After driving approx 400 miles at freeway speeds the engine shuts off. With semis honking as vehicle slowed while trying to pull over. No warning lights before or after stopping. Dash screen displayed "put car in park and press start". This happened twice in 2023. Took to dealer while car was under extended warranty. Was told 'could not duplicate' issue. They also informed me that this was something they never heard of before. We only do two highway trips per year. Took another trip couple of weeks age and vehicle shut off and we had to pull over 7 times going and returning. Dealer says I need a new transmission but can't guarantee that will fix the problem. I'm terrified to put this car on the freeway again. I thought we were going to be killed and possibly injure others on several of the occasions while trying to stop safely. I feel that Chrysler knows about this problem (from what I've recently read on the internet) and at least should know how to correct it. Also, it seems there was a class action suit that has been settled that we were unaware of. This vehicle has less than 60K miles and as we only drive 5-7000 miles per year didn't plan on having to junk it and buy another at this time. I've tried calling Chrysler Corp 800-992-1997. I was given a case # but they just seem to lie and never follow up with the dealer like they said. Now when I call and talk to someone, they put me on hold and kick me into the 'how did we do survey'. Realizing that sometimes you just don't always a ample time to safely get to the side of the road I'm at a loss as to how to proceed. The date listed below is the first time of incidence.
I bought this van about 2 years ago . In the last 6 -8 months. The anti-freeze in the coolant kept emptying, car never hot but disappeared. Turns out it's been going to the engine. I have a friend that is having same issue and should be a manufacturer defect.
Engine code P0303 (misfire cylinder 3 was triggered) car was found to be losing coolant through the cylinder. Chrysler dealer declared complete engine failure due to block damage. Stellantis indicated repair exceeded value of the vehicle.

Since acquiring the vehicle, I have encountered a persistent problem that poses significant safety concerns. 1. **Component/System Failure:** The vehicle experiences choking and emits a strong oil smell into the cabin. This odor is highly concerning as it leads to symptoms such as burning eyes, dry throat, red skin, coughing, nausea, and headaches. This indicates a potential failure or malfunction in the vehicle's ventilation or emissions system, possibly allowing oil or coolant fumes to enter the cabin. The component/system is available for inspection upon request. 2. **Risk to Safety:** The presence of these noxious fumes not only affects my health but also poses a serious safety risk to myself and passengers. The symptoms induced by the odor impair my ability to drive safely and could lead to accidents or injuries. Additionally, exposure to such fumes over time may have long-term health consequences. 3. **Confirmation:** While the problem was not confirmed by the dealer, multiple independent repair shops and the manufacturer have acknowledged the issue. Mechanics from these establishments have inspected the affected component/system and confirmed its malfunction. 4. **Inspections:** The component/system has been inspected by mechanics from various repair shops and by representatives from the manufacturer. 2017 Chrysler Pacifica with roughly 134,000 miles recently started have idling issues. The vehicle has had regular maintenance and always taken into shop when issues arise. When vehicle started idling rough we found out the spark plugs (all 6) needed replaced and should fix the issue. Replaced all 6 plugs within the same week of finding out they needed replacement. Then within a week while driving the engine starts over heating. Heating to upwards of 270 degrees F. As soon as we were able to get off the road and to a safe location to view the engine it was apparent the coolant was empty. Please keep in mind this is checked on a regular basis as is the oil in the vehicle. We replaced the coolant and drove back home. All seemed well. Then the very next day the engine would not start or even turn over. We looked again and the coolant was almost non existent. Had the vehicle towed to repair shop. The coolant had been leaking into the engine causing it to burn up. In the process the engine was seized come to find out and also the head gasket needed replacement. Doing more research once the coolant starts burning in the engine the engine will more than likely need replacement. We are being told to be prepared for the whole engine to need replacement due to coolant issues. If this would have been recalled we would not be in the position to have to replace our entire engine. It would be greatly appreciated if this can be looked into further as we have missed the deadline for the class action lawsuit for this issue.

The contact owns a 2017 Chrysler Pacifica. The contact stated that the vehicle failed to start. The contact stated that on the second attempt to start the vehicle, the brake pedal was stuck on the floorboard and failed to return to its normal position. The contact stated that the vehicle could not be jumpstarted. An independent mechanic was contacted and advised the contact to depress the brake pedal harder to start the vehicle. The contact stated that a friend assisted in depressing the brake pedal harder, and the vehicle started; however, the windshield wipers engaged independently, Additionally, the steering wheel made an abnormal sound and became difficult to turn. The contact stated upon starting the vehicle on another occasion, the rotary gear shift dial lights failed to illuminate, and the message "Service Shifter" was displayed. The contact was able to drive to the residence. The vehicle was not diagnosed or repaired by an independent mechanic or a dealer. The contact related the failure to NHTSA Campaign Number: 19V348000 (Electrical System) however, the VIN was not included. The manufacturer was not notified of the failure. The failure mileage was 81,270.

