Below is the email that I sent to both the manufacturer and the dealership: I hope your Christmas Eve was better than mine. My son and I were driving up to the Poconos [XXX] to go skiing and I noticed the sonar detection light come on. When we were driving back from the Poconos, the transmission would not catch around the second or third gear. The RPMs would shoot up. The dash / warning lights lit up. I pulled over to the side of the road and I called Lexus dealership, service center, and road side assistance. However, due to the fact that it was [XXX], the offices were closed. I managed to make it to my parent’s home, but it was a very dangerous ride on [XXX]. The odometer stated that we were going 50 mph, but we were crawling and the RPMs were 4000. The RPMs at rest were always over 1000. Additionally at times, the gear would not shift into second or third and/or bottom out. Obviously the transmission is beyond malfunctioning. Below are pictures including: 1.) check your engine: A malfunction in the electric control of the engine, throttle or automatic transmission has been detected... 2.) a malfunction in the Intelligent Clearance, Sonar System has been detected 3.) traction control turned off 4.) Secondary Collision Break System malfunction, 5.) System Malfunction 6.) Drive-Start Control Malfunction. The vehicle has just over 3000 miles. I am beyond distraught. My phone number is [XXX] . However, I think it's best if we communicate through email at this point. I planned a family ski trip to Killington, VT for the New Year. I would like to request a loaner vehicle so I can salvage this holiday. I look forward to hearing from you and your service department. The car comes with proactive driver assist. The system when activated will cause the car to unexpectedly turn the wheel and lunge usually left if the system suspects you are going off of track. It works even on unmarked 2 lane country roads. Very disconcerting and dangerous. My wife was then subsequently in a serious accident on a slippery and snowy night that totaled the car. We are not convinced that the proactive driver assist system was the cause of the accident but can't help but think it may have been a contributing factor to the accident. Her car swerved into the oncoming lane on a unmarked (snow covered) highway.

While driving on a local highway recently, my forearm brushed the automatic shift knob while reaching to place my phone on the wireless charging pad. The transmission shifted into neutral, resulting in an unexpected loss of speed. It took me a few seconds to realize what had happened - I shifted back into Drive, and resumed my trip. No accident resulted from this, but it has happened twice in the month that I have owned the car. I reported the issue to the dealership thinking my car had a problem - they said that this is the way the car is designed to function. I found a section in the Owner's Manual which confirms this - shifting from Drive to Neutral while stationary requires the knob to be held to the left for a couple seconds; for some reason, doing this while moving requires only a tap (see attached photo of the owner's manual). This seems like a design issue - the unexpected loss of power from bumping the shift knob to the left (no forward or backward motion is needed) could easily result in an accident. I don't understand why they specifically created a system where a "push and hold" is not required to shift into neutral when the vehicle is moving. I reported this issue to corporate Lexus today. They said they will consider this feedback in subsequent model designs, but I wonder whether a recall is appropriate. That's why I'm reporting the issue to you, as well.

Structure
The material (most likely) plastic used as a trim on the vehicle is melted or started to melt. This is the trim (b-pilar) between the driver's door and the rear passenger's door. We believe the quality of the material used for the trim is defective and should be addressed by the manufacturer.

Possibly battery issue. Four (4) different tow truck drivers over 4 days could not gain entry. Safety hazard as a child or disabled person could have been locked inside. Tow driver could not “pop” the door lock. Despite pulling up on the inside handle, the door would not open. Towed to dealer. Thus far dealer has no solution. Dealer said they are contacting Lexus. No warning lights, etc. Vehicle is a plug-in hybrid. Drive Battery was at 100%.

We just pick up this car last week from the dealership here at San Antonio. TX. As we were driving there is a downpour/rain due to hurricane beryl, turn on the wiper and did not work, just got stuck in the middle of the windshield. i almost got hit on the road because i can see in front of me. Maybe it is also my fault by not checking this things before i drove off. At this moment the car is back at the dealership and my service advisor says that the motor is defective and they will order the parts.

I have purchased a new Lexus RX 350 2024 from South County Lexus in Mission Viejo in March 2024. The contact owns a 2024 Lexus RX450. The contact stated that upon attempting to enter the vehicle, the vehicle failed to unlock with the key fob, prompting the contact to attempt to manually unlock the vehicle; however, the vehicle failed to unlock. The contact called Roadside Assistance and was determined that the battery was drained and was able to jumpstart the vehicle after using a Slim Jim to open the door. There was no warning light illuminated. The vehicle was taken to the dealer, who was unable to duplicate the failure. The contact was informed that leaving the key near the vehicle would result in inadvertent battery discharge. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 1,500. The VIN was not available.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2025 Lexus NX350, NX250, 2023-2026 RX350, and 2024-2026 TX350 vehicles. The rearview camera image may not display when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Consequence & remedy
Consequence: A rearview image that does not display reduces the driver's view behind the vehicle, increasing the risk of a crash.
Remedy: Dealers will update the rearview camera software or replace the rearview camera, as necessary, free of charge. Owner notification letters were mailed May 15, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26LB02 and 26LA02.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Consequence & remedy
Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.
Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

The second-row center seat belt webbing may be damaged. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 209, "Seat Belt Assemblies."
Consequence & remedy
Consequence: A damaged seat belt may not adequately restrain an occupant, increasing the risk of injury during a crash.
Remedy: Dealers will inspect the seat belt webbing and replace the seat belt assembly if necessary, free of charge. Owner notification letters were mailed March 20, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TA03 and 25LA03.
Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2024 Lexus RX 350, RX 350h, 2024-2025 NX 250, NX 350, and NX 350h vehicles. The head restraints on the driver and front passenger seats may be inadvertently removed without pressing the lock release button. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) Number 202, "Head Restraints."
Consequence & remedy
Consequence: A head restraint that is inadvertently removed and not reinstalled can increase the risk of injury during a rear-end crash.
Remedy: Dealers will replace the driver and front passenger seat head rest assemblies, free of charge. Owner letters were mailed February 14, 2025. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are 24LB06 and 24LA06.
