ON FEBRUARY 24, 2016, CAR BRAKES ON MY 2007 TOYOTA CAMRY HYBRID LOCKED UP WHEN WE TRIED TO STOP AND ABOUT 4 LIGHTS CAME UP ON THE DASHBOARD, ABS,VSC,WARNING LIGHT AND ANOTHER ONE. WE BARELY MANAGED TO GET OUT OF HARM'S WAY BUT AFTER THE GRINDING NOISE HAD LOST ALL EFFECTIVE POWER TO THE BRAKES. THE CAR ACTUALLY CAUGHT FIRE DUE TO THE INTENSE HEAT OF THE GRINDING OF THE PADS ON THE ROTORS BUT WAS SAFELY PUT OUT. AFTER REPLACING THE PADS, ROTOR AND CALLIPERS WE STILL WOUND UP TAKING THE CAR TO A DEALER BECAUSE THE BRAKE ACTUATOR OR SOMETHING RELATED WAS STILL NOT LETTING THE BRAKES WORK. THE DEALER HAS GIVEN US A LAME EXCUSE OF WHY THIS HAS HAPPENED AND IS BLAMING IT ON SOMETHING ELSE AND WANTS TO CHARGE US A LOT OF MONEY TO REPAIR IT. UPON RESEARCH, WE ARE FINDING OUT ABOUT THE ZE1 CAMPAIGN WHERE THEY HAD ACKNOWLEDGED THIS DEFECT. AND WERE REPAIRING ALL THE CARS THAT CAME IN WITH THE DEFECT. THIS IS VERY DANGEROUS AND SOMETHING SHOULD BE DONE ABOUT IT. CONTACTING TOYOTA HAS NOT HELPED AT ALL. WE ASKED FOR AN EMAIL ADDRESS BUT WERE TOLD THAT THESE ARE HANDLED BY MAIL ONLY. WE ARE IN THE PROCESS OF SENDING THEM A LETTER ANYWAY. *JS

SKID OFF ROAD AND CAR EVENTUALLY STOPPED BUT BRAKES WERE ALL THE WAY TO THE FLOOR. I GOOGLED THE WARNING LIGHTS AND FOUND THAT IT WAS A RECALL ON THE BRAKE SYSTEM. I DID NOT RECEIVE A RECALL NOTICE IN THE MAIL IF THERE WAS IN FACT ONE. CALLED THE TOYOTA DEALER WHERE I PURCHASED IT AND THEY TOLD ME TO BRING IT IN AND THEY WOULD LOOK AT IT TO CONFIRM THAT IT IS INDEED A RECALL ISSUE.

WITHIN A MINUTE AFTER THE ABS LIGHT CAME ON ALONG WITH CHECK PARKING BREAK AND CHECK VSC SYTEM AND THE GENERAL HAZARD LIGHT ALL WERE ON. I TRIED TO BREAK AND PULL OVER AND WHEN I DID I REALIZE I COULD NOT STOP. THE ASSIST BREAKS HAD BEEN COMPLETELY DISABLED AND I HAD TO PUSH VERY VERY HARD TO SLOW DOWN. I HAD TO ALSO USE THE PARKING BREAK TO ASSIST ME . WHEN I TURNED THE CAR OFF AND BACK ON AGAIN THE BREAKS ARE BACK AS IF NOTHING WAS WRONG. THIS HAS HAPPENED A FEW TIMES EACH WITH A VERY FRIGHTENING WITHOUT WARNING EXPERIENCE. MY MECHANIC SAYS THERE ARE NO CODES SHOWING WHAT IS HAPPENING. THIS HAS THE POTENTIAL TO KILL SOMEONE. ESPECIALLY A NEW DRIVER OR AN ELDERLY PERSON THAT SIMPLY WOULD NOT HAVE THE GREAT DEAL OF POWER NEEDED OR THE REFLEXES TO SLOW DOWN WHEN THE SYSTEM MALFUNCTIONS. THIS SHOULD BE A RECALL!!

