MY CAR HAS THE STICKY DASH ISSUE THAT MANY TOYOTAS HAVE. I FIRST NOTICED A 'PLASTIC / RUBBER' SMELL IN THE CAR A MONTH AFTER I BOUGHT IT; HOWEVER, THE TOYOTA DEALERSHIP SAID THAT IT WAS THE 'NEW CAR SMELL'. LATER I WAS TOLD IT WAS AN ONGOING ISSUE WITH MY CABIN INLET FILTER. FINALLY THE DASH STARTED BECOMING SOFT AND GOOEY. I LATER FOUND OUT THAT MY CAR WAS PART OF A RECALL AND TOYOTA AGREED TO REPLACE THE DASH - 9 YEARS LATER. I HAVE BEEN BREATHING THE TOXIC FUMES FROM THE MELTING DASH FOR 9 YEARS, 150K PLUS MILES.
WHEN I PURCHASED THE CAR IN 2015. I WAS INFORMED THAT A RECALL WAS GOING TO TAKE PLACE ON THE DASH BOARD. THIS WAS DUE TO THE FACT THAT SUN WAS MAKING A STICKY GLAZE TO FORM. THIS IS HAZARDOUS BECAUSE IT OFTEN CREATES A BLINDING GLARE DURING THE DAY WHILE DRIVING AND EVEN WHILE PARKED.

WHEN OUR CAR WAS IN MOTION, WE EXPERIENCED VERY LOUD AUDIBLE GRUNT ALONG WITH VERY POOR BREAKING. EXTREMELY SCARY SITUATION ESPECIALLY SINCE IT STARTED SUDDENLY, WITHOUT WARNING WHILE DRIVING AND THE BRAKES DID NOT WORK AT ALL DURING THE FIRST INCIDENT. THE WARNING LIGHTS DID NOT COME ON AND LUCKILY WE WERE NOT YET ON THE EXPRESSWAY. WE MADE IT HOME AND HAD IT TOWED TO THE REPAIR SHOP THE NEXT DAY. WE DROVE IT AROUND THE PARKING LOT WITH THE MECHANIC AND THE BRAKES FAILED AGAIN. WE HAD TO HAVE THE ABS BRAKE SYSTEM (ABS PUMP/MODULE ASSEMBLY) REPLACED AT A COST OF $2,263. WE CONTACTED TOYOTA WHO SAID THEY COULD NOT REIMBURSE FOR THE ISSUE. WE HAVE READ ONLINE THAT OTHER ISSUES HAVE HAPPENED TO CONSUMERS AND IT SEEMS THAT ANYTHING THAT RESULTS IN TOTAL BRAKE FAILURE SHOULD WARRANTY A RECALL. PLEASE ADVISE IF YOU ARE ABLE TO CONSIDER THIS SITUATION AND IF YOU NEED ANY FURTHER INFORMATION. SINCERELY, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6). *TR
CHECK VSC SYSTEM ERROR, TRACTION CONTROL LIGHT, ABS LIGHT AND EMERGENCY "!" LIGHT ILLUMINATED WHEN CAR WAS IDLE. BRAKES BECAME "SOFT". NEXT DAY, LIGHT WAS OFF UNTIL AFTER DRIVING A FEW MILES, AGAIN SAME LIGHTS CAME ON AND BRAKES BECAME SOFT AGAIN AT A STOP LIGHT
I HAVE A FOUL ODOR FROM MY A/C UNIT CAUSED BY MOLD/MILDEW. TOYOTA DID A FOAM/VENT TREATMENT, BUT THE PROBLEM IS STILL THERE. THEY WILL NOT REPAIR THE ISSUE.

