THE SUBFRAME OF MY 2009 MAZDA6 COMPLETELY ROTTED OUT FROM THE INSIDE AND THE VEHICLE BROKE DOWN DURING NORMAL USE. ON WEDNESDAY, MAY 2ND, A MAZDA DEALERSHIP REPLACED MY ROTORS, PADS, AND NOTICED THE CAR WAS PULLING TO ONE SIDE SO THEY CORRECTED THE ALIGNMENT. DURING THIS TIME THEY IDENTIFIED A NORMAL LEVEL OF RUST ON THE SUBFRAME BUT WAS NOT CONCERNED BY ANY OTHER SIGNS OF FAILURE. LESS THAN 10 MILES AFTER LEAVING THE DEALERSHIP THE SUBFRAME CRACKED, MY FRONT PASSENGER WHEEL CV AXIL DISCONNECTED AND THE CAR WAS INOPERABLE. THANKFULLY I WAS GOING ONLY 35 MPH, DOWN A STRAIGHT, PAVED CITY ROAD, AND NOT ON THE HIGHWAY. I FEAR THAT AT HIGHER SPEEDS THE CAR WOULD HAVE GONE OFF ROAD OR WORSE FOR WHEN THE SUBFRAME BROKE THE CAR JERKED HEAVILY TO THE RIGHT THEN COMPLETELY STOPPED. I BELIEVE THE DEALERSHIP PROVIDED QUALITY SERVICE AND DUE TO THE TYPE OF PHYSICAL FAILURE OF ROTTING FROM THE INSIDE THE DANGER IT POSED TO MYSELF AND MY FAMILY IN THE CAR WAS HIDDEN. THE DEALERSHIP INFORMS ME THAT THERE IS A BACKORDER (400ISH) FOR THE PART AND THAT PARTS MAY NOT BE AVAILABLE FOR SEVERAL WEEKS. SEARCHING ONLINE THERE SEEMS TO BE MANY SIMILAR CASES REPORTED WITH MODELS AROUND THIS TIMEFRAME. DUE TO THE TYPE OF FAILURE, NUMBER OF SIMILAR CASES, AGE OF THE CAR, TYPE OF USE, I AM SHOCKED THAT THERE IS NOT A RECALL.

I WAS DRIVING TO A MEDICAL APPT ON LOCAL ROADS WHEN I NOTICED A THUMPING SOUND FROM THE FRONT LEFT, AS IF THE TIRE WERE GOING FLAT. I STOPPED AND INSPECTED THE TIRE AND FOUND NO PROBLEMS, SO I CONTINUED ON TO THE APPT LESS THAN A MILE AWAY. LEAVING THE APPT I FELT THE CAR SUDDENLY PULLING SHARPLY TO THE RIGHT, AND THE DSC-OFF LIGHT CAME ON. TURNING RIGHT CAUSED A LOUD SQUEAKING NOISE. I DROVE IMMEDIATELY TO A MAZDA DEALERSHIP THAT WAS FORTUNATELY ONLY A MILE OR SO AWAY. THEY EXAMINED THE CAR AND FOUND THAT IT IS UNSAFE TO DRIVE DUE TO SEVERE RUSTING OF THE FRONT SUBFRAME (RIGHT SIDE). THEY SAID THAT IF I HAD HIT A POTHOLE THE RIGHT FRONT TIRE MAY HAVE BROKEN OFF, ENDANGERING MY LIFE AND THE LIVES OF OTHERS. THE DEALERSHIP WAS UNABLE TO LOCATE A USED SUBFRAME, AND NEW SUBFRAMES ARE ON BACKORDER UNTIL JUNE - LEAVING ME TO FIND ALTERNATE MEANS OF TRANSPORTATION FOR AT LEAST A MONTH. LIVING IN A NORTHERN CLIMATE I EXPECT CARS TO RUST, BUT A 9 YEAR OLD CAR WITH ONLY 70,000 MILES SHOULD NOT HAVE DETERIORATED TO SUCH A LEVEL THIS QUICKLY.

WHILE THE CAR WAS BEING LOOKED AT THE MECHANIC NOTICED THAT THE RIGHT HAND SIDE OF THE SUB FRAME WAS RUSTING AWAY. THIS TRANSPIRED JANUARY 18, 2018. THE REPAIR SHOP HAS BEEN TRYING TO GET A REPLACEMENT PART WITHOUT SUCCESS. I AM NOW BEING TOLD THAT THE EARLIEST THEY COULD GET ONE IS THE MIDDLE OF MAY. MY INSPECTION EXPIRED THE END OF MARCH. I FIND IT VERY DIFFICULT TO BELIEVE THAT MAZDA DOESN'T HAVE THIS PART AVAILABLE ANYWHERE. I FORTUNATELY HAVE NOT HAD AN ACCIDENT WHILE DRIVING THIS CAR.

