After several attempts to restart the vehicle, the contact was able to start the vehicle and drove home. The vehicle was towed to a dealer and the contact was informed that the TCM needed to be replaced. The vehicle was not repaired. The manufacturer was contacted and stated that the VIN was not under recall. The failure mileage was 143,245.

I purchased a 2016 Ford Focus in September of this year from a used car dealer. I drove the car back and forth to work approximately 10 miles each way, per day. I took a day off before Thanksgiving and drove my vehicle longer than usual. Holiday traffic was bad and there was a lot of stop and go traffic, my car sputtered and shook and stalled on a busy street. Luckily, I was close to my house and was able to pull over. Again, a few days ago I drove for about an hour and the car again sputtered when moving from a complete stop. I went online and googled "Ford Focus transmission issues" and saw that there has been a lawsuit and settlement. I called Ford and was told that it was not under recall and that I would have to take the car I just purchased, to be fixed and I would have to pay for it. Please recall these transmissions before someone gets killed.

The vehicle was taken to an independent mechanic who informed her that the clutch needed to be replaced. The vehicle was repaired but the failure recurred. The vehicle was taken to another independent mechanic who informed her that the failure was due to the clutch. The contact called the manufacturer who informed her that the repairs were not covered and referred her to the NHTSA Hotline. The failure mileage was approximately 23,000.

The manufacturer was made aware of the failure and confirmed that the vehicle was not included in NHTSA Campaign Numbers: 18V845000 (Power Train) nor 18V169000 (Power Train). Several people have told me these type of cars are known to have horrible transmissions. I can’t afford a $3,000 + repair on this when the mileage isn’t even bad enough to have a transmission issue. This is my significant others only way to make income. It is not safe to drive the car in this condition.

The contact owns a 2016 Ford Focus. The contact stated that while at a complete stop, the vehicle would shake without warning. The contact took the vehicle to the dealer where the seal and clutch assembly were replaced as well as the TCM (Transmission Control Module). Recently, the same failure recurred, and the contact was given the same diagnosis from the dealer. Upon investigation, the contact linked the failure to NHTSA Campaign Number: 18V845000 (Power Steering). The manufacturer was notified of the failure and informed her that there were no recalls or warranty on the vehicle. The manufacturer agreed to cover half the cost of the repair. The vehicle was not yet repaired. The failure mileage was 24,991. The contact stated that from a complete stop and upon acceleration, the vehicle would begin to shake. The force of the shaking varied but would increase over time. The shaking negatively affected the drivability of the vehicle. The contact was informed by the dealer that the issue would only get worse if no repairs were made. Half of the costs of repair was covered by the manufacturer due to the exact same problem happening within a year.

Consequence & remedy

Consequence: An engine that stalls while driving increases the risk of a crash.

Remedy: Dealers will update the powertrain control module (PCM) software, free of charge. Owner notification letters were mailed July 29, 2026. Consequence & remedy

Consequence: A transmission fluid leak in the presence of an ignition source such as hot engine or exhaust components can increase the risk of a fire.

Remedy: Dealers will update the software, free of charge. Additionally, dealers will inspect Focus vehicles and replace the clutch assembly as necessary, free of charge. Ford Motor Company (Ford) is recalling certain 2016-2018 Ford Focus and 2018-2022 Ford EcoSport vehicles. The oil pump drive belt or drive belt tensioner may fail, resulting in a loss of engine oil pressure.

Consequence & remedy

Consequence: A loss of oil pressure can result in an engine stall and a loss of power braking assist, increasing the risk of a crash.

Remedy: Dealers will replace the oil pump tensioner assembly and oil pump drive belt, free of charge. If engine failure occurs due to a damaged oil pump belt and/or oil pump belt tensioner, Ford has authorized dealers to replace the engine assembly as an interim repair before the availability of remedy parts for the final repair. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 206, "Door Locks and Door Retention Components."

Consequence & remedy

Consequence: With only a single press needed, the driver may accidently unlatch the hatchback, increasing the risk of injury to any unsecured passengers in the back.

