TOYOTA PRIUS 2007: WE OWN THIS VEHICLE WHICH HAS LESS THEN 80K MILES ON IT AND THE HEADLIGHTS TURN OFF WHEN IN USE. THEY DO NOT BOTH GO OUT, IT IS USUALLY FIRST THE PASSENGER SIDE, THEN I WILL HAVE TO TURN THEM OFF AND BACK ON TO GET IT WORKING AGAIN. THEN, AFTER A FEW MINUTES, THE DRIVER'S SIDE LIGHT TURNS OFF. WHO HAS EVER HEARD OF HEADLIGHTS DYING OUT SO EARLY? I READ THAT THERE WAS A SIMILAR PROBLEM ON THE 2005 MODEL SO I CALLED MY LOCAL LANSING, MI TOYOTA DEALERSHIPS SERVICE DEPARTMENT. THEY SAID THAT IT IS LIKELY THAT THEY ARE JUST GOING OUT. I INFORMED HIM OF THE MILEAGE AND HIS RESPONSE WAS "WELL, THEY ARE UP TO FOUR YEARS OLD." AGAIN, I ASKED WHO HAS EVER HEARD OF BUYING A NEW VEHICLE SUCH AS WE DID WITH THIS ONE, AND THEN THE HEADLIGHTS LASTING ONLY A FEW YEARS, WELL UNDER 100K, NOT TO MENTION THE PRIOR RECALL? THEY SAID WELL IF IT IS A COMPUTER SOFTWARE PROBLEM, INSTEAD OF JUST REPLACING THE BULBS YOU ARE LOOKING AT A THOUSAND PLUS DOLLARS FOR REPAIR. I BELIEVE THIS IS JUST A WAY FOR THEM TO NOT LOSE MONEY AS THERE ARE OTHER DANGEROUS PROBLEMS WITH THESE MODELS. I HAVE CHILDREN AND I DO NOT WANT TO BE DRIVING DOWN THE ROAD IN THE EVENING AND THEN I SUDDENLY LOSE THE ABILITY TO HAVE HEADLIGHTS ALTOGETHER! IS THAT WHAT IT WILL TAKE FOR A RECALL TO HAPPEN? I HOPE YOU CAN HELP. *TR

WHEN YOU TRY TO TURN THE HIGH-BEAMS ON IN THIS CONDITION THE ENTIRE DASH LIGHTING SYSTEM BLACKS OUT. THIS HAS HAPPENED ON NUMEROUS OCCASIONS BEFORE WE ACTUALLY UNDERSTOOD THEY PROBLEM. IT WAS VERY INFREQUENT AT FIRST, STARTING ON THANKSGIVING 2009. IT HAS HAPPENED TWICE THIS PAST WEEKEND. I HAVE CALLED A TOYOTA DEALER AND THEY TELL ME WE NEED TO REPLACE THE HEAD LAMPS. I FIND IT EXTREMELY DIFFICULT TO BELIEVE THAT THE ENTIRE LIGHTING FAILS, BECAUSE OF A BAD HEADLIGHT, LET ALONE ADD IN THE ATTEMPT TO SWITCH ON THE HIGH BEAMS AND THE ENTIRE DASH LIGHTS GO OUT. WE HAVE NOT BEEN ABLE TO GET THE CAR TO A DEALERSHIP YET DUE TO SCHEDULING. *TR

