The "Hill Assist" option was not operational while driving on a slope and the brake lights remained illuminated, even when the brake pedal was not depressed. An independent mechanic was contacted and associated the failure with the brake pedal failure. The contact associated the failure with NHTSA Campaign Number: 22V011000 (Services Brakes, Hydraulic). The dealer was contacted; however, the vehicle was not diagnosed or repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 200,000.

The vehicle was taken to an independent mechanic to be diagnosed. The contact was informed that the brake fluid reservoir was not properly filled. The mechanic topped off the brake fluid reservoir. The vehicle was repaired. The contact stated that the loss of brake fluid had become a recurring failure. The vehicle had been taken to a local dealer in July 2023 to be repaired under NHTSA Campaign Number: 23V162000 (Service Brakes, Hydraulic) and the failure started after the recall repair was performed. There was no warning light illuminated. The contact swerved to avoid crashing into the animal, and the vehicle crashed into a telephone pole, then rolled over two times. The air bags deployed. The contact was able to exit the vehicle. The contact was examined by emergency units and was advised he had not sustained major injuries. The contact sustained whiplash, a laceration and bruise on the right knee, and a swollen neck. The contact was transported to the residence. A police report was filed. The vehicle was towed to a tow yard, where it was totaled. The contact later received a recall notification of NHTSA Campaign Number: 23V162000 (Service Brakes, Hydraulic) that he related to the failure. The dealer was notified of the failure. The contact desperately depressed the brake pedal, but the vehicle would not stop. The contact down shifted the vehicle and pulled the activated the parking brake. The vehicle came to a stop. The contact shut off the vehicle and restarted the vehicle and drove to the residence. The contact received notification of the NHTSA Campaign Number: 23V162000 (Service Brakes, Hydraulic) however, the parts to do the recall repair were not yet available. The contact stated that he noticed fluid on the ground of his driveway. The contact then stated that upon inspecting under the hood of the vehicle, he noticed a drip located on the brake line. Additionally, the contact stated that on another occasion while driving at an undisclosed speed, the brake warning light illuminated. The contact stated that after coming to a stop, he depressed the brake pedal however, the brake pedal extended down to the floorboard. The contact had to apply pressure to the brake pedal to stop the vehicle. The contact notified the dealer of the failure, and the contact was provided an estimate for the diagnostic test. The contact was informed that parts were not yet available for the recall repair. The vehicle was not diagnosed or repaired. Ford Motor Company (Ford) is recalling certain 2015-2018 Focus and 2013-2016 Fusion vehicles previously repaired incorrectly under recall numbers 18V169 and 18V845. The clutch may fracture, resulting in damage to the transmission assembly and possibly a transmission fluid leak.

Consequence & remedy

Consequence: A transmission fluid leak in the presence of an ignition source such as hot engine or exhaust components can increase the risk of a fire.

Remedy: Dealers will update the software, free of charge. Additionally, dealers will inspect Focus vehicles and replace the clutch assembly as necessary, free of charge. Interim letters, notifying owners of the safety risk, were mailed August 5, 2026. Additional letters will be sent once the remedy is available. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 26S43. Consequence & remedy

Consequence: A brake fluid leak will increase brake pedal travel and extend the distance needed to stop the vehicle, increasing the risk of a crash.

Remedy: Dealers will replace the front brake hoses, free of charge. Owner notification letters were mailed June 27, 2023. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 23S12.

Ford Motor Company (Ford) is recalling certain 2013-2019 Escape, 2013-2018 C-Max, 2013-2016 Fusion, 2013-2021 Transit Connect, and 2015-2018 Edge vehicles. The bushing that attaches the shifter cable to the transmission may degrade or detach.

Consequence & remedy

Consequence: A damaged or missing bushing may prevent the vehicle from shifting into the intended gear, and cause the vehicle to move in an unexpected direction. Additionally, the vehicle may roll after the driver selects the 'Park' position. Either scenario increases the risk of a crash or injury.

