473654, 10478195, 10478053, 10437229, 10489425, 10510722, 10639984, 10648277, 10499778, 10712626, 10723196, 10787530, 10808124, 10838803, 11071646, 11098144, 11099507, 11102659, 11160767, 11192607, 11287550, 11287606
