On [XXX] while driving this vehicle owned by the state of Arizona I was involved in a collision at 40 mph. The airbags did not deploy and I sustained serious brakes and internal bleeding. When describing what happened, try to answer these questions as specifically as possible: What component or system failed or malfunctioned? Is it available for inspection upon request? the engine, when I am at a traffic light or in line at school to pick up my daughter, the car starts to shake suddenly, what I have to do is put it in NEUTRAL so that the movement decreases. Also when I'm driving on the highway they change speeds unpredictably. The engine light comes on intermittently. How was your safety or the safety of others put at risk? It puts my safety and that of my family at risk because if I don't put it in NEUTRAL it can accelerate and collide with the car behind or in front if the distance in the school line is tight. And on the road, having this unpredictable change in speed puts our lives in danger since many cars go at speeds of more than 75 miles per hour. Has the problem been reproduced or confirmed by an independent dealer or service center? The problem was confirmed by an independent mechanic. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No Were there warning lights, messages or other symptoms of the problem before the failure and when did they first appear? When the engine light came on.
