I WAS DRIVING MY 2007 TOYOTA CAMRY HYBRID AND WENT TO BRAKE AND MY DASH EMERGENCY LIGHTS, BRAKE LIGHT, CHECK VSC LIGHT, ABS, AND TRACTION CONTROL LIGHT WENT ON AND MY BRAKES DID NOT WORK. I WAS ABLE TO SLOW DOWN WITHOUT INCIDENT. TOOK IT TO TWO MECHANICS THEN FINALLY TO A TOYOTA DEALERSHIP. THE TOYOTA DEALERSHIP COULD NOT FINS ANYTHING WRONG WITH IT AND SENT ME ON MY WAY. TWO WEEKS LATER IT HAPPENED AGAIN. MY WIFE WAS DRIVING JUST OVER 40 MPH AND APPLIED THE BRAKES AND THEY DID NOT WORK. SHE WAS ABLE TO SWERVE INTO A PARKING LOT AND CRUISE TO A STOP. THE BRAKES WORK INTERMITTENTLY AND COME AND GO. MOVING THE CAR TO A PARKING SPOT A FEW MINUTES LATER THE BRAKES DID NOT WORK WHEN I STARTED TO MOVE THE CAR, BUT WHILE MOVING IT THE WARNING LIGHTS WENT OFF AND THE BRAKES STARTED TO WORK AGAIN. THIS IS A RECURRING PROBLEM THAT YOU DO NOT KNOW WHEN THE BRAKES WILL GO OUT. THE IS A SERIOUS ISSUE THAT NEEDS TO BE LOOKED AT IMMEDIATELY. *TR

TRAVELLING ABOUT 30 MPH I STEPPED ON THE BRAKES. BRAKE PEDAL FELT 'HARD' AND CAR BARELY DECELERATED, NEARLY HITTING ANOTHER CAR AS A RESULT. JUST MISSED CAR, BUT VERY SHAKEN I TOOK IT TO A NEARBY SHOP. THEY SAID THE BRAKE ACCUMULATOR WAS FAILING, THAT PART WOULD BE ABOUT $2600 AND LABOR OVER $300. *TR

THE BRAKES DO NOT RESPOND AND STEERING IS VERY DIFFICULT. THEN AFTER A FEW SECONDS, ALL THE WARNING LIGHTS GO OFF AND EVERYTHING IS FINE. THIS HAS HAPPENED 4 TIMES. I'M GRATEFUL IT'S ONLY OCCURRED IN PARKING LOTS BUT AFTER READING ABOUT THIS HAPPENING TO OTHERS, I'M VERY AFRAID THIS WILL HAPPEN AT HIGHER SPEEDS. THE DEALERSHIP CAN'T REPLICATE MY EXPERIENCE SO THEY SAY EVERYTHING IS FINE. WHEN I ASK ABOUT ACTUATOR ISSUES THAT OTHERS HAVE HAD, THEY DENY ANY KNOWLEDGE OF A PROBLEM. *TR

I TOOK THE CAR TO THREE DEALERSHIPS AND THEY ALL SAID MY ABS ACTUATOR WAS BAD AND NEEDED TO BE REPLACED. THE PART COSTS $2,600 JUST TO BUY FROM TOYOTA AND THEN COAST AN ADDITIONAL $800 IN LABOR TO INSTALL. THAT IS INSANE. I LOOKED ON THE INTERNET AND HER ARE TONSSS OF CAMRYS WITH THIS PROBLEM. CLEARLY THIS AS A DESIGN FLAW AND I SHOULD NOT HAVE TO PAY TO REPLACE A PART THAT TOYOTA FAILED TO DESIGN SAFELY. NOT I CAN'T SAFELY DRIVE WITH THE BROKEN PART.... *TR

