Yesterday (XXX) while starting my car the warning lights came on notifying me of a potential issue with multiple safety systems including the brake system.  My parking brake would not show that it was released and when put in gear the brakes were very mushy and the vehicle did not feel that it could be driven safely.  I requested Triple AAA to tow the car to the Honda dealership in Lumberton North Carolina this morning (XXX) as I did not feel that it was safe to drive.  A diagnostic test was performed that indicated that the Electric Servo Brake System / Electric Brake Booster System Malfunction was found and that the vehicle needs an Electric Servo Pedal Simulator Unit replaced. This component functions cooperatively with the Regenerative Brakes together with a structure in which pedal force is not influenced by braking pressure control. I have researched this issue and found that this particular problem and the associated part has been recognized as an ongoing problem with some Honda Hybrid models and that it has recently been recalled in Japan, and many Honda owners in the UK have also been experiencing the same malfunction to this part.  This appears to be a systemic issue with a part involving select Honda Hybrid models that appears to occur as early as after 20,000 miles and oftentimes shortly after the vehicle involved is out of warranty.  American Honda has not officially recognized this problem as a safety issue. Some time in 2021 is the estimated date of occurrence of the event I am describing next. I was cruising on a freeway [XXX] ), traveling at about 65 miles an hour. It was a clear day with light traffic, around noon. There was no car in front of mine. There were no cars adjacent to mine either. Then I saw with my eyes a partially deflated mylar balloon (silvery and shiny) floating across (left to right) my path of travel. I estimate that the vertical position of the balloon was less than the height of a typical bus or a semi-truck. Its float path was rather random - up and down. Its distance from my car was about 5 to 10 car length (very rough estimate) when I first noticed it. Since I easily recognized it as a light-weight balloon (typical party balloon), I determined that it would not do any harm to my car even if timing was such that my car came in contact with it. So, I did not take any evasive action - I did not apply brakes and I did not change to another lane. But this is when my CR-V's automatic braking system engaged with a sudden deceleration, "BRAKE" warning light on the dash along with a series of beeping. The whole event lasted a few seconds after which the car responded normally to my gas-pedal input. Luckily there was no car following me when this automatic braking occurred. This was a one-time occurrence to date. Since then I have not had similar unexpected automatic braking due stray objects. I did have had several occurrences of "BRAKE" warning light and beeping due to my car coming in close proximity to other cars in front of me. In these instances, I cannot say with certainty whether or not brakes themselves were automatically applied. In these instances, I applied brakes myself (albeit later than what the car's computer determined to be appropriate). The contact stated that while driving approximately 60 MPH, the Forward Collision Avoidance: Automatic Emergency Braking system erroneously activated without a person or other vehicle nearby. The contact was able to immediately release the brake pedal and the vehicle accelerated normally. There were no warning lights illuminated. The vehicle was taken to the dealer who was unable to identify the cause of the failure. The vehicle was not repaired. The failure recurred while driving approximately 40 MPH. The manufacturer was not made aware of the failure. The approximate failure mileage was 15,000.

Twice, I've been driving down the road, and the emergency braking system has engaged with no car near me. I get thrown against the seatbelt hard enough that it triggers the seatbelt to lock. I'm sure I would have been rear-ended if someone had been behind me. My foot was still on the gas both times it happened. I'm now scared to drive my car because I don't know when it will suddenly break hard and, maybe, hard enough to trigger airbags.

For the 2nd time in a span of ~6 months, the 2020 Honda CR-V randomly slowed down/tried to come to an abrupt halt in the middle of the freeway. Terrifying experience both times as it could have easily resulted in being in a fatal collision. I took this to the dealership immediately after the first time it happened but they deemed it as a "phantom issue". This happened again TODAY and I don't know what to do. Battery meter seemed to be on the lower end but was not empty. Things seemed to be normal.

The front passenger seat weight sensor may crack and short circuit, which can cause the air bags to deploy unintentionally during a crash.

Consequence & remedy

Consequence: Air bags that deploy unintentionally during a crash increase the risk of injury.