Vehicle steering gear is binding internally (confirmed by local service department). The safety concern with this is when steering at highway speeds the steering wheel locks and requires excessive force to move, which has not yet but almost caused an accident. There are no signs of outside force influencing this issue (ex. front end damage front collision or wheel mis-alignment). I have read online in an owners forum of several other cases where this is happening and that they recommend filling out a report with NHTSA as to make you aware of this in case of it being a larger issue. Vehicle has not been repaired yet as the part required is around $2,800 plus labor on top of that. I'm not filling this out to complain, but rather to make you aware of a potential safety issue of this particular model vehicle. Thank you for your time.
Engine head gasket failure requiring a complete engine replacement
Cylinder head damage at 101,000 miles. Valve guides and seals degraded to where it misfires. Problem confirmed via code scanner and leak-down test with a local mechanic.

Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.
Consequence & remedy
Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.
Remedy: Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

Consequence: A loss of power steering assist or a stall can increase the risk of crash.
Remedy: Chrysler will notify owners, and dealers will inspect the battery ground surfaces, clean the contact surfaces as needed, and properly reassemble the ground connection, free of charge. Vantage Mobility International, LLC (VMI) is recalling certain 2017 Chrysler Pacifica lowered floor vehicles modified by VMI. The rear pinion bracket on both sides of the vehicle may crack, causing the axle not to be centered and the wheel to wobble.
Consequence & remedy
Consequence: A wheel wobble may cause a possible loss of control, increasing the risk of a crash.
Remedy: VMI will notify owners, and dealers will replace the pinion brackets, free of charge. The recall began on August 14, 2017. Owners may contact VMI customer service at 1-800-488-9082. VMI's number for this recall is PAC20170728.

Vantage Mobility International, LLC (Vantage Mobility) is recalling certain 2017 Chrysler Pacifica lowered floor vans modified by Vantage Mobility for wheelchair accessibility. The affected vans have rear brake lines that might have been crossed during the modification. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 135, "Light Vehicle Brake Systems."
Consequence & remedy
Consequence: The crossed rear brake lines will cause the rear anti-lock brake system to be ineffective, increasing the risk of a crash.
Remedy: Vantage Mobility will notify owners, and dealers will correct the rear brake line attachments, free of charge. The recall began on July 3, 2017. Owners may contact Vantage Mobility customer service at 1-800-488-9082. Vantage Mobility's number for this recall is Pac-20170525.

Consequence & remedy
Consequence: If passengers occupying the third-row and/or second-row center seats cannot use a seat belt, there is an increased risk of injury in the event of a crash.
Remedy: Chrysler will notify owners, and dealers will replace the third-row and, if necessary, second-row center overhead seat belt stow bezel, free of charge. The recall began on July 14, 2016. The Office of Defects Investigation (ODI) is opening this Preliminary Evaluation to assess the scope, frequency, and potential safety consequences of alleged defects related to the electric power steering (EPS) system in model year (MY) 2017–2018 Chrysler Pacifica vehicles. ODI has received 94 Vehicle Owner Questionnaires (VOQs) reporting a condition in which the steering system exhibits a sticking sensation during gradual turns, particularly when the steering wheel is positioned near the 11 o'clock or 1 o'clock position. Complainants allege that during these events, increased steering effort is required to return the wheel to center. In many cases, this is followed by a sudden reactivation of EPS assistance, which may result in an oversteer condition. This behavior could increase the risk of a crash, especially at higher speeds. ODI has identified multiple VOQs describing similar symptoms. This investigation will assess the frequency, root cause, and potential safety-related consequences of the alleged EPS malfunction. On February 11, 2022, Fiat Chrysler Automobiles (FCA) issued a safety recall (NHTSA Recall 22V-077) on all 2017-2018 Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016 through August 7, 2018. The recall addresses ignition “OFF’ fires that originate at the high voltage (HV) battery pack assembly. An ignition ‘OFF’ vehicle fire can result in an increased risk of occupant injury, injury to persons outside the vehicle, and property damage, with little to no warning. At the time of the recall filing the root cause of the vehicle fires was unknown and under investigation by FCA and suppliers. A software remedy designed to improve thermal event detectability by monitoring battery diagnostics is currently being installed on subject vehicles as an interim remedy. As of November 23, 2023, the root cause of the battery failure is unknown. From December 2022 through December 2023, NHTSA has received four consumer complaints (VOQs) alleging vehicle thermal events involving recalled 2017 and 2018 Chrysler Pacifica PHEVs. Additional information collected from complainants confirmed a thermal event originating near or at the HV battery pack after the Recall 22V077 remedy had been applied to the vehicle. Follow up meetings with FCA and suppliers in November and December 2023 to discuss the ongoing root cause investigation and post-recall remedy incidents indicated a recent increase in HV battery thermal events. Furthermore, a review of NHTSA complaint data indicated the post-recall HV battery thermal event complaint rate now exceeds pre-recall levels. The Office of Defects Investigation (ODI) is opening this Recall Query (RQ) to review the effectiveness of the original recall remedy, understand the root cause of the battery fires, investigate additional reports of Pacifica PHEV HV battery fires, and to increase monitoring of the manufacturer root cause investigation.
