The contact stated that while driving at various speeds, the vehicle unintendedly decelerated and lost power. The contact stated that the failure occurred while driving on a busy highway, which could have caused someone to be seriously injured. The contact stated that the incident was significantly scary, and the Police Officer arrived after 21 minutes. The contact stated that there were no warning lights illuminated prior to the failure. The contact stated that the check engine warning light illuminated after the failure occurred. The vehicle was towed to the dealer, where it was diagnosed that the fuel pump had failed, causing metal intrusion in the entire fuel system. Biodiesel deposits may form on the pump drivetrain roller components, which could lead to failure of the high pressure fuel pump.
Consequence & remedy
Consequence: High pressure fuel pump failure can cause a loss of drive power, increasing the risk of a crash.
Remedy: Dealers will update the powertrain control module (PCM) software, free of charge. Owner notification letters were mailed mailed April 7, 2026. Ford Motor Company (Ford) is recalling certain 2020-2022 F-250 SD, F-350 SD, and F-450 SD vehicles. The 360-degree view camera system may not display a rearview image properly in certain lighting conditions.
Consequence & remedy
Consequence: A rearview camera image that does not display properly can reduce the driver’s view behind the vehicle, increasing the risk of a crash.
Remedy: Dealers will update the image processing module software, free of charge.This will be a phased campaign, with the remedy becoming available in different phases based on model years. Interim letters notifying owners of the safety risk were mailed on October 30, 2025. Additional letters will be mailed once the remedy is available on each phase. A software error may cause the rearview camera to display a blank image, or the image may remain on the display after the backing event has ended.
Consequence & remedy
Consequence: A rearview camera that displays a blank image can reduce the driver's view behind the vehicle. A rearview image that continues to be displayed after the vehicle is shifted out of reverse may distract the driver. Both of these situations can increase the risk of a crash.

As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 202, "Head Restraints."
Consequence & remedy
Consequence: Without instructions, the head restraints may be improperly adjusted, increasing the risk of injury in the event of a crash.
Remedy: Ford will mail the Owner's Manual addendums that describe how to adjust and remove the head restraints for the center seating positions, free of charge. Consequence: A fractured driveshaft can cause a loss of drive power, or a loss of vehicle control if the driveshaft contacts the ground. Additionally, unintended movement could occur while parked if the parking brake is not engaged. Any of these scenarios can increase the risk of a crash.

Consequence: Over cured tires may develop a break in the sidewall, resulting in sudden air loss or belt edge separation which could lead to tread/belt loss. Either condition can cause a loss of vehicle control, increasing the risk of a crash.

The transmission torque converter damper plate was not tempered, resulting in a brittle damper plate that may fracture and cause the engine to stall or require increased brake pedal effort in order to stop the vehicle.

Consequence: An engine stall or requiring additional braking effort to stop the vehicle can increase the risk of a crash.

The Office of Defect Investigations (ODI) has received 27 complaints alleging diesel fuel leaks due to a fracture in the secondary fuel filter housing in model year (MY) 2015-2021 Ford Motor Company (Ford) F-250, 350, 450, and 550 Super Duty vehicles equipped with 6.7L diesel engines. Of these complaints, 12 describe an engine compartment fire. The proximity of the secondary fuel filter to high heat sources creates a potential for autoignition thermal events and fires. ODI has contacted the vehicle manufacturer, Ford, and the filter manufacturer, Allevard Sogefi USA (Sogefi), and learned that the low-pressure fuel system is capable of a maximum operating pressure of 8.5 Bar, while the secondary fuel filter is only tested to seven (7) Bar during the manufacturing process. Additionally, changes starting in 2022 by both Ford and Sogefi align with a lack of similar complaints on MY 2022 and newer vehicles. ODI is opening this Preliminary Evaluation (PE) to evaluate the severity of the potential problem and to fully assess the potential safety-related problems. On September 23, 2020, Ford Motor Company (Ford) filed a noncompliance recall report under 49 CFR Part 573 (NHTSA Recall No. 20V-575), recalling 620,246 Model Year (MY) 2020 Ford F-150, F-250, F-350, F-450, F-550, Explorer, Mustang, Transit, Expedition, Escape, Ranger and Edge, and Lincoln Nautilus and Corsair vehicles for a noncompliance with Federal Motor Vehicle Safety Standard (FMVSS) 111, Rear Visibility. Ford's recall described the noncompliance as insufficient electrical conductivity within the printed circuit board (PCB) internal to the camera leading to intermittent rearview camera operation. The supplier of the cameras was Magna Electronics.According to Ford's chronology in the Part 573, during the period February-April 2020, Ford identified and monitored warranty claims pertaining to intermittent or inoperative rearview cameras. Ford reviewed supplier product changes, focusing on changes introduced by the supplier in the October-November 2019 time frame. The Part 573 stated that the issue was brought to Ford's Critical Concerns Review Group (CCRG) for review on May 12, 2020.On July 7, 2020, NHTSA's Vehicle Research and Testing Center (VRTC) informed the Office of Defects Investigation (ODI) of a trend of MY 2020 Escape vehicles with rearview camera malfunctions. ODI contacted Ford the next day, providing five Vehicle Owner Questionnaires (VOQs) alleging inoperative rearview cameras. On July 15, 2020, Ford met with NHTSA to discuss the rearview camera issue. In a subsequent August 24, 2020 meeting, ODI and NHTSA's Office of Vehicle Safety Compliance (OVSC) met with Ford to discuss the recall status and additional VOQs. Despite this, Ford began developing ways to prevent uncommanded tailgate openings from various human, mechanical or electrical causes. Based on testing, Ford issued a Customer Satisfaction program, 21M05-S1/S2/S3, to further address the potential of unintended tailgate openings.
