A WARNING ICON APPEARED ON MY DASH IN ADDITION TO A MESSAGE READING: "SERVICE ADVANCED TRAC & SERVICE POWER STEERING. I HAD THIS OCCUR THREE TIMES PREVIOUSLY AT LOW SPEEDS IN PARKING LOTS, BUT NEVER AT SUCH HIGH SPEEDS. I SAFELY EXITED THE INTERSTATE, CAME TO STOP, TURNED MY ENGINE OFF, AND RESTARTED MY CAR. THE WARNING MESSAGE DISAPPEARED AND I REGAINED POWER STEERING. I THEN WENT BACK ONTO THE INTERSTATE UP TO 65 MPH WHEN THE ISSUE HAPPENED AGAIN. I WAS ABLE TO CAREFULLY DRIVE HOME THE REST OF THE WAY WITHOUT POWER STEERING WHICH TOOK A SIGNIFICANT AMOUNT OF EFFORT TO TURN THE STEERING WHEEL. I TOOK THE CAR IT INTO THE DEALER THE NEXT DAY. THEY DIAGNOSED IT AS AN "INTERNAL MOTOR FAULT IN STEERING RACK". THEY HAD TO REPLACE THE STEERING RACK AND GEAR ASSEMBLY AND DO A WHEEL REALIGNMENT. TOTAL COST OF REPAIR WAS $1700. I WAS FORTUNATE TO NOT BE INVOLVED IN AN ACCIDENT WHEN SUDDENLY LOSING POWER STEERING ON THE EXPRESSWAY. FORD NEEDS TO FURTHER INVESTIGATE THIS AND REPAIR IT ACROSS ALL VEHICLES AFFECTED BEFORE SOMEONE GETS SERIOUSLY INJURED. *TR

MY WIFE WAS DRIVING ARE 2011 FORD FUSION THE OTHER DAY ON HER WAY BACK FROM A TRIP. NOTE WE JUST PURCHASED THIS CAR 35 DAYS AGO ROUGHLY. THE FUSION HAD 30,XXX MILES ON IT WHEN PURCHASED AND THE TIME OF THE INCIDENT WHEN THE POWER STEERING FAILED WAS 31,XXX MILES WE HAD NOT PUT MORE THEN 1000 MILES ON THIS CAR SINCE WE BOUGHT IT. THE WIFE WAS DRIVING THROUGH A CONSTRUCTION ZONE ON HER WAY HOME. SHE WAS A LITTLE MORE THEN A 100 MILES AWAY FROM ARE HOME. SHE WAS ON THE INTERSTATE WHEN SHE SUDDENLY REALIZED THE POWER STEERING LIGHT CAME ON SAYING IT NEEDED TO BE SERVICED. SHE WAS DRIVING IN SINGLE LANE OF TRAFFIC IN A 65 MPH ZONE BECAUSE OF THE CONSTRUCTION. LUCKILY THE POWER STEERING HELD UP LONG ENOUGH FOR HER TO GET OFF AT THE NEAREST EXIT OF THE INSTATE BEFORE IT FAILED COMPLETE IN THE PARKING LOT OF A GAS STATION. THINGS LIKE THIS ARE NO JOKE AND I CAN NOT BELIEVE FORD ISN'T DOING ANYTHING ABOUT THIS. I'M THANKFUL MY WIFE WAS NOT HURT OR ANYONE ELSE LIKE ON COMING TRAFFIC OR A CONSTRUCTION WORKER. BUT I'M VERY VERY ANGER AND HOPE FORD WAKES UP AND RECALLS THIS STEERING ISSUE BEFORE ITS TO LATE FOR SOME UNLUCKY PERSON!! THIS SHOULD NOT BE HAPPENING ON ANY CAR LET ALONE CLOSE TO BRAND NEW CARS WITH LESS THEN 100K ON THEM. I HAVE OWNED AND DROVE MANY FORD VEHICLES IN MY LIFE BUT I DON'T KNOW IF I WILL EVER BUY ANOTHER FORD CAR BECAUSE OF THIS CLEAR SAFETY ISSUE. MY WIFE IS FAIRLY STRONG AND SHE COULD BARELY MOVE THE CAR STEERING WHEEL AFTER IT LOST POWER TO THE WHEEL AND I EVEN MOVED THE CAR SO IT COULD BE TOWED AND I STRUGGLED TO TURN THE WHEEL I COULD NOT IMAGINE IF THE POWER STEERING WENT OUT AND I WAS NOT READY I WOULD PROBABLY LOSE CONTROL IF I WAS ON THE INTERSTATE OR IN CLOSE TRAFFIC LET ALONE TRYING TO MAKE A HARD TURN IN A CORNER. I HOPE EVER ONE THE BEST OF LUCK WITH THIS AS I KNOW I'M NOT ALONE AND PLEASE FILL OUT THE REPORT TO GET THIS RECALLED. THANKS!! *TR

