The contact safely pulled over to the side of the road. The vehicle was towed to the dealer, where it was diagnosed and determined that the steering gear motor had failed and needed replacement. I am reporting a serious and ongoing safety issue involving my 2015 Ford Fusion. The vehicle experiences intermittent power steering failure while driving. The steering assist will suddenly disengage and re-engage without warning, making the vehicle extremely difficult to control. This condition becomes even more dangerous in wet weather, where the vehicle is sometimes nearly undriveable. Additionally, I am now experiencing a popping noise in the steering wheel. This raises concern of a possible mechanical failure consistent with known recalls involving steering wheel bolt defects that could result in the steering wheel detaching. These issues are consistent with known recalls, including: - Power Steering Failure Recall (15V340000) - Steering Wheel Bolt Recall (18V167000 / 18S08) Despite these known safety defects, Ford has refused to repair the issue, stating that the recall has expired. This presents a serious safety hazard, including risk of loss of steering control, crash, injury, or death. I am requesting immediate investigation and enforcement of manufacturer responsibility for this safety-related defect.

My steering gear motor attachment bolts have fractured due to severe corrosion, which is the exact defect described in Safety Recall 19S26. This physical failure was confirmed by a Ford dealership technician on 1/11/2024. The technician documented the fractured bolts and recommended a full steering gear replacement. Despite this, Ford is denying the recall repair because this specific VIN was not included on the original mailing list. The vehicle is now undrivable due to a total loss of power steering assist. Before it failed completely, the car was making whining and grinding noises during low-speed turns. This failure occurred in Illinois, which is a high-corrosion 'Salt Belt' state specifically covered by the 19S26 bulletin. By denying this safety repair, Ford is forcing an unsafe vehicle to remain on the road or operate with manual steering, which is a major crash risk. The failure was physically confirmed and documented by a Ford dealer. I want to note that while the previous recall 18S08 for the steering wheel bolt was performed, that was an unrelated interior part and did not prevent this exterior motor bolt failure. I am requesting that the NHTSA investigate why a vehicle with a confirmed 19S26 failure in a covered state is being denied the mandated safety remedy.

In addition, the contact heard an abnormally loud bang. The power steering failure warning light was illuminated. The vehicle was driven to the residence. The contact diagnosed the vehicle with rack and pinion failure and detached bolts. The contact stated that the bolts were corroded, and the rack and pinion gear had detached. The contact stated that the rack and pinion needed to be replaced. The vehicle was not repaired. The contact referenced NHTSA Campaign Number: 19V632000 (Steering); however, the VIN was not associated with the recall. The manufacturer was made aware of the failure, and a case was filed. I have lost power steering in the vehicle. The dash is popping up Service Advance Trac and Hill assist unavailable. I was looking into sensors and rack and pinion when I seen a recall, 19S26, which was listed for 2013-2016 ford fusions and Lincolns, but the recall isn’t specific to my VIN. As of now, the dealership of course won’t fix what is an expensive repair, although it does appear that my car should be included. The information cited salt corrosion in winter as a contributor. It is now late December in Northeast Ohio.

Consequence & remedy
Consequence: A transmission fluid leak in the presence of an ignition source such as hot engine or exhaust components can increase the risk of a fire.
Remedy: Dealers will update the software, free of charge. Additionally, dealers will inspect Focus vehicles and replace the clutch assembly as necessary, free of charge. Interim letters, notifying owners of the safety risk, were mailed August 5, 2026. Additional letters will be sent once the remedy is available. Exposure to certain environmental conditions, such as high temperatures, high humidity, and salt air can cause the brake pedal bumper to corrode and separate from the brake pedal.

Consequence: A separated brake pedal bumper can cause the brake lights to illuminate continuously, causing confusion to other drivers and increasing the risk of a crash. On automatic transmission vehicles, a missing brake pedal bumper can allow the driver to shift the vehicle out of the "PARK" position without depressing the brake, allowing the vehicle to rollaway, increasing the risk of injury.

Ford Motor Company (Ford) is recalling certain model year 2014-2015 Ford Fusion vehicles manufactured July 27, 2013, to October 31, 2014, 2015 Ford Fusion Hybrid vehicles manufactured February 24, 2014, to October 31, 2014, and 2015 Ford Fusion Energi vehicles manufactured July 14, 2014, to October 31, 2014. In the affected vehicles, the key may be able to be removed even while the transmission is not in Park. Thus, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) No. 114, "Theft Protection and Rollaway Prevention."
Consequence & remedy
Consequence: If the key can be removed despite that the transmission is not in Park, a driver may think the vehicle is safety parked, when it is not, increasing the risk of a vehicle rollaway that could cause the vehicle to strike and injure the driver, other vehicle occupants, or bystanders.

On August 8, 2022, the Office of Defects Investigation (ODI) opened this investigation based on 50 Vehicle Owner Questionnaires (VOQs) alleging front brake jounce hose ruptures on model year (MY) 2013-2018 Ford Fusion and Lincoln MKZ vehicles. A ruptured front brake hose results in a brake fluid leak. As a result, the driver may experience an increase in pedal travel and a reduction in the rate of deceleration, increasing the risk of a crash. It should be noted that the subject vehicles identified fell outside the scope of NHTSA Safety Recall 20V-469, which addressed front brake hoses rupturing on certain MY 2015-2018 Ford Edge and 2016-2018 Lincoln MKX vehicles. The result causes a brake fluid leak, lengthening the distance needing to stop the vehicle, increasing the risk of a crash.On March 10, 2023, Ford Motor Company (Ford) submitted to NHTSA Safety Recall 23V-162 (Ford Recall 23S12) to address front brake jounce hoses that may rupture. Ford found the rupture occurs from a localized failure of the internal reinforcement braid due to cyclic fatigue through tensile/bending and torsional inputs during suspension and steering articulations. The supplier for the front brake jounce hoses is Hitachi. This recall affects the subject vehicles equipped with non-hybrid engines and the suspect front brake jounce hoses (a total of 1,280,726 vehicles). The recall covers subject vehicles built from February 3, 2012 to July 19, 2017 when the suspect hoses were used in vehicle production. The remedy is for dealerships to replace both front brake jounce hoses with new hoses that have a revised braid material at no cost. The vehicles equipped with hybrid engines are excluded from the recall because the internal hose pressure is much lower due to the regenerative braking provided by the hybrid brake system. No failures of brake hoses on hybrid vehicles have been reported to ODI or Ford.Ford's actions address ODIs concerns at this time. Hybrid vehicles will be monitored for related failures. On March 12th, 2018, Ford Motor Company submitted a Defect Information Report to NHTSA describing a defect in the steering wheel fastening system of approximately 1,301,986 model year (MY) Ford Fusion and Lincoln MKZ. On March 1, 2018, Ford discovered a design change that affected the MY 2014-2018 subject vehicles. The affected population are MY 2014-2016 Fusion and MKZ with manual and power adjust steering columns and MY 2017-2018 Fusion and MKZ equipped with manual adjust steering columns only. The affected populations have steering shafts with a reduced amount of threads for the steering wheel bolt to engage. The thread length was decreased by 5mm in the affected models. Ford stated that another contributing factor was the placement of a nylon locking patch torque retention on the fastening bolt.As a result, the steering wheel bolt may not maintain torque allowing the bolt to loosen and eventually separate from the steering shaft, resulting in a loose or detached steering wheel. The two crashes and one injury noted in Ford?s Defect Information Report are from two legal claims. The first accident is a claim where the driver reported the steering wheel twisted freely on the shaft which caused the driver to lose control.An injury was alleged but no details were provided.
