I'VE HAD NUMEROUS PROBLEMS SINCE THE FIRST DAY I DROVE IT TO WORK. THE MOST SERIOUS INCIDENT OCCURRED AUG. 7, 2013, WHEN THE VEHICLE SUDDENLY SHUT DOWN WHILE MY WIFE AND I WERE DRIVING ON THE 101 FREEWAY IN AGOURA, CA. A SIMILAR INCIDENT OCCURRED ON OCT. 16, WHILE LEAVING WORK, AND ON OCT. 21, WHILE DRIVING TO WORK. ON ALL THREE OCCASIONS THE CAR WOULD SUDDENLY STOP, A STOP SAFELY NOW READ-OUT WOULD APPEAR, FOLLOWED BY A MECHANICS WRENCH SYMBOL. SEEKING TO LEARN IF OTHERS HAD THIS PROBLEM I CAME ACROSS THE ODI PRELIMINARY EVALUATION DOCUMENT. MY WIFE WERE LUCKY TO HAVE COME OUT UNSCATHED IN THE FREEWAY INCIDENT AND FRANKLY FORD COULDN'T GIVE [XXX] ONE AND I CURRENTLY OWN 4 FORDS. PLEASE STICK IT TO THEM BEFORE SOMEONE ISN'T LUCKY ENOUGH TO SURVIVE A "LIMP" CONDITION AS THEY DESCRIBE IT. AS FOR ME, I'M CURRENTLY SEARCHING FOR A LEMON-LAW LAWYER. INCIDENTALLY, THE OTHER 4 FORD VEHICLES, I NEVER HAD A PROBLEM WITH THEM. [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6). *TR

THIS IS FOR THE FORD FOCUS ELECTRIC. DRIVING IN THE LEFT LANE OF A 4 LANE ROAD IN MODERATE TRAFFIC, MY WIFE WAS SLOWING TO MAKE A LEFT HAND TURN ONTO A 4 LANE HWY. SUDDENLY THE VEHICLE CHIMED ONCE, SHOWED AN ORANGE CAUTION SIGN IN THE LEFT INFO DISPLAY THAT READ "PLEASE STOP SAFELY NOW," AND IMMEDIATELY SHUT DOWN MECHANICALLY. SHE INSTANTLY LOST CONTROL OF THE STEERING, BRAKES OR ENGINE AND THE CAR CAME TO AN ABRUPT STOP NEARLY CAUSING A COLLISION. SHE PUT THE CAR INTO PARK AND TRIED TO TURN IT OFF (THE INTERIOR ELECTRONICS THAT ARE POWERED BY THE STANDARD LEAD ACID BATTERY WERE ALL STILL ON AND FUNCTIONING) AND RESTART THE MOTOR, BUT THE VEHICLE WOULD NOT TURN OFF. IT WAS LUCKY IT STOPPED BEFORE THE INTERSECTION, IF IT HAD HAPPENED 3 SECONDS LATER IT WOULD HAVE BEEN PERPENDICULAR TO TRAFFIC ON A HIGHWAY THAT'S MOSTLY TRAFFICKED BY BIG RIGS AND DELIVERY TRUCKS AND SHE VERY WELL COULD HAVE BEEN KILLED. THERE WAS NO WAY FOR HER TO SAFELY GET OUT OF THE CAR, THERE WAS ONCOMING TRAFFIC AND WHEN THERE WASN'T THERE WERE CARS HONKING AND SWERVING TO THE LEFT AND RIGHT OF THE VEHICLE. WE HAD THE VEHICLE TOWED TO THE DEALERSHIP WHERE THEY DIAGNOSED THE PROBLEM. IT WAS A FAILED COOLING PUMP SO THE VEHICLE SHUT DOWN THE BATTERY PACK AND EVERYTHING DRIVEN BY IT (IE STEERING BRAKES AND THE MOTOR) TO PREVENT OVERHEATING. THIS HAS NOT BEEN THE ONLY CAUSE OF THE "STOP SAFELY NOW" SIGN AND SHUTTING DOWN THE CAR. INSTANTLY CUTTING OFF THE DRIVER FROM ALL THE BASIC CONTROLS OF A VEHICLE IS A VERY DANGEROUS WAY TO DEAL WITH FAULT CODES. *TR

I PREVIOUSLY REPORTED TO THE NHTSA TWO PREVIOUS OCCASIONS THAT THIS CAR SUDDENLY LOST POWER AND CAME TO AN ABRUPT STOP. THE SAME FAULT OCCURRED ON 10/25/13. I HAVE CONTACTED ALBANY FORD OF CALIFORNIA AND THE CAR WILL BE FLAT BEDDED BACK TO THEM TOMORROW FOR FURTHER ATTENTION. AFTER THE PREVIOUS OCCASION THEY HAD THIS CAR TO CORRECT THIS FAULT, NO PARTS WERE REPLACED OR REPAIRED BUT CODES WERE DOWNLOADED AND A DIAGNOSTIC TEST WAS PERFORMED. OBVIOUSLY THIS DID NOT CURE THE PROBLEM. I FILLED OUT A FORM ON THE FORD OWNER WEB SITE YESTERDAY AND HAVE HAD AN ACKNOWLEDGEMENT BY E-MAIL. THIS IS AN OBVIOUS SAFETY PROBLEM WHICH WAS NOT ADEQUATELY ADDRESSED PREVIOUSLY. *TR

