REACTION CARRIER REPLACED AND TRANSMISSION RECONDITIONED AT 36,663 MI AFTER CAR STOPPED IN THE MIDDLE OF THE STREET. CAR HAD TO BE TOWED IN. AT 62K MILES ENGINE LIGHT ON AND IDLES ROUGH. REPLACED THROTTLE BODY. AT 67K WON'T ENGAGE IN REVERSE AND MAKES CLUNKING SOUND FROM REVERSE TO DRIVE. REPLACED REVERSE HOUSING, REACTION SHELL, ETC. AT 75K HAD A REACTION CARRIER FAILURE, LOST POWER BRAKES AND LOST POWER TO THE CAR. ALMOST WAS REAR ENDED. HAD TO STOP IN MIDDLE OF PARKING LOT DURING CHRISTMAS RUSH. WAITED 5 MIN AND RESTARTED. ENGINE LIGHT STAYED ON, BUT WAS ABLE TO GET TO DEALERSHIP. WAS TOLD THAT THROTTLE BODY NEEDS TO BE CLEANED EVERY 15K MILES UNDER SERVICE MAINTENANCE. THIS WAS THE FIRST TIME I HEARD THIS! NOT REALLY SURE WHY IT HAD TO BE CLEANED, THOUGH, JUST HAD THE THROTTLE BODY REPLACED 13K MILES AGO. IT SEEMS THAT THERE ARE MAJOR FLAWS IN THE DESIGN OF THE ENGINE. OBVIOUSLY, THE DEALERSHIP HAS BEEN PUTTING BAND AIDS ON MY PROBLEM BECAUSE THEY DON'T KNOW HOW TO FIX. Additionally, GM is recalling certain 2007-2008 Cadillac Escalade, Escalade ESV, Escalade EXT, Chevrolet Avalanche, Silverado 1500, Suburban, Tahoe, GMC Sierra 1500, Yukon, Yukon XL vehicles originally sold, or ever registered, in the states of Arizona, Arkansas, Delaware, District of Columbia, Illinois, Indiana, Kansas, Kentucky, Maryland, Missouri, Nebraska, Nevada, New Jersey, New Mexico, North Carolina, Ohio, Oklahoma, Pennsylvania, Tennessee, Virginia, and West Virginia or "Zone B." Consequence & remedy
Consequence: An inflator explosion may result in sharp metal fragments striking the driver or other occupants, resulting in serious injury or death.
Remedy: GM will notify owners, and dealers will replace the front passenger air bag inflator with an alternate one, free of charge. Interim letters notifying owners of the safety risk were mailed on February 22, 2021. Second notification letters will be mailed once the remedy is available. Owner notification letters were mailed on March 1, 2021. Owners may contact Chevrolet customer service at 1-800-222-1020; Cadillac customer service at 1-800-458-8006; and GMC customer service at 1-800-462-8782. GM's number for this recall is N212328760.

A RECALL WAS IMPLEMENTED IN 2008 TO ADD A FUSE TO THE CONTROL CIRCUIT HARNESS TO ADDRESS THE POTENTIAL CONSEQUENCES OF A PRINTED CIRCUIT BOARD (PCB) ELECTRICAL SHORT. HOWEVER, THERE HAVE BEEN NEW REPORTS OF THERMAL INCIDENTS ON HWFS MODULES AFTER THIS IMPROVEMENT WAS INSTALLED. THESE INCIDENTS RESULTED FROM A NEW FAILURE MODE ATTRIBUTED TO THE DEVICE'S THERMAL PROTECTION FEATURE. THE SIGNIFICANCE VARIES FROM MINOR DISTORTION TO CONSIDERABLE MELTING OF THE PLASTIC AROUND THE HWFS FLUID CHAMBER.
Consequence & remedy
Consequence: IT IS POSSIBLE FOR THE HEATED WASHER MODULE TO IGNITE AND A FIRE MAY OCCUR.
Remedy: DEALERS WILL PERMANENTLY DISABLE AND REMOVE THE HEATED WASHER FLUID MODULE. AN UPDATED PAGE FOR THE OWNER MANUAL WILL BE PROVIDED AND INSERTED IN THE OWNER MANUAL TO DOCUMENT THAT THE FEATURE HAS BEEN PERMANENTLY DISABLED AND REMOVED FROM THE VEHICLE. IN A LETTER DATED AUGUST 27, 2008, GENERAL MOTORS CORP. (GM) NOTIFIED NHTSA THAT IT WILL CONDUCT A SAFETY RECALL (NHTSA RECALL NUMBER 08V-441) TO ADDRESS A SAFETY RELATEDDEFECT THAT CAN RESULT IN ENGINE COMPARTMENT FIRE IN APPROXIMATELY 850,000 VEHICLES EQUIPPED WITH A HEATED WINDSHIELD WASHER FLUID OPTION.THE SUBJECT VEHICLES OF THIS INVESTIGATION COMPRISE OVER THREE-QUARTERS OF THE RECALL POPULATION. ACCORDING TO GM, A SHORT CIRCUIT ON THE PRINTED CIRCUIT BOARD FOR THE WINDSHIELD WASHER FLUID HEATER MODULE MAY OVERHEAT THE CONTROL-CIRCUIT GROUND WIRE.