THIS VEHICLE WAS SUBJECT TO A CUSTOMER SATISFACTION PROGRAM BECAUSE DRIVER-SIDE SUN VISORS WOULD STOP STAYING UP AND DROP DOWN WITHOUT WARNING. WE HAD OUR HIGHLANDER VISOR REPLACED UNDER THIS LATE 2012 PROGRAM. IT WAS AN EXTREME SAFETY HAZARD. NOW THE CAMRY WHICH IS OUR CREAM PUFF HAS THE PROBLEM AND TOYOTA SAID THE CSP HAS EXPIRED. THEY TREATED IT AS A COSMETIC ANNOYANCE BUT IT'S A HUGE SAFETY ISSUE FOR DRIVERS 6' TALL OR MORE. THIS HAPPENS IN MANY SITUATIONS BUT IS MOST DANGEROUS ON THE HIGHWAY. WE WISH TO REGISTER A COMPLAINT ABOUT EXCESSIVE OIL CONSUMPTION IN OUR 2008 TOYOTA CAMRY HYBRID AND WHICH FELL OUTSIDE THE CRITERIA FOR THE APPROPRIATE REMEDY WHICH IS TO REBUILD THE ENGINE ACCORDING TO TOYOTA TSB 0094-11 "2AZ-FE ENGINE OIL CONSUMPTION". IN THE TSB, IT IS NOTED 'SOME 2006 ' 2011 MODEL YEAR VEHICLES EQUIPPED WITH THE 2AZ-FE ENGINE MAY EXHIBIT ENGINE OIL CONSUMPTION. THE PISTON ASSEMBLY HAS BEEN CHANGED TO MINIMIZE OIL CONSUMPTION.' WE HAD OUR 2008 TOYOTA CAMRY HYBRID TESTED FEBRUARY 2015, WHICH WAS WITHIN THE EXTENDED WARRANTY PERIOD OF 10 YEARS OR 150,000 MILES. ACCORDING TO THE DEALER (TRI COUNTY TOYOTA, LIMERICK PA), OUR CAR DID NOT MEET THE CRITERIA FOR AN ENGINE REBUILD WHICH IS 'CONSUMPTION OF MORE THAN A QUART OF OIL OVER 1200 MILES.' ACCORDING TO THIS CRITERION, AN ENGINE WHICH WAS BURNING 3 QUARTS OF OIL BETWEEN OIL CHANGES (5,000 MILES) COULD MATHEMATICALLY FAIL THIS TEST BUT IS CLEARLY DEFECTIVE. THAT IS THE SITUATION WE NOW FIND OURSELVES IN. OUR CAR HAS BEEN SERVICED ACCORDING TO THE MANUFACTURER'S RECOMMENDATIONS FOR ITS ENTIRE LIFE BUT HAS NOW USED 3 QUARTS OF OIL BETWEEN OIL CHANGES (5,000 MILES). IT IS NOT LEAKING THROUGH SEALS TO THE EXTERIOR, AND IS THEREFORE BURNING EXCESSIVE OIL, INDICATIVE OF THE FUNDAMENTAL PROBLEM WHICH IS THE SUBJECT OF THE ABOVE-MENTIONED TSM. WE THEREFORE REQUEST THAT TOYOTA REBUILD THE ENGINE OF OUR CAR, FREE OF CHARGE, WITH THE MODIFIED PISTON ASSEMBLY TO RECTIFY THE SITUATION.
WAS DRIVING NEAR 10 MILES PER HOUR, CITY STREETS (ALL THREE TIMES) . I WENT TO BRAKE, THE CHECK VSC LIGHTS AND OTHERS WENT ON AND THE BRAKES FAILED. STEERING WAS DIFFICULT, CAR SEEMED TO SHUT DOWN. HAPPENED THREE TIMES IN THE LAST TWO WEEKS. *TR