I HAVE A TOYOTA PRIUS 2007. I HAVE 2 SAFETY ISSUES THAT I HAVE COMPLAINED TO THE DEALER ABOUT BUT THEY ARE UNABLE TO FIX SO THEY IGNORED MY CONCERNS AFTER REALIZING THEY COULD NOT FIX THE PROBLEM. FIRST PROBLEM IS THE HEAD LIGHTS. WHEN DRIVING AT NIGHT THE HIGH BEAMS DO NOT SHINE ON THE ROAD BUT SHINE INTO THE TREES. THEY SAY THERE IS NO ADJUSTMENTS FOR IT. ALSO PEOPLE THINK I AM SHINING MY HIGH BEAMS AT THEM WHEN I AM NOT. THEY ARE JUST TOO HIGH....ALSO THE HEADLIGHTS DO NO ILLUMINATE THE SIDES OF THE CAR AND ROAD SO WHEN MAKING TURNS AT NIGHT YOU CANNOT SEE WHERE YOU ARE TURNING. I HAVE TO APPROACH TURNS VERY VERY SLOWLY BECAUSE I CANNOT SEE THE ROADS WHEN TURNING. EVERYONE THAT DRIVES MY CAR COMPLAINS ABOUT THIS NOTICEABLE PROBLEM HOWEVER TOYOTA WONT ADDRESS IT BECAUSE THEY SAY THERE IS NO ADJUSTMENT. 2ND PROBLEM THEIR SLIP DIFFERENTIAL. WHETHER THERE IS SNOW ON THE GROUND OR NOT WHEN THE TIRES START TO SLIP THEY LOOSE ALL POWER TO THE WHEELS. I HAVE BEEN IN INTERSECTIONS WHEN THIS HAS OCCURRED AND WAS ALMOST HIT. EVEN WHEN TRYING TO DRIVE OVER THE STOP IN THE CAR WASH...SOMETIMES I AM UNABLE TO POWER OVER THE BUMP. I GET STUCK OFTEN. AGAIN TOYOTA SAYS THIS IS A NORMAL FUNCTION OF THE CAR. NOT SOMETHING I WOULD EXPECT FROM A $24,000 CAR. OUR VALUES HAVE PLUMMETED AND THERE IS NO RECOURSE FOR IT. *TR

IN THE SPRING OF 2009 THE HEADLIGHTS OF MY 2007 PRIUS STARTED TO GO OUT WHILE I WAS DRIVING. IF I FLICKED THE LIGHTS OFF AND ON THEY WOULD COME BACK FOR A LITTLE WHILE. HOWEVER, THIS CAUSED ME TO GET PULLED OVER ON TWO OCCASIONS FOR A MISSING HEADLIGHT. THIS HAPPENED AT LEAST 20 TIMES THAT I WAS ABLE TO NOTICE (BECAUSE OF A LACK OF ILLUMINATION WHILE DRIVING AT NIGHT) AND WAS CITED BY THE POLICE TWICE BEFORE IT WAS REPAIRED. THE TOYOTA DEALERSHIP SAID THAT THIS WAS A COMMON PROBLEM BECAUSE OF DEFECTIVE LIGHT BULBS. HOWEVER, TOYOTA WOULD NOT REPLACE THE DEFECTIVE BULBS BECAUSE BOTH RARELY WENT OUT AT THE SAME TIME. HOWEVER, I FELT THIS WAS A SAFETY HAZARD AND HAD TO PAY $150 PER LIGHT, PLUS LABOR TO CHANGE THE LIGHT BULBS. *TR

THE HID HEADLIGHTS ON MY 2007 TOYOTA PRIUS TOURING EDITION WILL STOP WORKING WHILE DRIVING. EITHER THE LEFT, RIGHT OR BOTH HEADLIGHTS WILL TURN OFF WITHOUT WARNING. WHEN I TURN THE LIGHTS OFF AND THEN ON AGAIN, BOTH BULBS WORK FINE. THE LIGHTS DO NOT FLICKER WHILE ON OR BEFORE THEY ARE ABOUT TO TURN OFF. I HAVE BEEN PULLED OVER BECAUSE OF THIS AND HAVE ALSO HAD THE LIGHTS TURN OFF WHILE DRIVING IN MOUNTAIN ROADS. THE ARE HIGH INTENSITY DISCHARGE LAMPS. HIGH INTENSITY DISCHARGE LIGHTS DO NOT HAVE A FILAMENT. INSTEAD LIGHT IS CREATED FROM AN ELECTRICAL DISCHARGE BETWEEN TWO ELECTRODES IN A MICRO-ENVIRONMENT OF XENON GAS AND METAL HALIDE SALTS. TOYOTA SENT ME A LETTER TALKING ABOUT THE HID HEADLIGHT PRIUS PROBLEM, BUT DID NOT ACKNOWLEDGE ANY WRONG DOING OR NECESSITY OF A RECALL. I SUSPECT THE PROBLEM IS WITH THE RELATIONSHIP BETWEEN THE BALLAST THAT CREATES THE ENERGY AND THE BULBS. *TR

