The contact owns a 2014 Ford Fusion. The contact stated while driving at an undisclosed speed, there was white smoke coming from the exhaust, and the check engine warning light was illuminated. The contact also stated that the vehicle was consuming an excessive amount of coolant; however, there were no visual coolant leaks found. The vehicle was towed to two independent mechanics for confirmation, where the vehicle was diagnosed with a coolant leak and a blown head gasket. The contact was informed that the engine needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure. The contact was informed that the repair was not eligible for coverage under the Technical Service Bulletin: 20-2100. The contact was advised to contact the NHTSA Hotline to report the failure. The dealer recommended the long block needed to be replaced. The vehicle was not repaired. The manufacturer was notified of the failure and informed the contact that the vehicle could not be repaired under NHTSA Campaign Number: 17V209000 (Engine and Engine Cooling) because the VIN was not included. The contact owns a 2014 Ford Fusion. The contact stated that the "Engine Coolant Overtemperature" message had flashed on the instrument panel while driving at an unknown speed. The contact added coolant to the coolant reservoir because the coolant level was low. Two days later, the vehicle started to jerk and hesitate upon the depression of the accelerator pedal with the check engine warning light illuminated. The vehicle was taken to the dealer where it was discovered that coolant had saturated the entire engine which resulted in engine failure. Upon investigation, the contact discovered NHTSA Campaign Number: 17V209000 (Engine and Engine Cooling) which was linked to the failure. The manufacturer was notified of the failure and the contact was informed that there were no recalls on the vehicle related to the engine. The vehicle remained in the possession of the dealer. The failure mileage was approximately 162,000.

The vehicle was diagnosed by an independent mechanic at the residence, and it was determined there was a blown head gasket and coolant intrusion into the engine. The vehicle was taken to an independent mechanic, where it was diagnosed that the engine had failed. The vehicle was taken to two other independent mechanics, where the same assessment was made. Additionally, the contact was informed that the generation of heat caused by the friction between the two pistons led to the engine failure. Additionally, the contact was informed that there was coolant intrusion in the engine. The vehicle was not repaired. The dealer was not notified of the failure. The manufacturer was notified of the failure and referred the contact to the NHTSA Hotline for assistance. The failure mileage was approximately 80,950.

Ford Motor Company (Ford) is recalling certain 2015-2018 Focus and 2013-2016 Fusion vehicles previously repaired incorrectly under recall numbers 18V169 and 18V845. The clutch may fracture, resulting in damage to the transmission assembly and possibly a transmission fluid leak.

Consequence & remedy

Consequence: A transmission fluid leak in the presence of an ignition source such as hot engine or exhaust components can increase the risk of a fire.

Remedy: Dealers will update the software, free of charge. Additionally, dealers will inspect Focus vehicles and replace the clutch assembly as necessary, free of charge. Interim letters, notifying owners of the safety risk, were mailed August 5, 2026. Additional letters will be sent once the remedy is available. Consequence & remedy

Consequence: A brake fluid leak will increase brake pedal travel and extend the distance needed to stop the vehicle, increasing the risk of a crash.

Ford Motor Company (Ford) is recalling certain 2013-2019 Escape, 2013-2018 C-Max, 2013-2016 Fusion, 2013-2021 Transit Connect, and 2015-2018 Edge vehicles. The bushing that attaches the shifter cable to the transmission may degrade or detach.

Consequence & remedy

Consequence: A damaged or missing bushing may prevent the vehicle from shifting into the intended gear, and cause the vehicle to move in an unexpected direction. Additionally, the vehicle may roll after the driver selects the 'Park' position. Either scenario increases the risk of a crash or injury.

Remedy: Dealers will replace the under hood shift bushing and add a protective cap over the shift cable bushing, free of charge. Interim owner notification letters, informing owners of the safety risk, were mailed July 1, 2022. Remedy parts are expected to be available in 4th Quarter 2022. Ford Motor Company (Ford) is recalling certain 2014-2015 Fusion, Lincoln MKZ, and 2015 Mustang vehicles originally sold or ever registered, in Texas, Louisiana, Mississippi, Alabama, Florida, Georgia, South Carolina, North Carolina, Virginia and Hawaii. Exposure to certain environmental conditions, such as high temperatures, high humidity, and salt air can cause the brake pedal bumper to corrode and separate from the brake pedal.

Consequence & remedy

Consequence: A separated brake pedal bumper can cause the brake lights to illuminate continuously, causing confusion to other drivers and increasing the risk of a crash. On automatic transmission vehicles, a missing brake pedal bumper can allow the driver to shift the vehicle out of the "PARK" position without depressing the brake, allowing the vehicle to rollaway, increasing the risk of injury.

