The contact owned a 2018 Ford Explorer. The contact stated that while driving at various speeds, a message displayed on several occasions instructing the driver to pull over and shut off the vehicle because the battery would shut off. Upon pulling over and turning off the vehicle, the vehicle failed to restart and required towing on several occasions. The vehicle was towed to the dealer, where a wiring failure was determined. The dealer attempted to repair the vehicle, but the failure persisted. While driving approximately 25 MPH, the infotainment screen turned blank, and a large wrench warning light illuminated. The vehicle independently accelerated, became uncontrollable, and drifted out of the intended lane. The contact regained partial control as the vehicle continued accelerating toward an embankment. The front driver's side wheel was caught in the gutter of the embankment, causing the driver's side of the vehicle to crash into the embankment. The contact's head struck the driver's side window, which was fractured, allowing branches to enter the cabin. The driver's side curtain air bags deployed for the second and third rows but failed to deploy properly for the front driver's side, causing the contact to hit her head a second time on a sail that had deployed. The contact became pinned between the driver's seat and the driver's side window area, with her hair caught and pulled in the seat belt pretensioner, causing significant pain. EMT personnel cut the contact's hair to free the contact from the vehicle. The contact was assisted from the vehicle, fitted with a neck brace, placed on a stretcher, and transported to the hospital. The contact sustained head and neck injuries, as well as a concussion. Additionally, the contact suffered from pre-existing auto-immune lumbar stenosis L1–L5, osteoarthritis L5–S1 with bone-on-bone contact, and sacroiliac joint disease, and experienced increased pain after missing a scheduled appointment for sacroiliac joint injections due to the crash. I am filing a safety complaint regarding unresolved recalls on my 2018 Ford Explorer (VIN: [XXX] ). FIRST RECALL: Over two years ago, I was notified to repair the exterior A-pillar trim/window shields. The dealer in Yakima, WA, placed me on a waiting list and said the part was unavailable. I have been waiting over two years with no resolution. Recently, while driving on the highway, this part fell off and struck the windshield of another vehicle behind me, creating a dangerous situation and potential for serious injury. The dealer refused to repair it for free, stating I must either wait longer or purchase the part myself, despite this being a manufacturer safety recall. This long delay and refusal put my safety and the safety of others at risk. It also caused stress, confrontation, and concern for my financial obligations, as I continue paying for a vehicle with unresolved safety defects. SECOND RECALL: The high-speed cooling fan motor relay recall was not properly addressed. The dealer only started the vehicle, stated the fan worked, and performed no inspection or repair, leaving a potential hazard unresolved. FORD RESPONSE: I reported these incidents to Ford Customer Service, but they said the situation did not qualify for opening a case. I strongly disagree, as these are confirmed safety recalls and one part already failed during driving. ADDITIONAL CONCERNS: I am still making monthly payments for this vehicle while unresolved recalls continue to appear. The ongoing failure to repair these defects, despite multiple reports, creates financial and safety burdens. I request NHTSA to investigate these matters, require Ford to complete all recall repairs properly, and hold the dealer accountable for their refusal to provide free safety repairs in a timely manner. The computer was acting up and it would do crazy things. Lock and unlock, go into that slow drive as i was pulling into a road. Etc. These would self correct, all but the abs in a matter of minutes. Oh i did have it checked. All that came up was right wheel speed sensor. I paid to have it fixed. Ford dealer told me his news, new sensor bad news still says it's bad. I was moving to another state. Stranger things kept happening. None could find out what was wrong. It started back firing so new plugs and coils. The shop told me after that it still had issues and while no code it was the transmission. It started to violently shake going 70mph. Scared me to death. I sold it and now car shopping. Never Ford again! I have always trusted them but they have compromised on quality, safety and customer loyalty.

Both driver and passenger low beam headlights started strobe, than completely stopped working while driving at night. This was extremely stressful and dangerous as I was no longer able to see the roads very well, my options were get a tow truck to pick up and bring my vehicle from Michigan back to Minnesota or sit on the side of the road until the morning also a safety risk. Upon inspection the following day, it was only the low beam that was not working and all other LED's were working. This is when I tried to replace both headlight modules, that did not fix the issue. I then bought new headlights thinking the low beam LED stopped working after going through 3 sets of headlights the issue was never resolved.