I WAS DRIVING DOWN THE ROAD TO GET TO WORK AND A ICON POPPED UP ON MY DASH IN ADDITION TO A COUPLE MESSAGES APPEARING WHERE MY ODOMETER READING TYPICALLY IS. THE MESSAGES READ : "SERVICE ADVANCED TRAC & SERVICE POWERSTEERING." THERE SEEMED TO BE NO DIFFERENCE IN THE CONTROL AND HANDLING OF MY CAR UNTIL I WENT TO PARALLEL PARK (MY STEERING FELT STIFF) SO I CONTINUED ONTO MY APPOINTMENT. AFTER RETURNING TO MY CAR AND TURNING IT BACK ON THE ICON WAS BACK AND THE MESSAGES RETURNED WITH THE ADDITION OF SAY "NOW" AFTER THE INITIAL MESSAGES. MY DRIVING EXPERIENCE HOWEVER WAS DIFFICULT. MY POWER STEERING WAS NOT WORKING AND I HAD TO EXERT A SIGNIFICANT AMOUNT OF EFFORT TO TURN THE STEERING WHEEL. IT ALSO FELT AS IF MY TURNING RADIUS WAS MUCH WIDER TO TURN CORNERS. MY DEALERSHIP CHECKED MY CAR AND GAVE ME AN ESTIMATE JUST UNDER $2000 TO REPAIR THE ISSUE INCLUDING THE INITIAL DIAGNOSIS OF THE ISSUE. THE REPORT INDICATED THAT THE "MODULE ON THE STEERING RACK" WAS AT FAULT, BUT TO REPAIR THAT WOULD REQUIRE THE REPLACEMENT OF THE WHOLE STEERING RACK. *TR

I WAS DRIVING TO WORK ON INTERSTATE-87 NORTH IN NEW YORK WITH NORMAL DRIVING CONDITIONS WHEN, ALL OF A SUDDEN, THE POWER STEERING LIGHT ON MY 2011 FORD FUSION CAME ON THE DASH INFORMING ME OF A POWER STEERING FAILURE. AT THE TIME, I WAS DRIVING IN THE LEFT HAND LANE OF THE THREE-LANE HIGHWAY AND WAS HAD TO NAVIGATE MY WAY ACROSS TWO LANES OF BUSY HIGHWAY TRAFFIC TO PULL ONTO THE SHOULDER. WITH MY HAZARD LIGHTS ON, I HAD TO FIGHT THE STIFF STEERING WHEEL TO GET TO THE SHOULDER. I PUT MY CAR IN PARK, TURNED THE CAR OFF, AND TURNED THE CAR BACK ON. THE ISSUE WENT AWAY AND I DROVE THE REST OF THE WAY TO WORK. I HAVE OWNED THIS CAR FOR ALMOST 2 YEARS AND HAD NOT HAD AN INCIDENT UNTIL JULY 2014. TWO TIMES PRIOR TO THIS STEERING FAILURE, THE ELECTRONIC POWER STEERING FAILED IN MY DRIVEWAY AS I SHIFTED THE CAR INTO REVERSE TO PULL OUT OF THE DRIVEWAY. THIS TRIP COULD HAVE EASILY TURNED INTO A SERIOUS ACCIDENT FOR MYSELF AND THOSE DRIVING AROUND ME. *TR

