NHTSA Safety Complaint Draft Vehicle Information: Make: Ford Model: F-150 Year: 2018 VIN: [XXX] Incident Summary: While driving my 2018 Ford F-150, the vehicle suddenly displayed a warning on the dashboard: “Steering Assist Fault – Service Required.” Within moments of this warning, I lost all power steering assist and was unable to control the vehicle, resulting in the truck veering off the road and into a ditch. This was a sudden and dangerous failure of the Electric Power Steering (EPS) system. I was fortunate not to have been injured, and no other vehicles were involved. However, this type of failure represents a major safety risk to me and to others on the road. I contacted Ford Motor Company and was assigned Case Number: [XXX]. Despite reporting the issue and explaining that the vehicle ran off the road, Ford refused to offer any assistance or goodwill repair. I was told the vehicle is outside of recall 24S45, even though it displays the exact same failure symptoms as recalled vehicles from later years. This failure could have resulted in serious injury or death. I am requesting that NHTSA investigate this steering system failure and expand any related recall coverage to include affected 2018 F-150 models like mine. Maybe there are others who are experiencing the same out there. My vehicle is well maintained, in fact I just put upwards to $8k into it this year.

Component/System That Failed: The 10R80 automatic transmission in my 2018 Ford F-150 3.5L EcoBoost has become essentially undrivable at 89,900 miles. The transmission will attempt to upshift from 1st gear through each gear unsuccessfully until it reaches 6th gear, at which point it refuses to downshift. The only way to exit 6th gear is by taking the vehicle completely out of drive. This transmission is available for inspection upon request. Safety Risk: This failure creates a major safety hazard. The inability to accelerate properly from a stop or to downshift while slowing down means the truck cannot respond to normal traffic conditions or emergency situations. This puts me, my passengers, and others at risk—especially when entering traffic, making turns, or attempting to stop. The erratic behavior also causes the vehicle to lurch or hesitate, increasing the chance of a rear-end collision or loss of control. Has the Problem Been Reproduced or Confirmed? Yes, the issue has been confirmed by a certified Ford dealership. They diagnosed internal transmission failure and quoted $8,876.27 for a full transmission replacement. Has the Vehicle or Component Been Inspected by Others? The vehicle has been inspected by a Ford dealership service department. No inspections have been done by law enforcement or insurance, and the manufacturer has not taken direct action, despite this being a well-documented issue with multiple TSBs and class action lawsuits tied to the 10R80 transmission. Warning Signs or Symptoms: The issue began gradually around 75,000 miles with hard or delayed shifting and worsened significantly by 89,900 miles. there was no warning light or messages until after the transmission gave out in the middle of a busy intersection. When the headlight switch is turned from "Autolamps" to "Headlamps On," the Daytime Running Lamps (DRLs) may remain fully bright instead of dimming to parking lamps as intended. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 108, "Lamps, Reflective Devices and Associated Equipment."
Consequence & remedy
Consequence: Daytime Running Lamps that do not dim may reduce the visibility of other drivers, increasing the risk of a crash.

Quickly moving the shift lever from Park to Drive may cause a loss of gear indication on the instrument cluster display and a momentary selection of an unintended gear, such as Reverse or Neutral.
Consequence & remedy
Consequence: Unintended gear selection may cause the vehicle to move in an unexpected direction, increasing the risk of crash or injury.

Ford Motor Company (Ford) is recalling certain 2018 F-150 vehicles with manual and power release tailgates. The tailgate latches may unlatch unexpectedly while driving.
Consequence & remedy
Consequence: Unexpected tailgate opening may allow unrestrained cargo to fall out of the truck bed and become a road hazard, increasing the risk of a crash.
Remedy: Dealers will replace the left-hand and right-hand tailgate latches, free of charge. An interim notification letter was mailed January 31, 2022. Consequence & remedy
Consequence: Over cured tires may develop a break in the sidewall, resulting in sudden air loss or belt edge separation which could lead to tread/belt loss. Either condition can cause a loss of vehicle control, increasing the risk of a crash.