Remedy: Dealers will replace the under hood shift bushing and add a protective cap over the shift cable bushing, free of charge. Interim owner notification letters, informing owners of the safety risk, were mailed July 1, 2022. Remedy parts are expected to be available in 4th Quarter 2022. Ford Motor Company (Ford) is recalling certain 2014-2015 Fusion, Lincoln MKZ, and 2015 Mustang vehicles originally sold or ever registered, in Texas, Louisiana, Mississippi, Alabama, Florida, Georgia, South Carolina, North Carolina, Virginia and Hawaii. Exposure to certain environmental conditions, such as high temperatures, high humidity, and salt air can cause the brake pedal bumper to corrode and separate from the brake pedal.

Consequence & remedy

Consequence: A separated brake pedal bumper can cause the brake lights to illuminate continuously, causing confusion to other drivers and increasing the risk of a crash. On automatic transmission vehicles, a missing brake pedal bumper can allow the driver to shift the vehicle out of the "PARK" position without depressing the brake, allowing the vehicle to rollaway, increasing the risk of injury.

Remedy: Dealers will replace the brake pedal bumpers and clutch pedal bumpers, free of charge. Interim owner notification letters notifying owners of the safety risk were mailed on March 4, 2022. Ford Motor Company (Ford) is recalling certain 2011-2014 Fiesta, 2013-2014 Fusion, 2015 Mustang, 2013-2015 Escape and C-Max, 2012-2015 Focus, 2014-2016 Transit Connect, 2013-2014 Lincoln MKZ, and 2015 MKC vehicles. These vehicles were previously recalled and repaired under recalls 15V-246, 16V-643 or 17V-210, however the repair may not have been completed properly. A component within the door latches may break making the doors difficult to latch and/or leading the driver or a passenger to believe a door is securely closed when, in fact, it is not.

Consequence & remedy

Consequence: A door that is not securely latched could open while the vehicle is in motion, increasing the risk of injury to a vehicle occupant.

Remedy: Ford will notify owners, and dealers will inspect the latch date codes to confirm if the repair was done correctly, and replace the side door latches if necessary, free of charge. Owners will also have the option to inspect their own vehicle's door latches and enter the date codes into a website to determine if the latches are correct. Ford Motor Company (Ford) is recalling certain 2014-2015 Fiesta, 2014-2016 Fusion and Lincoln MKZ vehicles sold, or ever registered in Alabama, Arkansas, Arizona, California, Florida, Georgia, Hawaii, Louisiana, Mississippi, New Mexico, Nevada, Oklahoma, Oregon, South Carolina, Texas, Utah, Washington, Puerto Rico, American Samoa, Guam, the Northern Mariana Islands (Saipan), and the U.S. Virgin Islands. A component within the door latches may break making the doors difficult to latch and/or leading the driver or a passenger to believe a door is securely closed when, in fact, it is not.

Consequence & remedy

Consequence: A door that is not securely latched could open while the vehicle is in motion, increasing the risk of injury to a vehicle occupant.

Ford Motor Company (Ford) is recalling certain 2013-2016 Ford Fusion and Lincoln MKZ, 2015-2016 Ford Edge, and 2016 Lincoln MKX vehicles originally sold, or currently registered in Connecticut, Delaware, the District of Columbia, Illinois, Indiana, Iowa, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Vermont, West Virginia, and Wisconsin. Road salt or other contaminants may corrode the electric power steering gear motor attachment bolts.

Consequence & remedy

Consequence: If the bolts corrode, the steering gear motor may detach from the gear housing resulting in a loss of power steering assist, requiring a greater steering effort, especially at lower speeds, and increasing the risk of a crash.

Remedy: Ford will notify owners, and dealers will replace the steering gear motor bolts and apply a wax sealer. If any of the steering gear motor attachment bolts are broken or missing, a new steering gear will be installed in the vehicle. These repairs will be performed free of charge. Ford Motor Company (Ford) is recalling certain 2013-2016 Fusion vehicles equipped with 2.5 liter engines. The bushing that attaches the transmission shifter cable to the transmission may degrade over time and cause the bushing to detach from the transmission. The condition could allow the driver to move the shift lever to Park and remove the ignition key, while the transmission may not be in Park, with no warning message or audible chime.