IN SEPTEMBER 2017 WHILE DRIVING ON A CITY STREET AT APPROXIMATELY 30 MPH THERE WAS A SUDDEN AND SIGNIFICANT LOSS OF STEERING CONTROL FOLLOWING A LOUD KNOCK ORIGINATING FROM THE FRONT END OF THE VEHICLE. UPON INSPECTION THE FRONT CROSSMEMBER/SUB-FRAME WAS COMPLETELY CORRODED TO THE POINT OF BREAKING. THE STEERING RACK BECAME TOTALLY SEPARATED FROM ITS MOUNTS ON THE PASSENGER SIDE RESULTING IN THE STEERING LOSS. THE FAILURE OCCURRED AT 85,583 MILES. SINCE THE FAILURE I HAVE ATTEMPTED TO GET THIS FIXED. HOWEVER, EVEN AFTER TRYING TO GET A REPLACEMENT SUB-FRAME FROM 4 DIFFERENT DEALERSHIPS, I HAVE BEEN UNABLE TO GET ONE FOR THE LAST 7 MONTHS. THEY ARE BACK ORDERED AND EVERY TIME THE DATE COMES AROUND, THE DATE IS PUSHED BACK FURTHER AGAIN, AGAIN, AND AGAIN. I HAVE REACHED OUT TO MAZDA'S NORTH AMERICAN CUSTOMER SERVICE (SERVICE REQUEST NUMBER 1-1245266941) AND IN THE LAST MONTH HAVE GOTTEN NO MORE THAN WE'LL LOOK INTO WHEN A REPLACEMENT WILL BE AVAILABLE. IN RESEARCHING THIS PROBLEM, IT IS APPARENT THAT THIS IS FAR FROM AN ISOLATED INCIDENT AS MANY OTHERS WITH THIS MODEL CAR REPORT FAILURES OF THIS EXACT NATURE. THE EMPLOYEES OF THE LOCAL MAZDA PARTS DEPARTMENTS ARE WELL AWARE OF THE COMPLAINTS AS WELL AND HAVE STATED TO ME THEY ARE SURPRISED THAT MAZDA HAS NOT INITIATED A RECALL OR TSB REGARDING THIS ISSUE. THESE FAILURES HAVE THE POTENTIAL TO CREATE A SIGNIFICANT RISK OF INJURY AND IT IS MY BELIEF THAT MAZDA IS NOT TREATING THIS SITUATION WITH THE SERIOUSNESS AND PROMPTNESS THAT IT WARRANTS. IT IS MY UNDERSTANDING THAT THIS SAME PART IS WAS INSTALLED ON NOT ONLY THE 2009 MODELS, BUT ALL MODEL YEARS THROUGH 2013. IF THERE IS NOT CORRECTIVE ACTION TAKEN, IT IS REASONABLE TO EXPECT THIS PROBLEM TO PERSIST FOR YEARS TO COME AS THE OTHER LATER MODELS AGE.

WHILE DRIVING AT HIGHWAY SPEED (55-65MPH) THE STEERING WHEEL BEGAN PROGRESSIVELY ROTATING COUNTER-CLOCKWISE WITH THE VEHICLE REMAINING STRAIGHT. THE DYNAMIC STABILITY CONTROL TURNED OFF AS IF THE VEHICLE WAS OUT OF CONTROL AND THE VEHICLE ACTED AS IF IT WERE ON ICY/SLICK SURFACES NO LONGER PROVIDING SUPPORT TO THE DRIVER. THE BRAKING SYSTEM BEGAN TO STRUGGLE NEARLY CAUSING AN ACCIDENT ON THE OFF RAMP OF THE EXPRESSWAY CAUSING A FISHTAIL WITH THE ABS ATTEMPTING TO ENGAGE. AT THIS POINT THE STEERING WHEEL WAS REGISTERING STRAIGHT' WITH THE STEERING WHEEL TURNED APPROXIMATELY 30 DEGREES LEFT. SOMEHOW AT SPEEDS LESS THAN 15 MPH THE VEHICLE WAS DRIVEN TO THE LOCAL MAZDA DEALERSHIP WHERE IT HAS A SERVICE HISTORY FOR AN EMERGENCY SERVICE INSPECTION. A TEST DRIVE WITH A SERVICE REPRESENTATIVE SUPPORTED THE OWNER'S COMPLAINTS OF A SEVERE SAFETY CONCERN WITH RIGHT TURNS CAUSING A SIGNIFICANT 'GRINDING'/METALLIC DRAGGING SOUND AS IF THE VEHICLE'S RIGHT UNDER-SIDE WAS CONTACTING THE GROUND. AFTER INSPECTING THE VEHICLE IN THE LIFT THE MECHANIC DETERMINED THERE WAS FRONT PASSENGER FRAME METAL CORROSION WHICH WAS BREAKING OFF IN CHUNKS. THE DEALERSHIP VOICED AWARENESS OF ANOTHER 2009 MAZDA6 WITH SIMILAR MILES (APPROXIMATELY 15,000 FEWER) HAVING NEARLY THE SAME ISSUE LAST FALL/WINTER. THE VEHICLE IN THIS REPORT HAS APPROXIMATELY 189,000 PRIMARILY HIGHWAY MILES AND IS OTHERWISE IN EXCELLENT CONDITION FOR ITS YEAR/MAKE/MODEL. THE AFOREMENTIONED FRAME FAILURE SHOULD NEVER HAPPEN AND IS BELIEVED TO BE A MANUFACTURER DEFECT WHICH SHOULD NOT BE THE RESPONSIBILITY OF THE OWNER. FORTUNATELY IN THIS CASE THERE WAS NOT AN AUTOMOBILE ACCIDENT/COLLISION, INJURY, OR DEATH THOUGH THIS COULD HAVE EASILY HAPPENED. I DO BELIEVE THERE IS AN EXTREMELY SERIOUS PROBLEM HERE AND NEEDS TO BE INVESTIGATED AS HUMAN LIVES ARE AT STAKE.

