TL*THE CONTACT WAS DRIVING A 1998 CHEVROLET G2500 VAN. WHILE DRIVING APPROXIMATELY 5 MPH THE CONTACT STATED THAT HE ENGAGED THE BRAKES AND CRASHED INTO THE VEHICLE IN FRONT OF HIM. TL*THE CONTACT WAS DRIVING A 1998 CHEVROLET G2500 VAN. WHILE DRIVING APPROXIMATELY 5 MPH THE CONTACT STATED THAT HE ENGAGED THE BRAKES AND CRASHED INTO THE VEHICLE IN FRONT OF HIM. HE STATED THAT THERE WAS NO DAMAGE TO THE OTHER VEHICLE. NO ONE WAS INJURED. A POLICE REPORT WAS FILED. THE CONTACT WAS DRIVING A COMPANY VEHICLE FOR TWO WEEKS AND EXPERIENCED BRAKE AND ACCELERATOR FAILURE PRIOR TO THE CRASH. THE VEHICLE WAS NOT REPAIRED. THE VIN WAS NOT AVAILABLE. THE FAILURE MILEAGE AND CURRENT MILEAGES WERE UNKNOWN.
