EARLY SIGNS WERE EXCESSIVE CLICKING SOUNDS COMING FROM THE BRAKE ACTUATOR ASSEMBLY ON THE RIGHT FRONT AREA OF THE ENGINE COMPARTMENT, AND AN INTERMITTENT HYDRAULIC SOUND WHEN THE BRAKE PEDAL WAS KEPT DEPRESSED. AFTER THAT I HAD THE ABS, BRAKE, WARNING, VST LIGHTS ALL ON WITH THE MESSAGE OF CHECK VST ON THE LCD OF THE CONTROL PANEL. TOOK THE CAR TO THE DEALER AND AFTER INSPECTION THEY DETERMINED THAT THE ABS ACTUATOR MODULE HAD TO BE REPLACED. THE QUOTE WAS FOR $2,600!!!!! I STARTED SEARCHING ONLINE AND FOUND OUT SEVERAL OTHER CAMRY OWNERS HAD THE SAME PROBLEM. I'M FILING THE COMPLAINT BECAUSE THIS IS A VERY CRITICAL COMPONENT IN TERMS OF SAFETY AND IT IS FAILING AT A VERY LOW MILEAGE (UNDER 100K) AND SOMETIMES EVEN UNDER 60K. MY '07 CAMRY HYBRID(CH) HAS STARTED TO EXPERIENCE BRAKE FAILURE WHILE DRIVING. THE FIRST DOCUMENTED INCIDENT: 03/09/12. WHEN I PUT THE CAR IN "PARK", TURNED OFF THE VEHICLE AND RESTARTED IT, I WAS ABLE TO RESUME DRIVING. I TOOK IT TO DEALER TO HAVE THE CAR DIAGNOSED. THE DIAGNOSIS: NOTHING WRONG WITH THE CAR, BRAKES, ETC(INV.#227594). OVER THE PAST 9 MONTHS, IT HAS OCCURRED A FEW MORE TIMES, USUALLY WHEN THE VEHICLE IS PULLING OUT OF THE GARAGE. WHEN IT HAPPENS,I TURN OFF THE VEHICLE, RESTART AND GO ON. ON 10/30/12 WHILE MY WIFE WAS DRIVING 35MPH, THE CAR SHUT DOWN, THE "CHECK VSC" LIGHT CAME ON AND THE BRAKES COMPLETELY FAILED. SHE CAUGHT A LEFT TURN SIGNAL AND COASTED INTO A PARKING LOT. OBVIOUSLY THIS WAS HORRIFYING FOR HER AND OUR KIDS. I CALLED AAA AND HAD THE CAR TOWED TO TOYOTA. MY SERVICE ADVISOR(SA) TOLD ME THEY HAVE BEGUN TO SEE THIS HAPPENING A LOT AND HE PREDICTED THE ABS BRAKE ACTUATOR (BA) FAILED. I TOLD HIM ABOUT THE INCIDENT IN MARCH, WHICH HE ACKNOWLEDGED KNOWING OF. 2 HOURS LATER HE CALLED TO CONFIRM THAT IT WAS THE BA. WE SPOKE ABOUT IT BEING A DEFECTIVE PART, HE AGREED AND WOULD CALL AND SPEAK TO TOYOTA NATIONAL(TN)TO SEE IF THEY COULD HELP OUT WITH THE $2200 REPAIR. THE NEXT DAY HE TOLD ME THAT TN WOULDN'T HELP, AND NEITHER WOULD THE DEALERSHIP. MY ONLINE RESEARCH DISCOVERED MANY CH HAVE HAD THIS PROBLEM. I DISCUSSED WITH THE SA THAT IF THE PART IS DEFECTIVE AND THEY ARE SEEING MORE AND MORE OF THIS, WHAT GUARANTEE IS THERE A NEW BA WOULDN'T REACT THE SAME WAY. HE COULDN'T GUARANTEE THAT IT WOULDN'T. INDICATORS: BIG RED BRAKES INDICATOR LIT UP, ABS LIGHT CAME ON, CHECK VSC SYSTEM LIGHT CAME ON, ANTI SKID LIGHT CAME ON, TRIANGLE WITH (!) LIGHT CAME ON, ORANGE CIRCLE WITH (!) LIGHT CAME ON. THE VEHICLE WAS TAKEN TO THE DEALER WHO UPDATED THE COMPUTER AND RETURNED THE VEHICLE TO ME. WITHIN THREE DAYS OF RECEIVING THE VEHICLE BACK, THE BRAKES FAILED AGAIN WITH THE SAME ABOVE CONDITIONS AND INDICATORS. THE VEHICLE WAS RETURNED TO THE DEALERSHIP AND REMAINS THERE PRESENTLY. THE DEALERSHIP ASSERTS THAT THEY ARE UNABLE TO FIND ANY FAULT CODES OR REPRODUCE THE PROBLEM AND ARE UNAUTHORIZED BY THE EXTENDED WARRANTY TO MAKE ANY REPAIRS WITHOUT PROVING A PROBLEM EXISTS. I CONTACTED TOYOTA USA AND REPORTED THE PROBLEM TO THEM AND AWAITING A RESPONSE. THE DEALERSHIP HAS A RECORD OF THE SEVERAL TIMES THE VEHICLE WAS BROUGHT TO THEM WITH THIS BRAKE FAILURE. I AM TERRIFIED OF AND WILL NOT DRIVE THE VEHICLE NOW UNLESS A PROBLEM IS FOUND AND CORRECTED. CAN YOU PROVIDE ASSISTANCE OR ADVISE ON WHAT CAN BE DONE TO FORCE TOYOTA TO REPLACE THE BRAKE SYSTEM IN THE VEHICLE? I NOTE I AM NOT THE ONLY PERSON WHO HAS REPORTED THIS FAILURE. Toyota Motor Company (Toyota) is recalling certain model year 2009-2011 Tundra, Sequoia, Corolla, Corolla Matrix and Scion xB, 2008-2011 Highlander and Highlander Hybrid, 2007 Camry and Camry Hybrid, 2009 Camry and Camry Hybrid, 2006-2011 RAV4, 2006-2010 Yaris, and 2009-2010 Scion xD and Pontiac Vibe vehicles. During the manufacturing of the Power Window Master Switch (PWMS), grease lubricant may have been inconsistently applied to the sliding electrical contacts.

