The contact owned a 2018 Ford Explorer. The contact stated that while driving at various speeds, a message displayed on several occasions instructing the driver to pull over and shut off the vehicle because the battery would shut off. Upon pulling over and turning off the vehicle, the vehicle failed to restart and required towing on several occasions. The vehicle was towed to the dealer, where a wiring failure was determined. The dealer attempted to repair the vehicle, but the failure persisted. While driving approximately 25 MPH, the infotainment screen turned blank, and a large wrench warning light illuminated. The vehicle independently accelerated, became uncontrollable, and drifted out of the intended lane. The contact regained partial control as the vehicle continued accelerating toward an embankment. The front driver's side wheel was caught in the gutter of the embankment, causing the driver's side of the vehicle to crash into the embankment. The contact's head struck the driver's side window, which was fractured, allowing branches to enter the cabin. The driver's side curtain air bags deployed for the second and third rows but failed to deploy properly for the front driver's side, causing the contact to hit her head a second time on a sail that had deployed. The contact became pinned between the driver's seat and the driver's side window area, with her hair caught and pulled in the seat belt pretensioner, causing significant pain. EMT personnel cut the contact's hair to free the contact from the vehicle. The contact was assisted from the vehicle, fitted with a neck brace, placed on a stretcher, and transported to the hospital. The contact sustained head and neck injuries, as well as a concussion. Additionally, the contact suffered from pre-existing auto-immune lumbar stenosis L1–L5, osteoarthritis L5–S1 with bone-on-bone contact, and sacroiliac joint disease, and experienced increased pain after missing a scheduled appointment for sacroiliac joint injections due to the crash. - I have experienced a sudden loss of engine power and speed on 5 occasions. Very scary and dangerous to completely lose power on the highway with cars behind you. Had to pull off onto the shoulder(s) in dangerous conditions, with two young kids in the backseat. Hazards, turn off the car, restart the car, etc. - When the power loss happens, the vehicle's dashboard lights turn on, including the engine and wrench icons along with the words "See Manual." - Based on my research, I believe it may be due to a defective throttle body and/or software, related to the "terrain management system." - Problem was not reproduced or confirmed by a dealer or independent service center - No inspections by the manufacturer, police, insurance representatives or others - No warning message is provided prior to the failure, which is another issue

On September 27th 2023 I brought my vehicle in because it was having shifting and vibration issues. To shop notice that the information I gave them was related to the transmission and with further review there was a recall on the transmission part of me coming into the shop. Which I wasn't aware of even when I brought my vehicle in months before that for recall maintenance. I had to pay for my transmission to be repaired we should have been done by Ford. The car was driving fine until early this year when it starts to do the vibration that was the issue previous when my transmission was going out. So I set up appointment at Ford dealership while I was in route to the dealership my car started to sputter and shake and fall out of gear. So I turned around tried to make it home in the car just stopped moving. I wasn't able to move forward or reverse there is no connection within the transmission for my car to move. I got my car towed to the dealership who diagnosed my car and my transmission and they stated that there was metal fragments found in the fluid level and I also said that the torque converter clutch performance never engage when it was testing it. I told him about my previous transmission issue with Ford and I also States them that four has have many federal recalls dealing with their transmissions in their vehicle.

The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. Since the car was new I experienced occasional situations where the car would not accelerate. I thought it might be the transmission slipping as it seemed to "bottom out" when hitting the gas. The car would not respond and I would coast onto the side of road for fear of getting hit. After a minute or so, the gas pedal would work and I could keep moving. I reported it to the dealer and at that time it was winter and the roads were icy so they were not able to test it. Nothing showed up on diagnostics. It has been happening more frequently (twice since June 2022). Upon further research it seemed to be the throttle body so this week I took it to the Ford dealer where I purchased the car and told them what was happening. They confirmed that it is a bad throttle body. They are now replacing it at a cost of $781. I have found numerous sites on the internet that this is a well-known problem and in some cases it seems that Ford issued an extended warranty on defective throttle body assemblies and sensors for 10 years or 150,000 miles from the original purchase date. So why does this not apply to me? This is very dangerous and I was lucky to not get hit. When the car stops accelerating in the middle of traffic, it would be easy to get rear ended or worse.