Remedy: Dealers will replace the brake pedal bumpers and clutch pedal bumpers, free of charge. Interim owner notification letters notifying owners of the safety risk were mailed on March 4, 2022. Ford Motor Company (Ford) is recalling certain 2011-2014 Fiesta, 2013-2014 Fusion, 2015 Mustang, 2013-2015 Escape and C-Max, 2012-2015 Focus, 2014-2016 Transit Connect, 2013-2014 Lincoln MKZ, and 2015 MKC vehicles. These vehicles were previously recalled and repaired under recalls 15V-246, 16V-643 or 17V-210, however the repair may not have been completed properly. A component within the door latches may break making the doors difficult to latch and/or leading the driver or a passenger to believe a door is securely closed when, in fact, it is not.

Consequence & remedy

Consequence: A door that is not securely latched could open while the vehicle is in motion, increasing the risk of injury to a vehicle occupant.

Remedy: Ford will notify owners, and dealers will inspect the latch date codes to confirm if the repair was done correctly, and replace the side door latches if necessary, free of charge. Owners will also have the option to inspect their own vehicle's door latches and enter the date codes into a website to determine if the latches are correct. Ford Motor Company (Ford) is recalling certain 2013-2016 Ford Fusion and Lincoln MKZ, 2015-2016 Ford Edge, and 2016 Lincoln MKX vehicles originally sold, or currently registered in Connecticut, Delaware, the District of Columbia, Illinois, Indiana, Iowa, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Vermont, West Virginia, and Wisconsin. Road salt or other contaminants may corrode the electric power steering gear motor attachment bolts.

Consequence & remedy

Consequence: If the bolts corrode, the steering gear motor may detach from the gear housing resulting in a loss of power steering assist, requiring a greater steering effort, especially at lower speeds, and increasing the risk of a crash.

Remedy: Ford will notify owners, and dealers will replace the steering gear motor bolts and apply a wax sealer. If any of the steering gear motor attachment bolts are broken or missing, a new steering gear will be installed in the vehicle. These repairs will be performed free of charge. Ford Motor Company (Ford) is recalling certain 2013-2016 Fusion vehicles equipped with 2.5 liter engines. The bushing that attaches the transmission shifter cable to the transmission may degrade over time and cause the bushing to detach from the transmission. The condition could allow the driver to move the shift lever to Park and remove the ignition key, while the transmission may not be in Park, with no warning message or audible chime.

Consequence & remedy

Consequence: If the vehicle is exited without the transmission being in Park and without the parking brake being applied, the vehicle may unexpectedly move, increasing the risk of a crash.

Remedy: Ford will notify owners, and dealers will replace the shifter cable bushing, free of charge. The recall began September 10, 2019. Owners may contact Ford customer service at 1-866-436-7332. The bushing that attaches the transmission shifter cable to the transmission may degrade over time and cause the bushing to detach from the transmission. The condition could allow the driver to move the shift lever to Park and remove the ignition key, while the transmission may not be in Park, with no warning message or audible chime.

Ford Motor Company (Ford) is recalling certain 2013-2014 Ford Fusion vehicles equipped with a 1.6L GTDI engine and 6-speed manual transmission and 2015-2016 Ford Focus vehicles equipped with a 1.0L GTDI engine and 6-speed manual transmission. Also included are four 2017 Ford Focus vehicles that had clutch replacements with potentially affected parts. The clutch may fracture, resulting in damage to the transmission assembly and possibly a transmission fluid leak.

Consequence & remedy

Consequence: A transmission fluid leak in the presence of an ignition source such as hot engine or exhaust components can increase the risk of a fire.

Remedy: Ford will notify owners, and dealers will update the Focus vehicles with software that detects and prevents prolonged clutch slip, and will replace the clutch, as necessary, free of charge. Owners of Fusion vehicles will have their clutch assembly replaced free of charge. The recall began August 3, 2018. Owners may contact Ford customer service at 1-866-436-7332. Ford Motor Company (Ford) is recalling certain 2014 Escape, 2014-2015 Fiesta ST, 2013-2014 Fusion and 2013-2015 Transit Connect vehicles equipped with 1.6L GTDI engines. If the vehicle is started and driven with an insufficient level of coolant within the engine cooling system, the engine cylinder head may overheat, crack and leak oil.

Consequence & remedy

Consequence: If the cylinder head cracks and leaks oil, the oil may contact a hot engine or exhaust component, increasing the risk of a fire.

