WENT ABOUT 60 MI WHEN I NOTICED THE STEERING WHEEL WAS SHAKING A BIT. KEPT DRIVING FOR MAYBE 5 MI BEFORE I HEARD A NOISE FROM THE FRONT END NEAR THE WHEELS. DECIDED TO PULL OFF THE HIGHWAY AND GET TO A SERVICE STATION TO CHECK TIRE PRESSURE. CHECKED TIRE PRESSURE AND BEGAN TO BACK OUT WHEN THE RIGHT FRONT WHEEL FELL OFF. THERE WAS ONLY 1 LUG STUD REMAINING THAT WAS BROKEN OFF ABOUT 1/2 " ABOVE THE BRAKE DRUM. THE OTHER 4 LUG STUDS WERE MISSING, AND SEEMED TO BE SHEERED OFF ABOUT 1/2" BELOW THE DRUM. THANKFULLY, THE WHEEL DID NOT FALL OFF ON ROUTE 95, ELSE WE AND, MOST LIKELY OTHERS, WOULD HAVE BEEN SERIOUSLY INJURED OR KILLED. I HAD THE CAR TOWED TO MY BODY SHOP, ALSO THE DEALERSHIP THAT I BOUGHT THE CAR FROM AND HAD THE TIRE ROTATION DONE. LOOKING AT SOME FORUMS, IT SEEMS LIKE THIS IS NOT THE FIRST/ONLY TIME THAT A FORD EXPLORER HAS LOST A TIRE BY BROKEN LUG STUDS DISAPPEARING. I WILL GET THE VEHICLE REPAIRED AND THEN WILL SELL THE VEHICLE. I WILL ALSO HAVE THE OTHER LUG POSTS INSPECTED, BUT WHO KNOWS IF ANYONE WILL SEE ANYTHING. THEN SUDDENLY IT STARTED DRIVING IN REVERSE AT A VERY HIGH SPEED, CAUSING ME TO COLLIDE WITH AN ONCOMING TRUCK ON THE FREEWAY. UNABLE TO CONTROL THE CAR AND FEARING FOR MY LIFE, I TURNED THE IGNITION KEY, WHICH BROUGHT THE CAR TO A HALT. THE HIGH PATROL ARRIVED WITHIN ONLY A COUPLE OF MINUTES. WHEN ONE OF THE OFFICERS TURNED ON THE CAR SO AS TO DETERMINE WHAT THE PROBLEM WAS, THE CAR IMMEDIATELY ACCELERATED TO 80 MPH. HE TURNED OFF THE CAR AND DIRECTED EVERYONE TO STEP AWAY FROM THE CAR, CAUTIONING THAT THE CAR WAS DANGEROUS. TWO OFFICERS DISCONNECTED THE BATTERY AND SAID THAT THERE APPEARED TO BE A PROBLEM WITH THE ACCELERATOR. THEY THEN PUSHED THE CAR OFF THE FREEWAY TO THE NEAREST GAS STATION. FORTUNATELY, I DID NOT GET HURT. BUT THERE WAS SIGNIFICANT BODY DAMAGE TO MY CAR AND TO THE OTHER TRUCK WITH WHICH I COLLIDED. HOWEVER, THE DRIVER OF THAT TRUCK DROVE OFF, THUS I WAS UNABLE TO GET HIS INFORMATION. I HAD THE CAR IMMEDIATELY TOWED TO MY NEIGHBORHOOD FORD DEALERSHIP, WHICH DETERMINED THAT THE ACCELERATOR PEDAL WAS DEFECTIVE. WHEN I ADVISED THEM THAT THE EMERGENCY BRAKE HAD FAILED, I WAS TOLD THAT NOTHING WORKS WHEN THE ACCELERATOR PEDAL BREAKS. THIS IS A SERIOUS PROBLEM. THE CAR WAS OUT OF CONTROL ON THE FREEWAY, DRIVING IN REVERSE, AND IT COULD HAVE INJURED MANY PEOPLE AND CAUSED A LOT MORE DAMAGE. I PURCHASED A 2003 EXPLORER IN AUGUST 2006; MILEAGE WAS APPROXIMATELY 45,000. BY OCTOBER THE CAR WOULD NOT START AND THE ALTERNATOR WAS REPLACED. A FEW WEEKS LATER, I WAS BACK BECAUSE MY CAR WAS JERKING. NOTHING SHOWED UP ON THE DIAGNOSTIC TOOL AT THAT TIME. HOWEVER, I WAS TOLD THE STARTER WOULD HAVE TO BE REPLACED AS WOULD THE RECENTLY INSTALLED ALTERNATOR. I RETURNED LESS THAN