WHILE DRIVING I-405N AT APPROX 63MPH, MY CEL & TC LAMP ILLUMINATED AND CAR STALLED OUT LEAVING ME WITH NO STEERING FUNCTIONS, THEN SUDDENLY IT TURNED BACK ON AND RED-LINED FOR ALMOST 20SECS STRAIGHT LEAVING ME AT WOT SPEEDING STRAIGHT TOWARDS THE VEHICLE IN FRONT OF ME. THE DASH FLASHED SOMETHING IN THE INSTRUMENT CLUSTER RIGHT BEFORE IT SHUTS OFF BUT TOO QUICKLY FOR ME TO BE ABLE TO READ IT, THE WHITE BOX THAT HIGHLIGHTS THE SELECTED GEAR TURNS ORANGE AND HIGHLIGHTS ALL GEARS, THEN TACHOMETER AND SPEEDOMETER FALL TO ZERO. THIS HAS HAPPENED ON MULTIPLE OCCASIONS. I WAS ABLE TO RECORD THE THE DASH DISPLAY ON MY PHONE WHEN IT WAS OCCURRING BACK -TO-BACK. I PULLED FAULT CODES(U0101,U1110,U1120,U11BC,U0121,U0100,U212). I TOOK THE CAR TO DEALERSHIP 2 WEEKS PREVIOUSLY, THEY JUST RESET MY CEL LAMP AND SAID BRING IT BACK IF IT HAPPENS AGAIN. THIS IS DANGEROUS AND NEEDS TO BE TAKEN CARE OF BEFORE IT CAUSES SERIOUS INJURY OR DEATH! I BELIEVE THE PROBLEM IS WITH THE ECM AND TCM PROGRAMMING, OR THE MODULES THEMSELVES. INCLUDED IS SCREENSHOT FROM THE VIDEO I RECORDED AS IT STALLED ON THE FWY FOR THE 3RD TIME IN 10 MINUTESS.

I HAVE BEEN TOLD THAT THIS TRANSMISSION HAS BEEN VERY TROUBLESOME SINCE IT`S INTRODUCTION AND I AM WONDERING IF A RECALL IS PENDING?? IF NOT, IT SHOULD BE!!!!!!! THE LAST TIME I LET THE VEHICLE WITH MY DEALER , THEY SAID THAT THEY "COULDN'T DUPLICATE THE PROBLEM", EVEN THOUGH THE TECHNICIAN THAT ALWAYS WORKS ON MY VEHICLES, ADMITS THAT HE HEARD AND FELT THE PROBLEMS THAT I DESCRIBE. I MADE A COMPLAINT TO CHRYSLER AND THEY BASICALLY TOLD ME TO GET LOST ! THEY SAID THAT THE SERVICE MANAGER, (AT THE DEALERSHIP), SAID THAT THERE IS NOTHING WRONG WITH MY VEHICLE, THAT IS THE WAY THIS TRANSMISSION IS SUPPOSED TO OPERATE !!!!!!! UNBELIEVABLE !!!!! *TR

THE VEHICLE'S ENGINE WAS COLD AND IT HAD BEEN SITTING IN A PARKING LOT FOR FOUR DAYS. I WAS ATTEMPTING TO MERGE INTO TRAFFIC FROM A TOLL BOOTH. THE VEHICLE WOULD NOT ACCELERATE PAST APPROXIMATELY 10 MPH AND IT LOST SPEED BUT THE RPMS WERE INCREASING. THIS WENT ON FOR APPROXIMATELY FIVE SECONDS AND THEN THE CHECK ENGINE LIGHT CAME ON AND I WAS ABLE TO ACCELERATE INTO TRAFFIC. I DROVE FOR APPROXIMATELY 30 MILES AT 70 MPH (AFTER CALLING A SERVICING DEALER WHO TOLD ME I WOULD BE OKAY DRIVING THE CAR UNLESS THE CHECK ENGINE LIGHT STARTED TO FLASH) THEN STOPPED FOR A TRAFFIC LIGHT. AT THE NEXT TWO TRAFFIC LIGHTS THE VEHICLE WOULD NOT ACCELERATE WITH CARS BACKED UP BEHIND ME AND HONKING. THE MPH WERE AT 15 BUT THE RPMS WERE AT 2200. THIS WENT ON FOR ABOUT FIVE SECONDS AT EACH LIGHT THEN HAPPENED AGAIN AS I TRIED TO MERGE ONTO ANOTHER HIGHWAY THAT WAS UNDER CONSTRUCTION WITH CONCRETE BARRIERS ON BOTH SIDES AND HAD NO WHERE FOR ME TO WAIT FOR ACCELERATION. THE SERVICING DEALER SAID THEY COULDN'T GET MY CAR IN FOR 4 DAYS. TWO DAYS LATER THE CHECK ENGINE LIGHT WENT OFF. THE NEXT DAY THE SERVICING DEALER SAID THEY COULD NOT DIAGNOSE THE PROBLEM WITHOUT THE CHECK ENGINE LIGHT ON. THEY DROVE IT FOR THREE DAYS AND THEN GAVE IT BACK TO ME UNDIAGNOSED. THE NEXT DAY THEY SAID BRING IT BACK IN BECAUSE DODGE ENGINEERS READ THE COMPUTER CODE AND TOLD THEM TO REPLACE THE TRANSMISSION. THE SERVICING DEALER DISCOVERED METAL SHAVINGS IN THE PAN WHILE REPLACING THE TRANSMISSION. THE SERVICING DEALER DID NOT KNOW WHAT CAUSED METAL SHAVINGS IN THE PAN BUT ONE OF THEIR EMPLOYEES TOLD ME ON THE SIDE THEY SEE THE PROBLEM A LOT AND DRIVERS WILL CALL BECAUSE THEY HEAR A LOUD BOOM FROM THE CAR .I AM A WEEK OUT WITH THE NEW TRANSMISSION AND STILL NOTICE IT DOES NOT UPSHIFT SMOOTHLY WHEN THE ENGINE IS COLD. NEITHER THE SERVICING DEALER OR THE SELLING DEALER WILL HELP ME WITH AN EXTENDED WARRANTY. *TR