Nov 14, 2012

Toyota is recalling certain model year 2004-2009 Prius vehicles manufactured August 6, 2003, through March 30, 2009 and FCHV-adv vehicles manufactured December 12, 2008, through September 13, 2011. During manufacturing, a scratch may have occurred inside of the electrically driven water pump at the coil wire. The coil wire may corrode at the scratched portion.

Consequence & remedy

Consequence: The corroded coil wire may break and the water pump could stop. The corroded coil wire could cause a short circuit between coil wires and a possible open fuse, creating a stall-like condition of the hybrid system while the vehicle is being driven. This may increase the risk of a vehicle crash.

Remedy: Toyota will notify its owners, and dealers will replace the electric water pump, free of charge. The recall began on January 17, 2013. Owners may contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331.

The Office of Defects Investigation (ODI) received a January 2013 defect petition requesting that it open an investigation on Model Year (MY) 2004-2009 Toyota Prius vehicles to address a possible manufacturing safety defect related to the steering column intermediate shaft.During the defect petition evaluation, ODI: 1.Reviewed the petition and its enclosures; 2.Reviewed the subject steering system layout; 3.Examined the Petition Vehicle?s history, including its repair history; 4.Reviewed and followed-up potentially related consumer complaints; 5.Assessed Safety Recalls 06V-188 and 12V-537; and, 6.Requested and examined complaint, claim, field report, service history, and warranty information from Toyota.ODI's review revealed that the petition vehicle had been recalled (Recall 06V188) to remedy a defect in the system identified in the petition with the remedy taking place prior to the failure that prompted the petition. Circumstances point to incorrect reassembly of the petitioner's vehicle by those who performed the recall remedy.ODI examined the issues raised by this petition to identify any related safety defect trend in the general subject vehicle population. That review identified a low rate of related consumer complaints (twenty-five over a eight year period from a population of over 600,000 vehicles) and no pattern that would identify either a safety defect not already covered by the two existing steering safety recalls by Toyota or a systematic problem with either recall remedy.Based on this evaluation, ODI does not believe that additional investigation will lead to a finding that a defect related to motor vehicle safety exists in the subject vehicles. Therefore, in view of the need to allocate and prioritize NHTSA?s limited resources to best accomplish the agency's safety mission, the petition is denied. The denial of this petition does not constitute a finding by NHTSA that a safety-related defect does not exist. The agency reserves the right to take further action if warranted by the circumstances. For more information, see the Federal Register notice in the public file for this action (https://www.nhtsa.gov/recalls#vehicle, select NHTSA ID, enter: DP13001)The ODI reports cited above can be reviewed on-line (https://www.nhtsa.gov/recalls#vehicle) under the following NHTSA ID numbers:10407520, 10418710, 10429551, 10473654, 10478195, 10478053, 10437229, 10489425, 10510722, 10639984, 10648277, 10499778, 10712626, 10723196, 10787530, 10808124, 10838803, 11071646, 11098144, 11099507, 11102659, 11160767, 11192607, 11287550, 11287606