I HAVE BEEN USING THIS CAMRY SINCE MID 2006. SO FAR IT HAS NOT HAD MANY MAJOR ISSUES. HOWEVER OUT OF THE BLUE ON MARCH 23RD 2014, WHEN I WAS PLANNING TO DRIVE TO A TEMPLE ALMOST 50 MILES FROM MY HOUSE WITH MY WIFE, 2 MONTH OLD DAUGHTER, MOTHER AND MOTHER-IN-LAW IN THE CAR, JUST AS WE PULLED OUT OF THE DRIVE WAY, THE CHECK VSC MESSAGE CAME ON AND THE BRAKE LIGHT, THE CAR SLIPPING LIGHT AND THE BAS LIGHTS CAME ON THE DASH AND AS I TRIED TO BRAKE, THE BRAKES COMPLETELY FAILED. LUCKILY SINCE I WAS ONLY ROLLING OUT OF MY DRIVE WAY (IN REVERSE), THE CAR ONLY ROLLED A LITTLE BIT AND THERE WAS NO CROSS TRAFFIC ON THE ROAD. I HAD TO PRESS MY PARKING BRAKES TO BRING THE CAR TO A HALT. I WAS SCARED TO DEATH WITH MY FAMILY MEMBERS IN THE CAR THERE WAS NO WAY I WAS DRIVING IT IN THAT CONDITION. I TRIED SHUTTING OFF THE CAR AND WAITING FOR 5 MINUTES BEFORE TURNING IT ON.THE MESSAGE CAME ON AGAIN AND THE BRAKES WERE UNRESPONSIVE. I IMMEDIATELY MOVED MY FAMILY TO ANOTHER CAR TO CONTINUE OUR TRIP. I THEN CAUTIOUSLY MOVED THE CAR BACK ON TO THE DRIVEWAY WITH MY LEFT LEG ON THE EMERGENCY BRAKES SO THAT I DON'T RUN INTO MY HOUSE. AFTER SEARCHING THE WEB AND SEEING MANY SUCH REPORTS, THE NEXT DAY (TODAY 3/24) I TRIED TO MOVE THE CAR AGAIN AND SEE IF THE MESSAGE DISAPPEARED. HOWEVER IT WAS THERE AND WHEN I REVERSED A LITTLE BIT, SUDDENLY THE MESSAGE AND THE LIGHTS DISAPPEARED AND THE BRAKES WERE WORKING AS NORMAL. I AM STILL NOT DRIVING THE CAR, THANKFULLY I HAVE 3 CARS SO I AM USING THE OTHER CARS. I HOPE TOYOTA DOES A RECALL ON THIS BECAUSE I DON'T WANT TO SPEND $3500+ ON A CAR THAT MAY NOT BE MORE THAN $8000 VALUE TODAY. I WAS PLANNING TO GET A 2014 SIENNA SOON BUT READING ABOUT THE SAFETY ISSUES WITH TOYOTA'S MY FAITH AND LOYALTY IN THIS BRAND IS NOW SEVERELY SHAKEN. *TR

15V689000 · Visibility:power Window Devices And Controls

Oct 22, 2015

Toyota Motor Company (Toyota) is recalling certain model year 2009-2011 Tundra, Sequoia, Corolla, Corolla Matrix and Scion xB, 2008-2011 Highlander and Highlander Hybrid, 2007 Camry and Camry Hybrid, 2009 Camry and Camry Hybrid, 2006-2011 RAV4, 2006-2010 Yaris, and 2009-2010 Scion xD and Pontiac Vibe vehicles. During the manufacturing of the Power Window Master Switch (PWMS), grease lubricant may have been inconsistently applied to the sliding electrical contacts.

Consequence & remedy

Consequence: If the sliding electrical contacts are not protected by lubricant, debris and moisture that get into the switch may cause a short circuit and the switch assembly may overheat and melt, increasing the risk of a fire.

Remedy: Toyota will notify owners, and dealers will inspect the switch, applying a lubricant if no abnormality is found. If abnormality is found, the PWMS circuit board will be replaced, free of charge. The recall is expected to begin December 20, 2015. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is C0M.