THE FIRST TIME THIS HAPPEN, I WAS IN A COSTCO PARKING LOT, SO I DID NOT THINK MUCH OF IT. THE SECOND TIME THIS HAPPEN, JULY 12TH, 2020, I WAS CROSSING A BUSY ROAD OF TRAFFIC NEAR MY HOME AND WAS ALMOST INVOLVED IN A SERIOUS ACCIDENT AS A RESULT OF THIS FAILURE. AS I WAS CROSSING THE 2 LANES OF TRAFFIC TO MAKE A LEFT HAND TURN, MY VEHICLE STALLED AND I WAS ALMOST SIDE SWIPED BY AN ONCOMING VEHICLE. AFTER THIS INCIDENT, I HAD MY VEHICLE TOWED TO A FORD DEALERSHIP FOR INSPECTION. UPON EXTENSIVE ANALYSIS OF THE VEHICLE, THE TECHNICIANS COULD NOT FIND ANYTHING WRONG WITH THE VEHICLE INCLUDING NO FAILED SENSORS IN THE COMPUTER AND THE TECHNICIANS WERE NOT ABLE TO REPLICATE THE CONCERN. ON AUGUST 19TH, 2020, MY CHECK ENGINE LIGHT CAME ON AND I IMMEDIATELY RETURNED THE VEHICLE TO FORD DEALERSHIP HOPING THIS WAS A RESULT OF THE ISSUES I HAD BEEN HAVING. I HAVE HAD TWO CARS WITH ADAPTIVE CRUISE AND THEY HAVE BOTH WORKED DOWN TO ZERO MPH. DURING OUR FIRST DRIVE USING THE SO-CALLED "ADAPTIVE CRUISE", WE WERE FOLLOWING A CAR ON THE FREEWAY GOING 70MPH. RATHER SUDDENLY, THAT CAR SLOWED. OUR FORD WAS SLOWING DOWN BEAUTIFULLY; YOU COULD FEEL AND SEE THE EFFECTS OF THE AUTOMATIC BRAKING. THEN, SUDDENLY AND WITHOUT NOTICABLE WARNING TO US, THE BRAKING QUIT AND OUR CAR WAS COASTING TOWARDS THE CAR IN FRONT. WE NARROWLY MISSED A LOW SPEED REAR-END COLLISION. I WOULD CALL THIS CRUISE CONTROL A "PARTIAL" ADAPTIVE CRUISE CONTROL. I REALLY FEEL THIS ASPECT OF THE ADAPTIVE CRUISE SHOULD BE MORE PROMINENT IN ALL OF THE LITERATURE AS WELL AS ON THE WINDOW STICKER.. I CANNOT IMAGINE WHAT WOULD HAVE HAPPENED HAD THE DECELERATION BEEN EXTREMELY FAST FROM 70MPH AND OUR CRUISE CONTROL DISENGAGED AND THE BRAKING SUDDENLY QUIT! THIS IS VERY DANGEROUS WHEN YOU ARE USED TO A CAR THAT WILL CONTINUE BRAKING ALL THE WAY TO A STOP. I DO CONSIDERABLE RUSH HOUR DRIVING AND AM IN STOP AND GO TRAFFIC CONSTANTLY. OTHER CAR MANUFACTURERS ARE ABLE TO BUILD CARS WITH A CRUISE THAT GOES DOWN TO ZERO. WHY NOT FORD? I WOULD EVEN HOPE THAT A SOFTWARE/FIRMWARE UPDATE COULD MAKE MY CAR CRUISE GO DOWN TO ZERO. THAT WOULD BE SUCH A GREAT SAFETY UPDATE TO OFFER CUSTOMERS. I BELIEVE STANDARDS SHOULD REQUIRE ZERO MPH CAPABILITY FROM ADAPTIVE CRUISE CONTROLS AND WOULD HOPE FORD COULD BE MADE TO FIX THIS RATHER DANGEROUS DEVICE. ANY MANUFACTURER USING SUCH A PARTIAL ADAPTIVE CRUISE SYSTEM SHOULD BE MADE TO NOTE THAT PROMINENTLY ON THE WINDOW STICKER AND IN THE LITERATURE.

Consequence & remedy

Consequence: A detached roof rail cover can create a road hazard, increasing the risk of a crash.

Remedy: Dealers will inspect the roof rail covers and as necessary, repair or replace the push-pins, rail clips, or roof rail covers, free of charge. Interim letters, notifying owners of the safety risk, were mailed August 28, 2026. Additional letters will be sent once the remedy is available, anticipated in January 2027. Ford Motor Company (Ford) is recalling certain 2017-2019 Explorer vehicles. The rear suspension toe links may fracture.

Consequence & remedy

Consequence: A rear toe link fracture can result in a loss of steering control, increasing the risk of a crash.

Remedy: Dealers will replace the toe links, free of charge. Owner notification letters were mailed on March 16, 2026. Owners may contact Ford Customer Service at 1-866-436-7332. Ford's number for this recall is 26S08. This recall is an expansion of previous NHTSA recall number 21V537. Ford Motor Company (Ford) is recalling certain 2016-2019 Lincoln MKC, 2016-2023 Explorer, 2019-2020 Fusion, 2019-2024 Ranger, 2020-2022 Lincoln Corsair, Escape, 2021-2024 Bronco, Bronco Sport, and 2022-2024 Maverick vehicles. The engine block heater may crack and develop a coolant leak, causing it to short circuit when the block heater is plugged in.