WHILE DRIVING THE VEHICLE AT 25 MPH I LOST THE POWER STEERING. I WAS ABLE TO SAFELY PULL THE VEHICLE INTO A PARKING LOT AND PLACE THE AUTO IN PARK. I TURNED THE CAR OFF AND BACK ON AND RECEIVE A SERVICE MESSAGE TO HAVE THE ADVANCE TRAC SYSTEM AND THE POWER STEERING SYSTEM SERVICED SOON. I ALLOWED THE CAR TO SIT FOR A FEW MINUTES, TURNED THE CAR OFF AND ON AGAIN AND THIS TIME THE WARNING MESSAGES CHANGED TO HAVE THE ADVANCE TRAC SYSTEM SERVICED AND THE POWER STEERING SYSTEM SERVICED NOW. I WAS UNABLE TO DRIVE THE AUTO. I CALLED A TOW AND THE CAR IS NOW SITTING AT FIRESTONE AUTO REPAIR. I'M ON THE PHONE WITH FORD NOW. THEY HAVE A RECALL FOR THIS ISSUE BUT ONLY FOR 2011'S MANUFACTURED AT THE KANSAS CITY PLANT. MY CAR WAS MANUFACTURED AT THE HERMOSILLO PLANT IN MEXICO. I'M ON HOLD NOW WITH FORD CUSTOMER SERVICE WAITING TO DISCUSS THIS TO SEE IF THEY ARE WILLING TO BROADEN THE RECALL TO COVER THE MEXICO PLANT. I DRIVE ON INTERSTATES IN CHICAGO EVERY DAY. IF THIS HAPPENS DURING A DAILY RUSH HOUR COMMUTE I WILL BE IN SERIOUS TROUBLE. THE FIRESTONE AUTO REPAIR DOESN'T REALLY KNOW HOW TO APPROACH THIS ISSUE. I HAVE READ THAT IT MAY HAVE SOMETHING TO DO WITH THE ROAD ANGULAR SENSOR OR THE VOLTAGE ON THE BATTERY. SUPPOSEDLY THE DEALER THAT I PURCHASED THIS AUTO FROM (MILADY NISSAN IN CRYSTAL LAKEK) GAVE ME A NEW BATTERY. I HAVE HAD THE CAR FOR 5 MONTHS. *TR

15V793000 · Fuel System, Gasoline:storage:auxillary Tank

Nov 24, 2015

Ford Motor Company (Ford) is recalling certain model year 2010-2011 Ford Fusion vehicles manufactured July 21, 2008, to March 4, 2011, and 2010-2011 Mercury Milan vehicles manufactured July 23, 2008, to December 10, 2010. The affected vehicles have fuel vapor canisters whose purge valves may not operate properly resulting in abnormal pressure changes in the fuel tank.

Consequence & remedy

Consequence: The pressure changes may cause the tank to crack and leak fuel. A fuel leak in the presence of an ignition source increases the risk of a fire.

Remedy: Ford will notify owners, and dealers will update the vehicle powertrain control modules and inspect the vehicle for any diagnostic trouble codes (DTCs) for this issue and perform a leak test on the Canister Purge Valve (CPV). The fuel tank will be inspected for cracks. The CPV and fuel tank will be replaced as necessary. These repairs will be performed free of charge. The recall began on January 15, 2016. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 15S34.