Consequence & remedy

Consequence: If the vehicle is exited without the transmission being in Park and without the parking brake being applied, the vehicle may unexpectedly move, increasing the risk of a crash.

Remedy: Ford will notify owners, and dealers will replace the shifter cable bushing, free of charge. The recall began September 10, 2019. Owners may contact Ford customer service at 1-866-436-7332. Consequence: A steering wheel detachment can cause a loss of steering control, increasing the risk of a crash.

Ford Motor Company (Ford) is recalling certain 2013-2014 Ford Fusion vehicles equipped with a 1.6L GTDI engine and 6-speed manual transmission and 2015-2016 Ford Focus vehicles equipped with a 1.0L GTDI engine and 6-speed manual transmission. Also included are four 2017 Ford Focus vehicles that had clutch replacements with potentially affected parts. The clutch may fracture, resulting in damage to the transmission assembly and possibly a transmission fluid leak.

Consequence & remedy

Consequence: A transmission fluid leak in the presence of an ignition source such as hot engine or exhaust components can increase the risk of a fire.

Remedy: Ford will notify owners, and dealers will update the Focus vehicles with software that detects and prevents prolonged clutch slip, and will replace the clutch, as necessary, free of charge. Owners of Fusion vehicles will have their clutch assembly replaced free of charge. The recall began August 3, 2018. Owners may contact Ford customer service at 1-866-436-7332. Ford Motor Company (Ford) is recalling certain 2014 Escape, 2014-2015 Fiesta ST, 2013-2014 Fusion and 2013-2015 Transit Connect vehicles equipped with 1.6L GTDI engines. If the vehicle is started and driven with an insufficient level of coolant within the engine cooling system, the engine cylinder head may overheat, crack and leak oil.

Consequence & remedy

Consequence: If the bolts corrode, the steering gear motor may detach from the gear housing resulting in a loss of power steering assist. Loss of power steering assist would require a higher steering effort, especially at lower speeds, which may increase the risk of a crash.