Consequence & remedy

Consequence: A detached roof rail cover can create a road hazard, increasing the risk of a crash.

Remedy: Dealers will inspect the roof rail covers and as necessary, repair or replace the push-pins, rail clips, or roof rail covers, free of charge. Interim letters, notifying owners of the safety risk, were mailed August 28, 2026. Additional letters will be sent once the remedy is available, anticipated in January 2027. Ford Motor Company (Ford) is recalling certain 2016-2019 Lincoln MKC, 2016-2023 Explorer, 2019-2020 Fusion, 2019-2024 Ranger, 2020-2022 Lincoln Corsair, Escape, 2021-2024 Bronco, Bronco Sport, and 2022-2024 Maverick vehicles. The engine block heater may crack and develop a coolant leak, causing it to short circuit when the block heater is plugged in.

Consequence & remedy

Consequence: An electrical short circuit can increase the risk of a fire.

Remedy: Owners are advised not to plug in their block heater until the vehicle is remedied. Dealers will replace the block heater, free of charge. Owners will also have a alternative option to replace engine block heater element with a threaded blanking plug, and remove the block heater electrical cord. Interim letters, notifying owners of the safety risk, were mailed December 3, 2025. Additional letters will be sent once the final remedy is available, anticipated September 2026. Ford Motor Company (Ford) is recalling certain 2011-2019 Explorer vehicles. The A-pillar trim retention clips may not be properly engaged, allowing the trim to detach.

Consequence & remedy

Consequence: A detached trim piece can fall off the vehicle, becoming a road hazard and increasing the risk of a crash.

Remedy: Dealers will inspect and replace the A-pillar trim as necessary, free of charge. This will be a phased campaign, with the remedy becoming available in different phases based on model years. Consequence: A seat back with an improperly assembled recliner mechanism may have reduced strength and may not adequately restrain an occupant in a crash, increasing the risk of injury.

Consequence & remedy

Consequence: A fuel leak in the presence of an ignition source can increase the risk of a fire.

PE23001 · Windshield Trim Molding Item Detachment

On January 27, 2023, the Office of Defects Investigation (ODI) opened PE23-001 to investigate allegations of A-pillar trim panel detachment on model year (MY) 2011-2019 Ford Explorer vehicles. At the time of opening, the office had received 164 Vehicle Owner Questionnaires (VOQs) reporting this failure. ODI has received 671 VOQs regarding this failure, including 1 alleged crash and 2 alleged injuries. Some consumers have reported that the failure occurred while they were driving at highway speeds, with the detached trim panel entering the path of travel of the vehicle behind them, requiring the driver of the following vehicle to take evasive action. It is noteworthy that motorists who experience this type of road hazard are less likely to accurately identify the subject vehicle the item detached from and report the event to the NHTSA. In its April 17, 2023 response to ODI’s information request, Ford provided 175 consumer complaints (including field reports) and 14,162 warranty claims related to A-pillar trim panel detachment. Ford relayed that the subject vehicles utilize a plastic A-pillar bracket, which is bolted to the vehicle structure during assembly. The A-pillar trim panel is attached to the bracket utilizing 7 plastic clips. Contributory factors which have been identified by Ford as leading to the subject defect include incomplete installation at the assembly plant, inadequate part stack up, damage to fasteners during installation, and failure to follow proper repair procedure during windshield replacements or other services. On January 19, 2024, Ford filed safety recall 24V-031 for MY 2011-2019 Ford Explorer vehicles to address the subject defect. In the Part 573 Safety Recall Report, Ford identified that a detached A-pillar trim panel can create a road hazard for other road users, increasing the risk of a crash. The recall remedy includes an inspection of the existing A-pillar trim panel and, if necessary, the replacement of the component. The remedy A-pillar trim panels will be installed with an additional adhesive for robustness and utilize an inspection procedure to ensure full engagement of all the retention clips during installation. In view of the recall action being taken by Ford, ODI is closing this Preliminary Evaluation. The agency reserves the right to take additional action if warranted by future circumstances.
