OnStar module has failed and malfunctioned. Vehicle is available for inspection. Vehicle now sates it needs an important safety update from GM, but vehicle will not update. Vehicle has not been to a dealership at this time, because I can not get a dealership to give me an estimate for repairs, not will they provide a loaner. But the error code has been verified in writing by OnStar- yet they refuse to just send a new module to preferred dealer so I can get it repaired. Absolutely no warnings on vehicle. Forward emergency braking system engaged when not needed and almost put myself and my grandkids through the windshield. (even though we were all properly restrained) I have since had to disengage this feature because it has now happened 2 times. There are thousands of persons with these same issues as can be found on the OnStar community or Chevrolet Forums.

(1) On Saturday 8/23/25, the braking system failed while driving on gravel roads going downhill. 
 (2) I could not stop the vehicle going downhill. I thought I was going to crash. As I started to go downhill @ ~5 mph, I put my foot on the brake pedal, the car would not brake. Pressing hard or lightly didn’t make a difference. It felt like an anti-lock brake (ALB) pulse 1X and then gave up releasing all braking even though the foot is on the brake. The 2-way road is heavily used, smooth, wide, and maintained. Other vehicles were going down the gravel fine. The grade is ~10%. (3) The problem was reproduced numerous times on 8/24/25, while my husband drove on gravel going downhill and attempting to slow down or stop the car. This gravel is with potholes and some rocks starting w/ grades 5-6%. I was a passenger in the car. At one point I thought we were going to hit the on-coming car, because the brakes were not working properly. (4) An appointment is being made w/ the dealer to inspect the car. (5) (On 8/23) I am not aware of warnings or messages. (On 8/24) A “ABS” alert appeared on the display sometimes. Additional Info: (6) On 6/15/25, I drove on gravel with 5-10% grades w/o issues for ~6 miles. When I pressed on the brake pedal, the car slowed down without issues. ALB did not engage. (7) I have recently read break failure stories that are similar from other Equinox EV owners. "... my car has had two separate brake failures..." "… now going down a fairly steep gravel hill daily, i experience very little braking..." "… I pressed the brakes and there was no perceived braking for at least 20 feet... " ... brake failure this past weekend while descending a dirt/stone road… The ABS, would not shut off for our entire descent, which lasted for about 2 full minutes. During this time, we could not stop the car and were accelerating out of control…."

I experienced a brake failure this past weekend in my 2025 Chevrolet Equinox EV LT while descending a dirt/stone road. The moment we started descending, the ABS system engaged and did not brake the car adequately. In my opinion, the ABS system should not have engaged in this terrain. The ABS, would not shut off for our entire descent, which lasted for about two full minutes. During this time, we could not stop the car and were accelerating out of control. We got going far faster than I was comfortable with, and my passenger & I thought we were going to end up crashing. After about a minute of inability to brake properly or stop, we ended up getting a flat tire. The flat tire, combined with the terrain flattening, eventually helped us come to a complete stop. After stopping, the car was also having software issues (infotainment system resetting), and the infotainment system was very hot to the touch, suggesting an electrical or software malfunction tied to the braking system. It was a very scary situation, but my passenger and I were not injured. This failure left me and my passenger completely out of control of the vehicle, with a high risk of collision, rollover, or serious injury. In my opinion, a braking system that does not stop the vehicle when the pedal is fully applied constitutes a critical safety defect. Chevrolet has been contacted, but the dealer refused to treat this as a warranty safety issue so far. The vehicle remains available for inspection.

I purchased this vehicle new. Within weeks started having issues with the OnStar system. Obviously with the module not working, other things began happening. I can't see status of charge. I can't find charging stations when driving. My hands free doesn't work because google is always off line. Can't use OnStar system because it doesn't work. Came out one morning to find my vehicle unlocked and all the windows down. I live in Florida, have never once used the defrost system or set the vehicle temperature above 72. Turned the car on one day and the defrosters magically turned on and the temperature went up to 76 full blast. The the forward braking system started slamming the brakes when I was already stopping. One day it slammed us so hard that my grandchildren screamed because their car seat belts cut into their shoulders. Contacted OnStar numerous times. Contacted dealerships numerous times. After weeks and weeks, I finally have a dealer willing to look at it. I have been provided one fault code by OnStar. There are so many people with issues and research shows the module is and has been on back order for months and months. Vehicle also makes noises like it is trying to lock or unlock whenever I open a door. There were no warnings for any of it. Just began. Safety is at risk because vehicle malfunctions. OnStar confirmed there is an error. Going to dealer finally this week. No warning lamps and issues got bad enough for concern approximately one month ago but were noticed within weeks of purchasing vehicle which was purchased [XXX]. The 2025 Chevrolet Equinox EV LT FWD was purchased with an HD Surround Vision system that advertises multiple camera views including standard and overhead front/rear perspectives and rear hitch area monitoring, but lacks any convenient dashboard control to access these features when needed. Currently, drivers must shift into reverse gear then back to drive to activate the cameras, creating safety hazards when maneuvering in tight parking spaces, street parking, or narrow garages where visibility is most critical. Despite eight months since purchase and three service visits to Capitol Chevrolet Austin, no solution has been provided, with the dealership service team showing reluctance to address this functionality issue. This matter requires escalation to appropriate stakeholders, as customers should not pay for advertised safety features that lack practical usability—the current implementation renders the HD Surround Vision system ineffective for its intended purpose and creates the very safety risks it was designed to prevent, necessitating the implementation of proper dashboard controls to access all camera views without requiring potentially dangerous gear changes.

