The 6-speed automatic transmission on my 2016 Honda Pilot EX-L (103,200 miles) has experienced a severe mechanical failure casing dangerously hard, violent gear shifts during both acceleration and deceleration. Critically, no dashboard warning lights or malfunction indicator lamps illuminated while this dangerous driving condition persisted, leaving me with no advance warning of a safety hazard. A subsequent diagnostic scan at an Authorized Honda dealership revealed hidden diagnostic trouble code P0776 (pressure control solenoid "B" performance or stuck off) and a failing torque converted. The severe, unpredictable jerking and hesitation during shifting creates an immediate traffic hazard during acceleration in traffic. Approximately 45 days prior, an out-of-town Honda dealership performed a prescribed triple fluid flush when the issue first arose. Then, 5 days later, my local Honda dealership performed the prescribed software update for shifting issues, which failed to remedy the underlying defect again. The dealership has now quoted $11,000 + for a full replacement of the transmission and torque converted. American Honda corporate has opened Case Number: 16606142 to track this defect.

The contact stated that the failure had occurred after the vehicle had been driven for two to three hours. In addition, the contact stated that while reversing, the rear-view camera screen flashed on and off. The vehicle was taken to the dealer; however, the failure was not duplicated. The transmission fluids were flushed. The contact stated that the transmission fluids had been flushed and refilled four times within the year. The contact referenced an unknown recall repair for the rear-view camera; however, the VIN was not associated with a recall related to the failure. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The manufacturer was made aware of the failure but provided no assistance. The failure mileage was 60,000.

I heard knocking as I was leaving gas station. I was scared about what was going wrong and if it was safe to drive home. I was alone and it was after dark. I took the next day off and took the car to my mechanic. He said it sounded like rod knock but didn’t want to do the engine replacement so he sent me to another mechanic who also said it sounded like the engine needed to be replaced. I didn’t have the money or the time to deal with it so it has been parked for the last five months. Today I took it to the dealer (that I bought it from new) for a diagnostic inspection. They stated that the noise was coming from main shaft, unable to repair, will need to replace engine. Cost estimate to replace engine is $15,964.36. I do not remember a check engine light coming on, only the lights for the driving sensing items. Which I read were also problems on this vehicle. Photos of the inspection report have been uploaded. A mistake was made on the mileage it is 103,203 not 103,703. Thank you for your time.

To Whom It May Concern, I am submitting this complaint to report ongoing engine and emissions system problems with my 2016 Honda Pilot that raise serious safety concerns and may be related to a broader defect currently under investigation. I purchased this vehicle from a dealership in April of 2024 and the issues began shortly afterward at approximately 67,000 miles. The vehicle currently has approximately 74,600 miles and has received regular oil changes and routine maintenance. The vehicle has repeatedly displayed emissions system warning messages and engine fault codes. It has experienced persistent engine misfiring even after a mechanic performed repairs on cylinder 1. In addition, the vehicle exhibits rough engine shaking during driving and idling, engine stalling, and rough or delayed gear changes continuously since 2024. Despite attempted repairs, the problems continue to occur to present. The stalling, misfiring, and unstable engine operation create a potential safety risk, particularly while driving at speed or in traffic. The rough shifting and loss of smooth power delivery further impact safe vehicle operation. Given the relatively low mileage, proper maintenance, and the recurrence of these symptoms, this does not appear to be normal wear and tear. I am aware that similar engine-related complaints have been reported by other owners of 2016 Honda Pilot vehicles and that this model year is included in an ongoing federal investigation into potential engine defects. I am requesting that this complaint be reviewed as part of that investigation and considered for further action, including possible recall or service remedy, to address the safety risks posed by these engine and emissions system failures. Honda is aware of thousands of 9 speed transmission failures. but worst of all they tell you that you failed to change trans fluid as scheduled, but there is no schedule printed in their owner's manual, nor in their vehicle service maintenance manual. the service manual only speaks to the quality of fluid required, and that it not be mixed with other brands of trans fluids. it then goes on to say "HAVE A DEALER CHECK THE FLUID LEVEL AND REPLACE IF NEEDED." I have owned cars with sealed trans that required no maintenance, which is not the case with the Honda's 9speed trans. The Honda 9 speed is a sealed unit. I am currently face with an estimated bill of $10,600 to replace my failed trans. There is a reaccuring problem with the 9 speed, that Honda should pay for. Honda (American Honda Motor Co.) is recalling certain 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2016 and 2018-2019 Pilot, 2017 and 2019 Ridgeline, and 2018-2019 Odyssey vehicles. Due to a manufacturing error, the connecting rod bearing in the engine may wear and seize, damaging the engine.
Consequence & remedy
Consequence: A damaged engine may run improperly or stall while driving, increasing the risk of a fire, crash, or injury.

