The contact stated that the TPMS warning light was illuminated, indicating that both front driver's side tire and the front passenger's side tire were underinflated; however, upon taking the vehicle to an independent mechanic, the contact was informed that the tire pressure for both tires were correct. The contact stated that the failure had been reoccurring. The dealer and the manufacturer were not notified of the failure. The vehicle was not diagnosed or repaired. The van was parked and ignition turned off. Airbags deployed without warning. No collision, no bumpy road conditions, and no warning prior to deployment. Van was inspected at repair shop through insurance where they determined it was a manufacturer issue. When we contacted the manufacturer they stated since there was no recall they could not help us. Police was called and report made. Also have witnesses that van was parked and turned off at the time. Thankfully no one was in vehicle or could have been worse. Front airbags are the only ones that deployed.

The contact owns a 2009 Chrysler Town and Country. The contact stated while driving 10 MPH and pulling into a parking lot, the vehicle inadvertently stalled and the ignition key was released. There were no warning lights illuminated. The contact stated that she reinserted the key into the ignition switch and restarted the vehicle; however, the failure reoccurred as she was attempting to pull into a parking space. The vehicle was towed to an independent mechanic who diagnosed that the camshaft had failed and needed to be replaced. The contact stated that the vehicle was repaired; however, the failure reoccurred twice while driving from the independent mechanic to her residence. The vehicle was taken back to the independent mechanic. The contact contacted an unknown dealer who advised that they were not aware of that specific failure. The manufacturer was not notified of the failure. The vehicle was not repaired. The failure mileage was approximately 174,000.

The contact stated when she approached the vehicle, she saw the front driver’s and passenger’s side airbags had deployed independently, shattering the front windshield and rear passenger’s side windows. The contact stated no one was injured nor sought medical attention. The contact towed the vehicle to a local dealer where it remained. The vehicle was not diagnosed or repaired. The manufacturer had been informed of the failure. The failure mileage was approximately 140,000.

TPMS (Tire Pressure Monitor System) gives false warnings of tires losing pressure. The flashing Yellow dashboard warning lights and audible alarm sounds and the dash display switches to show the four tire pressures. One by one the pressure of each of the four tires switches to horizontal lines until all four tire pressures are shown as horizontal lines. Meanwhile after many seconds the Warning Light becomes a steady yellow light. This steady yellow light and false flat tire readings continue for many miles (which can be over 50 miles). Sometimes parking and turning off the ignition resets the system. Sometimes restarting the vehicle just has it continue with the false readings. The false readings happen on smooth expressways and other roads as well. Sometimes they happen several times within a week. Other times weeks go by between false readings. The false audible and warning light are safety hazards because they are distracting, especially when it happens in a construction zone. This is a one owner vehicle and has been the subject of numerous complaints to the selling dealer about this issue. The dealer has been unable to repair the defect despite changing the module and tire sensors. Their history confirms failed repair efforts to fix the problem for several years at least back to November 3, 2016 despite being shown videos of the problem while the problem takes place.

The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The contact stated that he placed duct tape on the steering to cover the emblem screw holes. The contact stated that he was concerned about the airbag deploying while driving over a bump. The contact attempted to call the manufacturer but was unable to speak to anyone. The dashboard lights keep going on and off the speedometer won't work when this is happening also the windshield wipers go on automatically with me putting them on and won't stop. Sometimes while this occurs our automatic door won't operate or radio. We got a new alternator a new battery a new iod fuse and it's still happening i wasn't sure if this was a normal occurrence for chryslers so I did some research my van isn't the only one with the same issues we unfortunately bought it used a few months ago and can't take it back to the lot.

These vehicles may have a loose or missing brand emblem in the center of the steering wheel. The loose emblem or the securing nuts for the emblem within the driver air bag module may become projectiles in the event of a driver air bag deployment.

Consequence & remedy

Consequence: Projectiles as a result of driver air bag deployment in the event of a crash can increase the risk of injury.

Remedy: Chrysler will notify owners, and dealers will replace the driver's air bag cover with a redesigned cover, free of charge. The recall began February 11, 2021. Owners may contact FCA US LLC customer service at 1-800-853-1403. Chrysler's number for this recall is W09.

In the affected vehicles, the air bag control units may corrode and fail.

Consequence & remedy

Consequence: If the air bag control unit fails, the air bags may not deploy in the event of a crash, increasing the risk of occupant injury. Additionally, the air bags may inadvertently deploy, increasing the risk of a crash.

Freedom Motors Inc., (Freedom Motors) of Savaria Vehicle Group (formerly The Liberty Motor Company) is recalling one model year 2009 Volkswagen Routan and certain model year 2008-2010 Dodge Grand Caravan and Chrysler Town & Country wheelchair accessible vehicles modified by The Liberty Motor Company from February 2008 through January 2011. Over time and with use, the vehicle's rear axle may develop cracks which could lead to axle failure.

Consequence & remedy

Consequence: Failure of the rear axle may increase the risk of a crash.

