I am driving the vehicle and suddenly the information center and entertainment center will suddenly go black without warning. This will stay black for approximately 10 seconds. Followed by 2 chimes. A series of 4 lights on the drivers side front dash will come on. I believe this is related to the forward collision system(the lights that come on). The entertainment and information center then come back on without any errors or messages. I noticed this issue occurred all spring, summer and fall of and part of winter of 2025. Then it stopped thru mid april 2026 and then started again. The adaptive cruise is also shutting off without warning saying it's temporarily unavalable.

On October 8, 2025 My cousin, [XXX] , and I were on highway Mile 113, 65 North and were about to exit when an ambulance exited onto the highway ahead causing the traffic to suddenly stop. As the traffic stopped, I hit my brake, which usually jolts when I hit it but it didn't this time, the brake didn't feel the same, the brake was soft and it felt like my car wasn't stopping. The warning red light that usually goes off didn't and I hit a Jeep at a speed of about 55 to 60 mph yet none of my airbags diployed. We smelled fire and saw some smoke come in from the vents but couldn't see over the hood that had been bent up, both of our seatbealts had been stretched out so that when we took them off, we could not put them back on, and the adjuster I had on my seatbealt to keep it off my shoulder completely broke off. I pull over as far as I could safely since I couldn't see. The driver of the Jeep got out of her car and inspected it, she came back to the passenger side of my car and said that she was in a rental, there was no damage and couldn't afford to turn in accident in so she had to leave, and left the scene. Both my cousin and I were feeling pain in our chest so when 1st responder came, he called for an ambulance. My car was towed and we were both taken to Eskenazi ER where my cousin was treated and released the same day. I was admitted into the hospital ICU and had emergency cervical fusion spinal cord surgery on October 9, 2025 and wasn't released until October 17, 2025 with ongoing treatment for therapy and loss of mobility of my right arm which is due to nerve damage and abdomen bruising, I am still recovering and have ongoing treatments for my spine. I noticed the alarm didn't sound when it should have times before but since nothing was said about any problem when I took my car in for service in September, I didn't think that there would be an issue like this. The contact owns a 2025 Chevrolet Trax. The contact stated that on two separate occasions while driving at highway speeds during sunset, the Forward Collision Avoidance feature engaged unintendedly. Due to the failure, the vehicle independently came to an abrupt stop with a RED light flashing on the windshield. The vehicle was taken to a dealer to be diagnosed. The contact was informed that the diagnostic test indicated two lost connections, and the electrical system was reset. After the system reset the message "Service Side Detection System" was displayed. The manufacturer was notified of the failure but offered no assistance. The contact was referred to the NHTSA Hotline for assistance. The vehicle was not repaired. The failure mileage was approximately 22,000.

She slammed on her breaks again and my breaking system kicked in again but I also slammed my foot on the break. My car did not stop immediately it felt like the braking system had control over how fast I could stop my car. When my car was towed to the shop I had mentioned it to the shop workers but not much was said after that

26V539000 · Back Over Prevention: Sensing System: Camera

General Motors, LLC is recalling certain 2024-2025 Buick Envista, 2024-2026 Buick Encore GX, Chevrolet Trailblazer, and 2025 Chevrolet Trax vehicles. The rearview camera screen may display a distorted or blank image.

Consequence & remedy

Consequence: A rearview image that does not display correctly reduces the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will inspect and replace the rearview camera, if necessary, free of charge. Owner notification letters are expected to be mailed beginning September 16, 2026. Owners may contact Chevrolet customer service at 1-800-222-1020 or Buick customer service at 1-800-521-7300. GM's number for this recall is A262554900. Vehicle Identification Numbers (VINs) involved in this recall became searchable on NHTSA.gov on August 20, 2026.