I WAS DRIVING A RENTED 2014 DODGE CHARGER ON A RURAL ROAD AND NEEDED TO TURN AROUND. I BEGAN A THREE POINT TURN AND WHEN I ATTEMPTED TO SHIFT THE TRANSMISSION IN TO REVERSE THE SHIFTER WOULD NOT GO IN TO THE REVERSE POSITION. AS A RESULT I WAS STUCK CROSSWISE ACROSS THE ROAD, IN DANGER OF BEING STRUCK BROADSIDE BY ANY ONCOMING TRAFFIC. FORTUNATELY THERE WASN'T ANY TRAFFIC. I CONTINUED TO ATTEMPT TO SHIFT IN TO REVERSE AND AFTER SEVERAL MORE ATTEMPTS I WAS FINALLY ABLE TO. THIS WAS JUST ONE OF SEVERAL TIMES DURING THE WEEK I RENTED THIS VEHICLE THAT I WAS UNABLE TO PUT THE SHIFTER IN TO THE GEAR I NEEDED, WHETHER IT BE DRIVE OR REVERSE. THIS IS A POTENTIALLY HAZARDOUS FLAW. THE SHIFTER ON THIS VEHICLE IS EXTREMELY VAGUE AND FREQUENTLY OVERSHOOTS THE DESIRED POSITION. THE SHIFTER SEEMS TO BE ELECTRIC RATHER THAN MECHANICAL WHICH WOULD ACCOUNT FOR THE ABSENCE OF A MECHANICAL LOCKING OF THE SHIFTER. *TR

EVER SINCE PURCHASING THIS VEHICLE THERE HAS BEEN A CLUNKING NOISE AND THE WHOLE CAR MOVES WHENEVER PUTTING IN GEAR OR PUTTING IN PARK. AND THERE IS A LOUD CLUNK. I WENT TO PURCHASE GAS AT A SHELL STATION( I ONLY USE SHELL PREMIUM GAS) AND WENT INSIDE TO PAY FOR THE GAS. I CAME BACK HIT THE FUEL DOOR RELEASE BUTTON AND WENT TO THE PUMP. I WENT TO PLACE THE NOZZLE IN MY TANK AND THE METAL PLATE THAT COVERS THE FUEL TANK WOULDNT NOT MOVE. I CALLED MY HUSBAND AND HE CAME TO HELP ME AND THERE WAS NO WAY TO GET THE PLATE OPEN. I CALLED THE DEALERSHIP WHERE I BOUGHT THE CAR AND WAS TOLD THAT THEY HAD NEVER HEARD ANYTHING LIKE THAT HAD I TRIED TO PUT DIESEL IN MY TANK. I AM A FORMER TRUCK DRIVER AND KNOW THE DIFFERENCE BETWEEN DIESEL AND REGULAR GAS. AND THAT WAS NOT THE ISSUE. THE DEALERSHIP ALSO COULD NOT FIGURE OUT WHAT THE PROBLEM IS. I WENT TO A STATION THAT HAS A MECHANIC AND HE USED A TOOL AND WAS ABLE TO UNLOCK IT SO THAT WE COULD FILL UP THE TANK. I AM NOT CURRENTLY DRIVING THE CAR UNTIL THE DEALERSHIP CAN GET IT IN TO BE REPAIRED BECAUSE IF I DID RUN OUT OF GAS THERE IS NO WAY FOR ME TO PUT GAS IN THE VEHICLE. SO I AM STUCK WITHOUT A VEHICLE EVEN THOUGH I HAVE A BRAND NEW CAR WITH LESS THAN 5,000 MILES ON IT. THIS IS RIDICULOUS BECAUSE NO ONE SEEMS TO KNOW HOW TO FIX THE PROBLEM. AND ALL THE DEALERSHIPS IN THE HOUSTON AREA HAVE A 4 -5 DAY WAIT TIME FOR THEM TO EVEN LOOK AT MY CAR. *TR