TWICE IN THE PAST TWO DAYS, I WAS DRIVING DOWN THE ROAD AND MY ABS AND CHECK VSC LIGHTS CAME ON. THE FIRST TIME, NOTHING APPEARED TO HAPPEN TO THE VEHICLE OTHER THAN THE LIGHTS. TODAY HOWEVER, I WAS DRIVING AT HIGHWAY SPEED WHEN THE LIGHTS CAME ON. LUCKILY, I WAS APPROACHING CONGESTED TRAFFIC AND I WAS ABLE TO SLOW DOWN NATURALLY. AS I CAME TO THE TRAFFIC AND WAS DRIVING AT ROUGHLY 15MPH, I TRIED TO DEPRESS THE BRAKE AND IT WENT DIRECTLY TO THE FLOOR AND DID NOT APPLY ANY BRAKE PRESSURE OR STOP THE CAR IN ANY WAY. I PUMPED THE BRAKES WHILE VEERING ONTO THE SHOULDER. THE CAR DID NOT STOP AT ALL AND I WAS FORCED TO COAST TO A STOP ON THE SHOULDER OF A 5 LANE HIGHWAY, WHERE SPEEDS OFTEN REACH 70MPH OR HIGHER. I RESTARTED THE VEHICLE, THE LIGHTS TURNED OFF AND I WAS ABLE TO DRIVE THE REST OF THE WAY HOME. I CALLED LOCAL TOYOTA DEALERSHIPS AND MADE AN APPOINTMENT TO CHECK THE CAR. THEY ARE GOING TO CHARGE ME $110 FOR THE DIAGNOSIS OF THE VEHICLE, WHICH IS ALL I KNOW AT THIS POINT. I ALSO INDEPENDENTLY RESEARCHED THE WARNING LIGHTS AND FOUND ARTICLES STATING THAT THIS IS HAPPENING FAIRLY FREQUENTLY AND THAT THE NHTSA IS INVESTIGATING. THAT ARTICLE ALSO STATED THAT THE REPAIR CAN COST UPWARDS OF $2,200. THE CAR IS ABSOLUTELY UNFIT FOR THE ROAD. I CAN'T DRIVE A VEHICLE WHERE THE BREAKS COULD STOP WORKING AT ANY MOMENT AND WITH VERY LITTLE WARNING. THERE DOES NOT APPEAR TO BE ANY ISSUE WITH THE BREAKS THEMSELVES, WHEN WORKING PROPERLY. IT APPEARS TO BE PURELY A COMPUTER PROBLEM THAT CAUSES THE BREAKS TO NOT ENGAGE.

15V689000 · Visibility:power Window Devices And Controls

Oct 22, 2015

Toyota Motor Company (Toyota) is recalling certain model year 2009-2011 Tundra, Sequoia, Corolla, Corolla Matrix and Scion xB, 2008-2011 Highlander and Highlander Hybrid, 2007 Camry and Camry Hybrid, 2009 Camry and Camry Hybrid, 2006-2011 RAV4, 2006-2010 Yaris, and 2009-2010 Scion xD and Pontiac Vibe vehicles. During the manufacturing of the Power Window Master Switch (PWMS), grease lubricant may have been inconsistently applied to the sliding electrical contacts.

Consequence & remedy

Consequence: If the sliding electrical contacts are not protected by lubricant, debris and moisture that get into the switch may cause a short circuit and the switch assembly may overheat and melt, increasing the risk of a fire.

Remedy: Toyota will notify owners, and dealers will inspect the switch, applying a lubricant if no abnormality is found. If abnormality is found, the PWMS circuit board will be replaced, free of charge. The recall is expected to begin December 20, 2015. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is C0M.

12V491000 · Visibility:power Window Devices And Controls

Oct 10, 2012

Toyota is recalling certain model year 2007-2009 Camry, Camry Hybrid, RAV4, Corolla, Corolla Matrix, Tundra, Sequoia, Highlander, Highlander Hybrid, Yaris, Scion xB, Scion xD and Pontiac Vibe vehicles. The power window master switch assemblies in some of these vehicles were built using a less precise process for lubricating the internal components of the switch assemblies. Irregularities in this lubrication process may cause the power window master switch assemblies to malfunction and overheat.

