These safety features have failed on my 2016 Honda Pilot, Touring, AWD. The failures include Forward Collision Warning, Lane Departure Warning, Lane Keeping Assistance and Adaptive Cruise Control. When I reported this to my local Honda Dealership they did a plug in indicator test for $195.00. All they could tell me was that I needed a new camera. The cost would be $2000.00 plus. One of the reasons we purchased this vehicle was because of these safety features. We are both drivers [XXX], and thought these were excellent safety features considering [XXX]. However, the cost to fix seemed excessive. We are still able to drive the car but without the safety features that originally led us to buy this vehicle. We delayed reporting this failure thinking there would be a recall that would cover the failures. No recall has been reported to this point. Summary of issue The vehicle developed an engine/emission warning light and failed mechanically without any crash or impact. The car drove fine until about one week ago. I have driven the vehicle normally and performed regular maintenance. Events and observations A warning light appeared indicating engine and emission problems. I took the vehicle to two separate mechanics: Mechanic A diagnosed engine damage and catalytic converter issues. Mechanic B diagnosed engine damage and a possible fuel pump problem. I am not a mechanic and cannot independently interpret technical findings, but both inspections indicate serious engine-related failures. The vehicle has approximately 80,000 miles and was purchased for more than $20,000 less than two years ago. No collisions, impacts, or abuse occurred that could explain the failure. I believe this is a potential manufacturing defect. I contacted a Honda dealer about the problem; they stated it is not a manufacturing defect and declined to accept responsibility for repair. I believe the dealer is avoiding responsibility. Requested action I request NHTSA open an investigation (or add my report to any existing investigation) into this vehicle’s engine and related components to determine whether a safety defect or noncompliance exists. I request guidance on next steps and that this complaint be considered in any recall evaluation.

While driving my 2016 Honda Pilot (VIN: [XXX], approximately 125K miles) on the freeway at 65 mph, the engine seized and the vehicle would no longer accelerate. The check engine light began flashing, the oil pressure light came on, and the vehicle began making loud grinding noises. After pulling over safely, the vehicle was no longer drivable and required towing. Upon inspection by Northampton Car Care (6115 Root Road, Spring, TX), it was determined that the engine failure is consistent with the connecting rod bearing failure and crankshaft defect described in NHTSA Recall 23V-751. My VIN does not currently appear in the recall database; however, given that my vehicle experienced the exact failure mode described in the recall, I am requesting this complaint be considered under both Recall 23V-751 and NHTSA Investigation RQ24-013. What component or system failed or malfunctioned, and is it available for inspection upon request? The engine stalls out and shuts the car down. How was your safety or the safety of others put at risk? While driving down the road, if the engine shuts off, myself, passengers, and other vehicles could cause a fatal crash. Has the problem been reproduced or confirmed by a dealer or independent service center? Not yet Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? I have brought this in several times to try and fix this issue. The fuel pump has been replaced and the auto idle recall has been take care of. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? The check engine light comes on occasionally when it happens; sometimes when it does not. Other details: Several times now my car has randomly shut off especially when driving down the road. I wasn't sure what happened, but I finally figured out a way to replicate it. Basically if you drive it after about an hour of non-stop driving / engine being turned on, the engine overheats causing the car to stall out and eventually turn off. I timed this at 10:15AM to 11:25AM and my suspicions were correct. At 11:25AM, the car stalled and shut off while I was parked in my driveway.

I heard knocking as I was leaving gas station. I was scared about what was going wrong and if it was safe to drive home. I was alone and it was after dark. I took the next day off and took the car to my mechanic. He said it sounded like rod knock but didn’t want to do the engine replacement so he sent me to another mechanic who also said it sounded like the engine needed to be replaced. I didn’t have the money or the time to deal with it so it has been parked for the last five months. Today I took it to the dealer (that I bought it from new) for a diagnostic inspection. They stated that the noise was coming from main shaft, unable to repair, will need to replace engine. Cost estimate to replace engine is $15,964.36. I do not remember a check engine light coming on, only the lights for the driving sensing items. Which I read were also problems on this vehicle. Photos of the inspection report have been uploaded. A mistake was made on the mileage it is 103,203 not 103,703. Thank you for your time.

The contact owns a 2016 Honda Pilot. The contact stated that after attempting to use the Push-To-Start to start the vehicle, an abnormal clicking sound was heard and the engine failed to turn over. Several Unknown warning lights illuminated. The vehicle was towed to a local independent mechanic, where it was diagnosed with a seized engine that was possibly related to NHTSA Campaign: 23V751000 (Engine and Engine Cooling). The vehicle was later towed to a local dealer, where it was diagnosed at the consumer's expense as having a seized engine; however, the vehicle was not repaired due to the VIN not being included in NHTSA Campaign:23V751000(Engine and Engine Cooling). The manufacturer was made aware of the failure, but no assistance was provided. The failure mileage was approximately 78,962.

The affected vehicles have fuel tanks that were manufactured with insufficient welds which may separate and allow fuel to leak out.

Honda (American Honda Motor Co.) is recalling certain model year 2016 Honda Pilot 2WD vehicles manufactured May 4, 2015, to September 8, 2015 and 2016 Pilot 4WD vehicles manufactured May 7, 2015 to September 4, 2015. In the affected vehicles, when one of the safety systems such as tire pressure monitoring, anti-lock braking or electronic stability control malfunctions, there is potential that the instrument panel will not illuminate the corresponding warning light, however the warning lamps will illuminate when the ignition is turned off and then turned back on. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 126, "Electronic stability control systems", number 135, "Light vehicle brake systems" and number 138, "Tire pressure monitoring systems".

Consequence & remedy

Consequence: If a safety system cannot immediately warn the driver when needed, the driver may be at increased risk of a crash.

Consequence & remedy

Consequence: If the third row seatbelt is trapped, the occupants may not be restrained properly, increasing the risk of injury.

The Office of Defects Investigation (ODI) has received 414 reports of connecting rod bearing failures in the 3.5L V6 engine used in the following vehicles: Model Years (MY) 2018-2020 Acura TLX, MY2016-2020 Acura MDX, MY2016-2020 Honda Pilot, MY2018-2020 Honda Odyssey, and MY2017-2019 Honda Ridgeline.  The subject engine failures were analyzed as part of a Recall Query (RQ24013).  It was determined that these engine failures are outside the scope of Honda Recall 23V-751. Further, evidence gathered in conjunction with RQ24013 does not suggest this failure is caused by the same crankshaft manufacturing defect that is addressed by Honda in Recall 23V-751.  Based on analysis of data from RQ24013, NHTSA has closed that investigation. On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. On November 8, 2024, the Office of Defects Investigation (ODI) opened Recall Query RQ24013 to assess the scope of Honda recall 23V-751 after receiving complaints that alleged connecting rod bearing failures in vehicles that were not within the scope of recall 23V-751. In November 2023, Honda recalled 248,999 various Model Year (MY)2016–2020 Honda and Acura vehicles equipped with the 3.5L V6 engine. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation.  NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008). The Agency reserves the right to take additional action if warranted by new circumstances.
