I BELIEVE IT IS THE VEHICLE'S RIGHT REAR WHEEL THAT SPINS AND SPEEDS UP GOING OVER (STONE, BUMP, DEVIATION IN ROAD) AND LUNGES FORWARD. I FELT LOSS OF CONTROL FOR A BRIEF SECOND BUT THOUGHT I WAS GOING TO CRASH INTO CAR AHEAD OF ME. CAR FREQUENTLY DOES THIS SINCE I PURCHASED IT IN 2007 AND WAS TOLD BY DEALER THAT THIS HAPPENS WHEN THE CAR CHANGES FROM STANDARD BRAKING SYSTEM TO OTHER BRAKING SYSTEM. THE COMPLAINT HAS NEVER BEEN REGISTERED UNTIL IT HAPPENED AGAIN AND IN FEB 2010 AND I HAD HEARD OF THIS HAPPENING TO THE PRIUS 2010. I HAVE SINCE HAD THE COMPLAINT PUT IN WRITING. THE DEALER STATED IT WAS BECAUSE I NEEDED NEW TIRES AND I TRIED TO EXPLAIN THAT THIS HAS HAPPENED SINCE I BOUGHT THE CAR. FREQUENTLY WHEN I START TO DRIVE, WHETHER THE ROAD IS WET OR DRY, THE CAR WARNING LIGHT SHOWS THAT IT IS SKIDDING. I CAN FEEL IT SKIDDING BUT I DO NOT UNDERSTAND WHY. THE DEALER STATED THERE IS NO FIX FOR THIS PROBLEM IT IS JUST HOW THE CAR IS. THEY STATED THAT IF TOYOTA GETS ENOUGH COMPLAINTS THEY WILL LOOK INTO IT. THEY STATED IF THIS IS A PROBLEM THAT TOYOTA FINDS AND THEY FIGURE OUT HOW TO FIX IT, I WILL BE ABLE TO HAVE THE PROBLEM RESOLVED. I HAVE NOT CONTACTED THE TOYOTA CORPORATION ON THIS PROBLEM. I BOUGHT NEW TIRES ON 2/8/2010 AT THE RECOMMENDATION OF THE DEALER. *TR

I ALSO NOTICED THAT THE TRACTION CONTROL INDICATOR LIGHT ON THE DASH ALWAYS TURNS ON WHEN THIS HAPPENS. I TOOK THE DEALER JUST AFTER I PURCHASED THE VEHICLE AND WAS TOLD THAT EVERYTHING WAS NORMAL. THIS HAS BEEN HAPPENING FOR THE LAST 2 AND A HALF YEARS. IT HAS HAPPENED 3 OR 4 TIMES THIS WEEK ALONE. I HAVE GOTTEN USED TO THIS HAPPENING BUT HAVE ALSO BEEN VERY CONCERNED THAT IT WILL CAUSE A REAREND COLLISION, OR SOMETHING EVEN WORSE. THIS NEEDS ATTENTION ASAP. THANK YOU *TR