Honda (American Honda Motor Co.) is recalling certain 2020-2022 Pilot, Accord, Civic sedan, HR-V, Odyssey, 2020 Civic coupe, Fit, 2021-2022 Civic hatchback, 2021 Civic Type R, Insight, 2020-2021 CR-V, CR-V Hybrid, Passport, Ridgeline, Accord Hybrid, 2020 Acura MDX, 2022 Acura MDX, 2020-2022 Acura RDX, and 2020-2021 Acura TLX vehicles. The front passenger seat weight sensor may crack and short circuit, failing to suppress the air bag as intended.

Consequence & remedy

Consequence: An air bag that deploys unintentionally during a crash can increase the risk of injury.

The fuel pump inside the fuel tank may fail.

Consequence & remedy

Consequence: Fuel pump failure can cause an engine stall while driving, increasing the risk of a crash.

Remedy: Dealers will replace the fuel pump module, free of charge. Owner letters were mailed September 6, 2024. Owners may contact Honda customer service at 1-888-234-2138. Honda's numbers for this recall are KGC and KGD. This recall is an expansion of NHTSA recall numbers 21V-215 and 20V-314.

Honda (American Honda Motor Co.) is recalling certain 2020-2022 CR-V Hybrid vehicles. The 12-Volt battery cable routed outside of the body frame is missing a fuse on the power circuit, which can allow the battery cable to short circuit or overheat during a crash.

Consequence & remedy

Consequence: An overheated battery cable or short circuit can increase the risk of a fire or injury in a crash.

Remedy: Dealers will replace the battery cable, free of charge. Owner notification letters were mailed January 29, 2024. Owners may contact Honda customer service at 1-888-234-2138. Honda's number for this recall is FGB.

The Office of Defects Investigation (ODI) has received 48 Vehicle Owner Questionnaire (VOQ) reports and some Early Warning Reporting (EWR) field reports alleging intermittent loss of motive power in Model Year (MY) 2020–2022 Honda CR-V Hybrid vehicles.  These incidents are alleged to occur without warning at highway speeds and are accompanied by a flashing check engine light immediately before the loss of motive power occurs.  Motive power can be restored by turning the ignition off and back on.  However, this action also clears the check engine light and any fault codes that were set.  The fault’s intermittent nature, combined with the lack of fault codes, often results in many visits to repair facilities yielding no repairs. Honda indicated it was unaware of any related complaint or repair trends. ODI is opening this Preliminary Evaluation (PE) to determine the scope and severity of the alleged fault and to fully assess the possible safety consequences. Forward Collision Avoidance: Sensing System: Radar

On February 21, 2022, the Office of Defects Investigation (ODI) opened Preliminary Evaluation (PE22003) to assess reports of inadvertent activation of the Collision Mitigation Braking System (CMBS), an automatic emergency braking (AEB) system, in model year 2017-2019 Honda CR-V and 2018-2019 Honda Accord vehicles.  The reports allege that activation of the AEB system occurs while driving with no apparent obstruction in the vehicle's forward path, resulting in sudden vehicle deceleration. Honda indicated that they were aware of a total of X,XXX reports that may relate to the alleged defect. Honda provided analysis of the alleged defect and alleges that some customers possibly had an inadequate understanding of the CMBS and its limitations.  However, many consumer complaints allege that Honda dealerships were unable to reproduce the condition or state that they were informed that this is considered normal CMBS operation. To date, ODI has received a total of 1,294 consumer complaints of inadvertent activation of CMBS in 2017-2022 Honda CR-V and 2018-2022 Honda Accord vehicles. A total of 31 complaints alleged a crash and 50 alleged an injury.  The Total column in the Failure Report Summary removes duplicate reports and shows the total number of reports with unique VINs from all Manufacturer, ODI, and EWR data sources.  In some cases, there were multiple reports associated with a particular vehicle in which recurring failures were alleged.  In total, there were X,XXX reports, 93 injury incidents and 47 crashes involving vehicles with unique VINs that may relate to the alleged defect. NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.
