12/2020 First the TPS sensor failed at 1,000 miles and almost caused an accident. Would not go over 25mph, was shifting hard, and the service engine light was on. Verified and replaced at dealership. Second, the rear view camera stopped working while backing up and almost caused an accident - no back windows - major safety issue. Replaced by me as they have no replacement for it even though its under recall. No warnings. Third, the transmission was shifting hard and fell out while I was driving on the freeway at 55,000. Damaged transmission and crankshaft and joints. Verified and replaced at dealership. No warnings, no lights Fourth, the transmission bolt was found loose, hard shifts, and has an internal breakdown at 71,000 miles. Verified first by independent shop - they tightened the bolt for me and is now sitting at the dealership waiting on another transmission that has been verified and approved under warranty. No warnings, no lights

I have had ongoing issues with the backup camera not related to current recall 25v-552. The image on the dash screen would flip between upside down and right side up. It still the flipped image after being “fixed”. After the dealership “fixed” that problem, it would flip and reverse the image. That was subsequently “fixed” by replacing the dash monitor. However, the poor picture quality during both day and night makes the camera unsafe to use. During the day the image may be completely or partially whited out with glare. At night there are lines and distortion across the screen. In. Addition, the new unit does not monitor van diagnostic information which could impact safety. The van is currently back in shop for another attempt to fix camera system.

The backup camera failed and the screen is black with an error message. In a large cargo van it becomes nearly impossible to see behind you. The problem was reported to the RAM dealer in Broomfield, they inspected the camera and agreed it needed to be replaced. This issue had been going on for a full year at least.

On [XXX], while driving my 2021 Ram ProMaster 2500 at highway speed in Florida, the van suddenly lost all power and came to a near stop, then re-engaged. Vehicles were in front and behind me, creating an immediate risk of serious collision, injury, or death. I pulled over ~30 minutes, restarted, but it could not exceed 20 mph while RPMs were high. AutoZone confirmed limp mode and advised a dealer. I drove it home to Port Charlotte, then had it towed to Gettel Dodge Chrysler Jeep Ram, Punta Gorda, FL, where after two weeks it was diagnosed with a failed transmission requiring replacement. The transmission can be inspected at the dealership. No warning lamps appeared before failure. Problem confirmed by AutoZone and the dealer. I contacted Stellantis (Case #[XXXX], contact Regina Knox; dealership contact Noel), but repair assistance was denied. The van also has two active recalls, including risk of rolling out of park, unremedied. This transmission failure constitutes a serious and imminent safety defect, creating a hazard to myself and the public. I request NHTSA investigate this defect, compel Stellantis to repair it at their cost, and consider any further regulatory action necessary to prevent injury or death. Recall Remedy Failure NHTSA Recall Numbers: 25V-720 (67C) & 25V-552 (59C) I am reporting a failure by FCA US LLC to provide a timely remedy for Recall 25V-720. As of Jan 2026, the remedy remains "under development" since Oct 2025. Specific Safety Concerns for RV Owners: My vehicle is a 2022 Thor Tellaro motorhome. The admitted "Risk of Vehicle Fire" is critical because the vehicle contains propane and lithium/AGM battery systems. Despite the Part 573 report on the NHTSA server listing a "Do Not Drive. Park Outside" advisory, the manufacturer has failed to check these boxes on the consumer-facing summary and has broken links ("File Not Found") for the 67C technical documents on their owner portal. This lack of transparency and the 90+ day delay for a fire-risk remedy has rendered my vehicle unusable. I request that NHTSA investigate why these safety advisories are being obscured from consumers and force the manufacturer to provide a buy-back or immediate repair.

While preparing for a trip, I noticed the upper kitchen cabinetry is detaching from the ceiling and in danger of falling. This section is part of the cabinetry housing the microwave. The detaching cabinet has a glass plate attached to the bottom as a heat guard for the stove directly below it. The stove also has a glass top. It has the potential to injury an occupant both by its weight and glass breakage during travel or when stationary. Occupants can be sitting directly across from the cabinet on the couch which has seat belts. There is a 16 inch aisle between the stove top and the edge of the couch. I'm not sure if this is the appropriate type of safety issue for your agency, but I felt it could surely injure occupants in the kitchen area.