LOST STEERING CONTROL AND CAR HAD TO BE TOWED BECAUSE OF SIGNIFICANT DAMAGES TO WHEEL AND ALSO POSSIBLY STEERING. SNOWY ROAD. I WAS ALREADY DRIVING VERY SLOWLY. NO REASON WHY THIS MUCH DAMAGE TO MY CAR OR THIS LEVEL OF IMPACT UNLESS CAR SUDDENLY ACCELERATED BEYOND MY CURRENT SPEED WHILE I BRAKED. CAR SEEMS TO ACCELERATE ON SLIPPERY, UNEVEN SURFACES AND EVS LIGHT COMES ON, EVEN WHEN BRAKES APPLIED. ACCELERATION IS BEYOND THAT SEEN WITH NORMAL SLIPPAGE. WHILE APPROACHING INTERSECTION, OUR 2007 TOYOTA PRIUS FAILED TO STOP. WHILE BRAKING, THE CAR CONTINUED TO ROLL - INTO THE SIDE OF A TRUCK. THE CAR WAS TOTALED. AT THE TIME, WE EXPLAINED THAT THE BRAKES WERE APPLIED, BUT THE CAR FAILED TO STOP. WE ASKED THE INSURANCE COMPANY AND TOYOTA IF THERE WERE ANY REPORTS OF PROBLEMS WITH THE PRIUS, AND WE WERE TOLD "NO REPORTED PROBLEMS" WITH THE CAR. PRIOR TO THE INCIDENT WE HAD JUST HAD THE 25,000 MILE MAINTENANCE DONE SO WE KNEW IT WAS NOT THE BRAKES. ABOUT A YEAR LATER, WE WERE INFORMED OF THE CAR MAT PROBLEM, AND NOW THERE IS TALK OF POSSIBLE ACCELERATOR/ELECTRONIC PROBLEMS. WE NOW HAVE A 2008 PRIUS, AND EXPERIENCE A MOMENTARY LOSS OF THE ELECTRONIC SYSTEM WHEN THE CAR HITS A BUMP. WE CONTACTED TOYOTA LAST WEEK, AND WERE PROMISED A CALLBACK IN TWO DAYS, BUT SO FAR WE HAVE NOT HEARD ANYTHING. SO UNTIL WE GET ASSURANCE THAT OUR CAR IS SAFE, WE HAVE DECIDED TO STOP DRIVING THE PRIUS. SUBSEQUENT TO THE ACCIDENT, MY BACK PAIN WENT FROM MANAGEABLE TO UNBEARABLE, WHICH LED TO SPINE SURGERY IN 1/09. MY PHYSICIANS BELIEVED THAT THE ACCIDENT MIGHT HAVE AGGRAVATED THE SYMPTOMS TO THE POINT THAT I REQUIRED SURGERY - POSSIBLY FROM THE IMPACT FROM THE AIRBAGS. *TR

HOWEVER, THIS CAUSED ME TO GET PULLED OVER ON TWO OCCASIONS FOR A MISSING HEADLIGHT. THIS HAPPENED AT LEAST 20 TIMES THAT I WAS ABLE TO NOTICE (BECAUSE OF A LACK OF ILLUMINATION WHILE DRIVING AT NIGHT) AND WAS CITED BY THE POLICE TWICE BEFORE IT WAS REPAIRED. THE TOYOTA DEALERSHIP SAID THAT THIS WAS A COMMON PROBLEM BECAUSE OF DEFECTIVE LIGHT BULBS. HOWEVER, TOYOTA WOULD NOT REPLACE THE DEFECTIVE BULBS BECAUSE BOTH RARELY WENT OUT AT THE SAME TIME. HOWEVER, I FELT THIS WAS A SAFETY HAZARD AND HAD TO PAY $150 PER LIGHT, PLUS LABOR TO CHANGE THE LIGHT BULBS. *TR

I WAS DRIVING 15 MILES AN HOUR AND GENTLY APPLIED THE BRAKES FOR A STOP SIGN. THERE WAS A SIGNIFICANT DELAY IN THE BRAKES WORKING AND I HAD TO SLAM DOWN ON THE PEDAL TO STOP. THIS HAPPENED ABOUT 10 TIMES LAST WINTER AND HAS HAPPENED ONCE SO FAR THIS WINTER. MOST OF MY DRIVING IS IN TOWN AND I TRY TO GO SLOW, PARTICULARLY IN THE COLD, SO I HAVE ALWAYS BEEN ABLE TO STOP IN TIME. I CALLED THE TOYOTA DEALERSHIP ABOUT THIS LAST YEAR AND WAS TOLD THAT IT HAPPENS TO ALL CARS IN COLD WEATHER AND THERE WAS NOTHING THEY COULD DO. WITH THE RECENT DISCUSSION OF THE BRAKE SYSTEM FOR TOYOTA I THOUGHT IT WAS BEST TO REPORT THIS IN CASE THAT WASN'T TRUE. *TR

