The contact owns a 2016 Lincoln MKX. The contact stated that while operating the vehicle and depressing the brake pedal, the vehicle failed to immediately stop with the message “Low Brake Fluid” displayed. Brake fluid was later added to the brake reservoir, and the vehicle was driven to the local mechanic, who diagnosed that the brake hoses were faulty and needed to be replaced. The brake hoses were replaced, and the vehicle was repaired. The contact previously received notification of NHTSA Campaign Number: 20V469000 (SERVICE BRAKES, SERVICE BRAKES, HYDRAULIC). Additionally, the contact stated that the back over prevention camera had intermittently failed. The cause of the failure was not yet determined. The manufacturer and local dealer were not yet contacted. The failure mileage was approximately 52,000.

I am writing to file a formal safety complaint regarding the extreme delay in the recall remedy for my vehicle. Despite the recall being issued months ago, Ford Motor Company/Lincoln has failed to resolve this critical safety issue in a timely manner. Here are my vehicle details: Year/Make/Model: 2016 Lincoln MKX VIN [XXX] NHTSA Recall Number: 25V544 Manufacturer Recall Number: 25S87 Component Affected: Rear Flexible Brake Hoses Description of the Safety Concern: The recall notice explicitly states that a failure in the rear flexible brake hoses can lead to a loss of brake fluid, resulting in increased brake pedal travel and significantly increased stopping distances, which greatly elevates the risk of a crash. Recently, I have started experiencing a noticeable softening in the brake pedal ("increased pedal travel"), which makes driving the vehicle extremely hazardous. When I contacted my local Lincoln dealership, I was informed that they do not have the necessary parts in stock and could not provide a definitive timeline for the repair. As a consumer, I am being forced to drive a vehicle with a known, severe braking defect because the manufacturer has failed to supply the remedy parts. This is a direct threat to my life, my family’s safety, and the safety of others on the road. Furthermore, this prolonged delay is causing me significant financial hardship and stress. I urgently request NHTSA to intervene and hold Ford Motor Company accountable for expediting the parts distribution for this critical safety recall. While driving home from work during rush hour, a warning light illuminated on my dashboard and my vehicle suddenly lost braking ability. When I pressed the brake pedal, the vehicle did not slow or stop. I was forced to pull onto the shoulder of the road and pump the brake pedal extremely hard multiple times before the vehicle finally came to a stop. This occurred in heavy traffic and created an immediate and severe safety risk. The vehicle was towed to a dealership for inspection. I was informed that only a “temporary fix” could be performed, and I was required to pay approximately $500 for this repair in order to regain use of the vehicle. The vehicle has not received a permanent repair, and the dealership has taken several months without completing a permanent fix. I have also been waiting several months for reimbursement of the almost $500 paid for the temporary repair, with no resolution. I am concerned that I was charged for a temporary repair related to a serious safety failure and that continued delays in both reimbursement and permanent repair place me at ongoing risk. I am filing this complaint due to the dangerous nature of the brake failure, the lack of a permanent repair, the extended delay in resolving a critical safety issue, and the financial burden placed on me for a temporary solution.

My car is currently under two recall notices however only one of the issues has been communicated to me of resolution. One being emissions control 25E02 and second being Recall 25S87 REAR BRAKE FLEXIBLE HOSE LEAK. Recall 1- emissions has been a problem for about 2 years now. Emissions are connected to my engine thus having my check engine light come on. My car failed an emissions test but was only 4 years old and the catalytic converters are not broken, have hole, anything leading to failed test, car repair fees and non-registered vehicle. The second recall is not being fixed, and I have had to replace my brake house, line and rear right brake twice this year. Lastly, I have collision warning however it does not work properly if at all. I was in a minor fender bender when I went to push on brakes the sound to warn me of a collision did not go off until after the crash! I am unsure of the process but if connected to my braking system it failed and if it is a preventative measure it sucks.

1EA21002 · Desiccated Air Bag Inflator Rupture
Opened Sep 17, 2021 · No close date supplied
Status: open (inferred from source dates) · Air Bags:frontal:driver Side:inflator Module; Air Bags:frontal:passenger Side:inflator Module
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced. In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators. It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.