During the previous repair, the block heater cable's splice connector may have been damaged.
Consequence & remedy
Consequence: A damaged cable could cause a resistive short, increasing the risk of overheated or melted wiring, increasing the risk of a fire.
Remedy: Ford will notify owners, and dealers will disable the engine block heater from being used. When replacement cables become available, a second notice will be mailed and dealers will replace the engine block heater cord, free of charge. The recall began May 31, 2019. During the previous repair, the block heater cable's splice connector may have been damaged.
Consequence: A damaged cable could cause a resistive short, increasing the risk of overheated or melted wiring, increasing the risk of a fire.
Remedy: Ford will notify owners, and dealers will disable the engine block heater from being used. Ford Motor Company (Ford) is recalling certain 2018 F-150 vehicles equipped with 3.5L engines. The cylinder heads may have been machined without lubrication supply holes for the camshaft bearing journals.
Consequence & remedy
Consequence: Without a proper supply of lubrication, the engine will fail, causing a stall and increasing the risk of a crash.
Remedy: Ford is not aware of any affected vehicles outside of dealer control. On July 29, 2024, the Office of Defects Investigation (ODI) opened PE24019 to investigate complaints of low-pressure fuel pump failures in MY 2021 Ford Bronco vehicles. Some consumers reported that the failure occurred without warning while driving, resulting in a complete loss of motive power with no ability to restart the engine. Ford determined that several other vehicle models contain low-pressure fuel pumps substantially similar to the 2021 Ford Bronco, including the 2022-2023 Ford Bronco, 2018-2023 Ford F-150, 2022-2023 Ford Expedition, 2019-2023 Lincoln Navigator, 2020-2023 Ford F-250-F-550, 2024 Ford Super Duty Chassis Cab, 2024-2025 Ford Ranger, 2020-2023 Ford Explorer, 2020-2023 Lincoln Aviator, and 2021-2025 Ford Mustang. ODI's investigation found that the affected vehicles may lose fuel pressure and flow from the fuel delivery module (consisting of a low-pressure fuel pump, jet pump, reservoir, and other parts) due to a low-pressure pump failure. This can cause the engine to stall while driving. The root cause is reduced internal clearance in the fuel pump, increasing friction and sensitivity to vapor lock. Fuel blockage from contamination inside the jet pump also contributed to the failure. Ford stated that the contamination source was unknown. On July 7, 2025, Ford filed recall 25V-455 for certain 2021-2023 Ford F-550 SD/F-450 SD/F-350 SD/F-250 SD, 2021-2022 Lincoln Navigator, 2021-2022 Ford Mustang, 2021-2022 Ford F-150, 2021-2023 Ford Explorer, 2022 Ford Expedition, and 2021-2023 Ford Bronco vehicles produced between July 1, 2021, and July 30, 2022. The recall also included certain 2021-2023 Lincoln Aviator produced between July 1, 2021, and December 21, 2022. Ford explained in its recall filingthat the production dates used for the recall were based on changes in the manufacturing process. Specifically, Ford determined that in June 2021, the supplier modified the jet pump manufacturing process, producing fuel pump chambers with a lower-than-specified internal clearance. In July 2022, the supplier implemented corrective actions to the manufacturing process by adding two additional vacuum pumps to reduce contamination during the production of the jet pump. ODI has identified approximately 1,000 reports involving vehicles covered in this investigation but excluded from recall 25V-455. However, even with such reports, these vehicles experienced comparatively lower failure rates than the vehicles that are subject to Recall 25V-455. Ford has explained that these vehicles were excluded from the recall population because they did not experience the manufacturing changes that prompted the recall. Specifically, according to Ford, vehicles built before July 1, 2021 (the recall production start date), were not affected by the supplier’s change to the jet pump manufacturing process. Even after closing this investigation, ODI will continue to monitor complaints for vehicles excluded from recall 25V-455 to consider whether they exhibit the same risks as the vehicles encompassed by the recall. The recall remedy is currently under development. In view of Ford’s recall action, ODI is closing this Preliminary Evaluation, but will continue to monitor the issue as a remedy is developed and implemented. On June 18, 2018, the Office of Defects Investigations (ODI) opened a Preliminary Evaluation (PE18-008) to investigate complaints alleging post-crash fires at the B pillar in the area of the seat belt pretensioners on MY 2014-2018 Ford F-150 Regular Cab, and Super Crew Cab Vehicles. On August 24, 2018, Ford Motor Company recalled (FSA 18S27 - NHTSA #18V568) the affected population. Ford's investigation found that some front seat belt pretensioners may generate sparks during deployment in excess of Ford's specification. Sparks in the pretensioner exhaust gases inside the lower portion of the B-pillar may subsequently ignite combustible components contained in the B-pillar, such as insulation or carpet, and may cause a subsequent vehicle fire. ODI also requested relevant information from other manufacturers to assess the potential scope of post-crash pretensioner fires.
