By August 1, 2026, the check-engine light was on in my 2019 Honda Odyssey. I reported that the light was steady and the van was running normally, with no noticeable drivability problems. On August 31, 2026, at 141,126 miles, an authorized Honda dealer recorded DTC P0430, performed an air/fuel test, documented that the fuel injectors failed, and recommended replacing them. The dealer noted that failed injectors can cause P0430. The injector-specific parts-and-labor estimate is $1,849.06. No injector repair or post-repair test is documented yet. There was no crash, fire, injury, fatality, or police report. Honda Service Bulletin 21-010 provides a 10-year/150,000-mile injector warranty extension for certain 2018-2019 Odysseys. For P0430, it directs an air/fuel test and injector replacement if the test fails. Honda says this vehicle's VIN is ineligible despite the dealer's matching diagnostic pathway and offered 50% goodwill. This bulletin is a warranty extension, not a recall. I ask NHTSA to examine whether the eligible VIN population adequately captures 2019 Odysseys with this documented failure pattern. The attached evidence summary identifies 27 public NHTSA complaints that report injector or matching-code concerns together with VIN-based coverage exclusion; those are owner allegations, not confirmed failure rates. Some other owners allege hesitation, power loss, or limp mode, but I have not experienced those symptoms. I am concerned that continued injector deterioration could create a future drivability or loss-of-power risk. Please compare Honda's production, warranty, goodwill, dealer-test, and failure-analysis data for included and excluded vehicles. The dealer findings and evidence summary are attached.

For about 3-4 years now we have had issues with either the accelerator and/or possibly the transmission. We are not exactly sure which because our dealer claims they can never recreate the issue which is hard to believe since the issue happens to us most times the car is driven. The complaint that we have is intermittent delayed/no acceleration when accelerator is pressed, followed by sudden surge/punch forward after releasing and pressing accelerator again. What makes it particularly frightening is when it happens and the car is exposed to other perpendicular traffic that may not have to stop. No warning lamps, messages or other symptoms of the problem occur or accompany the issue in question. The vehicle has only been looked at by our Honda Dealership.

What component or system failed or malfunctioned, and is it available for inspection upon request? - Fuel injectors - They can be inspected in needed How was your safety or the safety of others put at risk? - This issue can cause issues with the engine and ultimately make the vehicle unsafe to operate. Has the problem been reproduced or confirmed by a dealer or independent service center? Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? This is a known issue with honda vehicles including the odyssey for 2019. There is a Service Bulletin 21-010 that mentioned the issue but only covers some VINS but this is an issue with other VINS that the dealer refuses to cover. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? A Check engine light will come on.

Failed Component: Direct Fuel Injector Set. The components are currently on the vehicle and are available for inspection upon request. Safety Risk: The failing fuel injectors create a severe engine air/fuel imbalance, which causes engine hesitation, engine stumbling during acceleration, and unpredictable power loss. This puts my family at high risk of a collision, particularly when merging onto highways, crossing busy intersections, or accelerating from a stop. Furthermore, dumping unburnt fuel into the exhaust risks overheating the catalytic converter, creating a potential fire hazard. Dealer Confirmation: Yes. The problem was confirmed by AutoNation Honda 365 in Memphis, TN, on [XXX] (Mileage: 83,452). Manufacturer Inspection: The vehicle was inspected by factory-trained technicians at an authorized Honda dealership. A case was opened with Honda Corporate, but they refused to cover the repair, stating my specific VIN batch was excluded from their warranty extension, despite suffering the exact same failure. Warning Lamps & Symptoms: An "Emissions System Problem" warning and the Check Engine Light illuminated on [XXX]. Incident Description & Cause: Honda is fully aware of this dangerous defect. They issued Service Bulletin 21-010, acknowledging that "debris from the high-pressure pump or the fuel injector machining process can cause internal wear or clogging" in 2018-2019 Odysseys. Following the TSB 21-010 protocol, the dealer pulled code P219A and performed a "Cylinder AF Test." My vehicle failed the test (Cylinder 3 running rich). The Honda technician’s official notes state: "Fail air fuel test need injector set internal failure." Honda extended the warranty to 10 years/150,000 miles for this known manufacturing defect, yet they are arbitrarily excluding my 2019 Odyssey, which has the exact identical internal failure documented by their own dealer. XXX, I was driving my 2019 Honda Odyssey EX-L on a family trip to Florida when the dashboard lit up like a Christmas tree and the car went into neutral. We were on [XXX], close to [XXX]. My wife and I had to push the car off the side of the road and call a tow truck company. This occurred on a Friday night. The only place I was able to take the vehicle for repair was the nearest dealership. I told them what happened. The service writer tells me the issue is the fuel pump and high pressure fuel pump. The cost was $2,800. When I told them I was on vacation and this issue should've been resolved back in 2021 with the recall they told me nothing could be done. I had the recall work completed in 2021 with 12,000 miles on the van. The fuel pump & high pressure fuel pump problem occurred in June 2026 with 51,000 miles on the van. Water may enter into the rearview camera, which can cause the rearview camera image to fail to display when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."
Consequence & remedy
Consequence: A rearview image that does not display reduces the driver's view behind the vehicle, increasing the risk of a crash.
Remedy: Dealers will replace the rearview camera, free of charge. Owner notification letters were mailed August 24, 2026. Owners may contact Honda's customer service at 1-888-234-2138. Consequence: A rearview camera that does not display an image can reduce the driver's rear view, increasing the risk of a crash or injury.