I HAD PULLED THE CAR AROUND TO THE FRONT OF THE MOTEL THE CAR WAS STATIONARY, IN "PARK," WHEN THE "STOP SAFELY NOW" MESSAGE WAS DISPLAYED. I JUST POWERED THE CAR OFF AND POWERED IT BACK ON. THAT CLEARED THE SSN MESSAGE. THEN, I DROVE HOME, AS PLANNED, WITH NO RECURRENCES OF THE ERROR MESSAGE. TODAY, 10/24/2013, AT 4127 MILES, THE "STOP SAFELY NOW" MESSAGE WAS DISPLAYED (FOR ONLY THE SECOND TIME), WHILE I WAS DRIVING DOWN A HILL, AT ABOUT 30 MPH. THE CAR HAD BEEN SITTING IN THE PARKING LOT, AT WORK, AND THE SSN ERROR OCCURRED AFTER DRIVING ABOUT 1 MILE. THERE WAS NO PLACE TO PULL OVER, WITHOUT BLOCKING TRAFFIC, BUT, SINCE I WAS GOING DOWNHILL, I JUST CONTINUED COASTING, MADE MY USUAL RIGHT TURN AT THE BOTTOM OF THE HILL, AND PULLED OVER, AT A SPOT WHERE THERE WAS A VERY WIDE, PAVED SHOULDER. SINCE IT WAS TOO LATE IN THE DAY (5:30 PM) TO GET THE CAR (WITH THE POWER STILL ON AND THE SSN MESSAGE STILL DISPLAYED) TOWED TO THE LOCAL FORD DEALER (WHICH CLOSES AT 6 PM), I POWERED THE CAR OFF AND, THEN, POWERED IT BACK ON. THE SSN ERROR, AGAIN, WENT AWAY, AND I WAS ABLE TO SAFELY DRIVE HOME, WITH NO ADDITIONAL SSN MESSAGES. BECAUSE OF A CONSTRUCTION DETOUR, LATELY, I HAVE BEEN DRIVING TO WORK AND BACK, ON A MAJOR HIGHWAY, THAT HAS A SPEED LIMIT OF 65 MPH. NUMEROUS STRETCHES OF THAT HIGHWAY ARE BOTH UPHILL AND HAVE LITTLE OR NO ROOM TO PULL OVER. THE LAST PART OF MY TRIP HOME IS US A STEEP HILL, WITH LOTS OF CURVE AND MOSTLY VERY NARROW SHOULDERS. *TR

MY VEHICLE DISPLAYED A "STOP SAFELY NOW" WARNING DURING MY NORMAL MORNING COMMUTE. THE WARNING WAS PRECEDED BY A FAILURE OF THE RADIO PORTION OF THE SYNC SYSTEM. I BRIEFLY TURNED OFF THE VEHICLE AT A USUAL STOP ON MY ROUTE AND UPON TURNING IT ON, THE RADIO VOLUME, POWER AND TUNING WERE ALL UNRESPONSIVE. I ATTEMPTED TO RESET THE VEHICLE AT THE NEXT TRAFFIC LIGHT BY POWERING DOWN AND OPENING THE DOOR, BUT THE RADIO REMAINED ON EVEN AFTER THE VEHICLE WAS OFF AND THE DOOR WAS OPENED. I ATTEMPTED THIS SEVERAL MORE TIMES AT EACH SUCCESSIVE TRAFFIC LIGHT AND THE RADIO WOULD NOT SHUT OFF. AFTER ONE SUCH RESET, THE STOP SAFELY NOW WARNING WAS DISPLAYED ON THE DASH. I WAS STOPPED AT A TRAFFIC LIGHT AT THE TIME AND WAS ABLE TO RESET THE VEHICLE AND SAFELY ENTER A PARKING LOT AND WAIT FOR A TOW - WITH THE RADIO STILL ON. MY LOCAL FORD DEALER WAITED THREE DAYS TO DIAGNOSE THE ISSUE AND UNSURPRISINGLY DETERMINED IT WAS CAUSED BY A DEFECTIVE/DEAD 12 V BATTERY AND REPLACED THE BATTERY. SOFTWARE WAS ALSO UPDATED. UPON RETURNING HOME AND ATTEMPTING TO CHARGE, MY VEHICLE DISPLAYED A HIGH VOLTAGE SYSTEM FAULT AND HAD TO BE TOWED TO THE DEALER AGAIN. THE DEALER SAID THIS FAULT WAS CAUSED BY A LOOSE CONNECTION ON A TERMINAL BLOCK ON THE ELECTRIC MOTOR THAT WAS USED TO SIGNAL BATTERY CAPACITY TO THE ON-BOARD COMPUTER. *TR