Oct 10, 2012
Toyota is recalling certain model year 2007-2009 Camry, Camry Hybrid, RAV4, Corolla, Corolla Matrix, Tundra, Sequoia, Highlander, Highlander Hybrid, Yaris, Scion xB, Scion xD and Pontiac Vibe vehicles. The power window master switch assemblies in some of these vehicles were built using a less precise process for lubricating the internal components of the switch assemblies. Irregularities in this lubrication process may cause the power window master switch assemblies to malfunction and overheat.
Consequence & remedy
Consequence: If the switch overheats, it may melt, possibly resulting in a fire.
Remedy: Toyota will notify its owners, and dealers will inspect the switch and apply a special grease that inhibits heat build up or replace any switch damaged by the defect, free of charge. Toyota owners that experience a problem with a switch should not attempt to make repairs, but should contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331. General Motors will contact the Pontiac Vibe owners affected by this recall and GM dealers will inspect and repair switches as necessary. Those owners may contact Pontiac Customer Care at 1-800-762-2737.

Oct 5, 2009
TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.
Consequence & remedy
Consequence: A STUCK OPEN ACCELERATOR PEDAL MAY RESULT IN VERY HIGH VEHICLE SPEEDS AND MAKE IT DIFFICULT TO STOP THE VEHICLE, WHICH COULD CAUSE A CRASH, SERIOUS INJURY OR DEATH.
Remedy: TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. TOYOTA WILL BEGIN MAILING LETTERS TO OWNERS ON DECEMBER 31, 2009. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331, LEXUS AT 1-800-255-3987.

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

On June 25, 2014, Toyota initiated a Warranty Enhancement Program (ZE1) to extend warranty coverage for the brake actuator assembly and skid control ECU/brake pedal stroke sensor in certain MY 2007 to 2011 Toyota Camry hybrid vehicles. Toyota's program covers repairs related to two conditions:internal malfunctions of the Brake Actuator assembly; and overly sensitive monitoring logic for the Brake Pedal Stroke Sensor.Primary coverage provides warranty enhancement until November 30, 2015, with no mileage limitation. After the primary coverage period ends, secondary coverage is provided for 10 years or 150,000 miles from the date of first use, whichever occurs first. In addition, on July 10, 2014, Toyota initiated Limited Service Campaign E0U to replace the Brake Reservoir Tank in MY 2007 through 2011 Toyota Camry hybrid vehicles. The LSC will be available until June 30, 2017, and will only be available at an authorized Toyota dealer.ODI's analysis of failure data determined that the majority of records were related to the Brake Actuator condition, which appeared to be the causal component in 734 complaints/field reports and 864 warranty claims. The Brake Pedal Stroke Sensor (80 reports/189 claims) and Brake Reservoir Tank (91 reports/44 claims) conditions were identified in far fewer records.Analysis of the failure data indicate that each condition provides warning prior to any reduction in power assisted braking, with less than 10 percent of all reports and claims involving diagnostic codes or complaint narratives indicating increased braking effort or reduced brake effectiveness. In addition, most of the incidents in which reduced braking assist appears to have occurred involved operation in a reduced assist mode and not a sudden, complete loss of power assisted braking.ODI assessed reduced assist mode in a subjective driving evaluation which indicated that the brake system provided normal and harddeceleration capability with reasonable increases in pedal effort.Less than one percent of reports and claims analyzed by ODI included evidence indicating a complete loss of power assisted braking occurred and this mode requires operation for a significant period of time with multiple audible and visual warnings.Repair expense was cited in many of the complaints reviewed by ODI, especially those related to the Brake Actuator, which had repair costs exceeding $3,000 according to some complaints.High repair cost appears to have been a factor in the only crash identified by ODI for the fault conditions identified by Toyota, which involved a Brake Actuator failure that was diagnosed but the owner declined repair approximately eight months prior to the reported crash incident. This preliminary evaluation is closed. The closing of this investigation does not constitute a finding that a safety-related defect does not exist. For additional information regarding this investigation, including analysis of the Brake Actuator, Brake Pedal Stroke Sensor, and Brake Reservoir Tank fault conditions and failure experience, see Attachment A to this closing resume.