EA26004 · Timing Belt Failure

On December 18, 2025, the Office of Defects Investigation (ODI) opened PE25020 to investigate instances of timing belt failure in model year (MY) 2015-2017 Ford Fiesta and MY 2015-2018 Ford Focus vehicles manufactured by Ford Motor Company (Ford), equipped with a 1.0L engine and manual transmission (subject vehicles). Additionally, MY 2018-2021 Ford EcoSport and MY 2016-2018 Focus vehicles with a 1.0L engine and automatic transmission were analyzed as peer vehicles because they utilize a wet timing belt comprised of similar material as the timing belt in the subject vehicles. Through the PE, ODI investigated a potential failure/degradation of the timing belt that may affect oil pump function resulting in a lack of engine lubrication and potential loss of motive power. The complaints commonly allege a low engine oil pressure warning lamp immediately preceding a complete loss or reduction of motive power while the vehicle is in motion. Some complaints allege that a sudden loss of motive power or engine seizure occurred without warning or prior detectable symptoms. ODI’s analysis supports that this failure can occur without sufficient and advanced warning to the driver that a loss of motive power/engine seizure is imminent. ODI found that the timing belt material may degrade and create debris that clogs the mesh oil pump pick-up screen, resulting in reduced oil pressure in the engine. As additional debris accumulates on the screen, this can progress to a loss of motive power while driving due to a lack of engine lubrication. The failure data provided in response to this investigation revealed an average failure mileage of approximately 70,000 miles, and that 98% of the failures occurred prior to the 150,000 mile timing belt replacement interval per the vehicles’ maintenance schedule. Based on evidence collected via consumer surveys and an engine teardown, failures have occurred despite evidence of proper and routine oil maintenance. The oil pump pick-up screen is not serviceable, so debris accumulation cannot be inspected or manually removed without removing the engine from the vehicle. On June 2, 2026, Ford informed ODI of a non-safety customer satisfaction program for global vehicles with the 1.0L Fox Classic Timing Belt. This program will notify customers globally of a revised maintenance interval for the timing belt, reducing the original 150,000 mile interval down to 100,000 miles/6 years. This customer satisfaction program will also offer reimbursement to eligible customers who previously paid to have their engine repaired or replaced due to a timing belt related issue. Based on NHTSA’s analysis of the data, failure rates, information provided by Ford, preliminary engine teardown analysis, and precedent recalls regarding loss of engine oil pressure with the presence of driver facing warnings, ODI believes there is an unreasonable risk to motor vehicle safety. On December 18, 2025, the Office of Defects Investigation (ODI) opened PE25020 to investigate instances of timing belt failure in model year (MY) 2015-2017 Ford Fiesta and MY 2015-2018 Ford Focus vehicles manufactured by Ford Motor Company (Ford), equipped with a 1.0L engine and manual transmission (subject vehicles). Additionally, MY 2018-2021 Ford EcoSport and MY 2016-2018 Focus vehicles with a 1.0L engine and automatic transmission were analyzed as peer vehicles because they utilize a wet timing belt comprised of similar material as the timing belt in the subject vehicles. Through the PE, ODI investigated a potential failure/degradation of the timing belt that may affect oil pump function resulting in a lack of engine lubrication and potential loss of motive power. The complaints commonly allege a low engine oil pressure warning lamp immediately preceding a complete loss or reduction of motive power while the vehicle is in motion. Some complaints allege that a sudden loss of motive power or engine seizure occurred without warning or prior detectable symptoms. ODI’s analysis supports that this failure can occur without sufficient and advanced warning to the driver that a loss of motive power/engine seizure is imminent. ODI found that the timing belt material may degrade and create debris that clogs the mesh oil pump pick-up screen, resulting in reduced oil pressure in the engine. As additional debris accumulates on the screen, this can progress to a loss of motive power while driving due to a lack of engine lubrication. The failure data provided in response to this investigation revealed an average failure mileage of approximately 70,000 miles, and that 98% of the failures occurred prior to the 150,000 mile timing belt replacement interval per the vehicles’ maintenance schedule. Based on evidence collected via consumer surveys and an engine teardown, failures have occurred despite evidence of proper and routine oil maintenance. The oil pump pick-up screen is not serviceable, so debris accumulation cannot be inspected or manually removed without removing the engine from the vehicle. On June 2, 2026, Ford informed ODI of a non-safety customer satisfaction program for global vehicles with the 1.0L Fox Classic Timing Belt. This program will notify customers globally of a revised maintenance interval for the timing belt, reducing the original 150,000 mile interval down to 100,000 miles/6 years. This customer satisfaction program will also offer reimbursement to eligible customers who previously paid to have their engine repaired or replaced due to a timing belt related issue. Based on NHTSA’s analysis of the data, failure rates, information provided by Ford, preliminary engine teardown analysis, and precedent recalls regarding loss of engine oil pressure with the presence of driver facing warnings, ODI believes there is an unreasonable risk to motor vehicle safety. On September 20, 2023, the Office of Defects Investigation (ODI) opened PE23-015 to investigate instances of engine failure due to loss of oil pressure in model year (MY) 2018-2021 Ford EcoSport vehicles. Some consumers reported a low engine oil pressure warning lamp immediately preceding a complete loss or reduction of motive power while the vehicle was in motion. The investigation revealed that the engine oil pump drive belt tensioner arm may fracture, separate from the tensioner backing plate, and/or the oil pump drive belt may degrade and lose teeth, resulting in a loss of engine oil pressure. A loss of engine oil pressure can result in engine seizure, which can result in a loss of motive power and a loss of mechanical vacuum pump required to provide power braking assist, increase the risk of a crash. On December 22, 2023, Ford filed recall 23V-905 for certain MY 2016-2018 Ford Focus and 2018-2022 Ford EcoSport vehicles. The recall remedy provides the replacement of the oil pump drive belt tensioner assembly and oil pump drive belt with redesigned parts to improve robustness. In view of the recall action being taken by Ford, ODI is closing this Preliminary Evaluation. The agency reserves the right to take additional action if warranted by future circumstances. RQ23006 · Loss Of Motive Power