12V491000 · Visibility:power Window Devices And Controls

Oct 10, 2012

Toyota is recalling certain model year 2007-2009 Camry, Camry Hybrid, RAV4, Corolla, Corolla Matrix, Tundra, Sequoia, Highlander, Highlander Hybrid, Yaris, Scion xB, Scion xD and Pontiac Vibe vehicles. The power window master switch assemblies in some of these vehicles were built using a less precise process for lubricating the internal components of the switch assemblies. Irregularities in this lubrication process may cause the power window master switch assemblies to malfunction and overheat.

Consequence & remedy

Consequence: If the switch overheats, it may melt, possibly resulting in a fire.

Remedy: Toyota will notify its owners, and dealers will inspect the switch and apply a special grease that inhibits heat build up or replace any switch damaged by the defect, free of charge. Toyota owners that experience a problem with a switch should not attempt to make repairs, but should contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331. General Motors will contact the Pontiac Vibe owners affected by this recall and GM dealers will inspect and repair switches as necessary. Those owners may contact Pontiac Customer Care at 1-800-762-2737.

09V388000 · Vehicle Speed Control:accelerator Pedal

Oct 5, 2009

TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.

Consequence & remedy

Consequence: A STUCK OPEN ACCELERATOR PEDAL MAY RESULT IN VERY HIGH VEHICLE SPEEDS AND MAKE IT DIFFICULT TO STOP THE VEHICLE, WHICH COULD CAUSE A CRASH, SERIOUS INJURY OR DEATH.

Remedy: TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. TOYOTA WILL BEGIN MAILING LETTERS TO OWNERS ON DECEMBER 31, 2009. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331, LEXUS AT 1-800-255-3987.

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

On June 25, 2014, Toyota initiated a Warranty Enhancement Program (ZE1) to extend warranty coverage for the brake actuator assembly and skid control ECU/brake pedal stroke sensor in certain MY 2007 to 2011 Toyota Camry hybrid vehicles. Toyota's program covers repairs related to two conditions:internal malfunctions of the Brake Actuator assembly; and overly sensitive monitoring logic for the Brake Pedal Stroke Sensor.Primary coverage provides warranty enhancement until November 30, 2015, with no mileage limitation. After the primary coverage period ends, secondary coverage is provided for 10 years or 150,000 miles from the date of first use, whichever occurs first. In addition, on July 10, 2014, Toyota initiated Limited Service Campaign E0U to replace the Brake Reservoir Tank in MY 2007 through 2011 Toyota Camry hybrid vehicles. The LSC will be available until June 30, 2017, and will only be available at an authorized Toyota dealer.ODI's analysis of failure data determined that the majority of records were related to the Brake Actuator condition, which appeared to be the causal component in 734 complaints/field reports and 864 warranty claims. The Brake Pedal Stroke Sensor (80 reports/189 claims) and Brake Reservoir Tank (91 reports/44 claims) conditions were identified in far fewer records.Analysis of the failure data indicate that each condition provides warning prior to any reduction in power assisted braking, with less than 10 percent of all reports and claims involving diagnostic codes or complaint narratives indicating increased braking effort or reduced brake effectiveness. In addition, most of the incidents in which reduced braking assist appears to have occurred involved operation in a reduced assist mode and not a sudden, complete loss of power assisted braking.ODI assessed reduced assist mode in a subjective driving evaluation which indicated that the brake system provided normal and harddeceleration capability with reasonable increases in pedal effort.Less than one percent of reports and claims analyzed by ODI included evidence indicating a complete loss of power assisted braking occurred and this mode requires operation for a significant period of time with multiple audible and visual warnings.Repair expense was cited in many of the complaints reviewed by ODI, especially those related to the Brake Actuator, which had repair costs exceeding $3,000 according to some complaints.High repair cost appears to have been a factor in the only crash identified by ODI for the fault conditions identified by Toyota, which involved a Brake Actuator failure that was diagnosed but the owner declined repair approximately eight months prior to the reported crash incident. This preliminary evaluation is closed. The closing of this investigation does not constitute a finding that a safety-related defect does not exist. For additional information regarding this investigation, including analysis of the Brake Actuator, Brake Pedal Stroke Sensor, and Brake Reservoir Tank fault conditions and failure experience, see Attachment A to this closing resume.
