Had a indicator light for "Emission System" come on. Used fuel injector cleaner on next fill up and the light went out. Light came on again a few weeks later. Ran another bottle of injector cleaner and once again the light went out. A couple of months later the light can back on and the car gave an audible warning to pull over and shut off the engine and to make sure we were not near an ignition source. After a short time with the car off it was restarted. The "Emission System" light stayed on but the car drove ok. We took the car to the dealer. They checked everything and said that the issue was the fuel injectors needed to be replaced. They told us the parts are on a "National backorder" and they have no idea when they will become available. The car is undrivable and remains at the dealership.

Severe risk of carbon monoxide poisoning. At 5:00 am in the morning the alarm on my car went off by itself and there was no way I could turn it off or even start the car. I finally disconnected the battery to stop the car and later reconnected the battery assuming the issue was gone. When I went back to check on the car later, found that the car had started by itself and the garage was full of fumes. I installed a carbon monoxide sensor in all rooms and parked the car outside. The car now randomly tries to start itself(and all warning lights go on and dash indicates an error with all systems) and when it does start(by itself) there is no way to shut it off except for disconnecting the battery. The car is now at the dealership but a quick google search reveals that this is a frequent issue with 2016/2017 Honda Pilots and a part - 38320-TG7-A420 (remote control unit) needs to be replaced and that this part is under serious backorder nationally (indicating a serious problem). Requesting NHTSA to investigate this issue and part.

The connecting rod bearing has gone bad resulting in slippage and engine rod knocking. The bearing has gone bad because the crankshaft was manufactured/machined out of proper specifications per Honda America. The problem in question is already posted on the NHTSA website. “Engine Damage from Connecting Rod Bearing Wear” The noise occurred while driving to North Georgia with seven people in the 8 passenger car. Once we heard the knocking we immediately pulled over, 115 miles from home. Further driving could have caused the engine to seize, slip, or explode. The problem has been confirmed by both a certified master mechanic at the Honda dealership as well as an independent mechanic who worked for a dealer for a number of years. Yes the 2016 Honda Pilot has been inspected by the certified master mechanic at the dealer. There were no warning lights that came on until the time of the noise. Once the noise was apparent the oil lamp, check engine light, and other warning lights began to flash.

Since 2021 the vehicle has stalled everywhere we go and has a gas/electrical burn fume. The vehicle had stalled in the middle of the highway and didn't allow us to shift it into any gears while flashing all dashboard lights. The vehicle was towed to Legend driven Honda where they couldn't find any issues and said they just reset the transmission code. For our peace of mind, we thought it would be best to have the full work up on the transmission since the dealership seemed incompetent with our vehicle's malfunction. The vehicle continued to stall and even had trouble starting up at times with the continued gas fumes. Well, it wasn't long before the issue returned and we were back to where we started, along with our A/C unit failing on us. We left the vehicle idle in the driveway for a bit as it was a safety concern for our families' wellbeing. We later received a recall notice on the auto-idle stop mechanism which we figured must have been the issue all along. We took it back to the dealership where they did their full inspection and came up with our transmission needing its regular full work-up which we had proclaimed that we already had done in the earlier months. After the alleged recall fix, the problem continued. We took the vehicle back and were told it was our battery and recommended our timing belt be replaced otherwise it would cause major issues down the road(it was concerning to us as these issues were never brought up the multiple times we had brought it in for the initial issues). Surely, we had it all fixed and replaced for the safety of our families well being having had our vehicle stall numerous times on the road which has now caused our children to be frightened of driving anywhere. Well on return home from the dealership, the vehicle did not stall, 2 hours later went to turn it on, it did not want to start and made a loud knocking noise. Currently towed to a different dealership

While driving on the highway, the vehicle suddenly stalled, and many notifications came up on the dash. Some of the warnings include: emissions system warning, power steering system problem, trailer stability assist problem, power steering problem, hill start assist problem, vehicle stability assist problem, all wheel drive system problem. While driving on highway, the vehicle stalled, lost the ability to steer and brake, and had to roll to the side of the road. There was total engine failure. Dealership diagnostics indicate a problem with crank shaft/ connecting rod, and the entire engine needs to be replaced. There is an existing recall for my year, make, and model of vehicle 2016 Honda Pilot for this exact problem, but it has not been applied to my VIN yet. The recall needs to be extended to cover my vehicle. My family's safety was compromised by complete engine failure on a highway during rush hour.

The system works fine initially, but after 30 minutes of driving, the system becomes filled with static noise. The static noise starts out low, but quickly gets to the point where nothing else can be heard. This is across all input channels: AM/FM radio, Bluetooth, XM Satellite, etc.. The system then eventually shuts down and no sound can be heard.

Opened Aug 20, 2025 · No close date supplied

Status: open (inferred from source dates) · Engine

The Office of Defects Investigation (ODI) has received 414 reports of connecting rod bearing failures in the 3.5L V6 engine used in the following vehicles: Model Years (MY) 2018-2020 Acura TLX, MY2016-2020 Acura MDX, MY2016-2020 Honda Pilot, MY2018-2020 Honda Odyssey, and MY2017-2019 Honda Ridgeline.  The subject engine failures were analyzed as part of a Recall Query (RQ24013).  It was determined that these engine failures are outside the scope of Honda Recall 23V-751. Further, evidence gathered in conjunction with RQ24013 does not suggest this failure is caused by the same crankshaft manufacturing defect that is addressed by Honda in Recall 23V-751.  Based on analysis of data from RQ24013, NHTSA has closed that investigation. The significant number of reports of engine failure in vehicles outside the scope of RQ24013 presents a potential safety risk that warrants further investigation.  ODI is opening this Preliminary Evaluation (PE) to further evaluate the scope and severity of the potential problem and to fully assess the potential safety-related issues.  To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. To review the ODI reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

RQ24013 · Engine Failure

Opened Nov 8, 2024 · Closed Aug 20, 2025

Status: closed (inferred from source dates) · Engine

On November 8, 2024, the Office of Defects Investigation (ODI) opened Recall Query RQ24013 to assess the scope of Honda recall 23V-751 after receiving complaints that alleged connecting rod bearing failures in vehicles that were not within the scope of recall 23V-751. In November 2023, Honda recalled 248,999 various Model Year (MY)2016–2020 Honda and Acura vehicles equipped with the 3.5L V6 engine. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation.  NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008). The Agency reserves the right to take additional action if warranted by new circumstances.