On September 15, 2023, the Office of Defect Investigation (ODI) opened this Recall Query (RQ) after receiving 98 consumer complaints alleging loss of motive power in model year (MY) 2012-2018 Ford Focus vehicles. The RQ focused on the effectiveness of the remedies of Recall 18V-735 and 19V-515. In October 2018, Ford issued Recall 18V-735, which updated Powertrain Control Module (PCM) software in MY 2012-2018 Ford Focus vehicles equipped with 2.0L GDI engines built prior to April 13, 2017, and those with 2.0L GTDI engines built prior to February 2, 2018. The recall addressed vehicles experiencing a malfunctioning Canister Purge Valve (CPV) that can stick open and PCM software that does not adequately detect a stuck-open CPV condition. A CPV that is stuck open can cause excessive vacuum in the fuel vapor management system, potentially deforming the fuel tank, collapsing on itself. This deformation of the fuel tank can cause a Malfunction Indicator Light (MIL) to illuminate, an inaccurate or erratic fuel gauge indication, drivability issues, and/or loss of motive power. In July 2019, Ford issued Recall 19V-515, which addressed certain MY 2012 and MY 2017 Ford Focus vehicles equipped with 2.0L GDI and MY 2013-2014 Ford Focus vehicles equipped with 2.0L GTDI engines whose PCM software had been reprogrammed as a part of recall 18V-735. The updated software calibrations for these subject vehicles were not properly installed by Ford at the time of production. Approximately one year after this RQ was opened, on September 25, 2024, Ford initiated a customer satisfaction program (24N07) for MY 2012-2018 Ford Focus vehicles equipped with the 2.0L GDI and GTDI engines to—as ODI learned during this investigation—address a different issue with the CPV from the primary focus of the safety recalls (18V-735 and 19V-515).  Ford stated the CPV may not operate as intended in these vehicles.  A failed CPV on its own merit, may cause drivability concerns such as engine hesitation, running rough, or stalling at low speed, which will be accompanied by an illuminated MIL. This program provides a one-time replacement of the CPV for vehicles with 15 years of service or less (with no mileage restriction). This coverage will automatically transfer to subsequent owners. It is to be noted that the customer satisfaction program being taken by Ford to address this RQ investigation results in a much different and significantly less severe problem than the safety action taken to remedy 18V-735 and 19V-515.  ODI found no further issues with the combined scope and severity related to 18V-735 and 19V-515 therefore, ODI is closing this Recall Query.