Braun Corporation (Braun) is recalling certain 2018-2021 Braun-modified Honda Odyssey vehicles. The fuel tank assembly installed by Braun may be susceptible to the fuel tank overfilling into the charcoal canister. If the charcoal canister fills with a sufficient amount of fuel, fuel may leak out.
Consequence & remedy
Consequence: A fuel leak in the presence of an ignition source may increase the risk of fire.
Remedy: Braun will notify owners, and dealers will inspect vehicles. If evidence of a fuel leak is present, a new fuel tank assembly and a new Honda charcoal canister will be installed, free of charge. The recall began February 22, 2021. Owners may contact Braun customer service at 1-800-488-0359.

If this occurs, the outer door handle cables may freeze in cold temperatures, preventing the front and rear sliding door latches from securely latching.

Consequence: A power sliding door that fails to latch may open while moving, increasing the risk of injury.

Water may get into the rearview camera mounting holes and enter the camera lens and cause a distorted image, or no image at all. Additionally, if the rearview camera view mode is set to top-down view, or switched from that view to another mode in a bright environment, the image may not display. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rearview Mirrors."

Consequence: An intermittent electrical short could overheat the wire harness, increasing the risk of a fire.
Remedy: Honda will notify owners, and dealers will inspect the third row seat accessory power outlet wire harness for damage, replacing it as necessary. Braun Corporation (Braun) is recalling certain 2018-2019 Honda Odyessy vehicles modified to be equipped with an in-floor wheelchair ramp conversion. The rear wheel speed sensor harnesses may have been routed to the incorrect rear wheels, resulting in the possibility that the anti-lock brake system will not recognize each rear wheel speed properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 135, "Light Vehicle Brake System" and 404, "Electronic Stability Control Systems."
Consequence & remedy
Consequence: If the rear wheel speeds cannot be correctly monitored, the anti-lock brake system may not engage properly to prevent the wheels from locking up, increasing the risk of crash.

Additionally, Honda extended the warranty coverage to 10 years with unlimited mileage for vehicles requiring the second stage component replacement. ODI continues to receive complaints of the AIS failure to restart, with many complainants alleging the campaign countermeasures had been completed.NHTSA has reviewed Honda's Information Request Letter responses involving warranty claims, lawsuits, and consumer complaints in addition to ODI's own data. This Engineering Analysis is being opened to further consider the potential safety defect, including gathering additional data regarding Honda’s service campaign efficacy. ODI is additionally expanding the scope to encompass all vehicles addressed in Honda’s service bulletins 23-008 and 23-009 as well as newer model year vehicles, which are substantially similar to the vehicles covered by the service campaign. On November 8, 2024, the Office of Defects Investigation (ODI) opened Recall Query RQ24013 to assess the scope of Honda recall 23V-751 after receiving complaints that alleged connecting rod bearing failures in vehicles that were not within the scope of recall 23V-751. In November 2023, Honda recalled 248,999 various Model Year (MY)2016–2020 Honda and Acura vehicles equipped with the 3.5L V6 engine. Honda recalled these vehicles due to a crankshaft manufacturing defect that caused premature connecting rod bearing wear and engine seizure. While the out-of-scope failures involved in these complaints appear to have a very similar failure mechanism as the recalled engines, a thorough analysis of all available data from Honda has not indicated that the same manufacturing defect exists in the out-of-scope engines as the engines recalled by 23V-751. Based on this analysis ODI has determined that Honda has properly defined the scope of recall 23V-751. Consequently, ODI will close this RQ investigation. NHTSA will continue to investigate these complaints of engine failure as part of a new Preliminary Evaluation Investigation (PE25008). The Agency reserves the right to take additional action if warranted by new circumstances.
