IN JUNE OF 2019 AFTER RECEIVING THE NOTICE OF RECALL V53, I TOOK MY VAN TO THE DEALERSHIP TO FIX THAT RECALL AND ANY OTHER RECALLS IT MAY HAVE. I WAITED APPROXIMATELY AND 1 HOUR AND 30 MINUTES WHILE MY VEHICLE WAS BEING FIXED. I WAS TOLD THAT ALL WERE COMPLETED AND WAS HANDED THE KEYS. RIGHT AFTER THE ACCIDENT I RECEIVED AN EMAIL ABOUT THE RECALL. I CALLED THE DEALERSHIP AND THEY TOLD ME THEY DIDN'T FIX RECALL NUMBER V53. I CONTACTED CHRYSLER ABOUT THE ACCIDENT AND THEY SENT AN INVESTIGATOR OUT TO INSPECT THE VAN. IN RETURN I RECEIVED A LETTER FROM FCA SAYING THAT THEY COULDN'T FIND ANY ISSUES. I FORMALLY BELIEVE A MALFUNCTION HAPPENED AT THE TIME OF THE ACCIDENT. OVER THE COURSE OF SEVERAL MONTHS SINCE OWNING THE VEHICLE IT HAD SHOWED SIGNS OF AN EXISTING PROBLEM WHICH WE MADE COMPLAINTS AT THE DEALERSHIP AND NOTHING WAS EVER FOUND. WHEN YOU COME TO A STOP OR SLOW DOWN TO COME TO A STOP/YIELD AND ACCELERATE THE VAN WOULD BUCK AND OR JERK AS IF IT WERE TO LOOSE POWER AND SHUT OFF. THEN WHILE THEY HAD IT THEY HAD ISSUES WITH THE POWER STEERING CUTTING ON/OFF. SO THEY SAID IT WAS THE RACK & PINION. THEY COULDN'T DUPLICATE THE ISSUES WITH THE VAN JERKING INTO LOWER GEARS. WELL HERE WE ARE A MONTH LATER AND MY POWER STEERING IS STILL CUTTING OFF/ON WHILE DRIVING CAUSING NEAR CAR ACCIDENTS. THE VAN STILL JERKS SOMETIME WHEN TRYING TO GET INTO LOWER GEARS. CALLED CHRYSLER CONSUMER NUMBER AND THEY SAID THEY WOULD GET BACK WITH ME. POWER STEERING FAILING COMPLETELY. STARTED INTERMITTENTLY AND WAS FIRST ADVISED THAT A 'LOW BATTERY' CAUSED A POWER SPIKE IN THE POWER STEERING UNIT AND THEY CHARGED THE BATTERY. THEN IT HAPPENED MORE FREQUENT AND THEY DIAGNOSED IT TO THE POWER STEERING UNIT ITSELF WHICH TOOK A WEEK TO REPLACE. THEN TWO DAYS LATER THE POWER STEERING WENT OUT WHILE ENTERING A 90 DEGREE CURVE AT 35 MPH. AFTERWARDS I COULD SIT IDLING, WATCHING THE VOLTAGE WANDER BETWEEN 11 VOLTS AND 15 VOLTS, AND HAVE POWER STEERING, BUT AS SOON AS I TURNED ON THE HEADLIGHTS OR FOGLIGHTS OR PUT IT IN GEAR, THE VOLTAGE WOULD STABILIZE AT 14.6-14.8 VOLTS AND THE POWER STEERING WOULD TURN OFF. IT'S BEEN A WEEK AND THEY JUST CONTACTED CHRYSLER. I'M AT 15 DAYS IN THE SHOP BETWEEN FAILED SECONDARY BATTERY, AND A MAJOR SAFETY ISSUE WITH THE POWER STEERING AND NO END IN SITE. WHEN THE POWER STEERING FAILS, THERE IS PRACTICALLY NO WAY TO TURN THE VEHICLE UNLESS YOU HAVE SIGNIFICANT STRENGTH IN THE ARMS. A NORMAL SIZED WOMAN WOULD ONLY HAVE ONE RECOURSE AND THAT IS TO STOP ABRUPTLY. THIS WILL LEAD TO AN ACCIDENT. WHEN THE POWER STEERING WENT OUT ON ME AT 35 MPH, I HAD NO ABILITY TO ABRUPTLY STOP AS I HAD A CAR IMMEDIATELY BEHIND ME. THAT WOULD HAVE RESULTED IN BEING REAR ENDED. IF I HADN'T BEEN ABLE TO STEER IT BY SHEER BRUTE FORCE, WE WOULD HAVE CRASHED INTO THE TREES ALONG THE ROAD. WHEN THE VEHICLE WAS TOWED TO THE DEALERSHIP THE POWER STEERING WOULD FAIL WHILE SITTING STILL, MUCH LESS DRIVING LIKE IT DID WHEN IT FIRST STARTED. Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Consequence & remedy

Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy: Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

Chrysler (FCA US, LLC) is recalling certain 2017-2023 Pacifica Plug-In Hybrid Electric vehicles (PHEVs). An internal transmission wiring connector may short circuit, resulting in an unexpected engine shutdown.

Consequence & remedy

Consequence: An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash.

Remedy: Dealers will update the Power Inverter Module (PIM) software and, if necessary, update the instrument panel cluster (IPC) and the electronic control module (ECM) software, free of charge. The Office of Defects Investigation (ODI) is opening this Preliminary Evaluation to assess the scope, frequency, and potential safety consequences of alleged defects related to the electric power steering (EPS) system in model year (MY) 2017–2018 Chrysler Pacifica vehicles. ODI has received 94 Vehicle Owner Questionnaires (VOQs) reporting a condition in which the steering system exhibits a sticking sensation during gradual turns, particularly when the steering wheel is positioned near the 11 o'clock or 1 o'clock position. Complainants allege that during these events, increased steering effort is required to return the wheel to center. In many cases, this is followed by a sudden reactivation of EPS assistance, which may result in an oversteer condition. This behavior could increase the risk of a crash, especially at higher speeds. ODI has identified multiple VOQs describing similar symptoms. This investigation will assess the frequency, root cause, and potential safety-related consequences of the alleged EPS malfunction. On February 11, 2022, Fiat Chrysler Automobiles (FCA) issued a safety recall (NHTSA Recall 22V-077) on all 2017-2018 Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016 through August 7, 2018. The recall addresses ignition “OFF’ fires that originate at the high voltage (HV) battery pack assembly. An ignition ‘OFF’ vehicle fire can result in an increased risk of occupant injury, injury to persons outside the vehicle, and property damage, with little to no warning. At the time of the recall filing the root cause of the vehicle fires was unknown and under investigation by FCA and suppliers. A software remedy designed to improve thermal event detectability by monitoring battery diagnostics is currently being installed on subject vehicles as an interim remedy. As of November 23, 2023, the root cause of the battery failure is unknown. From December 2022 through December 2023, NHTSA has received four consumer complaints (VOQs) alleging vehicle thermal events involving recalled 2017 and 2018 Chrysler Pacifica PHEVs. Additional information collected from complainants confirmed a thermal event originating near or at the HV battery pack after the Recall 22V077 remedy had been applied to the vehicle. Follow up meetings with FCA and suppliers in November and December 2023 to discuss the ongoing root cause investigation and post-recall remedy incidents indicated a recent increase in HV battery thermal events. Furthermore, a review of NHTSA complaint data indicated the post-recall HV battery thermal event complaint rate now exceeds pre-recall levels. By letter dated November 30, 2017, the Center for Auto Safety (CAS) petitioned the National Highway Traffic Safety Administration requesting it open an investigation into allegations of engine stalling in the model year (MY) 2017 Chrysler Pacifica, a new vehicle platform.