THE HID HEADLIGHTS ON MY 2007 TOYOTA PRIUS TOURING EDITION WILL STOP WORKING WHILE DRIVING. EITHER THE LEFT, RIGHT OR BOTH HEADLIGHTS WILL TURN OFF WITHOUT WARNING. WHEN I TURN THE LIGHTS OFF AND THEN ON AGAIN, BOTH BULBS WORK FINE. THE LIGHTS DO NOT FLICKER WHILE ON OR BEFORE THEY ARE ABOUT TO TURN OFF. I HAVE BEEN PULLED OVER BECAUSE OF THIS AND HAVE ALSO HAD THE LIGHTS TURN OFF WHILE DRIVING IN MOUNTAIN ROADS. THE ARE HIGH INTENSITY DISCHARGE LAMPS. HIGH INTENSITY DISCHARGE LIGHTS DO NOT HAVE A FILAMENT. INSTEAD LIGHT IS CREATED FROM AN ELECTRICAL DISCHARGE BETWEEN TWO ELECTRODES IN A MICRO-ENVIRONMENT OF XENON GAS AND METAL HALIDE SALTS. TOYOTA SENT ME A LETTER TALKING ABOUT THE HID HEADLIGHT PRIUS PROBLEM, BUT DID NOT ACKNOWLEDGE ANY WRONG DOING OR NECESSITY OF A RECALL. I SUSPECT THE PROBLEM IS WITH THE RELATIONSHIP BETWEEN THE BALLAST THAT CREATES THE ENERGY AND THE BULBS. *TR

Toyota is recalling certain model year 2004-2009 Prius vehicles manufactured August 6, 2003, through March 30, 2009 and FCHV-adv vehicles manufactured December 12, 2008, through September 13, 2011. During manufacturing, a scratch may have occurred inside of the electrically driven water pump at the coil wire. The coil wire may corrode at the scratched portion.
Consequence & remedy
Consequence: The corroded coil wire may break and the water pump could stop. The corroded coil wire could cause a short circuit between coil wires and a possible open fuse, creating a stall-like condition of the hybrid system while the vehicle is being driven. This may increase the risk of a vehicle crash.
Remedy: Toyota will notify its owners, and dealers will replace the electric water pump, free of charge. The recall began on January 17, 2013. Owners may contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331.

TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.
Consequence & remedy
Consequence: A STUCK OPEN ACCELERATOR PEDAL MAY RESULT IN VERY HIGH VEHICLE SPEEDS AND MAKE IT DIFFICULT TO STOP THE VEHICLE, WHICH COULD CAUSE A CRASH, SERIOUS INJURY OR DEATH.
Remedy: TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. TOYOTA WILL BEGIN MAILING LETTERS TO OWNERS ON DECEMBER 31, 2009. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331, LEXUS AT 1-800-255-3987.

The Office of Defects Investigation (ODI) received a January 2013 defect petition requesting that it open an investigation on Model Year (MY) 2004-2009 Toyota Prius vehicles to address a possible manufacturing safety defect related to the steering column intermediate shaft.During the defect petition evaluation, ODI: 1.Reviewed the petition and its enclosures; 2.Reviewed the subject steering system layout; 3.Examined the Petition Vehicle?s history, including its repair history; 4.Reviewed and followed-up potentially related consumer complaints; 5.Assessed Safety Recalls 06V-188 and 12V-537; and, 6.Requested and examined complaint, claim, field report, service history, and warranty information from Toyota.ODI's review revealed that the petition vehicle had been recalled (Recall 06V188) to remedy a defect in the system identified in the petition with the remedy taking place prior to the failure that prompted the petition. Circumstances point to incorrect reassembly of the petitioner's vehicle by those who performed the recall remedy.ODI examined the issues raised by this petition to identify any related safety defect trend in the general subject vehicle population. That review identified a low rate of related consumer complaints (twenty-five over a eight year period from a population of over 600,000 vehicles) and no pattern that would identify either a safety defect not already covered by the two existing steering safety recalls by Toyota or a systematic problem with either recall remedy.Based on this evaluation, ODI does not believe that additional investigation will lead to a finding that a defect related to motor vehicle safety exists in the subject vehicles. Therefore, in view of the need to allocate and prioritize NHTSA?s limited resources to best accomplish the agency's safety mission, the petition is denied. The denial of this petition does not constitute a finding by NHTSA that a safety-related defect does not exist. Steering
The agency reserves the right to take further action if warranted by the circumstances. For more information, see the Federal Register notice in the public file for this action (https://www.nhtsa.gov/recalls#vehicle, select NHTSA ID, enter: DP13001)The ODI reports cited above can be reviewed on-line (https://www.nhtsa.gov/recalls#vehicle) under the following NHTSA ID numbers:10407520, 10418710, 10429551, 10473654, 10478195, 10478053, 10437229, 10489425, 10510722, 10639984, 10648277, 10499778, 10712626, 10723196, 10787530, 10808124, 10838803, 11071646, 11098144, 11099507, 11102659, 11160767, 11192607, 11287550, 11287606