Remedy: Ford will notify owners, and dealers will apply sealer and replace the steering gear motor bolts as required. If one or more of the steering gear motor attachment bolts are broken or missing, a new steering gear will be installed in the vehicle. These repairs will be performed free of charge. On August 8, 2022, the Office of Defects Investigation (ODI) opened this investigation based on 50 Vehicle Owner Questionnaires (VOQs) alleging front brake jounce hose ruptures on model year (MY) 2013-2018 Ford Fusion and Lincoln MKZ vehicles. A ruptured front brake hose results in a brake fluid leak. As a result, the driver may experience an increase in pedal travel and a reduction in the rate of deceleration, increasing the risk of a crash. It should be noted that the subject vehicles identified fell outside the scope of NHTSA Safety Recall 20V-469, which addressed front brake hoses rupturing on certain MY 2015-2018 Ford Edge and 2016-2018 Lincoln MKX vehicles. The result causes a brake fluid leak, lengthening the distance needing to stop the vehicle, increasing the risk of a crash.On March 10, 2023, Ford Motor Company (Ford) submitted to NHTSA Safety Recall 23V-162 (Ford Recall 23S12) to address front brake jounce hoses that may rupture. Ford found the rupture occurs from a localized failure of the internal reinforcement braid due to cyclic fatigue through tensile/bending and torsional inputs during suspension and steering articulations. The supplier for the front brake jounce hoses is Hitachi. This recall affects the subject vehicles equipped with non-hybrid engines and the suspect front brake jounce hoses (a total of 1,280,726 vehicles). The recall covers subject vehicles built from February 3, 2012 to July 19, 2017 when the suspect hoses were used in vehicle production. The remedy is for dealerships to replace both front brake jounce hoses with new hoses that have a revised braid material at no cost. The vehicles equipped with hybrid engines are excluded from the recall because the internal hose pressure is much lower due to the regenerative braking provided by the hybrid brake system. In December 2019, the Office of Defects Investigation (ODI) opened this investigation based on 235 Vehicle Owner Questionnaires (VOQs) alleging doors failed to latch when closed, and/or inadvertently opened while driving, on model year (MY) 2012-2014 Ford Fiesta, and 2013-2014 Fusion and Lincoln MKZ. Some VOQs reported the failure occurred after the latch had been remedied under a prior recall which involved fracture of the latch pawl spring tab due to thermally induced cyclic fatigue. Other VOQs alleged a similar failure in vehicles not previously recalled. A latch with a fractured pawl spring tab typically results in a ?door will not close? condition, however in certain somewhat rare circumstances, may result in the door coming open while driving. In response to ODI?s investigation, Ford has announced three field actions, as discussed below.On March 23, 2020, Ford submitted Safety Recall 20V-177 (Ford 20S15), a regional recall campaign to address door latch failures in vehicles not previously recalled under Safety Recalls 15V-246, 16V-643 and 17V-210. The recall affects vehicles currently or ever registered in certain high ambient temperature states (see 20V-177 for affected states). As a remedy, Ford will replace all four door latches free of charge.On August 24, 2020, Ford issued a non-safety regional field action (Ford 20R01) to address door latch failures on the same model/MY vehicles not covered under 20V-177 (i.e., not currently/ever registered in affected states). In response to ODI?s investigation, Ford has announced three field actions, as discussed below.On March 23, 2020, Ford submitted Safety Recall 20V-177 (Ford 20S15), a regional recall campaign to address door latch failures in vehicles not previously recalled under Safety Recalls 15V-246, 16V-643 and 17V-210. The recall affects vehicles currently or ever registered in certain high ambient temperature states (see 20V-177 for affected states). As a remedy, Ford will replace all four door latches free of charge.On August 24, 2020, Ford issued a non-safety regional field action (Ford 20R01) to address door latch failures on the same model/MY vehicles not covered under 20V-177 (i.e., not currently/ever registered in affected states). According to Ford, spring tab failure rates for vehicles covered by this action are about half that of the 20V-177 vehicles. Under this action, dealers will replace all door latches, free of charge, if either; a) the vehicle exhibits a related latch concern, or b) if the vehicle owner expresses a concern about the latches in their vehicles. This recall involves an inspection of the latches, with the option for a consumer self-inspection; owners could view the date codes on their latches (and confirm child safety locks are present on the rear doors) and then submit the information to Ford?s mobile app or website. A dealership inspection option was also available. Any vehicle found to have unreplaced defective latches requires the replacement of all door latches as directed by Ford.Regarding the 4,494 reports cited above, 1,485 involve scope-related failures (with 13% alleging door opening), and the remaining 3,009 reports involving remedy concerns (with 18% alleging door opening - note ODI was unable to verify the allegations of door opening). The three ODI injuries were all relatively minor. Ford?s reported injury incidences include one (unconfirmed) report of an ejection, three minor injuries with the others involving undisclosed injuries. On March 12th, 2018, Ford Motor Company submitted a Defect Information Report to NHTSA describing a defect in the steering wheel fastening system of approximately 1,301,986 model year (MY) Ford Fusion and Lincoln MKZ. On March 1, 2018, Ford discovered a design change that affected the MY 2014-2018 subject vehicles. The affected population are MY 2014-2016 Fusion and MKZ with manual and power adjust steering columns and MY 2017-2018 Fusion and MKZ equipped with manual adjust steering columns only. The affected populations have steering shafts with a reduced amount of threads for the steering wheel bolt to engage. The thread length was decreased by 5mm in the affected models. Ford stated that another contributing factor was the placement of a nylon locking patch torque retention on the fastening bolt.As a result, the steering wheel bolt may not maintain torque allowing the bolt to loosen and eventually separate from the steering shaft, resulting in a loose or detached steering wheel. The two crashes and one injury noted in Ford?s Defect Information Report are from two legal claims. The first accident is a claim where the driver reported the steering wheel twisted freely on the shaft which caused the driver to lose control.An injury was alleged but no details were provided. The second claim resulted in minor damage to the driver side of the vehicle including a punctured tire.