Remedy: Ford will notify owners, and dealers will install a coolant level sensor and the supporting hardware and software, free of charge. Interim notices informing owners of the safety risk were mailed April 21, 2017. The recall began on January 5, 2018. Owners may contact Ford customer service at 1-866-436-7332. In the event of a crash, heat from the deployment of a seat belt pretensioner may cause a front seat belt anchor cable to fail.

Consequence & remedy

Consequence: If the driver or front passenger seatbelt cables cannot properly restrain the front seat occupants, they would be at an increased risk of injury.

Ford Motor Company (Ford) is recalling certain model year 2013-2015 Ford Fusion and Lincoln MKZ vehicles manufactured February 3, 2012, to March 20, 2015, and 2015 Ford Edge vehicles manufactured February 26, 2015, to February 28, 2015, and originally sold, or currently registered in, Connecticut, Delaware, Illinois, Indiana, Iowa, Maine, Maryland, Massachusetts, Michigan, Minnesota, Missouri, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Vermont, West Virginia, Wisconsin and the District of Columbia. In the affected vehicles, snow or water containing road salt or other contaminants may corrode the electric power steering gear motor attachment bolts.

Consequence & remedy

Consequence: If the bolts corrode, the steering gear motor may detach from the gear housing resulting in a loss of power steering assist. Loss of power steering assist would require a higher steering effort, especially at lower speeds, which may increase the risk of a crash.

Remedy: Ford will notify owners, and dealers will apply sealer and replace the steering gear motor bolts as required. If one or more of the steering gear motor attachment bolts are broken or missing, a new steering gear will be installed in the vehicle. These repairs will be performed free of charge. Ford Motor Company (Ford) notified the agency on April 23, 2015, that they are recalling certain model year 2013-2014 Ford Fusion and Lincoln MKZ vehicles manufactured July 1, 2012, to May 31, 2013, and 2012-2014 Fiesta vehicles manufactured February 1, 2012, to May 31, 2013. On April 30, 2015, Ford expanded the recall to cover an additional 119,567 vehicles, including certain model year 2011 Ford Fiestas manufactured from November 11, 2009, to May 31, 2013 and certain model year 2013 Ford Fusion and Lincoln MKZ vehicles manufactured from February 1, 2012, to June 30, 2012. A component within the door latches may break making the doors difficult to latch and/or leading the driver or a passenger to believe a door is securely closed when, in fact, it is not.

Consequence & remedy

Consequence: A door that is not securely latched could open while the vehicle is in motion, increasing the risk of injury to a vehicle occupant.

Remedy: Ford will notify owners, and dealers will replace all four door latches with an improved part, free of charge. Interim notifications were mailed to owners on June 5, 2015. In the affected vehicles, the key may be able to be removed even while the transmission is not in Park. Thus, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) No. 114, "Theft Protection and Rollaway Prevention."

Consequence & remedy

Consequence: If the key can be removed despite that the transmission is not in Park, a driver may think the vehicle is safety parked, when it is not, increasing the risk of a vehicle rollaway that could cause the vehicle to strike and injure the driver, other vehicle occupants, or bystanders.

On August 8, 2022, the Office of Defects Investigation (ODI) opened this investigation based on 50 Vehicle Owner Questionnaires (VOQs) alleging front brake jounce hose ruptures on model year (MY) 2013-2018 Ford Fusion and Lincoln MKZ vehicles. A ruptured front brake hose results in a brake fluid leak. As a result, the driver may experience an increase in pedal travel and a reduction in the rate of deceleration, increasing the risk of a crash. It should be noted that the subject vehicles identified fell outside the scope of NHTSA Safety Recall 20V-469, which addressed front brake hoses rupturing on certain MY 2015-2018 Ford Edge and 2016-2018 Lincoln MKX vehicles. The result causes a brake fluid leak, lengthening the distance needing to stop the vehicle, increasing the risk of a crash.On March 10, 2023, Ford Motor Company (Ford) submitted to NHTSA Safety Recall 23V-162 (Ford Recall 23S12) to address front brake jounce hoses that may rupture. Ford found the rupture occurs from a localized failure of the internal reinforcement braid due to cyclic fatigue through tensile/bending and torsional inputs during suspension and steering articulations. The supplier for the front brake jounce hoses is Hitachi. This recall affects the subject vehicles equipped with non-hybrid engines and the suspect front brake jounce hoses (a total of 1,280,726 vehicles). The recall covers subject vehicles built from February 3, 2012 to July 19, 2017 when the suspect hoses were used in vehicle production. The remedy is for dealerships to replace both front brake jounce hoses with new hoses that have a revised braid material at no cost. The vehicles equipped with hybrid engines are excluded from the recall because the internal hose pressure is much lower due to the regenerative braking provided by the hybrid brake system. From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. RQ19005 · Door Latch Failures

