In 2023, the vehicle began exhibiting erratic transmission shifting behavior, including harsh and unpredictable shifts while driving. The vehicle was taken to a GMC dealer for diagnosis and repair. Recently, the vehicle developed additional drivetrain and engine control issues related to the crankshaft position sensor circuit. The vehicle stalled while driving on a highway mountain overpass with multiple vehicles traveling behind me. When the stall occurred, the vehicle shifted into park and would not restart for several minutes. This created a hazardous situation due to loss of propulsion in active traffic at highway speed and limited shoulder space. Following diagnosis, the servicing dealer determined that the engine wiring harness requires full replacement due to internal wiring integrity issues affecting critical engine control signals. The crankshaft position sensor and related wiring are essential for engine timing and propulsion. Loss or corruption of this signal can result in reduced engine power, erratic transmission behavior, stalling, or no-start conditions. The earlier erratic shifting behavior appears consistent with conditions described in GM Technical Service Bulletin 21-NA-149, which identifies engine harness-related causes for drivetrain and shifting concerns. This issue appears to involve degradation or defect within the engine wiring harness affecting engine management and transmission operation. Given that this condition resulted in a highway stall and temporary inability to restart, I am reporting this for investigation as a potential safety-related defect involving loss of propulsion.

The contact owns a 2022 GMC Yukon. The contact stated that the vehicle was consuming an excessive amount of oil. The vehicle was taken to the dealer, where it was diagnosed, and metal shavings were found in the engine oil. The dealer replaced the camshaft and lifters; however, the failure worsened. The contact stated while driving approximately 65-70 MPH, the vehicle stalled and lost power steering functionality. The contact's husband was able to pull over to the side of the road. There was no warning light illuminated. The vehicle failed to restart and was towed to the residence. The vehicle was then picked up and diagnosed by the dealer. The dealer determined that the engine had seized and needed to be replaced. The contact had opened a case with the manufacturer for buyback assistance due to safety concerns. The vehicle was not repaired. The failure mileage was approximately 46,000.

The engine threw a rod on January 23, 2026. The vehicle stranded me on the side of the road. We called the dealership which they came and towed back to the dealership. No dashboard warnings at all. Oil pressure was good. I heard a low tick to begin with then a slight squeaking noise. After the engine locked it was an abrupt halt with the wheels locking up but didn't come to a complete stop. The noise was very loud after that and that's when I pulled off the highway. The dealership checked it out and did find that the motor needed to be replaced. We just had the SUV in the dealership on October 2025 and they found no need for a new motor, just a change in the oil viscosity.

The low oil warning light was illuminated approximately every 1,500 miles after an oil change. After each oil change, the level on the oil dipstick was low. The vehicle was taken to an independent mechanic, where an oil consumption test was performed. The mechanic informed the contact that the vehicle was consuming approximately 25 percent of the engine oil. The vehicle was not repaired, and the contact was referred to a dealer for assistance. The vehicle was not diagnosed or repaired. The contact was informed that an additional oil consumption test was needed. The manufacturer was made aware of the failure and referred the contact to the NHTSA Hotline for assistance. The approximate failure mileage was 87,000.

On 15-Jan-26 (approximately 5 weeks after the vehicle was serviced for the NHTSA #25V274) while doing highway speeds the vehicle suddenly started huffing and loosing propulsion. After the dealership did their initial inspection found that lifters in at least one bank were damaged. The suddenness, the randomness and severity of this unprovoked failure (I wasn’t towing anything or going uphill) combined by the fact that the dealership previously gave this vehicle a clean bill of health makes me have no confidence in the road worthiness of this vehicle.