15V340000 · Steering:electric Power Assist System

EA17004 · Power Steering Failure

Opened Nov 20, 2017 · Closed Feb 9, 2024

Status: closed (inferred from source dates) · Steering; Steering:electric Power Assist System; Steering:rack And Pinion; Steering:rack And Pinion:rack

On September 28, 2016, the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE16-011) to investigate claims of power steering assist loss on model year (MY) 2010 Ford Fusion vehicles.  During the PE, ODI received 718 unique claims related to the alleged problem with 598 reports indicating the power steering assist failed while driving at various speeds with no warning.  Ford Motor Company (Ford) provided 1,851 unique reports and 1,720 warranty claims related to the alleged problem.  It should be noted that as the result of investigation PE14-030, Ford conducted a safety recall (15V-340) ofMY 2011-2012 Ford Fusion and other vehicles using the same electric power assist steering unit to rectify incidents of loss of power steering assist. On November 20, 2017, PE16-011 was upgraded to this Engineering Analysis (EA17-004) to further assess the scope, frequency, and consequence of this failure.  This was based on an analysis of the subject vehicle complaint data that had indicated an increased likelihood of failure as the vehicles age.  At the time, the rate of occurrence of this failure was comparable to previous investigations involving loss of power steering assist including the previously mentioned PE14-030. During the EA, ODI’s analysis of the alleged defect in the subject vehicles identified the following:  testing showed minor steering effort changes between “assist” and “no assist”, no root cause identified, a declining failure trend, the ramp-down or reduced assist provided when a DTC is set, a lower failure rate than the recalled Fusion vehicles, and the high age of the vehicles. A safety-related defect trend has not been identified.  Accordingly, this Engineering Analysis is closed.  See the attached Engineering Analysis Closing Report for details.  The closing of this investigation does not constitute a finding by NHTSA that a safety defect does not exist, and ODI will take further actions if warranted by future circumstances.  To review the reports cited in the Closing Resume ODI Report Identification Number document, go to NHTSA.gov.

PE14030 · Electric Power Steering Failure

Opened Oct 2, 2014 · Closed Sep 10, 2015

Status: closed (inferred from source dates) · Steering:electric Power Assist System

The Office of Defects Investigation (ODI) opened a preliminary evaluation to investigate complaints of the loss of power steering while driving. The subject vehicles are equipped with a rack mounted Electric Power Assisted Steering (EPAS) unit. Some of the complaints alleged that the steering assist failed and resulted in increased steering efforts that contributed to a loss of control and in some cases a crash. The EPAS system was manufactured by TRW Automotive for Ford and was designated as Generation I.These units were first produced and put into vehicle production in July 2008 beginning with the model year (MY) 2010 Ford Fusion and Mercury Milan. The system is comprised of a mechanical steering rack-and-pinion gear with torque sensor, electric assist motor, motor position sensor and a power steering control module (PSCM). The PSCM is self-monitoring and designed to detect faults within the system and can store diagnostic trouble codes (DTCs) if a problem is detected. There are 29 potential DTCs the system can detect. Many of these allow the system to go into reduced-assist steering mode by using the default vehicle speed signal should the live speed signal be lost, if voltage to EPAS system falls below 10.7 or goes above 17 volts, the temperature at the PSCM goes above 105C (221F), or if high friction is detected. Some DTCs will cause the system to go into manual steering mode (no assist). On July 1, 2015, Ford filed a Defect Information Report (DIR) identifying a safety related defect in TRW GEN I EPAS systems installed in some of the subject vehicles (NHTSA Recall 15V-340, Ford No. 15S18).The DIR indicated that intermittent electrical connections caused by quality issues associated with conformal coat contamination and ribbon cable pin misalignment within the steering gear motor may lead to a loss of the motor position sensor signal, resulting in a sudden loss of power assist while driving. The recall covers approximately 393,623 vehicles built from September 2010 through February 2012:2011-12 Fusion built SEP 2010 - FEB 2012; 2011-12 Lincoln MKZ built SEP 2010 - FEB 2012; 2011 Milan built SEP 2010 - DEC 2010; 2011-13 Taurus built SEP 2010 - FEB 2012; 2011-13 Flex with 3.5L GTDI engine built SEP 2010 - FEB 2012; and 2011-13 MKS, MKT with 3.5L GTDI engine built SEP 2010 - FEB 2012.The repair consists of updating the PSCM software to prevent a loss of steering assist while driving. The revised software will provide audible and visual warnings to the driver. If one or more of the following DTCs are stored, C1B00-62, C200B-2F, C200B-61, C200B-62, C200C-2F, C200D-49, U0300-00, U2011-49, U2011-61, U3000-46, U3000-49, U3000-61, U3000-72, U3000-96, P07AE-09, the dealer will replace the EPAS system at no charge to the owner.ODI's analysis of failure data for this PE identified a total of 2,584 complaints to ODI and Ford and 9,967 Ford warranty claims related to the alleged defect. The recalled vehicles account for 1,634 of the complaints and 7,311 of the warranty claims.ODI's analysis identified 40 complaints alleging crashes that may be related to loss of steering assist while driving, 20 of which involved vehicles included in Ford's recall. The information available for the alleged crashes were not sufficient to assess specific causes of the incidents or to verify EPAS malfunctions in each case. Most of the crashes were low-speed impacts with curbs or roadside objects resulting in wheel or minor body damage.None of the injury claims indicated medical attention was required. In addition, statistical modeling of the failure data by NHTSA's National Center for Statistics and Analysis projected 10-year failur