The affected vehicles have fuel tanks that were manufactured with insufficient welds which may separate and allow fuel to leak out.

Honda (American Honda Motor Co.) is recalling certain model year 2016 Honda Pilot 2WD vehicles manufactured May 4, 2015, to September 8, 2015 and 2016 Pilot 4WD vehicles manufactured May 7, 2015 to September 4, 2015. In the affected vehicles, when one of the safety systems such as tire pressure monitoring, anti-lock braking or electronic stability control malfunctions, there is potential that the instrument panel will not illuminate the corresponding warning light, however the warning lamps will illuminate when the ignition is turned off and then turned back on. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 126, "Electronic stability control systems", number 135, "Light vehicle brake systems" and number 138, "Tire pressure monitoring systems".
Consequence & remedy
Consequence: If a safety system cannot immediately warn the driver when needed, the driver may be at increased risk of a crash.

Honda (American Honda Motor Co.) is recalling certain model year 2016 Honda Pilot vehicles manufactured May 4, 2015, to June 5, 2015. Due to an assembly issue, the third row seatbelt may be trapped between the rear seat and the rear sideliner.
Consequence & remedy
Consequence: If the third row seatbelt is trapped, the occupants may not be restrained properly, increasing the risk of injury.

The Office of Defects Investigation (ODI) has received 414 reports of connecting rod bearing failures in the 3.5L V6 engine used in the following vehicles: Model Years (MY) 2018-2020 Acura TLX, MY2016-2020 Acura MDX, MY2016-2020 Honda Pilot, MY2018-2020 Honda Odyssey, and MY2017-2019 Honda Ridgeline. The subject engine failures were analyzed as part of a Recall Query (RQ24013). It was determined that these engine failures are outside the scope of Honda Recall 23V-751. Further, evidence gathered in conjunction with RQ24013 does not suggest this failure is caused by the same crankshaft manufacturing defect that is addressed by Honda in Recall 23V-751. Based on analysis of data from RQ24013, NHTSA has closed that investigation. The significant number of reports of engine failure in vehicles outside the scope of RQ24013 presents a potential safety risk that warrants further investigation. ODI is opening this Preliminary Evaluation (PE) to further evaluate the scope and severity of the potential problem and to fully assess the potential safety-related issues. On June 3, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE22005 after the agency received VOQs and several field reports concerning the Auto Idle Stop (AIS) feature on 2016-2019MY Honda Pilot vehicles. The complaints allege that the engine fails to restart on its own from a complete stop at a traffic light or road intersection with the AIS function engaged. The subject vehicles are equipped with a 3.5L engine with a 9-speed automatic transmission. On January 6, 2023, Honda released service bulletins 23-008 and 23-009. These service bulletins address the failure to restart issue in the 2016-2019MY Honda Pilot vehicles, as well as the 2015-2020 Acura TLX, 2016-2020 Acura MDX, 2019-2022 Honda Passport, and 2020-2023 Honda Ridgeline vehicles. The repair outlined in the service bulletins is a two-stage countermeasure to remedy the issue. The first stage is a Programmed Fuel Injection (“PGM-FI”) software update. The second stage is only deployed if the software update fails to remedy the issue. The second stage, depending on the model and model year, consists of replacement of the starter assembly, starter relays, and a valve adjustment. Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. On November 8, 2024, the Office of Defects Investigation (ODI) opened Recall Query RQ24013 to assess the scope of Honda recall 23V-751 after receiving complaints that alleged connecting rod bearing failures in vehicles that were not within the scope of recall 23V-751. In November 2023, Honda recalled 248,999 various Model Year (MY)2016–2020 Honda and Acura vehicles equipped with the 3.5L V6 engine. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation. NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008). The Agency reserves the right to take additional action if warranted by new circumstances.