A MONTH LATER WITH THE SAME JERKING ISSUE, ONLY THIS TIME I NOTIFIED THE REPAIR SHOP THAT THE O/D LIGHT HAD BEEN COMING ON. FURTHERMORE, MY CAR HAD ACTUALLY STALLED IN THE MIDDLE OF TRAFFIC. AGAIN, NOTHING WAS FOUND. BY EARLY DECEMBER, I WAS BACK. MY CAR HAD STALLED ON A FEW MORE OCCASIONS AND WOULD NOT "PICK UP". THAT IS, EVEN THOUGH THE RPM'S WOULD REACH ANYWHERE FROM 2500 - 4000, MY CAR COULD NOT ACCELERATE ABOVE 25 - 30 MPH. SEVERAL DAYS LATER I WAS TOLD THAT THE TRANSMISSION NEEDED WORK. I GOT MY CAR BACK A FEW DAYS BEFORE CHRISTMAS AND DECIDED LAST MINUTE TO HEAD TO THE MOUNTAINS. AS SOON AS I REACH A SLIGHT ELEVATION, THE CAR CEASED TO ACCELERATE AND WHAT'S MORE, A THICK HEAVY CLOUD OF BLACK SMOKE CAME OUT OF THE REAR AND PERSISTED FOR AT LEAST TWO MILES. I HEADED BACK DOWN THE MOUNTAIN FOR HOME. I TOOK THE CAR BACK AND THEY KEPT IT FOR AT LEAST A WEEK AND A HALF TO REPAIR THE TORQUE CONVERTER AND OTHER TRANSMISSION RELATED ISSUE. IN EARLY FALL 2007, I TOOK THE CAR TO MY LOCAL FORD DEALER FOR SOME STRANGE NOISE COMING FROM THE ENGINE. IT ENDED UP COSTING $2500.00 TO REPAIR. I RETURNED IN NOVEMBER FOR MORE WORK, THIS TIME FOR THE SAME TORQUE CONVERTER AND SOLENOID ISSUE AS A YEAR AGO. DURING THIS TWO YEAR PERIOD, MY VEHICLE WAS UNDER EXTENDED WARRANTY. THE TOTAL REPAIR COST FOR THE FIRST YEAR OF OWNERSHIP IS ANYWHERE FROM $5000 - $9000, OF WHICH I PAID AT LEAST $2500 OUT OF POCKET. IN RESPONSE TO AN INFORMATION REQUEST (IR) LETTER SENT BY THE OFFICE OF DEFECTS INVESTIGATION (ODI), FORD STATED CONSUMER REPORTS OF HIGHER THAN EXPECTED EFFORTS WHEN SHIFTING INTO AND OUT OF PARK COULD BE A RESULT OF A GROOVE BEING WORN INTO THE SWING ARM RAMP OF THE STEERING COLUMN. THE SWING ARM IS A COMPONENT BOLTED TO THE STEERING COLUMN THAT ASSISTS IN SHIFTING THROUGH THE TRANSMISSION GEARS.AN ENGINEERING EVALUATION CONDUCTED BY FORD OF RETURNED STEERING COLUMNS FOUND THE GROOVE COULD BE CREATED OVER TIME IF CONSUMER SHIFT MORE QUICKLY FROM THE DRIVE OR REVERSE TO PARK THAN THE BRAKE SHIFT INTERLOCK (BSI) SOLENOID PIN RETRACTS AFTER THE BRAKE PEDAL IS APPLIED. IN NOVEMBER OF 2004, FORD RELEASED A REDESIGNED BSI SOLENOID WITH A SHORTER PIN RETRACTING TIME AS A SERVICEABLE COMPONENT. ON SEPTEMBER 6, 2005, ODI RECEIVED A PETITION REQUESTING THAT THE AGENCY INVESTIGATE ALLEGATIONS OF ENGINE SPARK PLUG EJECTION IN CERTAIN MODEL YEAR 1997 THROUGH 2004 FORD VEHICLES WITH TRITON V-8 AND V-10 ENGINES.ODI RECEIVED A TOTAL OF 474 NON-DUPLICATIVE COMPLAINTS ON THE SUBJECT VEHICLES WHERE THE COMPLAINANT, OR THE DEALER REPAIRING THE VEHICLE, REPORTED THAT A SPARK PLUG DETACHED FROM THE CYLINDER AND/OR EJECTED FROM THE ENGINE.AS OF DECEMBER 8, 2005, ODI IS NOT AWARE OF ANY ALLEGATIONS WHERE THE ALLEGED DEFECT RESULTED IN A LOSS OF VEHICLE CONTROL, A CRASH, AN INJURY, OR A FATALITY IN ANY OF THE 10,319,810 SUBJECT VEHICLES.IN ADDITION, ODI IS AWARE OF ONLY TWO INCIDENTS WHERE THE VEHICLE STALLED WITHOUT RESTART.INFORMATION CONTAINED IN THE ODI CONSUMER COMPLAINTS AND OBTAINED FROM 72 TELEPHONE INTERVIEWS WITH COMPLAINANTS SHOWED THE FOLLOWING:(1) 99% OF THE COMPLAINTS WERE ON MY 1997 TO 2002 SUBJECT VEHICLES.