ODI OPENED THIS INVESTIGATION AFTER RECEIVING REPORTS THAT BOTH HEADLIGHTS WOULD GO OUT, OR EXTINGUISH WHILE DRIVING.THE SUBJECT VEHICLES ARE EQUIPPED WITH HIGH INTENSITY DISCHARGE (HID) HEADLAMPS THAT OPERATE ON INDEPENDENT LAMPS AND ECU'S (ELECTRONIC CONTROL UNITS) FOR EACH HEADLIGHT, MEANING THAT EACH HEADLIGHT RUNS INDEPENDENTLY OF THE OTHER, AND A FAILURE IN ONE LAMP OR ECU DOES NOT NECESSARILY RESULT IN A FAILURE OF THE OTHER.DURING THE INVESTIGATION, A RANDOM SAMPLE OF FORTY CONSUMERS WAS CONTACTED BY ODI IN A TELEPHONE SURVEY TO VERIFY THEIR EXPERIENCES.THE CONSUMERS STATED IN COMPLAINTS TO TOYOTA THAT BOTH HEADLIGHTS FAILED AT THE SAME TIME.ODI DISCOVERED THROUGH TALKING WITH THESE COMPLAINANTS THAT IN FACT ONE HEADLIGHT WOULD BEGIN TO FLICKER AND THEN CUT OFF.THE OTHER HEADLIGHT WOULD STILL BE OPERATIONAL.IN A FEW CASES WHERE NO ACTION WAS TAKEN, SOME COMPLAINANTS STATED THAT AFTER A FEW MONTHS BOTH HEADLAMPS WOULD BEGIN TO FAIL RESULTING IN NO HEADLIGHTS; HOWEVER NONE REPORTED THAT BOTH LAMPS FAILED SIMULTANEOUSLY.COMPLAINANTS ALSO STATED THAT THEY COULD TOGGLE THE HEADLAMP SWITCH ON AND OFF TO TEMPORARILY RESTORE LIGHTING WHEN THIS CONDITION OCCURRED, EVEN IN CASES WERE BOTH HEADLAMPS WERE EXTINGUISHED.THE RESULTS OF THIS SURVEY ARE LARGELY CONSISTENT WITH ODI'S ASSESSMENT OF THE REPORTS AND WARRANTY DATA.TOYOTA SUBMITTED IN RESPONSE TO ODI'S INFORMATION REQUEST LETTER THAT, GENERALLY SPEAKING, ONLY ONE LAMP FAILED AT A TIME, AND THAT TOGGLING THE SWITCH RESTORED THE LAMP.TOYOTA REQUESTED A MEETING WITH ODI AND ON AUGUST 6, 2009, PRESENTED THEIR ASSESSMENT OF HEADLAMP FAILURE.TOYOTA CONFIRMED THAT A HEADLAMP COULD EXTINGUISH WHILE DRIVING DUE TO A BULB FAILURE, AND THAT IT COULD BE TEMPORARILY RESTORED BY TURNING THE HEADLIGHT SWITCH ON AND OFF, AND THAT SIMULTANEOUS FAILURE OF BOTH HEADLAMPS WAS RARE AND UNLIKELY.TOYOTA ALSO BRIEFLY DISCUSSED THEIR INTENTION TO CONDUCT A CUSTOMER SERVICE CAMPAIGN TO ADDRESS CONSUMER DISSATISFACTION WITH HID BULB FAILURES.SINCE THERE IS STILL ONE HEADLIGHT OPERATING WHEN THE OTHER ONE CUTS OFF, THERE IS STILL LIGHTING AVAILABLE TO GET THE VEHICLE TO A REPAIR FACILITY.THUS, A SAFETY DEFECT TREND HAS NOT BEEN IDENTIFIED AT THIS TIME AND FURTHER USE OF AGENCY RESOURCES DOES NOT APPEAR TO BE WARRANTED.ACCORDINGLY THIS INVESTIGATION IS CLOSED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT A SAFETY-RELATED DEFECT DOES NOT EXIST.THE AGENCY WILL TAKE FURTHER ACTION IF WARRANTED BY THE CIRCUMSTANCES.