PE18006 · Front Subframe Corrosion

Opened May 21, 2018 · Closed Nov 2, 2018

Status: closed (inferred from source dates) · Suspension; Suspension:front

In a letter dated September 14, 2018, Mazda North American Operations (Mazda) notified the National Highway Traffic Safety Administration (NHTSA) of a defect in the front subframe assemblies of model year (MY) 2009 through 2010 Mazda6 vehicles in certain states with high road salt use in winter months (Recall 18V-631). The recall covers approximately 48,814 vehicles that were sold or ever registered in the following states and District of Columbia:Connecticut, Delaware, Illinois, Indiana, Iowa, Kentucky, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Vermont, Virginia, West Virginia, and Wisconsin. Mazda indicated that the recalled vehicles may have insufficient paint coating on the front cross member, which after continuous use in salt states may cause premature corrosion of the cross member. As the corrosion progresses over time, the structural integrity of the mounting point at the passenger side of the lower control arm may deteriorate and cause the steering alignment to change. If the steering alignment changes significantly while the vehicle is in motion this may increase the risk of a vehicle crash.ODI's analysis of complaints it has received from the public (VOQ's) and Mazda complaint data submitted in response to the information request letter for PE18-006 identified a total of 53 non-duplicative reports alleging incidents in which front cross-member corrosion had progressed to the stage of complete or partial separation of the subject steering or suspension components and another 98 reports alleging premature and/or excessive corrosion of the subject components with no separation. Mazda's recall will inspect the subframe and apply structural reinforcements, anti-corrosive wax and an AC drain tube free of charge. In cases of excessive corrosion, the subframe will be replaced free of charge. All 53 complaints alleging complete or partial separations involved vehicles that will be covered under Mazda's recall, resulting in a failure rate of 108.6 reports per 100,000 vehicles (R/100k) in the recalled population. When all complaints are analyzed by region, 150 involve vehicles included in Mazda's recall (307.3 R/100k) and just 1 involved a MY 2009 through 2010 Mazda6 vehicle not included in 18V-631 (2.8 R/100k). The field data indicate that Mazda's recall adequately covers vehicles with a risk of experiencing premature corrosion-related failure of the subject steering and suspension components in the subject vehicles. Suspension:front

PE13015 · Door Latch Failures Due To Loose Screws

Opened May 9, 2013 · Closed Sep 30, 2013

Status: closed (inferred from source dates) · Latches/locks/linkages; Latches/locks/linkages:doors:latch

The Office of Defects Investigation (ODI) opened this investigation based on reports that the doors were failing to properly latch on model year (MY) 2009 Mazda6 vehicles.Complainants alleged that the door would fail to latch, or would not properly latch. ODI received a total of 9 complaints alleging this condition primarily on the front side doors, however any of the four side doors could be affected. In response to ODI's Information Request, Mazda provided 56 consumer complaints related to the alleged problem, and 329 warranty claims. On September 23, 2013, Mazda notified ODI that it would conduct a safety recall on model year 2009-2013 Mazda6 vehicles built from February 4, 2008 through August 24, 2012. This period covers the start of production of the MY 2009 Mazda6 through the end of production for the 2013 model year and totals a population of approximately 161,400 vehicles. Mazda determined that the door latch mounting screws may become loose due to lack of torque during manufacturing and/or unevenness of the door shell in the latch mounting area. If all three door latch mounting screws become loose, the door latch mechanism may not engage, potentially resulting in the doors opening while the vehicle is in motion. To remedy the condition, a thread locking adhesive will be applied to the latch screws on all side doors, and the screws will be tightened to the specified torque. This investigation is closed on the basis of Mazda's announcement of safety recall 13V-425.ODI reports cited above can be reviewed at http://www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification (ODI) numbers: 10406174, 10448036, 10492403, 10503682, 10508051, 10515258, 10521830, 10535228, 10544489.
