HE WAS ABLE TO MANEUVER ONTO THE SHOULDER TO AVOID HITTING THE VEHICLE IN FRONT OF HIM BUT THE BRAKES WERE NO LONGER OPERATING PROPERLY. THERE WAS NO LONGER ANY RESISTANCE IN THE BRAKE PEDAL AND MINIMAL STOPPING POWER. `HE WAS ABLE TO BRING THE VEHICLE INTO A PARKING GARAGE BY DRIVING SLOWLY AND IN LOW GEAR. I HAD THE VEHICLE TOWED HOME AND EXAMINED IT AND DETERMINED THAT THE BRAKE FLUID RESERVOIR WAS EMPTY AND THAT IT HAD LEAKED OUT OF A CORRODED AND BROKEN BRAKE LINE AT THE MIDPOINT OF THE CHASSIS. ALL OF THE METAL BRAKE LINES ARE CORRODED AND WILL NEED TO BE REPLACED. THERE WAS NO ADVANCE WARNING OF ANY IMPENDING BRAKE LINE FAILURE. THE VEHICLE HAS BEEN REGISTERED IN MARYLAND SINCE PURCHASED NEW AND THERE IS RELATIVELY LITTLE SNOW THAT FALLS IN THE WINTER AND AS A RESULT THERE IS ALMOST NO OTHER CORROSION ON THIS VEHICLE SO SEEING THE BRAKE LINES CORRODED LEADS ME TO BELIEVE THAT AN INFERIOR GRADE OF STEEL WAS USED ON THIS CRITICALLY IMPORTANT COMPONENT. TO MAKE MATTERS WORSE THE BRAKE LINE IN THE AREA OF THE FAILURE IS LOCATED ABOVE THE FRAME AND IS NOT VISIBLE FROM BELOW AND THEREFORE CANNOT BE INSPECTED FOR CORROSION. WHEN THE OIL IS CHANGED AND WHEN THE BRAKES ARE REPLACED THE BRAKE LINES ARE CHECKED BUT IN THE AREA OF THE FAILURE AS STATED THEY CANNOT BE INSPECTED. I BELIEVE GM SHOULD BE REQUIRED TO RECALL ALL VEHICLES WITH THIS TYPE OF METAL BRAKE LINE AND BE REQUIRED TO REPLACE THEM WITH A BETTER GRADE OF STAINLESS STEEL AS IT IS REASONABLY FORESEEABLE THAT FATALITIES WILL BE THE LOGICAL CONSEQUENCE OF THIS TYPE OF FAILURE IN A SIGNIFICANT PERCENTAGE OF THESE INCIDENTS. *TR

WHEN I CAME TO THE NEXT LIGHT ABOUT 200 YARDS FROM WHEN I NOTICED THE PROBLEM AND BEGAN TO TURN, THE CAR STALLED AND I LOST POWER STEERING AND POWER BRAKES. I WAS ABLE TO BRING THE CAR TO A STOP, BUT WAS NOW PERPENDICULAR TO ONCOMING TRAFFIC AND COULD NOT MOVE FORWARD OR BACKWARD. I ASKED THE THREE OLDEST CHILDREN TO EXIT THE VEHICLE IN THE ACTIVE ROADWAY AND PUSH THE TRUCK SLIGHTLY UPHILL TO THE SIDE OF THE RIGHT MOST LANE TO CLEAR THE INTERSECTION. I THEN HAD TO TOW THE CAR BACK TO THE HOUSE. I FOUND THAT THE FUEL PUMP FUSE HAD BLOWN AND REPLACED IT. DESPITE THIS THE CAR WAS STILL HARD TO START. I RESEARCHED THE PROBLEM ON THE INTERNET AND FOUND NUMEROUS REFERENCES TO THIS VEHICLE BLOWING FUSES, IN ALL CASES REQUIRING REPLACEMENT OF THE FUEL PUMP. I NOTED IN ONE COMPLAINT THAT THERE WAS AN NHTSA CAMPAIGN (05V155000) RELATED TO THIS PROBLEM ON THE SAME MODEL YEAR CHEVROLET SUBURBAN THAT RESULTED IN A RECALL. I AM CONCERNED THAT A RECALL FOR ANOTHER NEARLY IDENTICALLY CONSTRUCTED VEHICLE AFFECTING THE SAME COMPONENT INSTALLED IN MY VEHICLE WOULD NOT WARRANT NOTICE TO ME THAT I COULD SUFFER THE SAME FAILURE PLACING OCCUPANTS IN A VERY DANGEROUS SITUATION. HAD I BEEN NOTIFIED OF THE LIKELIHOOD OF THIS OCCURRENCE THAT COULD RESULT IN MY VEHICLE AND ITS OCCUPANTS BEING STRANDED IN AN ACTIVE ROADWAY, I WOULD HAVE INITIATED PREVENTATIVE REPAIRS. THIS RECALL WILL BE LAUNCHED IN THE "SALT BELT" STATES OF CONNECTICUT, DELAWARE, ILLINOIS, INDIANA, IOWA, MARYLAND, MASSACHUSETTS, MAINE, MICHIGAN, MINNESOTA, MISSOURI, NEW HAMPSHIRE, NEW JERSEY, NEW YORK, OHIO, PENNSYLVANIA, RHODE ISLAND, VERMONT, WEST VIRGINIA, WISCONSIN AND THE DISTRICT OF COLUMBIA ONLY.