PE13003 · Electronic Throttle Body Malfunction

Opened Feb 21, 2013 · Closed Feb 28, 2014

Status: closed (inferred from source dates) · Engine

On February 21, 2013, the Office of Defects Investigations (ODI) opened Preliminary Evaluation PE13-003 to investigate allegations of electronic throttle body (ETB) failures resulting in sudden reduction of engine power in model year (MY) 2009-2013 Ford Escape, Fusion, Mariner and Milan vehicles. During this investigation, Ford identified a condition in subject vehicles equipped with 2.5L and 3.0L engines that may result in a sudden reduction of engine power.According to Ford, the ETB internal motor contacts may develop a high resistance material buildup condition on the commutator, resulting in intermittent electrical connectivity and reduced engine power. When this condition occurs, the Malfunction Indicator Lamp (MIL) or Wrench light will illuminate and the vehicle may enter a limited limp home mode.Ford?s trade name for the feature is Failure Mode Effects Management (FMEM) mode.In this mode, engine power and vehicle speed are reduced, while full function of the power steering, power braking, lighting, and climate control systems are maintained.ODI?s complaint analysis indicate that the predominant failure mode involved reduced motive power associated with the limited limp home mode with engine speeds limited to approximately 900 RPM. Analysis of warranty claims provided by Ford identified 59,807 claims related to ETB replacements and approximately 50 percent of claims are associated with diagnostic trouble codes (DTC) P2111, Throttle Body Stuck Open, and P2112, Throttle Body Stuck Closed.Ford described several factors where the ETB motor may fail resulting in DTCs P2111 or P2112 but the failure is not an existing stuck open or closed ETB valve position.According to Ford, the ETB control strategy provides the driver with three FMEM modes that allow varying degrees of vehicle mobility depending on the severity of the fault detected.DTCs associated with stuck open or closed throttle valves are designated the highest failure severity resulting in engine speeds limited to high idle corresponding to the limited limp home mode.Vehicles are not likely to unexpectedly stall as a result of this condition, but drivers may characterize the reduced functionality as a stall, even though their vehicle may still has motive capability.Other FMEM limp modes may result in reduced engine performance but will maintain vehicle speed above 20mph. During this investigation, Ford and its suppliers, Delphi and Igarashi, updated the powertrain control module (PCM) software to include a throttle body motor cleaning cycle during key-on and modified the ETB internal motor components design, surface finish and material composition to improve durability. Additionally, Ford developed a remedy procedure and issued a special Customer Satisfaction Program (CSP) 13N03 extending the ETB warranty coverage and instructing dealers to update the powertrain calibration to improve vehicle performance in the event that intermittent electrical connectivity of the throttle body motor contacts occurs. The program extends the coverage for up to 10 years of service or 150,000 miles from the warranty start date of the vehicle, all vehicles are eligible for the program through January 31, 2015 regardless of mileage.Owners of the affected vehicles will be contacted by mail to take their vehicle to a Ford dealer who will reprogram the PCM to the latest calibration. The bulletin was sent to dealers on January 17, 2014 and the owner letter mailing began on January 27, 2014. See the investigative file for copies of Ford's bulletin and owner letter. This preliminary evaluation is closed. The closing of this investigation does not constitute a finding that

