We took our 2021 Pacifica Hybrid to the dealership for Recall 52D. The first charge at home after the recall melted one of the charging "pins" in the car, and on our home Level 2 charger. Our dealership now says it's the fault of our charger, and we'll have to pay $2000+ for a repair. Our charger handle is still fine, no damage, only the "pin" is melted. For safety reasons, we can't charge the vehicle, and aren't driving it, as it's still at the dealership.

The contact stated that the second-row seat failed to remain in the upright position, causing the seat to collapse forward or backward while the vehicle was being driven. The vehicle was inspected by a dealer who confirmed the seat failure but stated that the vehicle was not covered under NHTSA Campaign Number: 22V181000 (Seats). The contact received an estimate for the part; however, the part was on backorder. An independent mechanic later inspected the vehicle and suggested that the vehicle should be covered under the recall; however, inquiries to other dealers and the manufacturer resulted in consistent confirmation that the VIN was not under recall. A temporary repair was performed on the vehicle by the initial dealer; however, a permanent repair was not performed. The failure mileage was approximately 74,000.

Driving on the interstate at high speed, when check engine light comes on, warning message - Stop Safely Vehicle Will Shut Off - and red turtle warning indicator (limp mode). After a short time, vehicle will no longer accelerate. I pulled over to the shoulder, stopped and shut off engine. Restarted after a few minutes and everything appeared to work fine, with check engine light on. About 15 minutes later, it happened again. The dealership serviced the vehicle. Found error code P0A78 for driver moteo "A" inverter performance. Used Stellantis Technical Service Bulletin 08-253-24 to diagnose (both modules were out of date) and complete the indicated updates. This is a known issue that has affected many vehicles (per internet forums and that Stellantis has created a TSB. It is an extremely dangerous situation causing the vehicle to enter "limp mode" and requiring the driver to stop in a matter of minutes. Stellantis should recall all potentially affected vehicles to update the modules proactively.

It has experienced multiple high-voltage component repairs, including software modules for the power inverter. The vehicle is currently experiencing a recurring failure and is displaying active diagnostic trouble code P0A78 (Drive Motor "A" Inverter Performance), indicating a critical fault in the high-voltage hybrid propulsion system. Prior to the current failure, the check engine light illuminated, and the vehicle experienced propulsion system degradation. This failure mode risks a sudden shutdown of the hybrid drive system at highway speeds, creating a severe risk of rear-end collisions or being stranded in active traffic. After the inspection, they stated they could not determine the cause of the warning light and refused to complete the recall repair. The vehicle is available for inspection upon request. The recall involves a safety issue that could put me, my passengers, and others at risk. Because the recall has not been completed, I do not feel safe operating the vehicle. In addition, I have been unable to use the vehicle for work because it has not been repaired. Yes. The authorized Chrysler dealership confirmed that the warning light was present and performed a diagnostic inspection. However, they were unable to identify the cause of the warning light and refused to complete the recall repair. They informed me that the case had been escalated to a Chrysler/Stellantis engineer. es. The vehicle was inspected by an authorized Chrysler dealership. The dealership escalated the case to a Chrysler/Stellantis engineer, but after more than three weeks, no solution or repair has been provided. The dealership informed me that a warning light was illuminated and required diagnosis before the recall could be completed. After performing the diagnostic inspection, they were unable to determine the cause of the warning light. The recall repair has been delayed for more than three weeks without any resolution or estimated completion date.

I took the van to get the safety recall done in 2026, Feb 25. For the rear view back up camera. I took it to the same dealership that I bought the car from, Jones Junction. However, upon further complications with the back up camera, this issue has never been fixed or resolved. If anything, the issue has become worse. I am unable to use my camera 98% of the time. It freezes. It goes black or is unavailable to see anything out of. It's a safety recall for a reason. I want this issue fixed. As I have been driving like this with the issue for quite some time. What are my next steps? I prefer not to have to dealer hop if I can help it. But I might have to. Is there a patch they may have to do to re-install the software for the camera? Or would they have to go and flash it again and what ever else? My true gut tells me that I dont think this specific dealership ever did the recall, correctly.