ODI OPENED THIS INVESTIGATION AFTER RECEIVING REPORTS THAT BOTH HEADLIGHTS WOULD GO OUT, OR EXTINGUISH WHILE DRIVING.THE SUBJECT VEHICLES ARE EQUIPPED WITH HIGH INTENSITY DISCHARGE (HID) HEADLAMPS THAT OPERATE ON INDEPENDENT LAMPS AND ECU'S (ELECTRONIC CONTROL UNITS) FOR EACH HEADLIGHT, MEANING THAT EACH HEADLIGHT RUNS INDEPENDENTLY OF THE OTHER, AND A FAILURE IN ONE LAMP OR ECU DOES NOT NECESSARILY RESULT IN A FAILURE OF THE OTHER.DURING THE INVESTIGATION, A RANDOM SAMPLE OF FORTY CONSUMERS WAS CONTACTED BY ODI IN A TELEPHONE SURVEY TO VERIFY THEIR EXPERIENCES.THE CONSUMERS STATED IN COMPLAINTS TO TOYOTA THAT BOTH HEADLIGHTS FAILED AT THE SAME TIME.ODI DISCOVERED THROUGH TALKING WITH THESE COMPLAINANTS THAT IN FACT ONE HEADLIGHT WOULD BEGIN TO FLICKER AND THEN CUT OFF.THE OTHER HEADLIGHT WOULD STILL BE OPERATIONAL.IN A FEW CASES WHERE NO ACTION WAS TAKEN, SOME COMPLAINANTS STATED THAT AFTER A FEW MONTHS BOTH HEADLAMPS WOULD BEGIN TO FAIL RESULTING IN NO HEADLIGHTS; HOWEVER NONE REPORTED THAT BOTH LAMPS FAILED SIMULTANEOUSLY.COMPLAINANTS ALSO STATED THAT THEY COULD TOGGLE THE HEADLAMP SWITCH ON AND OFF TO TEMPORARILY RESTORE LIGHTING WHEN THIS CONDITION OCCURRED, EVEN IN CASES WERE BOTH HEADLAMPS WERE EXTINGUISHED.THE RESULTS OF THIS SURVEY ARE LARGELY CONSISTENT WITH ODI'S ASSESSMENT OF THE REPORTS AND WARRANTY DATA.TOYOTA SUBMITTED IN RESPONSE TO ODI'S INFORMATION REQUEST LETTER THAT, GENERALLY SPEAKING, ONLY ONE LAMP FAILED AT A TIME, AND THAT TOGGLING THE SWITCH RESTORED THE LAMP.TOYOTA REQUESTED A MEETING WITH ODI AND ON AUGUST 6, 2009, PRESENTED THEIR ASSESSMENT OF HEADLAMP FAILURE.TOYOTA CONFIRMED THAT A HEADLAMP COULD EXTINGUISH WHILE DRIVING DUE TO A BULB FAILURE, AND THAT IT COULD BE TEMPORARILY RESTORED BY TURNING THE HEADLIGHT SWITCH ON AND OFF, AND THAT SIMULTANEOUS FAILURE OF BOTH HEADLAMPS WAS RARE AND UNLIKELY.TOYOTA ALSO BRIEFLY DISCUSSED THEIR INTENTION TO CONDUCT A CUSTOMER SERVICE CAMPAIGN TO ADDRESS CONSUMER DISSATISFACTION WITH HID BULB FAILURES.SINCE THERE IS STILL ONE HEADLIGHT OPERATING WHEN THE OTHER ONE CUTS OFF, THERE IS STILL LIGHTING AVAILABLE TO GET THE VEHICLE TO A REPAIR FACILITY.THUS, A SAFETY DEFECT TREND HAS NOT BEEN IDENTIFIED AT THIS TIME AND FURTHER USE OF AGENCY RESOURCES DOES NOT APPEAR TO BE WARRANTED.ACCORDINGLY THIS INVESTIGATION IS CLOSED.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT A SAFETY-RELATED DEFECT DOES NOT EXIST.THE AGENCY WILL TAKE FURTHER ACTION IF WARRANTED BY THE CIRCUMSTANCES.
