On August 19, 2026, I was involved in a collision in my 2025 Ford F-150 Lightning after a suspected malfunction involving the automatic emergency braking, adaptive cruise control, and/or acceleration controls. Cruise control was on; BlueCruise was not. While on an exit ramp, I heard a loud warning beep, experienced automatic braking, and then felt the truck suddenly accelerate into the vehicle ahead. The unexpected braking and acceleration compromised my ability to control the truck and avoid a collision, putting me and occupants of the other vehicle at risk. I am concerned about driving this vehicle again, including with my daughter. Before the collision, I experienced three incidents in which the automatic brakes activated when no vehicles were around. I also reported a warning/error during a December 2025 service visit. The service record documents that visit; I do not know whether that issue is related to the collision. The loud warning beep occurred immediately before the collision. The truck has been at a Ford dealership for evaluation and is available for inspection upon request. The dealership has not confirmed or reproduced the suspected malfunction. The service manager told me its diagnostic testing would not necessarily show what happened during the incident and indicated limitations in obtaining crash-related data. I am requesting further investigation into the braking, cruise control, and acceleration systems. Ford Corporate and State Farm have been notified. I cannot confirm whether Ford Corporate, police, or insurance representatives have performed a technical inspection of these systems or retrieved event data. The cause remains unconfirmed.

On September 19, 2025, my wife was at the wheel of our 2025 Ford F-150 Lightning (VIN: [XXX] ), a vehicle we had leased just months earlier on June 22, 2025, from Sam Galloway Ford at [XXX] . Traveling at a mere 15-20 mph when she was involved in a routine multi-vehicle collision (no citations were issued). No collision warning was sounded. The front-end impact triggered an instantaneous and ferocious eruption of flames from the truck's forward section. The vehicle was swiftly consumed in a raging inferno, leading to its complete and irreparable destruction. My wife narrowly escaped with her life, scrambling from the cab amid the explosive intensification of the fire while relying on the urgent assistance of bystanders to reach safety—mere seconds from what could have been a fatal entrapment. She endured profound terror, acute anxiety, and an overwhelming fear for her very survival in the face of this abrupt and merciless blaze. Responding emergency crews, including the fire department, pinpointed the origin at the vehicle's front end, aligning with hallmarks of a catastrophic battery or electrical malfunction.