Chrysler (FCA US, LLC) is recalling certain 2020-2022 Pacifica Plug-In Hybrid Electric (PHEV) vehicles. The battery pack may cause a vehicle fire, even when the vehicle is parked with the ignition in the "Off" position.
Consequence & remedy
Consequence: A vehicle fire increases the risk of injury.
Remedy: Owners are advised not to charge their vehicles, and to park outside and away from structures, until their vehicle is repaired. Dealers will update the high voltage battery pack control module software. In addition, dealers will inspect and replace the battery pack assembly, as necessary. All repairs will be performed free of charge. Owner notification letters were mailed beginning June 23, 2026. Owners may contact FCA US LLC customer service at 1-800-853-1403. An improperly secured connector for the side curtain air bags may prevent the air bags from deploying. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard numbers 214, "Side Impact Protection" and 226, "Ejection Mitigation."
Consequence & remedy
Consequence: An air bag that does not deploy during a crash increases the risk of injury.

On July 22, 2022, the Office of Defects Investigation (ODI) opened investigation PE22-008 regarding complaints alleging a loss of motive power condition potentially due to a transmission malfunction in Model Years 2019-2021 Chrysler Pacifica Plug-In Hybrid Electric Vehicles (PHEV). On January 17, 2023, Fiat Chrysler Automobiles (FCA) issued safety recall (NHTSA Recall 23V-010) involving 2017-2023 Model Years (MY) Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016, through January 9, 2023. The recall states that the loss of motive power condition in these vehicles is caused by an internal transmission wiring connector that may short circuit, resulting in an unexpected engine shutdown. An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash. In discussions with FCA on Recall 23V-010, ODI confirmed the recall was related to investigation PE22-008. The remedy for 23V-010 is to update the Power Inverter module (PIM) and/or Instrument Panel Cluster (IPC) software to provide drivers a message stating “Stop Safely - Vehicle Will Shut Off Soon” followed by a chime and MIL, PRNDL blinking (on cluster and shifter knob), low SOC (State of Charge) warning and reduced power mode. The recall remedy does not replace the defective hardware that was described in Recall 23V-010, and the wiring connector short still exists on these vehicles. To date, ODI has received 132 reports for the population of vehicles subject to recall 23V-010. These reports allege a loss of motive power condition that reduces the vehicles speed, followed by dashboard warning lamps displaying messages associated with recall 23V-010. The presence of these warning lamps and messages indicates the vehicle likely had the recall remedy applied. Follow up discussions with FCA confirm their remedy does not prevent a loss of motive power, but instead limits torque to the engine and after an unspecified number of miles or period of time, the engine will shut down. When this occurs, FCA relies on the owner to transport the vehicle to a dealership after they experience a loss of motive power condition, at which time the defective hardware is replaced. Furthermore, the dashboard messages introduced in the recall remedy may cause the driver to pull over in an unsafe location. Pulling over in an unsafe location may expose the occupants to undue harm. The Office of Defects Investigation is opening this Recall Query (RQ) to assess the adequacy, frequency, and safety consequence of the remedy for recall 23V-010. On July 22, 2022, the Office of Defects Investigation (ODI) opened PE22008 to investigate allegation of loss of motive power, potentially due to transmission malfunction, in certain Chrysler Pacifica vehicles. In a January 17, 2023 letter to NHTSA, Fiat Chrysler Automobiles (FCA) submitted recall notice 23V-010 involving a transmission motor wiring defect in 67,118 model year (MY) 2017-2023 Chrysler Pacifica plug-in hybrid electric vehicles (PHEVs) manufactured from August 12, 2016 through January 9, 2023. FCA determined that an internal transmission connector short could result in an unexpected engine shutdown under certain conditions. Follow up correspondence with FCA confirmed this defect results in an electric motor position trouble code that was the basis for opening PE22008, therefore NHTSA is closing this investigation. The 23V-010 recall remedy is a software update to address the hardware failure associated with PE22008. When a hardware failure is detected, the vehicle limits motive power while instructing the driver to pull over to a safe location. NHTSA is concerned that drivers will pull over to an unsafe location when the warning message associated with the recall remedy displays on the dashboard. Due to the nature of the recall remedy and related reports, NHTSA has opened Recall Query RQ24004 to assess the recall remedy. The reports and failure report summary values included with this closing action are related to allegations of Recall 23V-010 remedy inadequacy. The Agency reserves the right to take additional action if warranted by future circumstances.
