The contact's brother rented a 2015 Nissan Versa Note through Turo. The contact stated that while her brother was driving at approximately 15 MPH in a tunnel, the transmission was slipping, the brake pedal was depressed to the floorboard, and the vehicle unexpectedly rolled backwards while on an incline. The check engine warning light was illuminated. The renter contacted the company, which stated that it was normal for the engine light to be illuminated. The local dealer was not contacted. I RENTED A 2015 NISSAN VERSA NOTE FROM BUDGET RENTAL OUT OF THE PROV RI AIRPORT ON 1-12-16 TO GO TO A WAKE & FUNERAL. AFTER LEAVING THE WAKE AND ABOUT 7:30PM NOT LONG AFTER STARTING THE VEHICLE AND TRAVELING 1/2 MILE I LOST BOTH THE ABILITY TO BRAKE AND STEER, I WASN'T GOING MORE THAN 10 MPH . THE VEHICLE LOST CONTROL OF BOTH THE STEERING AND THE BRAKES WERE HARD AND UNRESPONSIVE. THE VEHICLE CROSSED TO OTHER LANE AND SLOWLY BRUSHED AGAINST A TELEPHONE POLE. I GOT OUT AND DIDN'T SEE ANY DAMAGE. I DROVE THE VEHICLE APX 1.5 MILES TO MY BROTHERS HOUSE AND PARKED IT FOR THE NIGHT. THE NEXT MORNING I WAS LEAVING AND NOTICED THAT THERE WAS A SMALL DENT ON THE FENDER SO I WAS GOING TO LOOK AT THE POLE 1.5 MILES AWAY AND BACKED THE VEHICLE OUT OF THE DRIVEWAY AND STARTED TO PULL FORWARD AND 12-15 FEET GOING FORWARD THE STEERING AND BRAKES WOULD NOT WORK. I GOT OUT AND SAW THAT THE WHEELS WERE BOTH POINTED IN AND AFTER LOOKING CLOSER NOTICED THAT THE LEFT STERING CONNECTOR THAT WAS SUPPOSE TO BE ENGAGED TO THE RACK IN PINION STERING WAS NOT ENGAGED AND ONLY THE RIGHT FRONT WHEEL WOULD SPIN. I VIDEO TAPED THE TIRE TRACKS THAT THE VEHICLE TRAVELED AND THAT THE WHEELS WERE BOTH POINTED IN ( WHICH COULDN'T HAVE BEEN DRIVEN 1.5 MILES FROM THE ACCIDENT) CALLED BUDGET RENTAL, LET THEM KNOW , THEY DISPATCHED TOW DRIVER AND HE AND I WINCHED THE WHEELS SO THEY COULD BE PLACED ON DOLLY FOR TOW. I TOOK PICTURES OF TOW DRIVER. I INFORMED BUDGETS MECHANIC OF WHEEL ISSUE AND HE SAID HE WOULD HAVE NISSAN LOOK AT IT. I FELT JUSTIFIED THAT MY ACCOUNT OF THE INCIDIDENT WAS NOT IN QUESTION. I THEN VIDEOED THE POLE, TIRE TRACKS AND THE 1.5 MILE DRIVE TO MY BROTHERS HOME. Nissan North America, Inc. (Nissan) is recalling certain model year 2012-2015 Nissan Versa sedan vehicles manufactured June 9, 2011, to March 11, 2015, and 2014-2015 Nissan Versa Note vehicles manufactured April 23, 2013, to March 11, 2015. The affected vehicles have a center console trim panel that may catch the driver's shoe and delay the transition from the accelerator pedal to the brake pedal.

Consequence & remedy

Consequence: A delay in the application of the brake pedal would lengthen the distance needed to stop the vehicle and increase the risk of a crash.

Remedy: Nissan will notify owners, and dealers will modify the console trim panel, free of charge. The recall began on September 14, 2015. Owners may contact Nissan customer service at 1-800-647-7261.

Nissan North America, Inc. (Nissan) is recalling certain model year 2015 Nissan Rogue vehicles manufactured June 9, 2015, to June 10, 2015, and 2015 Nissan Sentra and Versa Note vehicles manufactured June 5, 2015, to June 13, 2015. In the affected vehicles, the driver side front and rear doors may not fully latch. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 206, "Door Locks and Door Retention Components."

Consequence & remedy

Consequence: A door that does not fully latch may result in the door opening while the car is moving. The distraction may increase the risk of a crash and/or a vehicle occupant may fall out of the vehicle if they are not wearing a seatbelt, increasing their risk of injury.

Remedy: Nissan will notify owners, and dealers will inspect the date, lot number, and cavity code on the left front and rear door latches, and replace them, as necessary, free of charge. The recall began on August 31, 2015. Owners may contact Nissan customer service at 1-800-647-7261.

EA21002 · Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. On August 11, 2015, Nissan North America, Inc., submitted a Defect Information Report (DIR) describing a condition that may impede the driver's ability to transition quickly and smoothly from the accelerator pedal to the brake pedal in certain model year (MY) 2012 through 2015 Nissan Versa Sedans and MY 2014 through 2015 Nissan Versa Note vehicles (NHTSA Recall No. 15V-507).According to Nissan's DIR, in certain circumstances related to driver foot position and shoe type, the right edge of the driver?s shoe may catch the edge of the center console lower trim panel.Nissan indicated that this condition could cause a slight delay in the smooth transition between the accelerator pedal and the brake pedal which may increase the braking distance; therefore increasing the risk of a crash.Nissan's recall instructs dealers to trim the console panel so that the leading edge is at a greater distance from the driver?s foot.Nissan had previously issued a service campaign (Nissan Campaign ID #P5308; TSB #NTB15-033) on April 16, 2015, to provide the remedy for the same set of vehicles before deciding to change the program to a safety recall.Of the approximately 421,305 vehicles covered by recall 15V-507, approximately 122,558 have already been repaired under the service campaign. ODI's investigation also examined allegations that an HVAC duct in the driver's footwell area may interfere with the driver's right foot in certain circumstances.ODI's analysis of all complaints related to foot obstructions in the subject vehicles identified 45 complaints, 7 crashes and 1 minor soft tissue injury.Thirty-five (35) of the complaints, all 7 crashes and the only injury were caused by the trim panel condition which is the subject of the recall.ODI will continue to monitor complaints related to the HVAC duct condition.This investigation is closed based on Nissan's recall.The VOQs associated with the closing of this investigation are: 10725339, 10723544, 10711794, 10691631, 10649259, 10640644, 10629161, 10614957, 10610356, 10606832, 10606766, 10606670, 10597004, 10573057, 10503811, and 10498000.