In December 2019, the Office of Defects Investigation (ODI) opened this investigation based on 235 Vehicle Owner Questionnaires (VOQs) alleging doors failed to latch when closed, and/or inadvertently opened while driving, on model year (MY) 2012-2014 Ford Fiesta, and 2013-2014 Fusion and Lincoln MKZ. Some VOQs reported the failure occurred after the latch had been remedied under a prior recall which involved fracture of the latch pawl spring tab due to thermally induced cyclic fatigue. Other VOQs alleged a similar failure in vehicles not previously recalled. A latch with a fractured pawl spring tab typically results in a ?door will not close? condition, however in certain somewhat rare circumstances, may result in the door coming open while driving. In response to ODI?s investigation, Ford has announced three field actions, as discussed below.On March 23, 2020, Ford submitted Safety Recall 20V-177 (Ford 20S15), a regional recall campaign to address door latch failures in vehicles not previously recalled under Safety Recalls 15V-246, 16V-643 and 17V-210. The recall affects vehicles currently or ever registered in certain high ambient temperature states (see 20V-177 for affected states). As a remedy, Ford will replace all four door latches free of charge.On August 24, 2020, Ford issued a non-safety regional field action (Ford 20R01) to address door latch failures on the same model/MY vehicles not covered under 20V-177 (i.e., not currently/ever registered in affected states). According to Ford, spring tab failure rates for vehicles covered by this action are about half that of the 20V-177 vehicles. Under this action, dealers will replace all door latches, free of charge, if either; a) the vehicle exhibits a related latch concern, or b) if the vehicle owner expresses a concern about the latches in their vehicles. Details can be found under NHTSA TSB No. 10190990, a copy of which is available in this investigative file available at NHTSA.gov.On June 8, 2020, Ford submitted Safety Recall 20V-331 (Ford 20S30) addressing vehicles previously remedied under NHTSA Recalls 15V-246, 16V-643 and 17V-210, where the repair may not have been properly completed, specifically not all of the door latches were replaced, or replaced correctly. This recall involves an inspection of the latches, with the option for a consumer self-inspection; owners could view the date codes on their latches (and confirm child safety locks are present on the rear doors) and then submit the information to Ford?s mobile app or website. A dealership inspection option was also available. Any vehicle found to have unreplaced defective latches requires the replacement of all door latches as directed by Ford.Regarding the 4,494 reports cited above, 1,485 involve scope-related failures (with 13% alleging door opening), and the remaining 3,009 reports involving remedy concerns (with 18% alleging door opening - note ODI was unable to verify the allegations of door opening). The three ODI injuries were all relatively minor. This recall involves an inspection of the latches, with the option for a consumer self-inspection; owners could view the date codes on their latches (and confirm child safety locks are present on the rear doors) and then submit the information to Ford?s mobile app or website. A dealership inspection option was also available. Any vehicle found to have unreplaced defective latches requires the replacement of all door latches as directed by Ford.Regarding the 4,494 reports cited above, 1,485 involve scope-related failures (with 13% alleging door opening), and the remaining 3,009 reports involving remedy concerns (with 18% alleging door opening - note ODI was unable to verify the allegations of door opening). On March 12th, 2018, Ford Motor Company submitted a Defect Information Report to NHTSA describing a defect in the steering wheel fastening system of approximately 1,301,986 model year (MY) Ford Fusion and Lincoln MKZ. On March 1, 2018, Ford discovered a design change that affected the MY 2014-2018 subject vehicles. The affected population are MY 2014-2016 Fusion and MKZ with manual and power adjust steering columns and MY 2017-2018 Fusion and MKZ equipped with manual adjust steering columns only. The affected populations have steering shafts with a reduced amount of threads for the steering wheel bolt to engage. The thread length was decreased by 5mm in the affected models. Ford stated that another contributing factor was the placement of a nylon locking patch torque retention on the fastening bolt.As a result, the steering wheel bolt may not maintain torque allowing the bolt to loosen and eventually separate from the steering shaft, resulting in a loose or detached steering wheel. The two crashes and one injury noted in Ford?s Defect Information Report are from two legal claims. The first accident is a claim where the driver reported the steering wheel twisted freely on the shaft which caused the driver to lose control.An injury was alleged but no details were provided.