Consequence & remedy

Consequence: An electrical short circuit can increase the risk of a fire.

Remedy: Owners are advised not to plug in their block heater until the vehicle is remedied. Dealers will replace the block heater, free of charge. Owners will also have a alternative option to replace engine block heater element with a threaded blanking plug, and remove the block heater electrical cord. Interim letters, notifying owners of the safety risk, were mailed December 3, 2025. Additional letters will be sent once the final remedy is available, anticipated September 2026. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 25SA4. Ford Motor Company (Ford) is recalling certain 2011-2019 Explorer vehicles. The A-pillar trim retention clips may not be properly engaged, allowing the trim to detach.

Consequence & remedy

Consequence: A detached trim piece can fall off the vehicle, becoming a road hazard and increasing the risk of a crash.

Remedy: Dealers will inspect and replace the A-pillar trim as necessary, free of charge. This will be a phased campaign, with the remedy becoming available in different phases based on model years. Owner notification letters were mailed between July 18, 2024 and December 16, 2025. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 24S02.

Ford Motor Company (Ford) is recalling certain 2016-2019 Explorer vehicles. The retention pins could loosen and allow the roof rail covers to detach from the vehicle.

Consequence & remedy

Remedy: Dealers will install push-pins and replace any damaged rail clips and roof rail covers, as necessary, free of charge. The interim owner notification letter was mailed on June 22, 2021. Also included are certain 2020 Ford Explorer and Lincoln Aviator vehicles equipped with rear outboard seats and manual seat back recliner mechanisms. The recliner mechanisms may be missing the third pawl required for seat back strength, resulting in a loose seat back. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints" and 207, "Seating Systems."

Consequence & remedy

Consequence: A seat back with an improperly assembled recliner mechanism may have reduced strength and may not adequately restrain an occupant in a crash, increasing the risk of injury.

Remedy: Ford will notify owners, and Ford and Lincoln dealers will inspect the seat structure and replace them, as necessary, free of charge. The recall began October 21, 2019. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 19C07.

PE23001 · Windshield Trim Molding Item Detachment

On January 27, 2023, the Office of Defects Investigation (ODI) opened PE23-001 to investigate allegations of A-pillar trim panel detachment on model year (MY) 2011-2019 Ford Explorer vehicles. At the time of opening, the office had received 164 Vehicle Owner Questionnaires (VOQs) reporting this failure. ODI has received 671 VOQs regarding this failure, including 1 alleged crash and 2 alleged injuries. Some consumers have reported that the failure occurred while they were driving at highway speeds, with the detached trim panel entering the path of travel of the vehicle behind them, requiring the driver of the following vehicle to take evasive action. It is noteworthy that motorists who experience this type of road hazard are less likely to accurately identify the subject vehicle the item detached from and report the event to the NHTSA. In its April 17, 2023 response to ODI’s information request, Ford provided 175 consumer complaints (including field reports) and 14,162 warranty claims related to A-pillar trim panel detachment. Ford relayed that the subject vehicles utilize a plastic A-pillar bracket, which is bolted to the vehicle structure during assembly. The A-pillar trim panel is attached to the bracket utilizing 7 plastic clips. Contributory factors which have been identified by Ford as leading to the subject defect include incomplete installation at the assembly plant, inadequate part stack up, damage to fasteners during installation, and failure to follow proper repair procedure during windshield replacements or other services. On January 19, 2024, Ford filed safety recall 24V-031 for MY 2011-2019 Ford Explorer vehicles to address the subject defect. In the Part 573 Safety Recall Report, Ford identified that a detached A-pillar trim panel can create a road hazard for other road users, increasing the risk of a crash. The recall remedy includes an inspection of the existing A-pillar trim panel and, if necessary, the replacement of the component. The remedy A-pillar trim panels will be installed with an additional adhesive for robustness and utilize an inspection procedure to ensure full engagement of all the retention clips during installation. In view of the recall action being taken by Ford, ODI is closing this Preliminary Evaluation. The agency reserves the right to take additional action if warranted by future circumstances. NHTSA is opening this audit query to investigate whether The Hertz Corporation (Hertz) has complied with the requirements of the National Traffic and Motor Vehicle Safety Act (Safety Act). The Safety Act requires, among other things, that a rental company shall not rent a vehicle subject to a safety recall unless the recall remedy has been performed. 49 U.S.C. § 30120(i).NHTSA is in receipt of information that indicates Hertz rented vehicles to customers without having performed required recall repairs. Accordingly, we are opening this audit query to seek additional information concerning this issue.
