1. What component or system failed or malfunctioned and is it available for inspection upon request? Component/System: Brake-by-wire hydraulic fluid loop, master cylinder assembly, or stroke simulator system on a 2026 Lexus RX 350h (VIN: [XXX]; 2,450 miles). Cash-purchased new in XXX. Weak, unrestricted hydraulic pressure. The brake pedal depresses to within 1 inch of the floorboard, bottoming out against the physical mechanical extension rod limit. A Reserve Distance Measurement confirms the vehicle fails to meet the minimum Lexus safety standard of 86 mm (3.39 inches) of pedal reserve distance. 2. How was your safety or the safety of others put at risk? Safety Hazard: Operating at less than 1 inch of pedal clearance instead of the factory-mandated 86 mm (3.39 inches) violates Federal Motor Vehicle Safety Standards (FMVSS) regarding pedal reserve margins. Imminent Risk: This drastically increases the risk of a catastrophic front-end or multi-fatal high-speed collision. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Dealer Visit: Park Place Lexus in [XXX] on XXX (at 2,450 miles) for low hydraulic pressure. In an emergency stop on dry pavement, my braked did not operate in a straight line. The vehicle swerved. My statement is attached as Page 4 to an uploaded form supplied by the dealership abbreviated as "UB Interview Guide". You have my permission to inspect the vehicle which is in the possession of the Toyota of Santa Barbara dealership. Toyota of Santa Barbara has not to date inspected or repaired my vehicle. I am not aware that anyone has inspected the vehicle. I consider the problem to be one that could cause death or serious bodily injury if the vehicle is driven without repair. A red box appeared on my dashboard during the incident, but I had no time to read or process whatever message may have appeared. Please see my statement prepared within 24 hours of the incident.

Un-Intended Accelerations, the accelerator pedals either stocked, jammed in the Carpet, Triggered Un-Intended Acceleration due to Safety Use cases not designed correctly Once Triggered the un-intended acceleration the brake system is not working •Make: Toyota •Model: Camry •Year: 2026 •VIN: [XXX] On Sunday, [XXX], at approximately [XXX], I operated the subject vehicle (2028 Toyota Camry, VIN: [XXX]) for an approximate distance of three to five meters along my residential driveway. My intention was to proceed forward briefly, then shift into reverse to continue my planned route. Upon releasing my foot from the accelerator pedal in preparation for this maneuver, the vehicle unexpectedly and rapidly accelerated without any input from me. The rate and intensity of acceleration was unprecedented in my experience operating motor vehicles. I immediately applied pressure to the brake pedal in an attempt to stop the vehicle; however, the braking system was unresponsive and failed to slow or halt the vehicle's forward motion. As a result of this uncontrolled acceleration, the vehicle collided with the front porch of my residence. Upon impact, all airbags deployed. The collision caused extensive damage to my property, including a fractured and displaced porch support column. I sustained multiple serious injuries during this incident, including a severe laceration to my left leg, burn injuries across both hands, body, and legs, loss of mobility in my left arm and neck, and significant pain in my lower back region. Since the incident, I have experienced persistent sleep disturbances and difficulty concentrating due to recurring mental imagery of the collision. The physical injuries and psychological impact have substantially affected my daily functioning and quality of life. Saturday may 23 at night my car started showing a message that states “Braking Power Low Stop in a Safe Place See Owner's Manual” I took it to Toyota on Tuesday may 2026 they said it was fine and nothing was wrong even after I said that message kept appearing at least five times each time I was driving the vehicle since Saturday. After they checked it the problem got worse and now it appears 95% of the time I’m driving. Not just that but on may 27 at approximately 9:10 pm my check engine turned on and also system lady took turned on and secondary collision break malfunction turned on. Along side many other warning comes and go but the main ones that stay are the check engine and the braking power low one. I am worried because this all started happening whenever I accelerated even just a little bit and it could end up causing accidents.

I purchased a 2026 Toyota Camry SE in November 2025. The hybrid battery has stop working several times also the steering system has failed as well. I am told the hybrid battery is defective. Toyota the Manufacture has refused to replace it. I am afraid for my safety.

The instrument cluster display may fail during vehicle startup, deactivating the hazard lights, turn signals, seat belt warning system, and smart key reminder. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 101, "Control and Displays," 108, "Lamps, Reflective Devices and Associated Equipment," 114, "Theft Protection and Rollaway Prevention," and 208, "Occupant Crash Protection."

Consequence & remedy

Consequence: An instrument cluster that fails to display critical safety information increases the risk of a crash or injury.

Remedy: Dealers will update the display software, free of charge. Owner notification letters are expected to be mailed September 21, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26TB16 and 26TA16.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2025-2026 Camry Hybrid and 2026 Corolla Cross Hybrid vehicles. A bolt inside the inverter may not have been tightened properly, causing incomplete contact at the inverter terminal.

Consequence & remedy

Consequence: Incomplete contact can cause a loss of motive power, increasing the risk of a crash. In addition, a loose bolt can cause an electrical short while the vehicle is powered on, increasing the risk of a fire.

Remedy: Dealers will replace the inverter assembly, free of charge. Owner letters were mailed July 23, 2026. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is 25TB15/25TA15.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.
