I WAS AT A COMPLETE STOP, MY FOOT WAS PUSHED DOWN ON THE BRAKE AND IT SUDDENLY RELEASED FROM MY FOOT FOLLOWED BY A WHOOSHING SOUND. MY BRAKE SYSTEM FAILED, AND I HAD TO USE MY EMERGENCY BRAKE TO STOP MY VEHICLE AFTER I STARTED DRIVING IT. I HAD IT TOWED TO A DEALERSHIP AND THEY DIAGNOSED IT AS OIL LEAKING FROM THE ENGINE INTO THE BRAKE BOOSTER. ON 7/15/2015 MY CHECK ENGINE LIGHT APPEARED, FOLLOWED BY THE ENTIRE VEHICLE SHAKING UNCONTROLLABLY. I TURNED THE VEHICLE OFF, RESTATED IT AND THOUGH THE CHECK ENGINE LIGHT STILL APPEARED, THE CAR NO LONGER SHOOK, AND THE CHECK ENGINE LIGHT WENT OFF A COUPLE HOURS LATER. ON 7/23/2015 THE CHECK ENGINE LIGHT REAPPEARED SO I TOOK IT INTO THE DEALERSHIP WHERE THEY ARE CURRENTLY TRYING TO DIAGNOSE WHAT IS WRONG WITH IT, BUT THINK IT HAS A VALVE PLUGGED OR AN ENGINE PROBLEM. BOTH OF THESE INCIDENTS OCCURRED AFTER I HAD AN OIL CHANGE COMPLETED ON THE VEHICLE.

I PURCHASED MY VEHICLE IN JUNE 2014 WITH ONLY 4 MILES ON IT. THE ONLY MINOR PROBLEM I NOTICED AFTER A FEW DAYS' USE IS THAT SOMETIMES IT IS VERY DIFFICULT TO SWITCH BETWEEN GEARS - IT'S ALMOST LIKE YOU HAVE TO JERK IT IN ORDER TO MOVE FROM ONE GEAR TO THE NEXT. I DIDN'T THINK THIS WAS A MAJOR ISSUE SO I DIDN'T THINK ANYTHING OF IT THOUGH THE PROBLEM PERSISTS FROM TIME TO TIME. THE MAJOR ISSUE IS THE FOLLOWING: AROUND MID-SEPTEMBER 2014, I WAS DRIVING TO WORK IN THE MORNING, AT APPROXIMATELY 35 MPH. AT ONE POINT, THE VEHICLE ACCELERATED ITSELF. THERE WERE NO VEHICLES IN FRONT OF ME SO NOTHING HAPPENED. I IMMEDIATELY PRESSED ON THE BRAKE PEDAL UNTIL I CAME TO A FULL STOP PULLING OVER ON THE RIGHT. I TURNED OFF THE VEHICLE AND RESTARTED IT. I DROVE TO WORK AND THE VEHICLE WORKED FINE THIS TIME. IT WAS A VERY SCARY SITUATION BECAUSE I WAS AFRAID IT WOULD HAPPEN AGAIN. I HAVE NEVER HAD ANY ISSUES WITH VEHICLES AND THINKING THAT IT WAS PROBABLY A ONE-TIME INCIDENT, I NEVER CONTACTED THE RETAILER NOR THE MANUFACTURER OF THE VEHICLE. THE PROBLEM HAS SINCE THEN OCCURRED THREE OR FOUR TIMES AFTER THAT. THE LAST TIME IT OCCURRED WAS EARLY MARCH 2015. I WAS ON THE SAME ROAD I WAS WHEN IT FIRST HAPPENED, DRIVING APPROXIMATELY AT THE SAME SPEED, WHEN THE CAR ABRUPTLY ACCELERATED. I ALWAYS LEAVE ENOUGH SPACE BETWEEN ME AND THE CAR IN FRONT OF ME SO THANKFULLY I WAS ABLE TO BRAKE AND COME TO A FULL STOP. MY PASSENGER SAW WHAT HAD TRANSPIRED AND ADVISED ME TO IMMEDIATELY TAKE MY VEHICLE TO THE RETAILER SO THEY CAN HAVE IT LOOKED AT. MY VEHICLE HAS ROUGHLY 8,300 MILES ON IT BECAUSE I JUST DRIVE TO WORK AND BACK, SCHOOL AND BACK AND GROCERIES, AND ALL PLACES ARE WITHIN 5 AND 25 MINUTES FROM ME. I ALWAYS OBEY SPEED LIMITS AND EVADE HIGHWAYS BECAUSE I NEVER KNOW IF AND WHEN THE PROBLEM WILL HAPPEN AGAIN, SO I FEEL UNSAFE DRIVING THE VEHICLE. *TR

RQ25002 · Recall 19V-293 Post Remedy Failures

Status: closed (inferred from source dates) · Power Train:automatic Transmission:lever And Linkage:floor Shift; Power Train:shift Linkage/cable/rod; Power Train:shift Linkage/cable/rod:retaining Fasteners/hardware

On June 30, 2025, the Office of Defects Investigations (ODI) opened RQ25002 to investigate and review the effectiveness of Recall 19V-293. This recall was intended to address deterioration and detachment of shifter cable bushings by replacing them with an improved bushing that is less susceptible to deterioration from chemicals, oils, and solvents in MY2013-2016 Dodge Dart vehicles equipped with automatic transmissions and manufactured by Fiat Chrysler Automotive US LLC (FCA US). If the shifter cable becomes detached from the transmission, the transmission gear position may not match the driver’s intended gear selection. This mismatch could result in unintended vehicle movement or rollaway when the driver believes the transmission is in PARK, increasing the risk of a crash and/or injury. ODI has received sixty-three (63) Vehicle Owner Questionnaires (VOQ) alleging worn or misadjusted shifter cables and/or shifter cable detachments in subject vehicles that had already received the remedy for Recall 19V-293. Complainants reported incidents of bushing deterioration and detachment, resulting in gear-selection mismatch despite the prior recall repair, raising concerns regarding the effectiveness and durability of the implemented remedy and the adequacy of the recall. ODI’s investigation sought to determine whether the remedy used in recall 19V-293 adequately addressed the cause of the failure and whether other vehicles using the same bushing design might be affected. In response, FCA US and Dura Automotive Systems Inc. (Dura) determined that the repair for Recall 19V-293 did not sufficiently address the issue, and therefore, a safety defect exists in the subject vehicle population. As a result, FCA US and Dura initiated the following recall actions: ·         Recall 25V-674 (FCA US) is intended to address MY 2013–2016 Dodge Dart vehicles repaired under 19V-293 or otherwise equipped with suspect service parts. Remedy is under development. ·         Recall 25E-065 (Dura) is intended to address the defect at the equipment level by recalling affected gearshift control cable assemblies distributed as service components. Remedy is under development. The recalls initiated by FCA US (25V-674), and Dura Automotive Systems Inc. (25E-065) were received prior to ODI sending an Information Request (IR) letter to FCA. These two recalls seem to resolve the concerns that prompted investigation RQ25002 by replacing the previously recalled and service-supplied shifter cable assembly with an improved design, currently under development. The vehicle scope concern identified in this query was addressed by the new recalls including all Model Year 2013–2016 Dodge Dart vehicles produced and equipped with the affected shifter cable assembly. Accordingly, ODI is closing Investigation RQ25002. The agency will continue to monitor field performance and recall completion data to evaluate the effectiveness of the new remedy. ODI reserves the right to take additional action if warranted by future circumstances. To review the ODI reports cited in the Closing Resume ODI Report Identification Number document, go to NHTSA.gov.
