Issue with the forward collision sensor, lane keep departure, and adaptive speed control. It has a mind of its own if it works or not. Some times it working then while drive the warnings and dings start going off while driving. There isn't a way to use cruise control if there is an issue with the system.

My current 2020 Pilot has presented a failure/ malfunction at the multipurpose camera in the front side resulting on the most important safety advance drivers assistance systems to not to work (Collision Mitigation System, road departure mitigation system, auto high beam, lane keeping assist, automative emergency breaking). This malfunction for this safety device could put at risk my family while driving since system got deactivated for no reason and will not respond to under unexpected situation while driving. I notice several messages on my dashboard then I confirmed safety componentes where bypassed and not activated not working properly. The problem was reporte at a dealer, then vehicle device was inspected and confirmed by the dealer that the safety multipurpose camera has a malfunction. Prior to the failure there were no messages or symptoms, just stops working without advice. Safety devices must he operative at all time, me vehicle is in very good condition and not more that 6 years old.

An internal failure on the front windshield camera assembly part #36160-TG7-A63 had caused the Advanced Driver Assistance Systems to fail. Which in turn has caused the following systems to fail: Adaptive Cruise Control, Automatic Emergency Braking, Blind Spot Warning, Forward Collision Warning, Lane Departure Warning, Lane Keeping Assistance, Parking Collision Warning, and Rear Cross Traffic Warning. Leaving these systems unrepaired due to a known and common hardware failure issue leads to this specific vehicle and the ones similar unsafe for the driver and occupants due to the safety systems being inoperable. The problem has been confirmed on two separate occasions by my local Honda dealer. The known issue with this specific camera assembly leads to a multitude of warning lights being displayed including: adaptive cruise control warning, lane keep assist, collision mitigation system problem, and auto high beam mitigation.

This car only has ~86,000 miles and this is unacceptable. This failure disables the Lane keep assist, lane departure, auto high/low beam, adaptive cruise control, auto braking safety features and causes the brake light system light to come on. Apparently, this is a common issue and should be investigated. The parts are on back order nationally and good luck finding a used one.

I am reporting a safety defect in my 2020 Honda Pilot EX-L ~86,300 miles. The rearview camera and electrical/infotainment system intermittently fail, showing static, disconnecting Bluetooth, and sometimes leaving the camera blank. This creates a serious safety risk when reversing, driving in tight spaces, or transporting my children. These issues began within the first year of ownership (2020–2021) and continued after Recall 23-047, during which cables were replaced and the system later rewired in August 2023. Despite these repairs, the defects persist. The dealership Pohanka Honda Fredericksburg confirmed the faults multiple times but now refuses to repair, quoting over $3,000. The vehicle has also experienced three dead batteries since 2020, leaving me stranded multiple times, including once during out-of-state travel. There were no warning lights for the battery failures, but infotainment and camera issues were noticeable immediately. The vehicle has been out of service for approximately three cumulative months, impairing reliability and my ability to perform daily work and family responsibilities. These defects have significantly impaired the use, value, and safety of the vehicle, especially given my responsibilities as a single mother, caregiver to a blind parent, and emergency department practitioner. The vehicle has been inspected by the dealer and reviewed by Honda corporate. I am seeking a proper remedy for a federally mandated safety feature (rear camera, FMVSS 111). The vehicle and affected components are available for inspection upon request. This complaint documents ongoing safety risks, repeated battery failures, and unresolved recall-related defects that began under warranty. Honda’s failure to properly repair these defects continues to create unsafe driving conditions and a serious risk of injury to myself, my passengers, and the public.

PE25008 · Engine Failure

Opened Aug 20, 2025 · No close date supplied

Status: open (inferred from source dates) · Engine

The Office of Defects Investigation (ODI) has received 414 reports of connecting rod bearing failures in the 3.5L V6 engine used in the following vehicles: Model Years (MY) 2018-2020 Acura TLX, MY2016-2020 Acura MDX, MY2016-2020 Honda Pilot, MY2018-2020 Honda Odyssey, and MY2017-2019 Honda Ridgeline.  The subject engine failures were analyzed as part of a Recall Query (RQ24013).  It was determined that these engine failures are outside the scope of Honda Recall 23V-751. Further, evidence gathered in conjunction with RQ24013 does not suggest this failure is caused by the same crankshaft manufacturing defect that is addressed by Honda in Recall 23V-751.  Based on analysis of data from RQ24013, NHTSA has closed that investigation. The significant number of reports of engine failure in vehicles outside the scope of RQ24013 presents a potential safety risk that warrants further investigation.  ODI is opening this Preliminary Evaluation (PE) to further evaluate the scope and severity of the potential problem and to fully assess the potential safety-related issues.  To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. RQ24013 · Engine Failure

Opened Nov 8, 2024 · Closed Aug 20, 2025

Status: closed (inferred from source dates) · Engine

On November 8, 2024, the Office of Defects Investigation (ODI) opened Recall Query RQ24013 to assess the scope of Honda recall 23V-751 after receiving complaints that alleged connecting rod bearing failures in vehicles that were not within the scope of recall 23V-751. In November 2023, Honda recalled 248,999 various Model Year (MY)2016–2020 Honda and Acura vehicles equipped with the 3.5L V6 engine. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation.  NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008). The Agency reserves the right to take additional action if warranted by new circumstances.