General Motors has decided that certain 2027 Chevrolet Bolt EV, 2025-2026 Cadillac OPTIQ, Chevrolet Colorado, Equinox EV, GMC Canyon, 2026 Buick Enclave, Envision, Cadillac CT5, Escalade, Escalade ESV, Escalade IQ, Escalade IQL, LYRIQ, VISTIQ, Chevrolet Blazer EV, Corvette Convertible, Corvette Coupe, Corvette Convertible E-Ray, Equinox, Silverado 1500, 2500, 3500, Silverado EV, Suburban 1500, Tahoe, Traverse, GMC Acadia, Hummer EV Pickup, Hummer EV SUV, Sierra 1500, 2500, 3500, Sierra EV, Terrain, Yukon, and Yukon XL vehicles. The radio may not have been set to the correct status to download the electronic owner's manual during production. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, “Occupant Protection.”

The pedestrian alert sound system may not produce a sufficient change in volume between the vehicle being stopped and moving at low speeds. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 141, "Minimum Sound Requirements for Hybrid and Electric Vehicles."

Consequence & remedy

Consequence: A vehicle that does not emit sufficient sound may fail to alert pedestrians of a moving vehicle, increasing the risk of a crash or injury.

Remedy: The body control module software will be updated by a dealer, or through an over-the-air (OTA) software update, free of charge. Owner notification letters were mailed January 29, 2026. General Motors, LLC (GM) is recalling certain 2025 Chevrolet Blazer, Blazer EV, and Equinox vehicles. The door strikers were not properly heat-treated, which can cause them to break.

Consequence & remedy

Consequence: A broken door striker can cause the door to open unexpectedly, increasing the risk of injury.

Remedy: Dealers will replace all four side door strikers and attachment bolts, free of charge. All affected vehicles are still in dealership inventory and therefore no owner notification letters are expected to be mailed. Owners may call Chevrolet customer service at 1-800-222-1020. EA26006 · Electronic Brake Module Component Failure

On April 11, 2024, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE24011 after receiving complaints concerning a loss of brake assist in 2023 MY Cadillac Lyriq vehicles. These complaints allege a hard brake pedal, followed by a “Brake System Failure” message at start up or while driving. GM shared records regarding several internal investigations for loss of brake assist due to alleged spindle fractures in the eBoost system. GM has stated that in the event of a spindle fracture, anti-lock brakes (ABS), stability control, and traction control remain functional until the vehicle comes to a complete stop. At this point, ABS, brake assist, stability control, and traction control functions are lost. The loss of these functions causes the vehicle to set one of three Diagnostic Trouble Codes (DTCs) based on failed performance criteria for the Electronic Brake Control Module (EBCM). These DTCs, in turn, activate several driver alerts: an audible chime, a visual “Service Brake System” message in the display, and multiple illuminated Malfunction Indicator Lights (MILs) for the ABS, Traction Control, and Brakes informing the driver of loss of functionality. The vehicle speed will also be limited to 70 KPH (43 MPH). Since opening PE24011, ODI has continued to receive reports of loss of brake assist. Some of these reports describe symptoms inconsistent with GM’s description of a spindle failure. These reports allege an immediate loss of brake assist functionality as opposed to the loss occurring after the vehicle comes to a complete stop. The sudden loss of brake assist during a braking event could result in extended braking distance, which increases the risk of a crash or injury. ODI believes further analysis is necessary to assess the potential for failures in GM’s eBoost system to resulting in safety-related outcomes. At this time, ODI is aware of the following vehicles that are equipped with this eBoost system: 2023-2026 MY Cadillac Lyriq, Chevrolet Colorado, and GMC Canyon; 2024-2026 MY Buick Enclave, Envision; Chevrolet Blazer EV, Equinox EV, Traverse; Cruise Origin; and GMC Acadia; 2025-2026 MY Cadillac Celestiq and Optiq. ODI is also aware of 2024-2026 MY Acura ZDX and 2024-2026 MY Honda Prologue vehicles that were built with this system. The Acura and Honda vehicles were manufactured by GM in a joint venture with Honda, and will be included in the Engineering Analysis as peer vehicles. ODI is opening this Engineering Analysis to further assess the safety-related consequences of a failed eBoost system. In addition, ODI is expanding the scope of the investigation to encompass all GM vehicles that share this eBoost system.