2022 GMC Yukon AT4 was brought into the dealership for the service recall on the engine. No issues and received the 0W40 "fix". Approximately 4-6 weeks later, I was traveling on the interstate at approx. 75MPH when the truck completely shut down mid-passing with a code stating "conditions for not for shifting" and I was lucky enough to get the truck to the shoulder of the road without being struck by another vehicle as the truck "shifted" to neutral and would not do anything. Once the truck was in park, it shut off and I was unable to get it started again. Received multiple codes (key fob battery needs replacing, battery low, etc.) while stuck on the side of the interstate for several hours until I was able to get it towed (at a charge of $500) to my dealership. It then sat in the parking lot for 3 days for diagnosis pending TAC deciding whether to tear the engine down or replace it. It was determined to meet the recall qualifications and the dealership has ordered a replacement engine and was told they would start at a estimated 4 week repair time but would probably take closer to 6-8 weeks. Dealership has provided a rental at no cost to me but did not refund my towing costs associated with the repair.

I purchased a Yukon Denali 6.2L in March of 2022 brand new from Bob Johnsons in Rochester NY. On [XXX] at 12,577 miles the engine went through a catastrophic failure, while driving, the vehicle began to make very loud noises. The vehicle was taken to Bob Johnsons GMC 4389 West Ridge Road Rochester, NY 14626. The repair notes #3 main bearing thrust portion had come apart. The vehicle was out of service from 4/3/2023 to 4/19/2023 for engine replacement. On [XXX] at 50,244 miles on the vehicle, I was driving down a busy highway during holiday traffic and the vehicle went into neutral and would not start, we immediately turned the flashing lights on, unrolled the windows and began to show distress to ensure we were not hit by passing cars. We made it to safety and called Onstar for assistance, on this occurrence due to the vehicle not starting we were unable to get the vehicle back in to the neutral position to be able to roll the vehicle. The vehicle was towed to Bob Johnsons GMC due to the Thanksgiving holiday, the vehicle was brought in 11/28/2025, and deemed to need another engine replacement. On November 28th I called to file a claim with General Motors and was called back by Stephanie on November 29th and was told they would get back to me if they had any options as now this is the 3rd engine replacement. On December 3rd, a new woman called also named Stephanie and advised me they were still undergoing options, as I requested a Buy Back with Trade Assistance. On December 9th, my final call from General Motors advisor Stephanie, I was told I do not qualify for a buy back because I placed too many miles on the vehicle between engine failures. The vehicle was out of service from 11/26/26-12/29/26 for engine replacement. On [XXX] 2 weeks and 4 days after the vehicle going back into service, the engine failed again, only driving approximately 245 miles. The original fuel tank may have been replaced with an incorrect fuel tank, which can allow fuel to leak in a rollover crash.

Consequence & remedy

Consequence: A fuel leak in the presence of an ignition source increases the risk of a vehicle fire.

General Motors LLC (GM) is recalling certain 2021-2024 Cadillac Escalade and Escalade ESV, Chevrolet Silverado 1500, Suburban, and Tahoe, GMC Sierra 1500, Yukon, and Yukon XL vehicles equipped with a 6.2L V8 gas engine. The connecting rod and/or crankshaft engine components may have manufacturing defects that can lead to engine damage and engine failure.

Consequence & remedy

Consequence: Engine failure increases the risk of a crash.

Remedy: Dealers will inspect the engine, and as necessary, repair or replace the engine. For vehicles that pass inspection, dealers will add higher viscosity oil, install 6 oil fill cap, replace the oil filter, and update the owner's manual. Repairs will be performed free of charge. General Motors, LLC (GM) is recalling certain 2020-2023 Cadillac CT4 and CT5; 2021-2023 Buick Envision; and 2022-2023 Cadillac Escalade and Escalade ESV, Chevrolet Silverado 1500, Suburban, Tahoe, GMC Sierra 1500, Yukon, and Yukon XL vehicles. The daytime running lights (DRLs) may not deactivate when the headlights are on. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 108, "Lamps, Reflective Devices, and Associated Equipment."

Consequence & remedy

Consequence: DRLs that do not deactivate as intended may result in a glare, reducing visibility, and increasing the risk of a crash.

Remedy: The body control module software will be updated by a dealer, or through an over-the-air (OTA) update, free of charge. Owner notification letters were mailed January 23, 2023. General Motors, LLC (M) is recalling certain 2021-2022 Cadillac Escalade, Escalade ESV, Chevrolet Suburban, Tahoe, GMC Yukon, and Yukon XL vehicles equipped with third-row seats. The rivet that retains the buckle to the mounting bracket in the left or right side third-row seat belt buckle assembly may have been improperly formed.