In December 2019, the Office of Defects Investigation (ODI) opened this investigation based on 235 Vehicle Owner Questionnaires (VOQs) alleging doors failed to latch when closed, and/or inadvertently opened while driving, on model year (MY) 2012-2014 Ford Fiesta, and 2013-2014 Fusion and Lincoln MKZ. Some VOQs reported the failure occurred after the latch had been remedied under a prior recall which involved fracture of the latch pawl spring tab due to thermally induced cyclic fatigue. Other VOQs alleged a similar failure in vehicles not previously recalled. A latch with a fractured pawl spring tab typically results in a ?door will not close? condition, however in certain somewhat rare circumstances, may result in the door coming open while driving. In response to ODI?s investigation, Ford has announced three field actions, as discussed below. On March 23, 2020, Ford submitted Safety Recall 20V-177 (Ford 20S15), a regional recall campaign to address door latch failures in vehicles not previously recalled under Safety Recalls 15V-246, 16V-643 and 17V-210. The recall affects vehicles currently or ever registered in certain high ambient temperature states (see 20V-177 for affected states). As a remedy, Ford will replace all four door latches free of charge. On August 24, 2020, Ford issued a non-safety regional field action (Ford 20R01) to address door latch failures on the same model/MY vehicles not covered under 20V-177 (i.e., not currently/ever registered in affected states). According to Ford, spring tab failure rates for vehicles covered by this action are about half that of the 20V-177 vehicles. Under this action, dealers will replace all door latches, free of charge, if either; a) the vehicle exhibits a related latch concern, or b) if the vehicle owner expresses a concern about the latches in their vehicles. Details can be found under NHTSA TSB No. 10190990, a copy of which is available in this investigative file available at NHTSA.gov.On June 8, 2020, Ford submitted Safety Recall 20V-331 (Ford 20S30) addressing vehicles previously remedied under NHTSA Recalls 15V-246, 16V-643 and 17V-210, where the repair may not have been properly completed, specifically not all of the door latches were replaced, or replaced correctly. This recall involves an inspection of the latches, with the option for a consumer self-inspection; owners could view the date codes on their latches (and confirm child safety locks are present on the rear doors) and then submit the information to Ford?s mobile app or website. A dealership inspection option was also available. Any vehicle found to have unreplaced defective latches requires the replacement of all door latches as directed by Ford.Regarding the 4,494 reports cited above, 1,485 involve scope-related failures (with 13% alleging door opening), and the remaining 3,009 reports involving remedy concerns (with 18% alleging door opening - note ODI was unable to verify the allegations of door opening). The three ODI injuries were all relatively minor. Ford?s reported injury incidences include one (unconfirmed) report of an ejection, three minor injuries with the others involving undisclosed injuries. Ford?s crashes involved two (unconfirmed) crashes alleging the incident door contacted another vehicle and three involving doors striking other property. The recalled vehicles include MY 2011-2015 Ford Fiesta, 2013-2016 Ford Fusion and Lincoln MKZ, 2015 Ford Mustang and Lincoln MKC, 2013-2015 Ford Escape and Ford C-MAX, 2012-2015 Ford Focus and 2014-2016 Ford Transit Connect.Ford's actions address ODI's concerns at this time. In an October 28, 2013 letter to NHTSA, Ford Motor Company (Ford) submitted a recall notice identifying a defect in the Powertrain Control Module (PCM) software in approximately 2,456 model year (MY) 2012 through 2014 Ford Focus Electric vehicles manufactured from September 15, 2011 through August 23, 2013 (Recall No. 13V-523).According to Ford, PCM software faults may cause a complete loss of powertrain torque. When this condition occurs, drivers receive a red triangle indicator and the message Stop Safely Now in the instrument cluster. In addition, vehicle brake and steering systems will continue to operate normally and the vehicle can be restarted after going through a shutdown process.Ford and its supplier, Magna Powertrain, upgraded the PCM software to address powertrain shutdown conditions and issued a technical service bulletin on September 23, 2013 instructing dealers to reprogram the PCM on all affected vehicles. The Office of Defects Investigation (ODI) analyzed complaint data provided by Ford as well as complaints submitted to ODI from consumers. In total, there were 171 unique reports indicating loss of motive power while driving, and the predominant failure mode identified involved loss of powertrain torque.Ford began owner notification on November 5, 2012.Owners of the recalled vehicles are notified to take their vehicle to a Ford or Lincoln dealer who will reprogram the PCM to the latest calibration. See the investigative public file for copies of Ford's bulletin and owner letter. This preliminary evaluation is closed. The ODI reports cited above can be viewed at www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the following identification numbers (ODI Nos.): 10536160,10534988,10534323,10531894,10525393,10525187,10523536,10521207,10516270,10512459,10512146,10509307,10543015,10542983,10542840,10542753,10538223,10537801,10537111,10537048,10536894,10536592,10536538,10536510,10526350,10524026,10522585,10521986,10521459,10519446,10545282.