Consequence & remedy

Consequence: THIS CAN CAUSE INCREASED STOPPING DISTANCES DURING LOW-SPEED BRAKE APPLICATIONS, WHICH COULD RESULT IN A CRASH.

Remedy: DEALERS ARE TO REMOVE THE WHEEL SPEED SENSOR AND THOROUGHLY CLEAN THE WHEEL SPEED SENSOR MOUNTING SURFACE ON THE BEARING, APPLY RUST INHIBITOR TO THE CLEANED SURFACE, GREASE THE MOUNTING SURFACE, REINSTALL THE WHEEL SPEED SENSOR, AND CHECK THE PEAK-TO-PEAK OUTPUT VOLTAGE TO ENSURE THE WHEEL SPEED SIGNAL IS WITHIN SPECIFICATIONS. THESE UNITS CONTAIN A COURTESY LIGHT OR LIGHTS THAT MIGHT OVERHEAT WHEN THE WIRE HARNESS IS EXPOSED TO EXCESSIVE MOISTURE AND ROAD SALT.

VEHICLE DESCRIPTION: PICKUP AND UTILITY TRUCKS. SOME OF THESE VEHICLES HAVE AN INTERNAL COMPONENT IN THE OUTBOARD SEAT BELT RETRACTORS FOR THE 2ND AND 3RD ROW OF SEATS THAT COULD BE CRACKED. WITH REPEATED ACTUATION OF THE LOCKING MECHANISM, THE CRACK COULD SPREAD TO THE POINT SUCH THAT THE SEAT BELT WOULD NO LONGER LOCK.

Consequence & remedy

Consequence: IN A CRASH, INCREASED OCCUPANT INJURY COULD RESULT FROM THE NO-LOCK CONDITION.

THIS INVESTIGATION INVOLVES THE GENERAL MOTORS CORPORATION (GM)THROTTLE BODY (TB) UTILIZED IN MODEL YEAR (MY) 1999-2002 SILVERADO, SIERRA, TAHOE, SUBURBAN, AVALANCHE AND YUKON (SUBJECT VEHICLE(S)) WITH 4.8L, 5.3L, AND 6.0L ENGINES. THE TB VALVE MAY INTERMITTENTLY STICK IN A CLOSED POSITION.IN SUCH A SITUATION, AN OPERATOR MAY APPLY ADDITIONAL ACCELERATOR PEDAL FORCE TO INCREASE ENGINE SPEED.THE APPLICATION OF ADDITIONAL ACCELERATOR PEDAL FORCE, TO OPEN A STUCK THROTTLE VALVE, MAY OPEN THE THROTTLE VALVE MORE THAN INTENDED AND, IN TURN, ACCELERATE THE ENGINE AND VEHICLE MORE THAN INTENDED AND REASONABLY EXPECTED BY THE DRIVER.ON AUGUST 1, 2002, THE OFFICE OF DEFECTS INVESTIGATION (ODI), OF THE NATIONAL HIGHWAY TRAFFIC SAFETY ADMINISTRATION (NHTSA), OPENED THIS ENGINEERING ANALYSIS.THE INVESTIGATION REVEALED THAT TWO FACTORS MAY CAUSE THE DEFECT: 1) TB MANUFACTURING PROCESS PROBLEMS; AND 2) ACCUMULATION OF DEPOSITS (A PCV SYSTEM BY-PRODUCT) ON OR AROUND THE TB THROTTLE VALVE AND BORE.ODI CONCLUDES THAT THE TB PERFORMS IN A DEFECTIVE MANNER AFTER ANALYZING GM?S DATA SUBMISSIONS, AND TEST DATA COLLECTED BY THE VEHICLE RESEARCH AND TEST CENTER (VRTC).FIFTY-NINE CRASHES ARE ALLEGED TO HAVE OCCURRED AS A RESULT OF GM?S TB DEFECT.THE MAJORITY OF THE CRASHES INVOLVE A SINGLE VEHICLE, ENGAGING IN A CLOSE QUARTER VEHICLE MANEUVER (OR LOW SPEED), AND CAUSING ONLY MINOR PROPERTY DAMAGE.FOUR OUT OF THE FIFTY-NINE CRASHES INVOLVE INJURY, AND THOSE INJURIES WERE MINOR.ALTHOUGH GM?S TB IS DEFECTIVE, BASED ON THE EVIDENCE GATHERED, ODI HAS FOUND THAT THE ACCELERATOR PEDAL FORCE REQUIRED TO OPEN A STUCK THROTTLE VALVE IS NEGLIGIBLE, AND THE THROTTLE VALVE DOES NOT STICK IN THE OPEN POSITION.THEREFORE THIS ENGINEERING ANALYSIS (EA02-015) WILL BE CLOSED AT THIS TIME.THE CLOSING OF THIS INVESTIGATION DOES NOT CONSTITUTE A FINDING BY NHTSA THAT A SAFETY-RELATED DEFECT DOES NOT EXIST.FOR A DETAILED DISCUSSION OF THE TB COMPONENT AND TESTING, PLEASE SEE THE ATTACHED REPORT.