EA12009 · Floor Mat Interference With Accelerator

Opened Dec 12, 2012 · Closed Jun 12, 2014

Status: closed (inferred from source dates) · Vehicle Speed Control; Vehicle Speed Control:accelerator Pedal

On May 27, 2014, Ford Motor Company submitted recall 14E-028 (Ford recall #14S07) to correct a safety-related problem with certain driver's side Ford optional all-weather floor mats sold for use in model year 2006 through 2011 model year Ford Fusion, Mercury Milan, Lincoln Zephyr, and Lincoln MKZ vehicles built at Hermosillo Assembly Plant (HAP) from November 9, 2007, to October 31 ,2010.NHTSA testing determined that the original accessory all-weather mat with a single retention clip (Figure 1), may interfere with accelerator pedal return to idle with the mat retained by the clip and rotated clockwise. Both the single- and double-clip (Figure 2) all-weather floor mats may interfere with the accelerator pedal return to idle when the mats are not secured by the retention clip(s).Testing showed that this can occur if the mats are moved forward from the clipped position approximately two inches into a position in which the rear edge of the mats will abut the retention clip(s) when installed singly and align with the clip(s) when stacked on another floor mat.The pedal interference incidents reviewed by the Office of Defects Investigation (ODI) include 1 crash and 9 incidents with allegations of brake friction materials smoking and/or requiring repair for heat related damage. The crash incident occurred after the parking brake was applied at speed, resulting in loss-of-control vehicle yaw event caused by lock-up of the rear tires. The spin-out resulted in a side impact collision with an oncoming vehicle in an opposing lane of traffic. For the 29 incidents with sufficient information available to make an assessment; 23 (79%) were controlled by the driver either shifting to neutral, turning the engine off, or both; 4 (14%) were ended by the driver either kicking the pedal free (2) or pulling the mat away from the pedal (2); and 2 (7%) were ended by the pedal freeing itself from the mat with no apparent driver action.None of the drivers who reacted to the incident by shifting to neutral or turning the engine off reported any difficulties with either action. The subject mats were sold by Ford and Lincoln dealers directly to customers as an optional accessory equipment item or, beginning in November 2007, the mats were also available as a factory installed accessory if ordered with a new vehicle.Ford estimates that approximately 82,579 affected driver's side all weather floor mats were sold worldwide; either over the counter through Ford dealerships or as original equipment.Owners that purchased the subject all-weather floor mats as a factory installed accessory with their new vehicle will be notified to return their all weather floor mats to their Ford or Lincoln Dealership in exchange for replacement mats.Owners of all other potentially affected vehicles will be notified that, if they have an affected Ford driver's side all weather floor mat, they should return their mats to the dealership in exchange for replacement mats. The VOQs associated with the closing of this investigation are: 10327597, 10332233, 10334211, 10335336, 10339330, 10348772 (and duplicate 10342572), 10372674, 10379562, 10415911, 10426255, 10434107, 10521053, 10554966.This investigation is hereby closed.