THE REAR BRAKE LINES MAY HAVE BEEN IMPROPERLY ROUTED AND NOT SECURED PROPERLY DURING THE ALTERATION. IF NOT ROUTED AND SECURED PROPERLY, THE BRAKE LINES COULD RUB AGAINST THE VEHICLE CAUSING A BRAKE FLUID LEAK.

Consequence & remedy

Consequence: A BRAKE FLUID LEAK COULD RESULT IN A LOSS OF BRAKING ABILITY, POSSIBLY CAUSING A CRASH.

Remedy: ELDORADO NATIONAL AMERIVAN DEALERS WILL INSPECT THE VEHICLES AND IF THE BRAKE LINES WERE IMPROPERLY ROUTED THEY WILL BE CORRECTLY REINSTALLED FREE OF CHARGE. CHRYSLER IS RECALLING 6,138 MY 2009 TOWN AND COUNTRY AND DODGE GRAND CARAVAN VEHICLES. UNUSED ELECTRICAL CONNECTORS FOR THE BLIND SPOT DETECTION SYSTEM MAY BECOME CORRODED AND COULD SHORT CIRCUIT, WHICH CAN CAUSE A VARIETY OF CONDITIONS.

Consequence & remedy

Consequence: IF THE FUSE FOR THIS CIRCUIT DOES NOT BLOW, THE CONNECTORS COULD OVERHEAT AND POTENTIALLY CATCH FIRE.

Remedy: DEALERS WILL SEAL THE POWER CIRCUIT AT THE SUBJECT CONNECTOR AND REPLACE THE FUSE FOR THIS CIRCUIT IF NECESSARY FREE OF CHARGE. THE RECALL BEGAN ON MARCH 9, 2009. OWNERS MAY CONTACT CHRYSLER AT 1-800-853-1403.

DP14004 · Totally Integrated Power Module Failure

In a letter dated August 21, 2014, the Center for Auto Safety (CAS) petitioned the National Highway Traffic Safety Administration (NHTSA) to initiate a defect investigation of alleged failures associated with the Totally Integrated Power Module (TIPM) installed in Chrysler SUV?s, trucks, and vans beginning in the 2007 model year. The petitioner alleges that TIPM defects may result in engine stall, airbag non-deployment, failure of fuel pump shutoff resulting in unintended acceleration, fire, and other symptoms. On September 25, 2014, the Office of Defects Investigation (ODI) opened DP14-004 to evaluate the petition for a grant or deny decision. ODI analyzed complaint data provided by CAS as well as complaints submitted to ODI from consumers. In total, there were 296 complaints submitted by the petitioner in the original petition and five supplements, including 271 complaints related to the subject vehicles equipped with TIPM-7. Approximately 3 percent of CAS complaints are related to vehicles equipped with TIPM-6 and ODI?s review of these complaints did not identify any safety defect trends. ODI conducted a detailed review of complaints narratives submitted by CAS and consumers including careful analysis of vehicle repair histories, warranty claims obtained from the manufacturer and any available Customer Assistance Inquiry reports (CAIR). Analysis of the field data indicated that MY2011-2013 Jeep Grand Cherokee and Dodge Durango vehicles exhibited significantly higher complaint rates related to fuel pump relay (FPR) failures than other subject vehicles. In a September 3, 2014 letter to NHTSA, Chrysler submitted a Defect Information Report (DIR) identifying a defect in the FPR within the TIPM-7 which can result in a no start or stall condition in approximately 188,723 model year (MY) 2011 Jeep Grand Cherokee (WK) and Dodge Durango (WD) vehicles manufactured from January 5, 2010 through July 20, 2011 (14V-530).In a February 24, 2015 letter, Chrysler submitted a second DIR expanding the scope of the FPR defect condition to include an additional 338,216 MY 2012 through 2013 Jeep Grand Cherokee vehicles manufactured from September 17, 2010 through August 19, 2013 and MY 2012 through 2013 Dodge Durango vehicles manufactured from January 18, 2011 through August 19, 2013 (15V-115).ODI analysis of the CAS allegations of TIPM defects resulting in stall while driving, airbag non-deployment, unintended acceleration, fire and other faults identified a single defect condition related to 1 of over 60 different circuits in the TIPM assembly. The most common effect of this defect condition, related to the fuel pump relay, was a no-start concern, but it could also result in stall while driving (76 complaints). No valid evidence was presented in support of claims related to airbag non-deployment, unintended acceleration or fire resulting from TIPM faults and these claims were found to be wholly without merit based on review of the field data and design of the relevant systems and components.Except insofar as the petitioner's contentions relate to the defect condition addressed by the Chrysler recalls, the factual bases of the petitioner's contentions that any further investigation is necessary are unsupported. In our view, additional investigation is unlikely to result in a finding that a defect related to motor vehicle safety exists or a NHTSA order for the notification and remedy of a safety-related defect as alleged by the petitioner at the conclusion of the requested investigation.
