Found out it was coolant leaking in to combustion chamber due to warped head/head gasket leak on rear side of the engine (cylinders 1,3,5). We have 85K miles on it and had to pay $5400 to repair and ~$1000 in rental car fees while in the shop for two weeks.

On the highway the temp gauge went all the way to hot and then immediately went back to normal temp. We pulled over on the shoulder of the busy highway and let it sit for a bit, and it never showed hot again and stayed in the normal range. Drove 5 more miles to home and checked the coolant and it was almost empty. We filled it and kept checking it every day. Had to add more every other day till we could get it in to a mechanic. a few days later the engine idled rough and check engine light came on. Took to mechanic and he said code P03305 showed as a mis fire in cylinder #3. He replaced the coil pack and spark plug and did a pressure test on the coolant system to see if he could find the leak. He said I could find a leak and bet that it was a crack head leaking coolant into cylinder #3. Ran better for another week or so while we were waiting for our appointment at the dealership to have them look into where the leak was. 2 days before our appointment at the dealership, we had the same rough idle and code P03050 saying misfire in cylinder #3. I replaced just the spark plug and then took it to the dealership. They misdiagnosed the problem as the reservoir was leaking. After they replaced it, we were back 2 weeks later with the coolant still leaking somewhere. They eventually found the head was warped due to excessive heat and coolant leaking into the #3 cylinder. This van has around 73,000 miles on it. After researching, it seems Chrysler has a faulty engine that is causing people to break down on the highway which is not a safe place to be. Please look into Chrysler's faulty 3.6 penstar motor problems.

The contact owns a 2017 Chrysler Pacifica. The contact stated that while driving at an undisclosed speed, the check engine warning light illuminated. The vehicle was taken to CarMax where it was diagnosed; however, the cause of the failure could not be determined. The vehicle was then taken to the dealer where it was diagnosed and determined that the oil needed to be changed. The vehicle was repaired; however, the failure recurred. The check engine warning light illuminated. The vehicle was taken back to the dealer where it was determined that either the fuel filler, filler neck, or filler tube failed and could not be repaired or replaced. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 56,054.

If this would have happened on the road my wife and kids would of been stranded. The vehicle is at a Chrysler Dealership and at first they thought it was the starter, but determined that it was the head gasket because there was fluid in the oil. the Engine light came on right before the car wouldn't start. I did see there is a class action suit for this type of van and year.
My car unexpectedly stalls while being driven.
Around 79k miles the car began hesitating and rough idling. From the OBDII sensor: Error Code P0301: Cylinder 1 misfire detected
I was driving about 2Omph and all of a sudden the van slowed down and I had a message on the car to shift to P to Park.it was weird because I was driving and it basically stopped on me and I had to shift the gear to P and press the start button to restart.

P0303 error code for misfire in the engine, 3rd Cylinder. Repair was completed by the dealer. It was 13,000 miles outside of drivetrain warranty. Now, the second failure is 3 months outside the 2year, unlimited mileage service warranty from when the head gasket and seat was replaced on the #3 cylinder. I just learned there was a class action ruling related to my model car and engine due to a faulty crankshaft. I have no idea if this could result in my current engine problems as I continue to have difficulty reaching the appropriate Chrysler representative to ask. I read in another class action lawsuit (which has been filed I believe by a law group on the East Coast, and then postponed due to COVID) for specific defects inside the engine resulting in misfires, hesitation during automatic shifting, and sudden increases in speed (which I have experienced during freeway driving). One of the symptoms of this issue is a ticking sound when the engine is turned off, which my car has always done, though I was told some years ago this is normal for this car. It took 3 weeks and about half a dozen calls to the dealer service department before I got a call back to schedule a diagnostic which is another 6 weeks out.

Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.
Consequence & remedy
Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.