Consequence & remedy

Consequence: If the sliding electrical contacts are not protected by lubricant, debris and moisture that get into the switch may cause a short circuit and the switch assembly may overheat and melt, increasing the risk of a fire.

Remedy: Toyota will notify owners, and dealers will inspect the switch, applying a lubricant if no abnormality is found. If abnormality is found, the PWMS circuit board will be replaced, free of charge. The recall is expected to begin December 20, 2015. Owners may contact Toyota customer service at 1-800-331-4331. Toyota is recalling certain model year 2007-2009 Camry, Camry Hybrid, RAV4, Corolla, Corolla Matrix, Tundra, Sequoia, Highlander, Highlander Hybrid, Yaris, Scion xB, Scion xD and Pontiac Vibe vehicles. The power window master switch assemblies in some of these vehicles were built using a less precise process for lubricating the internal components of the switch assemblies. Irregularities in this lubrication process may cause the power window master switch assemblies to malfunction and overheat.

Consequence & remedy

Consequence: If the switch overheats, it may melt, possibly resulting in a fire.

Remedy: Toyota will notify its owners, and dealers will inspect the switch and apply a special grease that inhibits heat build up or replace any switch damaged by the defect, free of charge. Toyota owners that experience a problem with a switch should not attempt to make repairs, but should contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331. General Motors will contact the Pontiac Vibe owners affected by this recall and GM dealers will inspect and repair switches as necessary. Those owners may contact Pontiac Customer Care at 1-800-762-2737.

TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.

Consequence & remedy

Consequence: A STUCK OPEN ACCELERATOR PEDAL MAY RESULT IN VERY HIGH VEHICLE SPEEDS AND MAKE IT DIFFICULT TO STOP THE VEHICLE, WHICH COULD CAUSE A CRASH, SERIOUS INJURY OR DEATH.

Remedy: TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. TOYOTA WILL BEGIN MAILING LETTERS TO OWNERS ON DECEMBER 31, 2009. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331, LEXUS AT 1-800-255-3987.

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

On June 25, 2014, Toyota initiated a Warranty Enhancement Program (ZE1) to extend warranty coverage for the brake actuator assembly and skid control ECU/brake pedal stroke sensor in certain MY 2007 to 2011 Toyota Camry hybrid vehicles. Toyota's program covers repairs related to two conditions:internal malfunctions of the Brake Actuator assembly; and overly sensitive monitoring logic for the Brake Pedal Stroke Sensor.Primary coverage provides warranty enhancement until November 30, 2015, with no mileage limitation.After the primary coverage period ends, secondary coverage is provided for 10 years or 150,000 miles from the date of first use, whichever occurs first.In addition, on July 10, 2014, Toyota initiated Limited Service Campaign E0U to replace the Brake Reservoir Tank in MY 2007 through 2011 Toyota Camry hybrid vehicles.The LSC will be available until June 30, 2017, and will only be available at an authorized Toyota dealer.ODI's analysis of failure data determined that the majority of records were related to the Brake Actuator condition, which appeared to be the causal component in 734 complaints/field reports and 864 warranty claims.The Brake Pedal Stroke Sensor (80 reports/189 claims) and Brake Reservoir Tank (91 reports/44 claims) conditions were identified in far fewer records.Analysis of the failure data indicate that each condition provides warning prior to any reduction in power assisted braking, with less than 10 percent of all reports and claims involving diagnostic codes or complaint narratives indicating increased braking effort or reduced brake effectiveness.In addition, most of the incidents in which reduced braking assist appears to have occurred involved operation in a reduced assist mode and not a sudden, complete loss of power assisted braking.ODI assessed reduced assist mode in a subjective driving evaluation which indicated that the brake system provided normal and harddeceleration capability with reasonable increases in pedal effort.Less than one percent of reports and claims analyzed by ODI included evidence indicating a complete loss of power assisted braking occurred and this mode requires operation for a significant period of time with multiple audible and visual warnings.Repair expense was cited in many of the complaints reviewed by ODI, especially those related to the Brake Actuator, which had repair costs exceeding $3,000 according to some complaints.High repair cost appears to have been a factor in the only crash identified by ODI for the fault conditions identified by Toyota, which involved a Brake Actuator failure that was diagnosed but the owner declined repair approximately eight months prior to the reported crash incident.This preliminary evaluation is closed.The closing of this investigation does not constitute a finding that a safety-related defect does not exist.For additional information regarding this investigation, including analysis of the Brake Actuator, Brake Pedal Stroke Sensor, and Brake Reservoir Tank fault conditions and failure experience, see Attachment A to this closing resume.