(2) MOST THE COMPLAINANTS REPORTED HEARING A LOUD POP WHILE DRIVING OR UPON STARTING UP THE VEHICLE FOLLOWED BY A LOUD, REPETITIVE CLICKING OR POPPING SOUND.(3) MANY OF THE COMPLAINANTS REPORTED THAT THE POPPING SOUND WAS ACCOMPANIED BY SOME LOSS OF VEHICLE POWER; HOWEVER, IN 99% OF THE INCIDENTS REPORTED, THE VEHICLE DID NOT STALL.IN THE VERY FEW INCIDENTS WHERE THE VEHICLE DID STALL, MOST VEHICLES COULD BE RESTARTED.(4) ONLY A SMALL PERCENTAGE OF THE COMPLAINANTS CITED THAT THEY SMELLED GAS OR A SLIGHT BURNING SMELL WHEN THE INCIDENT OCCURRED.(5) IN ALL BUT A VERY FEW INCIDENTS, VEHICLE DAMAGE WAS LIMITED TO THE ENGINE.IN ONE INCIDENT, THE COMPLAINT REPORTED THAT THE FUEL RAIL WAS DAMAGED AND REPLACED AFTER ONE OF THE SPARK PLUGS EJECTED FROM THE ENGINE; HOWEVER, THE COMPLAINANT REPORTED THAT THE DAMAGE DID NOT RESULT IN ANY TYPE OF FUEL LEAK OR FIRE.IN ANOTHER INCIDENT, THE ONLY INCIDENT WHERE A FIRE WAS ALLEGED, THE COMPLAINANT REPORTED THAT NO FLUID LEAK WAS OBSERVED, BUT THAT A FIRE RESULTED AFTER THE SPARK PLUG HAD EJECTED FROM THE ENGINE AND HE HAD RESTARTED THE VEHICLE AND DRIVEN TO ANOTHER LOCATION.NONE OF THE COMPLAINANTS REPORTED ANY DAMAGE TO THE VEHICLE HOOD.(6) ONLY TWO COMPLAINANTS REPORTED THAT THEY OBSERVED WHAT APPEARED TO BE SOME DROPS OF FUEL COMING FROM THE CYLINDER WHERE THE SPARK PLUG HAD FAILED OR ON THE SPARK PLUG ITSELF; HOWEVER, EACH OF THESE COMPLAINANTS REPORTED THAT THERE WAS NO SMOKE OR FLAMES AS A RESULT OF HIS INCIDENT.AS THE PETITIONER NOTED, AND ODI?S ANALYSIS SHOWED, IT IS POSSIBLE FOR A SPARK PLUG TO DETACH FROM THE ENGINE CYLINDER THREADS IN THE SUBJECT VEHICLES.HOWEVER, ODI?S ANALYSIS OF 474 COMPLAINTS DESCRIBING SUCH INCIDENTS FOUND ONLY A VERY FEW ALLEGED ANY SAFETY-RELATED CONSEQUENCES.NONE OF THESE SHOWED ANY EVIDENCE OF A SERIOUS SAFETY CONSEQUENCE.GIVEN THE LARGE POPULATION AND RELATIVELY LONG EXPOSURE TIME OF THE SUBJECT VEHICLES, THE COMPLAINT ANALYSIS INDICATES THAT THE RISK TO MOTOR VEHICLE SAFETY FROM THE ALLEGED DEFECT IS VERY LOW.IN VIEW OF THE FOREGOING, IT IS UNLIKELY THAT NHTSA WOULD ISSUE AN ORDER FOR THE NOTIFICATION AND REMEDY OF THE ALLEGED DEFECT AT THE CONCLUSION OF THE INVESTIGATION REQUESTED IN THE PETITION.THEREFORE, IN VIEW OF THE NEED TO ALLOCATE AND PRIORITIZE NHTSA?S LIMITED RESOURCES TO BEST ACCOMPLISH THE AGENCY?S SAFETY MISSION, THE PETITION IS DENIED.