AFTER A FEW VISITS TO MY DEALER AND AFTER THEY KEPT MY CAR FOR A WEEK, IT WAS THEN DETERMINED THAT THE VALVE BODY INSIDE THE TRANSMISSION NEEDED REPLACED, AND CHRYSLER WILL NOT GIVE RENTAL CARS OUT DUE TO THE VEHICLE STILL BEING DRIVABLE. (WTF?) THE PART NEEDED TO FIX THE TRANSMISSION IS ON BACKORDER AND CHRYSLER CURRENTLY DOES NOT KNOW WHEN THEY WILL HAVE IT IN BECAUSE IT HAS TO COME FROM A VENDOR, SO NOW I'M STUCK WITH A $40000 PAPER WEIGHT IN MY DRIVEWAY UNTIL CHRYSLER GETS OFF THEIR SORRY BUTTS TO GET THIS PROBLEM RESOLVED. DUE TO THIS PART BEING ON BACKORDER, THIS IS OBVIOUSLY A COMMON PROBLEM THAT I FEEL CHRYSLER SHOULD MAKE A RECALL. *TR

AFTER 4 DAYS, WE CALLED THE DEALERSHIP AND THEY SAID THAT THE SERVICE PERSON THAT TOOK CARE OF US FROM DAY 1 HAD BEEN TERMINATED. SPOKE TO A NEW SERVICE PERSON AND HE STATED THAT HE 'S NOT AWARE OF THIS PROBLEM. WE INSISTED THAT THE CAR BE DIAGNOSE AS SOON AS POSSIBLE AND GAVE US AN APPOINTMENT ON 7/19/14. IN THE MEAN TIME, THE CAR CANNOT BE DRIVEN TOO FAR. PERFORMANCE OF THE CAR IS VERY ERRATIC AND WOULD FEEL THAT IT WOULD SHUT OFF. ALTHOUGH, SERVICE PERSON STATED THAT IS OK TO BE DRIVEN. *TR

2EA21002 · Desiccated Air Bag Inflator Rupture
Opened Sep 17, 2021 · No close date supplied
Status: open (inferred from source dates) · Air Bags:frontal:driver Side:inflator Module; Air Bags:frontal:passenger Side:inflator Module
From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced. In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators. It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field. Lists of recall actions that may have used desiccated PSAN inflators as remedy parts, as well as the makes and models originally manufactured with them, is available with the downloadable version of this document (see nhtsa.gov/recalls?nhtsaId=EA21002 -- note this information is subject to change/revision as the investigation proceeds). This investigation does not supersede EA15-001, which remains open.

EA16002 · Vehicle Rollaway, Engine On
Opened Feb 3, 2016 · Closed Jun 24, 2016
Status: closed (inferred from source dates) · Power Train:automatic Transmission:gear Position Indication (prndl)
On April 22, 2016, Fiat Chrysler Automotive US LLC, (FCA) submitted a Defect Information Report (DIR) to NHTSA regarding a defect that could result in unattended vehicle rollaway with the engine running in approximately 811,586 model year (MY) 2012 to 2014 Dodge Charger and Chrysler 300 vehicles and MY 2014 to 2015 Jeep Grand Cherokee vehicles equipped with 8-speed transmissions and a monostable gear selector (NHTSA Recall #16V-240, FCA Recall #S27). According to FCA's DIR, The existing strategies built into these vehicles to deter drivers from exiting the vehicle after failing to put the transmission into PARK have not stopped some from doing so. Drivers erroneously concluding that their vehicle's transmission is in the PARK position may be struck by the vehicle and injured if they attempt to get out of the vehicle while the engine is running and the parking brake is not engaged. The vehicles recalled by FCA are equipped with Monostable electronic (shift-by-wire) gearshift assemblies supplied by ZF Group (see Figure 1).The subject Monostable shifter has a single neutral position that it snaps back to when the driver releases the shift knob.Although the Monostable gearshift has the familiar appearance of a conventional console mechanical gearshift assembly, it has an unfamiliar movement that does not provide the tactile or visual feedback that drivers are accustomed to receiving from conventional shifters.Consequently, the driver must take additional time to verify that the desired gear position was achieved by checking the PRNDL display on the shift knob or the Electronic Vehicle Information Center (EVIC) display.