Consequence & remedy

Consequence: If the switch overheats, it may melt, possibly resulting in a fire.

Remedy: Toyota will notify its owners, and dealers will inspect the switch and apply a special grease that inhibits heat build up or replace any switch damaged by the defect, free of charge. Toyota owners that experience a problem with a switch should not attempt to make repairs, but should contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331. General Motors will contact the Pontiac Vibe owners affected by this recall and GM dealers will inspect and repair switches as necessary. Those owners may contact Pontiac Customer Care at 1-800-762-2737.

09V388000 · Vehicle Speed Control:accelerator Pedal

Oct 5, 2009

TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.

Consequence & remedy

Consequence: A STUCK OPEN ACCELERATOR PEDAL MAY RESULT IN VERY HIGH VEHICLE SPEEDS AND MAKE IT DIFFICULT TO STOP THE VEHICLE, WHICH COULD CAUSE A CRASH, SERIOUS INJURY OR DEATH.

Remedy: TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. TOYOTA WILL BEGIN MAILING LETTERS TO OWNERS ON DECEMBER 31, 2009. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331, LEXUS AT 1-800-255-3987.

On June 25, 2014, Toyota initiated a Warranty Enhancement Program (ZE1) to extend warranty coverage for the brake actuator assembly and skid control ECU/brake pedal stroke sensor in certain MY 2007 to 2011 Toyota Camry hybrid vehicles. Toyota's program covers repairs related to two conditions:internal malfunctions of the Brake Actuator assembly; and overly sensitive monitoring logic for the Brake Pedal Stroke Sensor.Primary coverage provides warranty enhancement until November 30, 2015, with no mileage limitation. After the primary coverage period ends, secondary coverage is provided for 10 years or 150,000 miles from the date of first use, whichever occurs first. In addition, on July 10, 2014, Toyota initiated Limited Service Campaign E0U to replace the Brake Reservoir Tank in MY 2007 through 2011 Toyota Camry hybrid vehicles. The LSC will be available until June 30, 2017, and will only be available at an authorized Toyota dealer.ODI's analysis of failure data determined that the majority of records were related to the Brake Actuator condition, which appeared to be the causal component in 734 complaints/field reports and 864 warranty claims. The Brake Pedal Stroke Sensor (80 reports/189 claims) and Brake Reservoir Tank (91 reports/44 claims) conditions were identified in far fewer records.Analysis of the failure data indicate that each condition provides warning prior to any reduction in power assisted braking, with less than 10 percent of all reports and claims involving diagnostic codes or complaint narratives indicating increased braking effort or reduced brake effectiveness. In addition, most of the incidents in which reduced braking assist appears to have occurred involved operation in a reduced assist mode and not a sudden, complete loss of power assisted braking.ODI assessed reduced assist mode in a subjective driving evaluation which indicated that the brake system provided normal and harddeceleration capability with reasonable increases in pedal effort.Less than one percent of reports and claims analyzed by ODI included evidence indicating a complete loss of power assisted braking occurred and this mode requires operation for a significant period of time with multiple audible and visual warnings.Repair expense was cited in many of the complaints reviewed by ODI, especially those related to the Brake Actuator, which had repair costs exceeding $3,000 according to some complaints.High repair cost appears to have been a factor in the only crash identified by ODI for the fault conditions identified by Toyota, which involved a Brake Actuator failure that was diagnosed but the owner declined repair approximately eight months prior to the reported crash incident. This preliminary evaluation is closed. The closing of this investigation does not constitute a finding that a safety-related defect does not exist. For additional information regarding this investigation, including analysis of the Brake Actuator, Brake Pedal Stroke Sensor, and Brake Reservoir Tank fault conditions and failure experience, see Attachment A to this closing resume.