Consequence & remedy

Consequence: A seat belt with a deformed rivet may not properly restrain an occupant, increasing the risk of injury during a crash.

Remedy: Dealers will inspect the rivet head formation on both the left and right side third-row seat belt buckle assemblies and replace seat belt buckle assemblies, as necessary, free of charge. Interim owner notification letters informing owners of the safety risk were mailed September 28, 2022. Owner notification letters were mailed on November 1, 2022. On January 16, 2026, the Office of Defects Investigation (ODI) opened Recall Query RQ26001.  This investigation was opened to assess the remedy effectiveness of Recall 25V-274. ODI has received complaints alleging engine failure in vehicles after the recall remedy had been completed, whether that remedy involved an oil viscosity change or a complete engine replacement. General Motors (GM) issued Recall 25V-274 to address concerns of engine failure in the L87 engines produced between March 1, 2021 and May 31, 2024. GM cited the root cause as a supplier manufacturing and quality issue. GM’s recall remedy for Recall 25V-274 consisted of two different repairs, either an oil viscosity change or an engine replacement, based on the results of the remedy inspection procedure. ODI continues to receive reports of engine failures after the completion of the recall remedy. To date, ODI has received 499 complaints alleging post-remedy engine failure in vehicles subject to Recall 25V-274, 473 of which involved an oil viscosity change and 26 a complete engine replacement. Additionally, ODI has received 191 reports of L87 engine failures in engines produced after the production period cited in Recall 25V-274. GM reported receiving 6,953 complaints of post-remedy engine failure. ODI is opening this Engineering Analysisto continue investigating complaints of post-remedy engine failure. The Office of Defects Investigation (ODI) has received 1,157 reports of engine bearing failure failures in the L87 6.2L engine produced by General Motors (GM). Of the 1,157 reports received, 173 are outside of the scope of recall 25V-274. These engines are installed in Model Years (MY) 2019–2024 Chevrolet Silverado 1500 and GMC Sierra 1500, and 2021-2024MY Chevrolet Suburban and Tahoe, GMC Yukon/Yukon XL, and Cadillac Escalade/Escalade ESV vehicles. The subject engine failures were investigated as part of a Preliminary Evaluation (PE25001).  GM determined that these engine failures were due to multiple supplier manufacturing and quality issues. GM issued recall 25V-274 to address the supplier manufacturing and quality issues produced between March 1, 2021, and May 31, 2024. Based on the issuance of recall 25V-274, NHTSA has closed PE25001. ODI continues to receive a significant number of reports of engine failure in vehicles outside the scope of 25V-274.  This presents a potential safety risk that warrants further investigation.  ODI is opening this Engineering Analysis (EA) to further evaluate the scope and severity of the potential problem and to fully assess the potential safety-related issues of vehicles built outside the recall scope. On January 16, 2025, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE25001 after receiving complaints and field reports concerning engine failures in MY2019-2024 GM’s full-size truck and SUV T1XX platform equipped with the L87 6.2L V8 engine. These complainants alleged engine bearing failures resulting in knocking and growling noises from the engine’s bottom end and in some cases more severe consequences such as broken connecting rods leading to engine damage, or engine failure. If the engine fails during vehicle operation, the vehicle will lose propulsion, increasing the risk of a crash. On April 24, 2025, GM issued recall 25V-274 to address the concern with L87 engines produced between March 1, 2021, and May 31, 2024.  GM cited the root cause as a supplier manufacturing and quality issues. GM’s  recall remedy will be either an oil change to an oil with a different viscosity or engine replacement depending on the results of the remedy inspection procedure. To date, ODI has received 1,157 allegations of engine bearing failure. GM submitted 55,464 reports in total, with 29,752 unique VINs. ODI continues to receive allegations of L87 engine failures which fall outside of the scope of recall 25V-274. Based on GM’s recall, ODI will close this PE. NHTSA will continue to investigate complaints of engine failure outside the scope of recall 25V-274 as part ofEngineering Analysis (EA25007).