Chrysler (FCA US LLC) is recalling certain 2017-2018 Chrysler Pacifica vehicles equipped with the 8-passenger seating option. In certain passenger seating and vehicle situations, the second-row center seating position seat belt buckle could cause the left outboard seat belt to become unlatched.
Consequence & remedy
Consequence: Inadvertent unlatching of the left outboard seat belt can increase the risk of injury in the event of a crash.

The affected vans have rear brake lines that might have been crossed during the modification. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 135, "Light Vehicle Brake Systems."
Consequence & remedy
Consequence: The crossed rear brake lines will cause the rear anti-lock brake system to be ineffective, increasing the risk of a crash.

Chrysler (FCA US LLC) is recalling certain model year 2017 Pacifica vehicles manufactured February 22, 2016, through April 28, 2016. The center overhead seat belt stow clip may fracture and prevent use of the seat belt in the third-row and/or second-row center passenger seat (if so equipped).
Consequence & remedy
Consequence: If passengers occupying the third-row and/or second-row center seats cannot use a seat belt, there is an increased risk of injury in the event of a crash.
Remedy: Chrysler will notify owners, and dealers will replace the third-row and, if necessary, second-row center overhead seat belt stow bezel, free of charge. The recall began on July 14, 2016. Owners may contact Chrysler customer service at 1-800-853-1403. Chrysler's number for this recall is S42.

The Office of Defects Investigation (ODI) is opening this Preliminary Evaluation to assess the scope, frequency, and potential safety consequences of alleged defects related to the electric power steering (EPS) system in model year (MY) 2017–2018 Chrysler Pacifica vehicles. ODI has received 94 Vehicle Owner Questionnaires (VOQs) reporting a condition in which the steering system exhibits a sticking sensation during gradual turns, particularly when the steering wheel is positioned near the 11 o'clock or 1 o'clock position. Complainants allege that during these events, increased steering effort is required to return the wheel to center. In many cases, this is followed by a sudden reactivation of EPS assistance, which may result in an oversteer condition. This behavior could increase the risk of a crash, especially at higher speeds. ODI has identified multiple VOQs describing similar symptoms. This investigation will assess the frequency, root cause, and potential safety-related consequences of the alleged EPS malfunction. On February 11, 2022, Fiat Chrysler Automobiles (FCA) issued a safety recall (NHTSA Recall 22V-077) on all 2017-2018 Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016 through August 7, 2018. The recall addresses ignition “OFF’ fires that originate at the high voltage (HV) battery pack assembly. An ignition ‘OFF’ vehicle fire can result in an increased risk of occupant injury, injury to persons outside the vehicle, and property damage, with little to no warning. At the time of the recall filing the root cause of the vehicle fires was unknown and under investigation by FCA and suppliers. A software remedy designed to improve thermal event detectability by monitoring battery diagnostics is currently being installed on subject vehicles as an interim remedy. As of November 23, 2023, the root cause of the battery failure is unknown. From December 2022 through December 2023, NHTSA has received four consumer complaints (VOQs) alleging vehicle thermal events involving recalled 2017 and 2018 Chrysler Pacifica PHEVs. Additional information collected from complainants confirmed a thermal event originating near or at the HV battery pack after the Recall 22V077 remedy had been applied to the vehicle. Follow up meetings with FCA and suppliers in November and December 2023 to discuss the ongoing root cause investigation and post-recall remedy incidents indicated a recent increase in HV battery thermal events. Furthermore, a review of NHTSA complaint data indicated the post-recall HV battery thermal event complaint rate now exceeds pre-recall levels. The Office of Defects Investigation (ODI) is opening this Recall Query (RQ) to review the effectiveness of the original recall remedy, understand the root cause of the battery fires, investigate additional reports of Pacifica PHEV HV battery fires, and to increase monitoring of the manufacturer root cause investigation. By letter dated November 30, 2017, the Center for Auto Safety (CAS) petitioned the National Highway Traffic Safety Administration requesting it open an investigation into allegations of engine stalling in the model year (MY) 2017 Chrysler Pacifica, a new vehicle platform.
